Morphometry and histology of gonads from 13 children with dysgenetic male pseudohermaphroditism.

BACKGROUND: Dysgenetic male pseudohermaphroditism (DMP) is a sexual differentiation disorder characterized by bilateral dysgenetic testes, persistent müllerian structures, and cryptorchidism in individuals with a 46,XY karyotype. However, the histologic criteria for the diagnosis of DMP are poorly established. OBJECTIVE: To determine gonadal histology in children with DMP. PATIENTS AND METHODS: Between 1996 and 1998, 13 patients with DMP were evaluated on our service. The clinical diagnosis of DMP was based on a 46,XY karyotype, sex ambiguity, high levels of follicle-stimulating hormone and low levels of antimüllerian hormone, a decreased testosterone response to human chorionic gonadotropin stimulation without accumulation of testosterone precursors, and the presence of müllerian structures. Molecular sequencing the HMGbox region of the SRY gene did not reveal any mutations. Biopsies were performed for 22 of 26 gonads (patient age at the time of biopsy, 16 months to 10 years). Conventional microscopy was used to evaluate mean tubular diameter, tubular fertility index, and number of Sertoli cells per tubular profile. RESULTS: All 26 gonads were located outside of the labioscrotal folds. Their histologic features varied from only a reduction in tubular size to features of a streak gonad. Five of the 22 gonads grossly resembled a streak gonad. The mean tubular diameter was severely reduced (>30% reduction relative to the normal tubular diameter for the patient's age) in 4 gonads, markedly reduced (10%-30%) in 11 gonads, slightly reduced (<10%) in one gonad, and normal in one gonad. The tubular fertililty index, expressed as the percentage of tubular profiles containing germ cells, was severely reduced (<30% of normal values) in 9 gonads, markedly reduced (50%-30%) in 2 gonads, and normal in 6 gonads. The number of Sertoli cells per tubular profile was elevated in 16 gonads and normal in one gonad. Thin tubules surrounded by fibrous tissue were occasionally observed. CONCLUSION: The histologic findings confirmed the clinical diagnosis of DMP in every patient in the present series. However, gonadal histology was variable, and careful morphometric evaluation may be necessary to establish the diagnosis.

[Relationship between the quantitative morphometry and the clinical response to alpha adrenergic blocker in benign prostatic hyperplasia].

BACKGROUND: There have been few reports on the relationship between the prostatic tissue components and the effect of alpha adrenergic blocker on urination. The present study was undertaken to correlate the rates of the prostatic tissue components and the clinical response to alpha adrenergic blocker in benign prostatic hyperplasia. METHODS: 40 patients with symptomatic benign prostatic hyperplasia were treated with 0.2 mg/day tamsulosin, a alpha adrenergic blocker, for 4-8 weeks followed by transurethral resection of the prostate. The mean weight of resected prostatic tissues was 25.2 g (range 5-96 g). Specimens were stained with antibodies to muscle-specific actin and to prostate-specific antigen for the detection of the smooth muscle and the glandular epithelium, respectively. Double-immunoenzymatic stained tissue sections were divided into four groups: smooth muscle, connective tissue, glandular epithelium, and glandular lumen. The rates of the areas of respective prostatic tissue components were evaluated with computer-assisted quantitative color image analysis. RESULTS: The mean rate of the area of smooth muscle, connective tissue, glandular epithelium, and glandular lumen was 32.7%, 46.9%, 10.8%, and 9.6%, respectively. International prostatic symptom score, maximum flow rate and residual urine volume before the treatment were not related to the rates of respective prostatic tissue components. International prostatic symptom score and residual urine volume were reduced and maximum flow rate was increased after the treatment with tamsulosin. A significant relationship was proved between the rate of smooth muscle and the change in maximum flow rate. The large hyperplastic adenoma contained less smooth muscle and showed a lower sensitivity to alpha adrenergic blocker than the small one. CONCLUSION: The clinical response to alpha adrenergic blocker was related to the rate of smooth muscle in benign prostatic hyperplasia.

