Psychotropic drug use in the Boston area. A report from the Boston Collaborative Drug Surveillance Program.

During 1972, adults admitted to general medical and surgical wards in Boston area hospitals were interviewed to determine their use of prescribed psychotropic drugs prior to hospitalization. Patients hospitalized for psychiatric disorders or psychogenic ("functional") disease were excluded. Patients who had taken prescribed drugs that could not be identified were considered nonusers. Of the total sample, 20% gave a histroy of psychotropic drug use. Antianxiety drugs were taken by 15% of the patients and accounted for two thirds of total psychotropic drug use. Hypnotics were taken by another 4% of the sample. Slightly over half of antianxiety and hypnotic drug use was for a year or more. Use was more frequent in patients with potentially chronic medical disorders. The findings, in general, are consistent with other studies employing other methods to investigate the use of psychotropic drugs. Simple studies of use, however, do not provide a sole or adequate definition of treatment option, need, or efficacy.

Oral contraceptives and hepatitis. A report from the Boston Collaborative Drug Surveillance Program, Boston University Medical Center.

History of oral-contraceptive use was compared between 68 women with a hospital-discharge diagnosis of acute hepatitis and 1142 women who had been admitted for other reasons to the same hospitals during the same period. The risk of admission to hospital for hepatitis for users of oral contraceptives was estimated to be 3-3 times that for non-users (95% confidence interval, 1-8--6-3). This overall association was almost entirely attributable to a strong association among women under 25 years of age. Among users of oral contraceptives, duration of use tended to be shorter for hepatitis patients than for controls.

Relation between smoking and age of natural menopause. Report from the Boston Collaborative Drug Surveillance Program, Boston University Medical Center.

The relation of cigarette smoking to natural menopause has been evaluated in two independent sets of data. In both, at each age from 44 to 53 years, more current smokers were post-menopausal compared to "never" smokers. In the first set of data the age-standardised proportions of women who were postmenopausal were 35, 36, 43, and 49%, respectively, for never smokers, ex-smokers, and current smokers of 1/2 and greater than or equal to 1 pack per day. The corresponding proportions for the second set were 53, 59, 60 and 65%.

Drug-induced gastrointestinal bleeding. Report from the Boston Collaborative Drug Surveillance Program, Boston University Medical Center.

Rates of drug-induced gastrointestinal bleeding were estimated from data on 16 646 consecutively monitored medical inpatients who had no known predisposing illness. Heparin, warfarin, ethacrynic acid, steroids, and aspirin-containing drugs were associated with gastrointestinal bleeding and were estimated to account for about two-thirds of such bleeds. Major gastrointestinal bleeding, defined as bleeding severe enough to require transfusion, occurred in only 57 patients (0.3%).

Comparative studies of Hb Lepore Boston, Hb A2, and Hb A.

Several functional tests were performed to compare Hb A, Hb A2, and Hb Lepore Boston, which has a delta-beta crossover in the region of residues 87 to 116. Oxygen equilibrium curves determined by an automatic apparatus in 0.1 M potassium phosphate buffer, pH 7.0, at 20 degrees showed that the p50 was 5.8 mm Hg for Hb Lepore Boston, in contrast to 8.1 and 10.3 mm Hg for Hb A2 and Hb A, respectively. The n values (Hill coefficinets) of Hb Lepore Boston and Hb A2 were slightly smaller than that of Hb A. The effect of 2,3-diphosphoglycerate and inositolhexophosphate on the p50 of Hb Lepore Boston and Hb A2 was less than that on the p50 of Hb A. The molecular stability to mechanical shaking of Hb Lepore Boston and Hb A2 showed that the oxy forms of Hb Lepore Boston and Hb A2 denatured at a rate 3 times faster than that of Hb A. MetHb Lepore Boston was more unstable than MetHb A2 to mechanical shaking. These results indicate that, although the molecular stability of Hb Lepore Boston is more similar to that of Hb A2 than that of Hb A, the oxygen-binding properties of Hb Lepore differ from both Hb A and Hb A2.

A comparison of visual pathways in Boston and Midwestern Siamese cats.

