Sociodemographic and clinical characteristics associated with nocturnal enuresis among children in a tertiary care center in Cairo, Egypt: A cross-sectional study.

BACKGROUND: Nocturnal enuresis (NE) is a significant pediatric health concern that warrants increased attention from healthcare providers, educators, and society. This study aimed to determine the frequency of NE among children attending a tertiary care center in Cairo, Egypt, and to explore their sociodemographic characteristics and associated factors. METHODS: A cross-sectional study was conducted among 1587 children aged 6-16 years attending a tertiary care center in Cairo from December 2022 to February 2024. Data were obtained from parents via a structured questionnaire covering sociodemographic characteristics. Sleep problems were assessed via the Children's Sleep Habits Questionnaire. The multivariate logistic regression analysis was conducted to estimate the odds ratio and identify factors associated with NE. RESULTS: The total prevalence of NE among children attending a tertiary care center was 333 (21%). Our results indicated that being female and being older were associated with a lower likelihood of NE. Low socioeconomic status (OR = 5.559; 95% CI = 1.24-24.88), stressful events (OR = 3.697; 95% CI = 1.73-7.88), a family history of NE (OR = 26.231; 95% CI = 8.617-79.85), and sleep problems (OR = 13.233; 95% CI = 6.082-28.793) were associated with increased odds of NE. Furthermore, consuming caffeinated drinks before sleep, the presence of urinary tract infections, and intestinal infestations were also associated with NE. CONCLUSION: The findings of the present study confirm the multifactorial etiology of NE and identify several associated factors. Integrating these factors into screening and management programs may improve early identification and lead to more effective, tertiary care-specific preventive and therapeutic interventions.

Streptococcal impetigo and acute glomerulonephritis in children in Cairo.

Impetigo contagiosa in Cairo affected young children of both sexes, the face being the main site. Post-impetigo nephritis, confirmed by a low serum C3 level and by urinalysis, occurred in only 11% of cases. Streptococcus pyogenes strains were recovered from 84% of the skin lesions. Sixteen types were identified according to their T-protein, and most infections were associated with T3/B3264, T13/B3264, T5, T11, T12, T8/25/Imp 19 and T14/49; the majority of these types were also recovered from houseflies. The types isolated from cases of post-impetigo nephritis were T4 (M 60), T14/49 (M49), T8/25/Imp 19 (mostly M 55) and T11. Seventy percent of the patients infected with T4 (M 60) and 40% of those infected with T14/49 (M 49) developed nephritis. Strains isolated from the skin bore a closer resemblance to those isolated from the nose than to those found in the throat. The ASO response was poor in uncomplicated impetigo but the titre rose more aften in post-impetigo nephritis.

The clinical manifestations of systemic lupus erythematosus: a Cairo-Glasgow co-operative study.

The clinical features of 78 patients with SLE seen in Cairo and Glasgow are reviewed. Raynaud's phenomenon was recorded more frequently here than in previous series. The value of serial measurements of anti-DNA antibodies, C3 and C4 in the management of SLE is discussed. Although antibodies to native DNA paralleled the disease course in only a minority of SLE patients anti-DNA antibodies were present during all major SLE exacerbations and could be diagnostically useful. Serious systemic infections complicating the management of SLE patients could occur and their diagnosis is discussed.

Minilaparotomy for female sterilization: a feasibility study of a new technique.

Female sterilization via a minilaparotomy incision was performed on 30 patients at Al-Azhar University Teaching Hospital in Cairo, Egypt. A uterine sound fed through a Foley catheter was used as a uterine elevator. A 2 cm suprapubic transverse laparotomy was performed and sterilization was accomplished by fimbriectomy. Twenty patients received general anesthesia and 10 patients initially received local anesthesia. Surgical time ranged from 7 to 15 min, and the average duration of hospitalization was 8 hours. Blood loss was small, and no patient was transfused. One patient had a wound infection. This pilot study indicates that this procedure is relatively simple, safe and inexpensive.

[History of lumbar sympathectomy from its origin to the present].

