[Are automatic devices suitable for blood pressure determinations?].

Four automatic blood pressure recorders (Bosograph II, Bosch EBM 500, Elag BE 237R, and Physiometrics SR-2) were compared with the random zero sphygmomanometer. Since each of the automatic devices was calibrated for phase IV, diastolic blood pressure values were read off at phase IV and not phase V of Korotkow with the random zero sphygmomanometer. Blood pressure measurements were carried out in 80 patients with hypertension and in 20 normotensive healthy subjects. Readings with the automatic recorders and with the random zero sphygmomanometer correlated well in a wide range of systolic and diastolic blood pressure (r = 0.878--0.982, p less than 0.001). However, the automatic values were in most cases significantly lower than those obtained with the random zero device. For Physiometrics SR-2, where two diastolic readings were possible, only the first diastolic values correlated well with phase IV of Korotkow. The second values were significantly lower, with a difference of -11.1 mm Hg, and probably did not reflect the true diastolic blood pressure. These results show that each of the four automatic recorders is suitable for measuring blood pressure. However, with the Physiometrics SR-2, Only the higher diastolic reading should be used. The differences between the four automatic devices were minor and can therefore be ignored.

Doppler determined blood pressure recordings: the effect of varying cuff sizes in children.

Ten children ages 12 months to 14 years were studied to determine the effect of cuff size on blood pressure determined by the Doppler ultrasonic technique compared to blood pressure recordings by the auscultatory method. It was found that Doppler determined blood pressure varied with cuff size as it does with the auscultatory method. Too small a cuff will give an artificially high recording while too large a cuff causes too low a recording. The effect of the large cuff is less marked than the effect of too small a cuff.

[Problem of early registration of hypertension: combination of x-ray and blood-pressure determination].

In a program aiming at 30,000 blood pressure measurements per year 12,401 persons have been examined in the canton of Berne (population of approx. 1 million) from first of January 1976 to the end of April 1977. This group belongs to the active working part of the inhabitants (age 30 years and more). The introduction of this method was without any problems. 97 to 100 percent of the people participated on a voluntary base. A short personal history of hypertension and smoking habits were requested by a questionnaire. Some results obtained by the use of a general computerprogram are presented: 15.1 % of the screened population had a hypertension greater than or equal to 160/95 mmHg. 55.5 % were unknown to the examined persons. Connections with other findings than hypertension, such as pathologic images of the heart in the x-ray and smoking have been determined. 66.5 % showed one, two or three of the above mentioned findings. The advantages of this method are stressed.

Indirect blood pressure determination by Doppler technique in renal hypertensive cats.

The Doppler ultrasonic recording technique was used to measure systolic and diastolic blood pressures indirectly in renal hypertensive cats. The accuracy of the method was evaluated by comparing indirect blood pressures from one leg of a cat with direct pressure measurements from the other leg. A linear relationship existed between indirect and direct systolic and diastolic pressures. The consistency of the method was assessed by measuring blood pressure during a 5-h monitoring period in normotensive and renal hypertensive cats. No significant variation occurred over this period. The sensitivity of the method to blood pressure changes was determined also. A significant reduction in systolic and diastolic pressure induced by hydralazine, 10 mg/kg po, was recorded during a 5-h monitoring period. The development of renovascular hypertension was followed for approximately 70 days. Systolic pressure rose in a logarithmic fashion from 160 to a maximum of 240 mmHg. It was concluded that the Doppler ultrasonic technique is a simple and reliable method for recording indirect blood pressure acutely and chronically in conscious unrestrained cats.

[Evaluation of an automatic device for arterial blood pressure determination : the Arteriosonde 1217].

The reproductibility and reliability of measurements made with an automatic device for measuring blood pressure was assessed with reference to the results obtained by indirect auscultation with a mercury manometer. The results of the study of the average values of the correlation between successive values, and of the variability of the values of the systolic and diastolic blood pressure show that the measurement obtained by this method are very reproducible.

[Blood pressure determination in children: normal values for a given age group].

Blood pressure was measured under standardized conditions in 151 first-grade schoolchildren. The values of this small sample were normally distributed and statistical analysis was therefore performed. The systolic blood pressure of boys and girls was 102.0 +/- 9 mm Hg and the diastolic blood pressure 69.5 +/- 7.3 mm Hg. Boys and girls were also analyzed separately. The data fit in very well those from the literature, and it is concluded that the values from the literature, especially those of the "Task force of blood pressure control", can be used with high probability for Berne and Switzerland as well. Additional findings: The skin thickness of all children and of girls correlates with blood pressure in the normal range. The family history with regard to risk factors is probably only of importance if precise questions are asked, since general inquiries about obesity, myocardial infarction, and hypertension did not yield useful results.

[Comparative ultrasonic Doppler blood pressure determination on the upper and lower extremities as a possibility in the early detection and course observation of peripheral arterial circulatory disorders].

The comparative measuring of blood pressure on the arm and foot artery by means of the ultrasound technique after Doppler is a method for the diagnostics of the disturbance of the peripheral arterial blood supply relevant to practice. Its advantages are in the short time of examination, the simple manipulation, the little technical expenditure and the providing of clear measuring data. The course of examination used by us is explained, the normal and borderline values got are reported. Patients with haemodynamically effective disturbances of the arterial blood supply of the vessels of the leg have on the malleolar artery a by more than 10 Torr lower blood pressure than on the brachial artery. For recognition of early changes we propose a simple loading test in form of a position test. The comparative ultrasound measuring of the blood pressure after Doppler on the upper and lower extremity is suitable for the observation of the course and should stand in the first place of the functional diagnostics of peripheral angioorganopathies.