A genetic mutation in Siamese cats causes retinogeniculate fibers representing roughly the first 20 degrees of ipsilateral visual field in each eye to cross aberrantly in the optic chiasm and terminate in the wrong lateral geniculate nucleus (LGN). Previous investigations have shown that in the visual cortex this extra representation of ipsilateral visual field can be organized into one pattern in Boston Siamese cats, another in Midwestern. This finding was confirmed here. The possibility that the organization of the LGN might account for these two patterns was studied using combined anatomical and physiological methods. On the basis of microelectrode recordings from the visual cortex, 11 out of the 12 Siamese cats included here were Boston cats; one was Midwestern. The distribution of retinogeniculate terminals was examined in each cat using autoradiographic techniques following an eye-injection of 3H-proline. Overall, the LGN organization in Boston cats was similar to that of Midwestern: both lateral and medial normal segments of lamina A1 (mnA1) were present. In Boston cats, however, the mnA1 was remarkably small and shifted ventromedially in the nucleus to allow for the fusion between the medial borders of lamina A and the abnormal segment of A1. In the Midwestern cat this fusion was not apparent and the medial normal segment of A1 was significantly larger. These differences in organization of the LGN are consistent with those seen at the level of the visual cortex in Midwestern and Boston Siamese cats. It was not possible, however, to relate them clearly to the characteristic strabismus of these animals.

Abnormal interhemispheric connections in the visual system of Boston Siamese cats: a physiological study.

In the normal cat most visual fibers in the splenium of the corpus callosum are concerned with a representation of the vertical meridian of the visual field. These fibers for the most part originate from cortical regions forming the border between adjacent architectonic areas, such as the anatomical 17-18 border. Due to a genetic mutation in Boston Siamese cats, the anatomical 17-18 border now represents a region in the ipsilateral visual field roughly 20 degrees away from the vertical meridian, and the representation of the vertical midline is displaced from the border to regions within areas 17 and 18 proper. Do visual fibers in the Boston Siamese cat corpus callosum originate, as in normal cats, from the 17-18 border even though the vertical meridian is no longer represented there? The present paper deals with this question from a physiological standpoint. Single visual fibers in the corpus callosum of 11 Boston Siamese and two normal cats were recorded using extracellular microelectrodes. Receptive fields were mapped and their positions used to infer the likely cortical site of origin for each fiber. In confirmation of earlier findings, most callosal receptive fields in the normal cat were closely apposed to the vertical meridian, as would be expected if only those visual cortical regions which represent the vertical meridian, such as the border between areas 17 and 18, contribute to the corpus callosum. In Boston Siamese cats, however, an abnormally large amount of visual field was represented in the corpus callosum, with some receptive fields located as far as 25 degrees away from the vertical meridian. The representation of the vertical meridian was nevertheless substantial. The present findings suggest, therefore, that in Boston Siamese cats callosal fibers do not originate solely from the boundary regions between adjacent cortical areas, such as the anatomical 17-18 border, but originate also from other cortical regions, especially the vertical meridian representation, regardless of their location with respect to such boundaries.

Anatomy of interhemispheric connections in the visual system of Boston Siamese and ordinary cats.