Morphometry in clinical dermatology.

Rapid and simple methods are presented which allow the estimation of areas, area fraction and contour lengths in clinical dermatology. They are based on point counting and intercept measurements using simple grids made on transparent film or on overhead foils. Because of their ease of use, these grids allow the determination of the size as well as the irregularity of skin lesions even during daily clinical work. The applications presented here include the area determination of naevi, leg ulcers, wheal and flare reactions and migration areas in lymphocyte and macrophage migration assays. Additionally, the determination of area fraction in psoriasis and sebutape evaluation is described. Other possible applications such as estimation or contour length, and determination of irregularity and estimation of the volume of tumours are discussed.

Measurement variability and diagnostic sensitivity of gastric mucosal inflammatory cell morphometry.

OBJECTIVE: To assess the variability and sensitivity of morphometric measures of gastric mucosal lymphocyte and plasma cells to determine a systematic procedure for evaluating the density of these mononuclear inflammatory cells (MNC). STUDY DESIGN: Gastric biopsies of antrum (n = 3), incisura angularis (n = 2) and corpus (n = 3) from two controls and three patients with Helicobacter pylori-related gastritis (antral, diffuse or multifocal gastritis) were considered. In each biopsy, three fields from each of three sections were selected. In each field, stromal area was obtained by subtracting gland area (GA) from total area, and MNC were counted. Results were expressed as MNC/total mm2 and MNC/stromal mm2. Correlations with GA, coefficients of variation (CV), discriminant power analysis and analysis of variance were performed. RESULTS: Correlations always existed between GA and MNC/total mm2 and rarely between GA and MNC/stromal mm2. CV of MNC/stromal mm2 were lower (18%) than those of MNC/total mm2 (30%). High sensitivity (95.4%) and specificity (95.8%) were found for MNC/stromal mm2 but not for MNC/total mm2. Differences in MNC/stromal mm2 existed in all subjects (P < .0001). Highly significant differences in MNC/stromal mm2 were also found between normal and inflammatory states, gastric sites and sections. CONCLUSION: In contrast to MNC/total mm2, MNC/stromal mm2 is an unbiased estimate of MNC density. The following sampling procedure is proposed: two biopsies from each gastric site, two sections from each biopsy and two microscopic fields from each section.

Three-dimensional in vivo morphometry of trabecular bone in the OVX rat model of osteoporosis.

This paper describes the application of synchrotron radiation microtomography to osteoporosis research. By taking advantage of the high intensity, collimation, and monochromaticity of synchrotron radiation, we have been able to image the three-dimensional trabecular bone structure in living rats, thus providing serial data on the earliest architectural changes that occur with estrogen loss. Results from these in vivo animal experiments demonstrate that one of the earliest manifestations of estrogen loss, in addition to a decrease in the amount of trabecular bone, is decreased connectivity. We demonstrate that estrogen replacement therapy, when initiated soon after significant changes have occurred, restores bone mass to baseline levels but does not recover the trabecular connectivity. Even without an associated recovery in trabecular connectivity, finite element calculations on the three-dimensional images suggest that estrogen recovers the original structural modulus of elasticity. We believe the recovery of the elastic properties is due to an increase in trabecular thickness above baseline values.

Prognostic value of nuclear morphometry in colorectal cancer.

