[Information sources for the physician on the internet. Library catalogs, scientific bookshops and journal articles].

The fast availability of current medical literature is of paramount importance for physicians. Especially methods of online-retrieval in specialised libraries and of journal articles as well as the possibility of lending are of significance. Also, it should be possible to purchase books in online-bookshops. Therefore, the Internet with it's constantly increasing offers plays a more and more important role in scientific research.

Searching the databases: a quick look at Amazon and two other online catalogues.

The Amazon Online Catalogue was compared with the Library of Congress Catalogue and the British Library Catalogue, both also available online, by searching on both neutral (Gay, Lesbian, Homosexual) and pejorative (Perversion, Sex Crime) subject terms, and also by searches using Boolean logic in an attempt to identify Lesbian Fiction items and religion-based anti-gay material. Amazon was much more likely to be the first port of call for non-academic enquiries. Although excluding much material necessary for academic research, it carried more information about the individual books and less historical homophobic baggage in its terminology than the great national catalogues. Its back catalogue of second-hand books outnumbered those in print. Current attitudes may partially be gauged by the relative numbers of titles published under each heading--e.g., there may be an inverse relationship between concern about child sex abuse and homophobia, more noticeable in U.S. because of the activities of the religious right.

Unity from diversity: the evidential use of anecdotal reports of adverse drug reactions and interactions.

Anecdotal case reports contribute about one-third of the published literature on adverse drug reactions and interactions, but are regarded as providing poor-quality evidence. However, they can occasionally provide proof of cause and effect, and there are many other reasons for publishing them. Because an anecdote is a narrative, narratological paradigms from literature, art, and music can show how we can make evidential use of anecdotes. Useful paradigms are the dramatic unities (of time, place, and action), comprehensive catalogues, and pattern formations. Here I give examples of each of these types of paradigm and show how they can be used to interpret anecdotes about adverse drug reactions and interactions. The dramatic unities show how a proper classification of adverse drug reactions can be achieved, according to dose-relation, time-course, and susceptibility factors; use of this classification should improve the evidential use of anecdotal reports. A high background incidence of the effect (the medical equivalent of subplots, which violate the unity of action) makes it more difficult to detect adverse drug effects using anecdotal reports. To make best evidential use of the corpus of anecdotal reports of adverse drug reactions, comprehensiveness is important: each suspected adverse reaction should be reported in detail and reactions should be reported in sufficient numbers for proper classification and for patterns to be recognized. One form of pattern recognition, teleoanalysis of data, should, when possible, include not only randomized controlled trials and observational studies, but also case series and anecdotal reports.

TALON--the first five years.

The South Central Regional Medical Library Program, Region IX in the national network of medical libraries, was one of the last to be organized and funded. The particular makeup of this organization has permitted a good deal of flexibility in the identification and implementation of various types of network services and activities. The following is a brief account of some of these network functions during the first five years relating the successes and failures in network strategies and describing the further evolution of programs and refinement of a regional plan which will advance network services.