In Siamese cats, previous studies have shown that a genetic mutation causes retinogeniculate fibers in each eye which arise from the temporal retina representing the 20 degrees of ipsilateral visual field adjacent to the vertical meridian to cross aberrantly in the optic chiasm, thereby terminating in the wrong lateral geniculate nucleus. The abnormality is expressed subsequently at the level of the visual cortex. This paper presents anatomical evidence that the pattern of commissural visual connections in the "Boston" variety of Siamese cat also is highly abnormal in comparison to that of ordinary cats. The topographical distribution of neurons supplying visual fibers to the splenium of the corpus callosum was studied in Boston Siamese and ordinary cats using the method of retrograde transport of horseradish peroxidase (HRP) following localized cortical injections made through a recording micropipette. In ordinary cats, after an HRP injection at the border between cortical areas 17 and 18, which represents the vertical meridian of the visual field, HRP-labeled cells in areas 17 and 18 of the opposite hemisphere were found only immediately adjacent to the 17-18 border, thus confirming the results of previous investigations. In Boston Siamese cats, the border represents a region in the ipsilateral visual field roughly 20 degrees away from the vertical meridian, and the vertical meridian representation is displaced to sites within areas 17 and 18 proper. When HRP was injected at the 17-18 border, labeled cells in the opposite hemisphere were located well within area 17 near the suprasplenial sulcus, and also well within area 18; few labeled cells were found at the 17-18 border. When an HRP injection was placed at the vertical meridian representation, again few HRP-labeled cells were found at the opposite 17-18 border, but instead most were found in area 17 slightly medial to the border, and in area 18 slightly lateral to it. Thee findings were complemented in an autoradiographic study in which orthograde transport of tritiated proline after a localized cortical injection was used to demonstrate the distribution of callosal terminals. Thus the pattern of callosal connections revealed in Boston Siamese cats, although anatomically different from that of ordinary cats, was nevertheless consistent with the proposal that cortical sites representing similar visual field coordinates in each hemisphere are appropriately interconnected via the corpus callosum. The laminar distribution of callosal connections was examined briefly. Layer III pyramidal cells of areas 17 and 18 supplied the majority of terminals to the opposite 17-18 border. Pyramidal cells of Layers II and VI, and Layer IVa in area 18, made a smaller contribution. In areas 17 and 18, the same cortical layers (II, III, and VI; and IVa in 18) were again the major sites of callosal termination. A clear projection to the base of layer I was also noted. The laminar distribution of callosal connections in ordinary and Boston Siamese cats were not substantially different.

Agenesis of Corpus Callosum

Ethylisocyanide equilibria of hemoglobins M Iwate, M Boston, M Hyde Park, M Saskatoon, and M Milwaukee-I in half-ferric and fully reduced states.

The ethylisocyanide equilibria of all the five known hemoglobins M, namely Hb M Iwate (alpha287 Tyrbeta2), Hb M Boston (alpha258 Tyrbeta2), Hb M Hyde Park (alpha2beta292 Tyr), Hb M Saskatoon (alpha2beta263 tyr), and Hb M Milwaukee-I (alpha2beta267 Glu), were studied both in the half-ferric and fully reduced heme states. In the half-ferric state, no heme-heme interaction was observed for Hb M Iwate, Hb M Boston, and Hb M Hyde Park, but Hb M Saskatoon and Hb M Milwaukee-I show small but definite heme-heme interaction with Hill's n of 1.3. The beta chain mutants, Hb M Hyde Park and Hb M Saskatoon, have almost normal affinity for ethylisocyanide and a normal Bohr effect, whereas the alpha chain mutants, Hb M Iwate and Hb M Boston, have abnormally low affinity and almost no Bohr effect. Hb M Milwaukee-I showed a large Bohr effect and low affinity. These results are consistent qualitatively with those on oxygen equilibria reported previously. In the fully reduced state, in which all four hemes were in the ferrous state and capable of binding ethylisocyanide distinct differences were found in the extent of heme-heme interaction. Namely, the n values for proximal histidine mutants, Hb M Iwate and Hb M Hyde Park, were 1.1 and 1.0, respectively, whereas the distal histidine mutants, Hb M Boston and Hb M Saskatoon, showed high n values of 2.4 and 1.6, respectively. Hb M Milwaukee-I also exhibited a high n value of 2.0 The ethylisocyanide affinity of the four histidine mutants was high compared with that of Hb A, while that for Hb M Milwaukee-I was almost normal. All five Hbs M had approximately normal magnitudes of Bohr effect. In the half-ferric state, the proximal and distal histidine mutants of the same chain showed similar affinity for ethylisocyanide and Bohr effect, rather different from those of the mutants of the opposite chain. These differences seem to be derived from the difference of abnormal bonding of ferric iron to tyrosine or glutamic acid. On the other hand, the reduction of iron, which abolished the abnormal bonding and made all of the chains capable of binding ligand, extinguished the differences of alpha and beta chains, and the effect of amino acid side chains close to iron on ligand binding properties became clear. Proximal histidine, which is considered to trigger the transition between the T and R states, seems to be essential to the heme-heme interaction.

Planning for emergency medical services in Boston.