PURPOSE: In many types of cancer, certain morphometric characteristic of tumor cells correlate with patient survival. Our observations suggested that the survival of patients with colorectal carcinomas is negatively correlated with tumor-cell nucleus size. METHODS: We investigated relationships between postsurgery survival and nucleus morphometrics in 90 patients who had undergone resection for a colorectal tumor. The nucleus-size variables considered were maximum diameter, minimum diameter, perimeter, area, and form factor (means for 100 nuclei from each patient were used in all cases). RESULTS: Our results confirmed that patients with large maximum nucleus diameter (where large = greater than the first quartile) have significantly worse survival than patients with smaller maximum nucleus diameter (mean survival, 28 vs. 43 months). Similar results were obtained for the other nucleus-size variables. Stepwise Cox regression analysis was then performed, with postsurgery survival time as the dependent variable and the following candidate independent variables: age, gender, Dukes class, degree of histologic differentiation, the various nucleus-size variables, and relative frequencies of different nucleus shapes (spherical, oval, cylindrical, fusiform, and irregular). The variables selected for the prognostic model were Dukes class, relative frequency of irregular nuclei, and maximum nucleus diameter. CONCLUSIONS: These findings indicated that nucleus size and shape are useful predictors of survival. Even if Dukes class is known, consideration of nucleus size and shape significantly improves prediction of survival.

Morphometry separates squamous cell carcinoma of the vulva into two distinct groups.

The aim of the study was to investigate quantitatively the heterogeneity of vulvar squamous cell carcinoma. The material consisted of archive material from 36 patients with squamous cell carcinoma of the vulva treated at Department of Gynaecology and Oncology. Selected slides were stained with iodidium propide and immunostained for p53 and Ki-67 (MIB-1 clone). Percent of immunopositive nuclei, number of mitotic figures, DNA-ploidy, and geometric nuclear parameters such as area, perimeter, minimum, mean and maximum diameter, shape factor and folding factor were measured with applications running on AnalySIS pro 2.11 (Soft Imaging System GmbH, Germany) image analysis system. Cluster analysis disclosed 2 groups significantly different in respect to patient age, nuclear area, p53 expression and DNA-ploidy. No significant difference in proliferation markers nor clinical stage was found. In other studies also 2 distinct group of squamous cell carcinoma of the vulva are postulated; that groups differ in pathogenesis and epidemiology. Our study confirms this heterogeneity by an independent method. In the present study usefulness of image analysis for separating squamous cell carcinoma of the vulva into distinct groups was explored and proved.

Computer-assisted morphometry of cell-substratum contacts.

AIM: Quantitative analysis of size and shape of the cell-substratum contacts in Dictyostelium and comparison of these parameters between wild-type cells and the cells bearing cytoskeletal protein mutations. METHODS: Reflection interference contrast microscopy (RICM) was used to image the areas of contact between aggregation-competent Dictyostelium cells and weakly adhesive mica surfaces. The cell-substratum contact areas were automatically identified in RICM micrographs by digital image processing. Information about the size and shape of the contact areas was obtained by using the shape descriptors based on two-dimensional geometrical moment invariants. RESULTS: Lack of either of the two actin-crosslinking proteins, a-actinin and 120 kDa gelation factor, similarly affects the cell-substratum interactions of Dictyostelium cells. The shape descriptors, elongation and dispersion, of the contact areas were reduced by 10% to 30% in mutant cells when compared to the wild type, but the size of the contacts was not affected. CONCLUSION: Video microscopy combined with digital image processing and quantitative image analysis is capable of revealing small phenotypic effects of cytoskeletal protein mutations on the level of single cells. Such automated microscopic methods are expected to gain importance and find a widespread use in biomedicine.

DNA ploidy and nuclear morphometry for the classification of dysplastic nevi.

