[Characteristics of bone regeneration in distraction of bone fragments].

One of the features of distraction osteosynthesis consists in the presence of a zone of permanent growth between bone breaks. Electron microscopic examinations of the permanent growth zone at 14 and 21 days of distraction showed the presence in the zone of cells of three kinds: fibroblast-like cells, osteoblastic elements and little-differentiated preosteoblasts. Fibroblast-like cells and osteoblastic elements were found to have ultrastructural changes of the endoplasmic reticulum indicating the occurrence of intensive synthesizing processes. Specific conditions of blood supply cause hypoxic metabolism with typical enhanced formation of fibrillae which leads to the preservation of the fibrillar tissue throughout the entire period of distraction. The fibrillar tissue in diastace may be considered to be a special growth plate.

[Mucopolysaccharides, collagen and calcium of bone regenerate in replacing a bone defect by a method of unilocular compression-distraction osteosynthesis].

Intensive synthesis of collagen, which constitutes the basis of organic matrix in bone tissue, was observed in the regenerate developed during compression-distractional osteoxynthesis. The organic matrix of the regenerate was quickly mineralized; after termination of the distraction it contained more than a half of calcium as compared with the mature bone of healthy animals. Relatively low accumulation of mucopolysaccharides in the regenerate appears to depend on development of favourable conditions of its blood supply together with suitable fixation of bone fragments.

Bone regeneration within a coralline hydroxyapatite implant.

The hypothesis that incomplete resorption of osteons in an autogenous cortical bone graft may limit its replacement by new bone regeneration was explored by implanting a hydroxyapatite replica of a coral skeletal structure into bone gaps. This implant contained channels and interconnections similar to those in osteon-evacuated bone grafts. In 6 implanted mandibular defects in dogs, two of which were examined at two, 4, and 6 months, 11 percent, 46 percent, and 88 percent of the implant areas were filled with regenerated bone. The regenerated bone was a woven type at two months, but changed to a lamellar type by 6 months. In two implanted defects examined at 12 months, biodegradation of 29 percent of the implant had occurred. The bone regeneration was physiological, the implant was biocompatible, and the biodegradation began after the bone had regenerated.

[Animal experimental studies on bone regeneration following drilling of the bone].

Experiments in dogs showed that drilling at the surface of bone provokes damage which increases with the number of revolutions per minute. Polychrome sequence marking and histological tests demonstrated that filling up of bony defects started later with turbine-driven drills than with drills of 2,000 r.p.m. and has not even been completed after eight weeks. It is therefore recommended to abstain from using turbines in drilling at the surface of bone.

[Effect of direct current on bone regeneration].

The data on bone biocurrent and on the bone regenerating effect of low intensity direct current are discussed. The stimulator constructed by the authors is described. With its help an improved callus formation could be achieved on the osteotanised femur of the rabbit.

[Radiological interpretation of bone regeneration after cystectomy of odontogenic cysts].

38 patients who had undergone surgical intervention (Partsch II) for odontogenic cysts were examined radiographically as well as clinically. The extent of bone regeneration was estimated according to age, sex, location of the cyst in the jaw, cyst size, length of time elapsed after operation and function. To facilitate the interpretation of the results obtained, the patients were subdivided into five age groups. Bone regeneration was most frequent up to the age of 20 (85.7%). It occurred more often in women (in 11 of 16 patients = 68.75%) than in men (in 14 of 22 patients = 63.6%). Bone healing was most favourable in the anterior region of the mandible less, favourable in the posterior region of the maxillae and least favourable in the anterior region of the upper jaw. With increasing size of the cyst, the regenerative power of the bone decreased. An increase in regeneration was observed up to 3 years after surgical intervention.

[Effect of carbostimulin and vitamin D3 on activity of glycolytic enzymes of rabbit bones, regenerates and muscles].

Activity of glycolysis enzymes such as: phosphofructokinase, aldolase, phosphoglucomutase, was determined in the regenerate of a bone, bone fragments, native bone and, for the sake of comparison, in the animal muscles after the radius resection and feeding of carbostimulin and its mixture with vitamin D3 for 10 days. On the 12th day after the radius resection the phosphofructokinase activity in rabbits increases in the native and operated bones as compared to that in bones of nonoperative animals. In the group of rabbits which were fed on the mixture of carbostimulin and vitamin D3 the phosphofructokinase activity is 7-2 times as high. In the regenerate of these animals the activity becomes 12 times as high as that in the regenerate of the control rabbits. An analogous increase in the aldolase and phosphoglucomutase reaction rate is observed in the bones of the animals which received carbostimulin and vitamin D3 (4.5 and 7.5 times, respectively). The same tendency to the increase in the activity of these enzymes is observed in the regenerate tissue. In the muscular tissue at this stage of regeneration the activity of the studied enzymes which is usually high in norm decreases.

[Bone regeneration over plastic implants after mandibular resection breaking the continuity].

In young patients, bone regeneration may start from the region of the temporo-mandibular joint and from the peripheral mandibular stump in cases of continuity defects of the mandible. A resin graft implanted for soft tissue reconstruction is collar-like surrounded by bone which is developing into a functionally and anatomically full regenerate over many years, in correspondence to periosteal and desmal growth stimuli. In adults, regeneration is minimal.

The effect of systematic plaque control on bone regeneration in infrabony pockets.

A clinical trial was undertaken to test the hypothesis that periodontitis can be cured and that bone regeneration occurs in infrabony pockets in patients maintained on an optimal standard of oral hygiene. The material comprised 24 patients with advanced periodontal disease. After an initial examination, the patients were randomly distributed into one test group and one control group. All the patients were given instruction and practice in a proper oral hygiene technique, and then subjected to periodontal surgery using the modified Widman flap procedure. Following treatment, during a 2-year period the patients of the test group were recalled once every second week for professional tooth cleaning. The control patients were recalled once every 12 months for prophylaxis. The results showed that all osseous defects of the patients of the test group were refilled with bone. The control patients, on the other hand, could not maintain a high standard of oral hygiene, and exhibited a progressive deterioration of the periodontal tissues during the postsurgical observation time.

Alveolar bone regeneration after removal of inflammatory and traumatic factors.

In this investigation periodontal destruction was produced by a combination of inflammatory and traumatic factors. After the inflammation was resolved and the trama stopped, a significant amount of alveolar bone regeneration took place. The implications of these findings for the management of advanced periodontal disease are discussed.

A stimulator of stem cell proliferation in regenerating bone marrow.

A factor, capable of stimulating resting haemopoietic stem cells into DNA-synthesis has been extracted from regenerating bone marrow. It has a molecular weight in the range of 30.000-50.000 daltons and is not detectable in normal bone marrow. Used in combination with a stem cell proliferation inhibitor, previously described, it will restimulate proliferation in stem cells initially stopped by the inhibitor. Conversely, stimulation produced by this factor can be reversed by the addition of inhibitor. It is concluded that stem cell proliferation is controlled by an appropriate balance of endogenously produced stimulatory and inhibitory factors.