The results of a survey of 10,200 visits to 11 Boston hospital emergency rooms during a 9-day period in March 1972 are presented. The survey was designed to provide data on emergency room use to permit more informed planning by public agencies concerned with improving areawide emergency medical services. The 11 institutions surveyed provided virtually all of the emergency medical services in the city of Boston. A majority are teaching hospitals affiliated with one or more of the three medical schools in the area. Of the 11 hospitals, 3 accounted for 60% of all emergency room visits. Survey data were extracted from emergency room log sheets and hospital medical records of individual patients. Information collected included the residence pattern of patients within the geographic area, the patient mix by degree of urgency based on presenting complaints, mode of transportation to the hospital, and age and sex of the patients. Only 15 percent of the 10,200 visits were true emergencies. Fifty-seven percent were classified as urgant and 28% nonurgent. The mix among the 11 hospitals ranged from 7 to 22 percent in the emergency category, and 11 to 61 percent in the nonurgent classification. Trauma accounted for 19 percent of all admissions, with 3 percent attributed to fractures and 4 percent to head injuries. Fifty-six percent of the emergency cases required the services of an internist or pediatrician, 38 percent a surgeon, and 1 percent an obstetrician. The highest utilization rate--27 per 1,000 population--was recorded for the under 5 age group. Although the 65 and older age group had the lowest utilization rate of 6 per 1,000, this group had the highest rate of visits classified as emergencies. Children under 5 accounted for the highest proportion of nonurgent visits. The survey revealed that 30 percent of all hospital admissions were from the emergency room. One in four emergency patients lived outside the city of Boston. A neighborhood health center and a hospital general practice unit reduced hospital emergency room workloads appreciably, even when they were open only during daytime hours. Eighty-eight percent of all patients arranged for their own transportation, usually by private automobile. Of those arriving by ambulance, only 35 percent were classified as emergencies. The survey data reinforce the conclusion that major planning efforts should be concentrated on the management of the nonemergency patient. The data also emphasize the need for a single agency to be responsible for overall planning for emergency medical services on an area wide basis.

Adolescent

The return of Boston exanthem. Echovirus 16 infections in 1974.

Although the "Boston exanthem" has been seen rarely over the past 20 years, in the summer of 1974 we identified ten children, aged 1 week to 7 years with echovirus 16 infections. Seven of the children had rashes, and in four, the illness suggested roseola infantum. Five of these children were neonates, all with illnesses severe enough to mimic sepsis. In addition information obtained on echovirus 16 isolations occurring in 1974 elsewhere in the country showed an additional 27 isolations from eight states. It is postulated that more outbreaks of the Boston exanthem or other echovirus 16 illnesses will appear during the next several years, before the virus again "disappears".

Increase in suicide attempts by drug ingestion. The Boston experience, 1964-1974.

At the Massachusetts General Hospital between 1964 and 1974, there was an estimated 118% increase in emergency ward admissions for drug overdose suicide attempts and an estimated 257% increase in such attempts by individuals in the 20- to 29-year age range. Approximately 67% of overdoses seen in 1972 and 1974 occurred in individuals under age 30. An increase in drug overdose suicide attempts by males was noted. By 1972, males were making approximately as many attempts as females. Based on extrapolations from the Massachusetts General Hospital drug overdose data, and not including an estimate of non-hospital-treated suicide attempts, an estimated annual (1972) suicide attempt rate of 337/100,000 (0.38%) was obtained for Boston. The estimated suicide attempt-completed suicide ratio for the Boston area was 39:1. These estimates should be interpreted as giving an approximate order of magnitude and not as specifying precise rates.

Bacteremic superinfections of patients with bacteremia: occurrence, bacteriology, mortality, and duration of hospitalization at Boston City Hospital during 12 selected years between 1935 and 1972.

During the course of hospitalization of 6,414 patients with bacteremic infections identified at Boston City Hospital (Boston, Massachusetts) during 12 selected years between 1935 and 1972, 6.0% developed verified bacteremic superinfections with organisms not isolated or identified in the primary bacteremic infection. Analysis of these cases reemphasizes the increasing occurrence of serious hospital-acquired infections despite the successive introduction and intensive use of a large number of effective antibacterial agents. The bacteremic superinfections, like the primary hospital-acquired bacteremias, increased in incidence over the years, particularly since 1961; they were more frequent and were associated with a higher mortality rate and longer duration of hospital stay in the primary hospital-acquired cases than in the patients in whom bacteremia was considered to be community-acquired. The organisms in superinfections were similar to those in primary hospital-acquired bacteremias. Superinfection was more frequent among the patients who died than among the survivors.

