[Effects of captopril on microcirculation and tissue oxygen balance in patients with hypertension].

Bulbar conjunctival microcirculation, aggregation of platelets and red blood cells, skin oxygen balance were studied in 22 patients with Stage II hypertensive disease. Captopril was found to decrease not only blood pressure, but aggregation of platelets and red blood cells and to improve tissue oxygen supply. With inadequate doses of the drug, an extreme lowering in blood pressure resulted in deteriorated skin oxygen supply along with unaltered aggregation of cellular elements.

Effects of a human urinary kininogenase (SK-827) on cerebral microcirculation after glass bead-induced cerebral embolism in rabbits.

The pharmacological effects of a human urinary kininogenase (SK-827) on the cerebral microcirculation were studied intravital-microscopically in a rabbit with cerebral microvessel injury induced by glass bead injection into an internal carotid artery. Intravenous administration of SK-827 dilated pial arteries of the brain and increased cerebral hemoglobin content (IHb). SK-827 tended to dilate smaller arterioles more markedly than larger arterioles, and larger arterioles than small arteries. SK-827 appeared to have a vasodilating effects qualitatively different from those of nicardipine and CDP-choline. Findings obtained by concurrent hematological and biochemical examinations also suggested its beneficial effects on the function of platelets and erythrocytes.

[Laser therapy of duodenal ulcers: effect on indices of microcirculation, cell membrane permeability and homeostasis of trace elements].

AIM: To study the efficiency of including laser therapy (LT) in the duodenal ulcer (DU) treatment complex. MATERIALS, METHODS: 123 patients with duodenal ulcer were examined. LT was included in the therapy complex for 89 of them. 34 patients underwent only traditional drug therapy and formed a control group. LT was carried out by 2 ways: intracavitary--by ulcer defect irradiation with the light guide through the gastroscope biopsy canal and helium-neon laser (HNL), and noninvasive--through the front abdominal wall in the corresponding areas, which was made by infrared laser (IRL). RESULTS: The comparison of two groups established that by including LT in the therapy complex for DU patients the dynamics of clinical and laboratorial data is essential better than during traditional drug treatment (TMT). CONCLUSION: It was established that the inclusion of LT in the duodenal ulcer treatment complex has a positive impact on the course of the disease. This impact was displayed as faster reverse development of clinical symptoms, positive laboratory and instrumental data dynamics, blood serum microelements content normalization, membrane permeability increase by indicators of Na-Li counter-transport in the erythrocytes membrane and microcirculation improvement, mainly in the intravascular section.

[Microcirculation end effect of detralex in patients with chronic venous insufficiency of the lower extremities].

Fourteen patients with chronic venous insufficiency (CVI) of the lower extremities, class 0-4 according to the CEAP (1995) were examined for microcirculation (MC) and effect of detralex, using biomicroscopy of the conjunctival vessels and capillaroscopy of the nail matrix, with detailed computed morphometry. In 7 patients, CVI was associated with truncal arterial hypertension and in 5 with type 2 diabetes mellitus. It has been established that in CVI patients, there were remarkable disorders of MC and regional circulation. In addition to regression of the clinical manifestations of CVI, administration of detralex given in courses to 12 patients noticeably refined both regional circulation in the lower extremities and systemic MC, marked by abatement of perivascular edema, an increase in the number of the functioning microvessels and flow acceleration in them. Also, there was marked the lowering of intramuscular red blood cell aggregation.

[The state of hemodynamics and microcirculation in patients with hypertensive disease during long term controlled therapy with lisinopril].

Lisinopril (Diroton, 10-40 mg/day) was given for 6 months to 30 patients (mean age 57.2+/-0.64 years) with stage II hypertension (WHO, 1999). Left ventricular diastolic function and microcirculation were assessed by echocardiography and biomicroscopy, respectively. Treatment with lisinopril was associated with improvements of impaired left ventricular diastolic function, structural and functional state of the heart. Parameters of microcirculatory vascular bed also improved.

Microcirculation: target therapy in cardiovascular diseases - a clinical perspective.