OBJECTIVE: To examine the diagnostic value of DNA ploidy and nuclear morphometric features in sporadic dysplastic nevi as compared to those in compound nevi and melanoma. STUDY DESIGN: DNA ploidy profiles plus seven direct and three derived nuclear features were obtained in a series of 120 melanocytic skin neoplasms (30 dysplastic nevi [DN], 30 melanomas [MM], 60 compound nevi [CN]) and the results compared. RESULTS: DNA ploidy separated melanomas from benign melanocytic skin neoplasms with 96.5% accuracy in classifying the grouped cases. The derived nuclear shape factor Form PE and nuclear axis ratio were the most successful discriminants separating DN from MM but allowed only 73.3% correct classification of cases. Separation of DN from CN was best achieved using Form PE and mean nuclear area (74.4% correctly classified). Results from compound nevi in subjects < 25 years of age fell between those for DN and MM. CONCLUSION: Quantitative nuclear cytologic characteristics in sporadic dysplastic nevi span a range seen in common nevi through to those in thin melanomas. Cytologic changes in sporadic dysplastic nevi overlap those seen in other melanocytic skin neoplasms. Therefore, other reproducible morphometric features need to be assessed in order to further refine the histopathologic diagnosis of this entity.

Placental morphometry in pregnancies complicated by intrauterine growth retardation with absent or reversed end diastolic flow in the umbilical artery.

The aim of this study was to assess any possible correlation between villous tree architecture and its vascularization, and absent or reversed end-diastolic flow velocity (ARED) in the umbilical artery. The study group included seven pregnancies complicated by IUGR (estimated fetal weight < 10th percentile) and absent end-diastolic flow velocity in the umbilical artery. A gestational-age matched group of seven normally grown pregnancies was selected as control group. At delivery, the placenta was weighed and immersed in 10% neutral buffered formalin. A stratified random sampling procedure was used to obtain 12 blocks of full-thickness tissue per organ. A single random section was cut from each block. The following morphometric parameters were evaluated in each section: mean vessel diameter, volume density of the villous tissue, stem villi and terminal villi. Measurements were performed using a computerized Video Image Analysis system. No significant difference in mean vessel diameter was found between the two groups (37.1 microns versus 36.1 microns; p = 0.1). There was a significant reduction in the proportion of total villous tissue in the ARED group (43% versus 52%): this was due to a significant reduction in the volume of tissue occupied by the terminal villi (14.1% versus 18.4%). No significant difference was found in the proportion of villous tissue occupied by the stem villi (42% versus 40%). Several studies have investigated the anatomical and/or vasomotor bases of absent end diastolic flow velocity in the umbilical artery of fetuses with severe IUGR. Our observations of a significant reduction in the proportion of villous tissue occupied by the peripheral villi are consistent with the theory that failure of normal development of the terminal villous is responsible for the increased vascular resistance in IUGR pregnancies with ARED.

Morphometry of the mandible in prepubertal craniofacial microsomia patients following an inverted L osteotomy.

The aim of this study was to undertake finite element modeling of craniofacial microsomia (CFM) patients who exhibited a unilateral mandibular deformity that was surgically corrected by an inverted L osteotomy and autogenous bone graft. Preoperative, 1-year, and 3-year postoperative anteroposterior cephalographs of 14 consecutive children (mean age, about 9 years) with CFM were employed. All cephalographs were scanned, and 8 homologous mandibular landmarks were digitized in triplicate (digitization error < 1%; P > 0.05). Average mandibular geometries, scaled to an equivalent size, were generated using Procrustes superimposition and subjected to analysis of variance. Results showed that while the mean pre- and early postoperative mandibular configurations marginally failed to differ statistically, finite element modeling of the affected mandibular ramus showed a positive allometry (a lengthening by about 10% to 12%) and marked anisotropy, presumably reflecting the surgical procedure carried out in that region. For the preoperative and late postoperative means, the configurations were statistically different (P < 0.002), showing a lengthening of the ramus (about 14%) while the unoperated side had a high degree of isotropy. The early and late postoperative configurations were also statistically different (P < 0.002). The operated ramus showed an increase in length (about 15%), but the corpus showed a decrease in size (about 10%) and marked anisotropy that reflected remodeling associated with the surgical procedure. Although mandibular morphology is improved in CFM patients who are surgically treated by an inverted L osteotomy, some relapse is evident due to deficient growth, localized in the mandibular ramus and body of the affected side.