Role of heat-labile toxigenic Escherichia coli and Reovirus-like agent in diarrhoea in Boston children.

61 Boston children aged five years or less with acute diarrhoea were studied for evidence of infection with Escherichia coli strains that produce heat-labile enterotoxin (L.T.) or with a reovirus-like agent associated with childhood gastroenteritis. This represented the first evaluation of the prevalence of disease produced by these two agents in the same population. E. coli, isolated from acute-phase stool specimens, were tested in adrenal-cell tissue-culture and adult-rabbit ileal-loop assays for L.T. Acute and convalescent phase sera, collected from 31 children, were tested by the adrenal-cell assay for anti-L.T. activity. None of the 61 children demonstrated evidence of infection with L.T.-positive E. coli. Paired sera from 31 of the children studied were also tested for evidence of recent infection with the reovirus-like agent by determining titres of immunofluorescent-staining antibody to the serologically related Nebraska calf diarrhoea virus. 11 of the children (35%) had evidence of recent infection. These results suggest that an important proportion of endemic acute diarrhoea of young children in Boston is caused by the reovirus-like agent, and that disease caused by L.T.-producing E. coli is uncommon.

Use of bioelectrical impedance to predict leanness of Boston butts.

The objective of this research was to make available bioelectrical impedance technology for the prediction of kilograms of lean and kilograms of fat-free muscle of Boston butts. Seventy butts were removed from 70 pork carcasses according to standard procedures (NAMP, #406), with the exception that the fat was not removed. After the weight in kilograms (BUTT) and internal temperature in degrees centigrade (TEMP) were recorded, each butt was measured for resistance (Rs, ohms), reactance (Xc, ohms), and distance (L, centimeters) between detector terminals four different ways: parallel or perpendicular to the top of the carcass and on either lean surface or fat surface of the cut. Each cut was physically separated into lean, fat, and bone. Chemical composition (moisture, protein, and fat) was determined on the lean portion. Variable selection analysis was used to develop equations for predicting kilograms of lean and kilograms of fat-free muscle of Boston butts. Results of measurements of the four sites were quite similar; however, measuring perpendicularly on the lean surface is recommended. The prediction equation for kilograms of lean from measurements thus taken is as follows: .461-.0304 x TEMP + .576 x BUTT - .0118 x Rs + .00845 x Xc + .0630 x L. The respective coefficients of these independent variables for predicting kilograms of fat-free muscle are .537, -.0415, .479, -.0139, .00804, and .0764. In an industry application of these coefficients, recording temperature would not be imperative because the temperature range would be sufficiently narrow to render temperature of little practical influence when separating butts according to leanness.(ABSTRACT TRUNCATED AT 250 WORDS)

Sociocultural factors associated with smoking behavior by Puerto Rican adolescents in Boston.

This study examined cigarette smoking among Puerto Rican adolescents in the Boston area in its sociocultural context by describing who uses cigarettes, and by identifying factors that encourage or discourage cigarette use. The study, conducted from 1986-87, used two approaches. The first was to survey 605 Puerto Rican households with adolescents 11-20 years old. Households were identified from a random sample of census blocks in five neighborhoods. In each household, interviews were completed with one adolescent and his or her female caretaker. The second approach was to conduct ethnographic interviews with 40 adolescents selected through a purposive sample of survey respondents from three of the study neighborhoods. Bivariate and multivariate statistical analyses were used to examine the relationships between smoking behavior and characteristics of the adolescents. The results of these analyses were examined in light of the ethnographic data. Overall, 13.7% reported smoking in the month preceding the interview. Smoking was most prevalent among males 17-20. Patterns of smoking were associated with teens' social networks. The Puerto Rican adolescents in this study were more likely to smoke when their friends and household members smoked, if they participated in recreational activities such as sports or unsupervised games, and if they were not in school. These effects varied according to the gender of the respondent.