Microcirculation conduit, distribution, exchange and reception vessels usually retain a demand-dependent vascular-tissue match as well as a nutrient friendly capillary-matrix tissue match. Various stimuli can initiate a vascular-capillary matrix-tissue mismatch. Counter-regulatory mechanisms result in hyperplasia or apoptosis. Microvascular disease (MVD) as a consequence or outcome of supply-demand mismatch has clinical therapeutic and prognostic implications in the hypertensive syndrome and coronary artery disease (CAD) cases. Recognition of the role of apoptosis and MVD may initiate a paradigm shift in clinical practice. Digitalis and other anti-hypertensive agents have anti-apoptotic action and MVD blunting effects that can control LVH development to reduce congestive heart failure (CHF) progression.

Asian traditional medicine (ATM) based on in vivo microcirculation evidence.

Asian traditional medicine (ATM) (herbal medicine, acupuncture or moxibution) has gained some popularity among communities in Asia, but there are still few evidences to prove the effectiveness of such therapeutic measures. A symposium was held with aim at the effectiveness of Asian traditional therapies in views of in vivo microcirculation. This report is concerned with the symposium, including Asian activities for ATM.

[State of microcirculation channel in the area of laser "welded" anastomosis of the small intestine].

Experiments were conducted with CO2 and AIG laser on neodymium to study the condition of microcirculation and the degree of its disturbance in the wall of the small intestine in formation of a laser "welded" entero-enteral and termino-terminal anastomosis. The microcirculatory disorders and their extent were found to be directly dependent on the degree of the thermal effect and the width of the coagulation zone in the region of the suture. The results of the experiment confirmed that the suggested powers of the laser effect on the intestinal wall were optimal.

[Measuring microcirculation after replantation and revascularization].

Eighty-four patients were evaluated following replantation or revascularization. Extremity function in daily and professional life was altogether satisfactory. Microcirculation investigations in eight patients included thermography, laser-doppler-flowmetry and photoplethysmography. The rewarming period and postocclusive hyperemia after standardized cooling of the upper extremity were determined. In areas of hypesthesia, the rewarming period was clearly delayed. In addition, reactive hyperemia after occlusion of circulation was significantly reduced.

[Heterogeneity of small intestinal microcirculation].

INTRODUCTION: Microcirculatory disorders play important roles in the impairment of the mucosal barrier of the small intestine. Circulatory failures may increase the heterogeneity of microvascular perfusion, however, conventional methods are inadequate to describe an evolving heterogeneity in time or space. The authors aims were to examine and characterize the microcirculatory reactions in the mucosa and longitudinal muscle of the small intestine in clinically relevant experimental models of circulatory disorders. METHODS: Intravital videomicroscopy with orthogonal polarization spectral imaging technique was used to visualize the microcirculation. A mathematical approach was applied to describe the temporal variability in perfusion based on the calculation of average red blood cell velocity from the relative time periods of observed velocity, while spatial heterogeneity was calculated as a function of the perfused area. The authors' experiments were performed after 1. sham operation, 2. during hemorrhagic shock and crystalloid resuscitation, 3. during 2-hr endotoxin infusion, 4. in response to nitric oxide synthesis inhibition, and 5. after superior mesenteric artery occlusion and reperfusion. RESULTS: During hemorrhagic shock a fluctuating flow pattern appeared in the mucosa, the average red blood cell velocity decreased by 40% in the villi and by 60% in the muscle. Ischemia caused 20% flow reduction in both layers, while endotoxin caused a temporary 20% decrease in the mucosa, and a persistent, over 25% decrease in the muscle layer. Nitric oxide synthesis inhibition resulted in a 40% decrease in both structures. CONCLUSIONS: Heterogeneity of microcirculatory perfusion may appeared both spatially (between or within anatomical layers) and in time (periodically) after the insults. A redistribution of blood flow favouring the mucosa evolves in the small intestine after systemic circulatory disorders. Heterogenous perfusion-related microcirculatory changes can be characterized with mathematical approaches, and these calculations should be taken into account during the comparison of the consequences of distinct circulatory failures involving the small intestine.

Temperature related changes in pulpal microcirculation.

During dental treatment temperatures can be reached which might possibly damage the tooth pulp. To determine the effect of both thermal stimulation on pulpal microcirculation and local anesthesia on thermoregulation we measured the pulpal blood flow by using laser Doppler flowmetry. Experiments were carried out on lower incisors of Wistar rats anaesthetized with thiopental. The rats were divided into three groups, with one remaining untreated, and the others being either desensitized with capsaicin or sympathectomized with guanethidine. In a range between 33 degrees C and 42 degrees C there was no substantial change in blood flow, which, however, was the case below 33 degrees C. Up to 49 degrees C an increase could be recorded in both untreated and guanethidine pretreated animals, whereas the capsaicin groups showed almost no reaction. This increase in blood flow can be blocked reversibly by local anaesthesia. For this purpose we tested articain 5% and mepivacain 3%, both without constrictor. Intravital microscopic studies show that the temperature related increase in blood flow is also associated with plasma extravasation. From these results we draw the conclusion that pulpal thermoregulation is linked to nociceptive sensory neurons and can be described as "neurogenic inflammation".

