[Contributions of thoracic surgery in the Research Institute for Pulmonary Diseases and Tuberculosis since its founding and under the presence conditions in the treatment of specific and nonspecific lung diseases (author's transl)].

Thoracic surgery is done in the Research Institute for Pulmonary Diseases and Tuberculosis for about 25 years. 2463 lung resections on account of tuberculosis, 934 lung resections on account of intrathoracic tumors, among them 795 suffering from bronchial carcinoma, 422 operations because of nonspecific pulmonary diseases (bronchiectasis, lung abscess a.s.o.) and nearly the same number of operations because of spontaneous pneumothorax were performed in this time. Cystic lung disease, bullous emphysema, pulmonary mycosis and diseases of diaphragm and oesophagus were rarer indications for surgery. Since 1970 an increasing number of thoracic injuries by accidents were treated (70 patients). At the beginning collapse therapy (1953/54) was still in use (305 operations). In all the number of great thoracic operations 1978 was 5417. Besides, experimental investigations dealing with lung transplantation were done in dogs. These results are published in several papers. The technical experience gained by this research work could be utilized for clinical practice.

Decortication after 35 years.

A 66-year-old white woman had a paraffin plombage performed in 1940 for tuberculosis. In 1975, she had decortication for terminal respiratory insufficiency, with return to an active life. Thirty-five years of pulmonary compression does not produce irreversible changes in the pulmonary parenchyma.

[Mesotheliomas of the pleura].

The materials concerning 19 patients with pleural mesotheliomas are analysed. The patients were subdivided into three groups according to the clinico-roentgenological picture and course of the affection: 1) four patients in whom pleural mesothelioma proceeded without exudate accumulation; 2) seven patients in whom exudate accumulation in the pleural cavity was the main sign of the tumor; 3) 8 subjects in whom the tumor developed on the side of an artificial pneumothorax many years following its cessation. Among 19 patients only in 5 cases the tumor was suspected soon after their admission to the hospital. A needle biopsy of the pleura is felt to be the most effective method of diagnosis of pleural mesotheliomas, whereas in failure of the former -- pleuroscopy.

[Respiratory consequences of thoracopulmonary surgical operations in children and adolescents].

Spirographic investigations were carried out in a lot of 125 children and adolescents with surgical exeresis or collapse, performed between 1965 and 1972 for suppurations, tuberculosis and other thoraco-pulmonary diseases. Spirographic determinations before and 1--2 months after surgery showed that the postoperative sequelae were milder in children than in adults. Recovery of the ventilation function was more complete than that of the pulmonary circulation. The necessity of a correct postoperative care and systematic, prolonged respiratory kinesitherapy is emphasized.