The author first recalls the origins of the words "sympathetic" and "neurovegetative system" and then outlines the beginnings of surgery on the autonomic nervous system in general. This started with the section of the rami communicantes introduced by the Australians Royle and Hunter in surgery of the muscle tone. Then nearly at the same time in 1924. Adson in USA and J. Diez in Argentina resected the lumbar sympathetic chains; the latter mainly for treatment of thromboangeitis obliterans. From 1924 on, lumbar sympathectomy met with rapid success as shown at the Cairo Congress of 1935. Physiologic and pathologic effects of lumbar sympathectomy are analysed and the usual techniques are reviewed. The author then lists the specific postoperative complications, including the pain symdroms and the functional sexual disorders in males. Finally the principal indications where lumbar sympathectomy proved useful are named. Chronic obliterative arteriopathies are the first. For these lumbar sympathectomy still holds a palliative, supportive or absolute role. A scheme illustrates its beneficial actions.

RNA synthesis by nuclei and nucleoli from ischemic liver cells.

Nuclei and nucleoli were isolated from rat livers subjected to an interruption of the blood supply for periods of different duration, as well as after restoration of the blood supply. They were assayed for RNA synthesis under conditions of diverse ionic strengths, and in the presence of an exogenous template, such as poly d (A-T), and actinomycin to inactivate the endogenous template; alpha-amanitin was made used of to distinguish polymerase I and polymerase II dependent RNA synthesis. Nuclei and nucleoli from ischemic livers showed a severe impairment of RNA synthesis, which is likely to be due to decreased initiation frequency of the engaged polymerases, while free polymerases were essentially unchanged. Both form I and II polymerase were equally involved. After restoration of the blood supply RNA synthesis recovered with an overshooting well above normal levels of activity, lasting for at least 24 hours. Increased RNA synthesis was not followed by thymidine incorporation into DNA.

Multimodal analysis of CD38 in T-cell Acute Lymphoblastic Leukemia Identifies Combinatorial Therapeutic Strategies.

Outcomes for pediatric patients with refractory or relapsed T-cell acute lymphoblastic leukemia (T-ALL) are poor, underscoring the need for improved therapeutic strategies. CD38, a type II transmembrane glycoprotein, is a promising target in T-ALL, with clinical trials evaluating CD38-targeting immunotherapies in frontline and relapsed settings. However, the biological role of CD38 in T-ALL has not been systematically defined. We interrogated CD38 biology through multimodal profiling of pediatric T-ALL samples. Bulk RNA sequencing of 1,335 primary tumors revealed that CD38 expression varies across genomic and immunophenotypic subtypes in T-ALL. Flow cytometry of 150 primary samples and CITE-sequencing of 40 cases demonstrated broad surface expression of CD38. A transcription factor CRISPR-screen identified RUNX1, RUNX3, and TP53 as candidate positive regulators of CD38. Metabolomic profiling of cell lines further revealed disruption of the polyamine pathway following CD38 perturbation. Supporting this finding, co-targeting CD38 with difluoromethylornithine (DFMO), a polyamine metabolism disruptor, improved survival in preclinical models. Across transcriptomic datasets, including primary tumors, cell lines, and patient-derived xenograft models, IL32 expression consistently decreased following CD38 loss or negativity, supporting an association between CD38 and inflammatory signaling pathways. Additionally, CD38 and LCK expression were positively correlated across majority of genomic subtypes, implicating SRC kinase signaling. Consistent with this, daratumumab in cell lines increased LCK phosphorylation, and combination therapy with dasatinib improved survival compared to monotherapy. Collectively, these findings define previously unrecognized interactions between CD38 and targetable pathways and genes in T-ALL and identify rational combinatorial strategies to enhance CD38-directed therapies and reduce relapse risk.

[Persistency of serum antibodies against Entamoeba histolytica].

With the purpose to determine the duration of serum antibodies against E. histolytica by means of the contraimmunoelectrophoresis technique, 31 patients with the diagnosis of intestinal or liver invasive amebiasis were included in this study. In 92.3% of the 13 patients subjected to transversal study, no specific antibodies were found from 10 to 24 months after the occurrence of amebic disease. In 64.7% of 17 children with liver amebiasis who were studied longitudinally, no serum antibodies were found between 4 to 10 months after the disease. The possible factors involved in the varaibility of the duration of serum antibodies against E. histolytica are discussed.