Functional alterations in pulpal microcirculation in response to various dental procedures and materials.

Using the quantitative 15 microns microsphere injection method, the effects of several restorative procedures on pulpal microcirculation in dog canine teeth were investigated. Pulpal blood flow (PBF) decreased steadily as the remaining dentin thickness (RD) became smaller with crown preparation without water spray. One hour after the preparation wit 1/3 RD (approximately 1 mm) PBF was reduced to 10% of the control, indicating that dry deep preparation has a deleterious effect on PBF. A careful preparation to an RD of 1 mm under copious water spray had a negligible effect on PBF. Dry preparation to an RD of more than 50%, on the other hand, caused a significant increase in blood flow through shunt vessels, AVAs and "U"-turn loops as determined with the 9 microns and 15 microns microspheres. These shunt vessel activities were especially prominent in the apical portion of the pulp, suggesting that the activation of the shunt vessels may be one of the compensatory mechanisms of the pulp in response to inflammation. In rat incisor teeth observed by intravital microscope, shunt vessels opened up as the incisor tips were drilled. Impression procedures after tooth preparation with water spray using the copper band with wax caused severe flow fluctuation as compared to rubber base material impression. In anesthetics, the use of epinephrine was found to be the single most important factor affecting pulp circulation. Whether it is by infiltration or mandibular block, the use of epinephrine containing anesthetic caused a severe reduction in PBF.(ABSTRACT TRUNCATED AT 250 WORDS)

[The role of leukocytes in microcirculation dynamics in the norm and pathology].

The physiological functions grow to be more perfect and complicated in the process of evolution. However, a developing function could display some evidence of regress and, in certain cases, it can trigger serious diseases including autoimmune ones. Conventionally, we refer to the above cases as to "errors" of evolution. The present case study deals with a negative factor of leukocytes in the cerebral microcirculation. Leukocytes were shown, experimentally, to inhibit the capillary blood flow owing to their big volume (2-2.5-fold more versus that of erythrocytes). An increasing trend to adhesion due to hypoxia is responsible for shaping of leukocytes' conglomerates, which cause the occlusion of venules and of the smallest cerebral veins. It entails the effect known as "no re-flow" resulting in death.

Blood rheological abnormalities in the microcirculation during experimental traumatic shock.

OBJECTIVE: To find whether the principal indices of blood rheological disorders related to the microcirculation undergo drastic changes during the traumatic shock. METHODS: RBC aggregability and deformability, as well as the systemic hematocrit, were assessed in white laboratory rats with the techniques that provided us with direct and quantitative data. RESULTS: We found that in the experimental animals the RBC aggregability was sharply increased, the RBC deformability significantly decreased, while the systemic hematocrit underwent considerable lowering during the traumatic shock. CONCLUSION: The blood rheological properties are significantly disordered during development of the traumatic shock.

Direct beneficial effect of insulin on blood rheological disorders in the microcirculation.

Under both the in vivo and in vitro conditions we investigated the insulin effect on the most significant factor disturbing the blood rheological disorders in the microcirculation, the red blood cell aggregability. The in vivo studies we carried out in the 42 insulin treated diabetic patients (diabetes mellitus type II), as well as conducted the in vitro investigations of the blood both of the diabetic patients (24) and of the healthy people (20) where the insulin was added to the blood ex vivo. The RBC aggregability in blood investigated with the "Georgian technique" was found significantly enhanced, by about 100 per cents in the diabetic patients. Under the in vivo conditions insulin administered intravenously decreased the RBC aggregability almost to the normal level in diabetic patients. In addition, in the in vitro studies we found that the insulin lowered significantly the RBC aggregability when it was significantly enhanced by addition of Dextran-500, as well as in the blood of patients with the ischemic brain infarcts. The decreasing effect on the RBC aggregability was observed even in the healthy control group where the RBC aggregability was in a normal range.