[Coproantibodies in intestinal amebiasis].

E. histolytica tissue invasion induces IgG and IgM and probably IgA serum antibody formation of different types in the intestinal mucosa. 21 children with intestinal invasive amebiasis were studied. Samples were taken from peripheral blood and stools during diagnostic studies and three weeks later. Contraimmunoelectrophoretic antibody determinations and indirect hemagglutination tests IH were made in an attempt to demonstrate its true nature. During the acute phase (first sample), results showed antibodies against amoeba in stools by CIE in 57 per cent and by IH in 80 per cent; in serum, results were 38 and 76 per cent respectively. In the second sample and three weeks after diagnosis was established a decrease in the number of patients with coproantibodies against amoeba was observed and the number of patients with serum antibodies increased. We are also providing evidence that supports the IgG and IgA nature of coproantibodies against amoeba. These results suggest the presence of a local immunological reaction in invasive amebiasis.

Antibodies

[T and B lymphocytes in the peripheral blood of the in-utero malnourished new born infant].

The per centum distribution of T and B lymphocytes was determined in 30 full term newborns showing adequate development for their gestational age and in 42 full term newborns showing in utero malnutrition. A significant decrease (p less than 0.001) of both lymphocytic subpopulations was found in the in utero malnutrition group. The possibility of a decreased efficiency of the immune response in this group of patients is discussed.

B-Lymphocytes

[Rosette-forming lymphocytes in the peripheral blood of normal children].

The study included 180 healthy children distributted into age groups from newborns to 16 years of age. The subpopulations of early E and EAC (T and B) rosette forming lymphocytes were determined. It was found that the main difference is placed in percentual mean values of the newborn where the value of T lymphocytes is less than for the rest of the ages studied, remaining stable beginning at the age of one year. In newborns, the values of B lymphocytes are greater; later, they drop but remain stable also since the age of one.

Adolescent

[Rosette forming lymphocytes as prognostic indices in idiopathic thrombocytopenic purpura].

The study included 12 patients with acute ITP to whom subpopulations of rosette forming lymphocytes were determined in peripheral blood and in serial form, the number of platelets to establish a correlation as prognostic index in the evolution of the disease. No correlation was found between the progressive increment of platelets in the evolution of the disease. However, in patients with a tendency to remission, the percentage of E(T) rosette forming lymphocytes was low and normal that of EAC (B) lymphocytes. Dissimilarly, it was seen that in patients without tendency to remission, the percentage of T and B rosette forming lymphocytes was within normal limits. This seems to show that determination of the subpopulations of lymphocytes may be useful as a prognostic index in this stage of the disease.

[Concentration and activity of alpha-1-antitrypsin in neonatal hepatitis].

The concentration and activity of alpha 1-antitrypsin were studied in 20 children with the diagnosis of neonatal hepatitis. The concentration of alpha 1-AT was found normal or high in all of the patients (3.6 +/- 0.12 mg/ml). No case was found with PiZZ homozygote type alpha 1-AT deficiency. The activity of alpha l-AT was found decreased in 90% of the patients with neonatal hepatitis (control: 0.86 +/- 0.18 mg. trypsin inhibited by ml. of serum; neonatal hepatitis: 0.49 +/- 0.07 mg/ml.). The possibility that this may correspond to a disorder characteristic of neonatal hepatitis is mentioned.

[Rosette-forming peripheral lymphocytes, in children with measles].

During measles infection in human beings, depression of cellular immunity appears. The authors studied several groups of children with measles complaining of various degrees of malnutrition in terms of rosette forming lymphocytes in peripheral blood; marked reduction of T lymphocytes were seen in all of the groups during the acute stage of the disease, that persisted up to two weeks after the exanthematous stage. When comparison was established of eutrophic and 1st degree malnourished children with those affected with 3rd degree malnutrition, differences were found in the recovery of the number of rosett-E forming cells.
