Patients' comments on psychiatric inpatient treatment experiences: patient-therapist relationships and their implications for treatment outcome.

Only few researchers pay systematic attention to what patients may have to say about their treatment experience during psychiatric hospitalization. This study not only delineates the various therapeutic aspects of an open psychiatric inpatient unit based on the patients' comments, but also attempts to correlate the data with the objective outcome of treatment rated clinically by the therapists and independently by the author. Positive patient-therapist relationship has been found to be the most significant factor correlated closely with the treatment outcome. The correlation may support the relevance of the patients' comments regarding subjectively perceived therapeutic benefits vs. objective actual benefits.

Fundamental comments on cell polarography according to Velazquez and Rosado.

Some fundamental comments are made on cell polarography according to Velázquez and Rosado (Fertil Steril 23:562, 1972). The results which they have obtained cannot be reliable because fundamental mistakes are made against the principles of the electroanalytic techniques used. We comment upon the conductometry, the amperometric titrations, and the alternating-current polarography as carried out by Velázquez and Rosado.

Ejection fraction derived by M-mode echocardiography: a table and comments.

Left ventricular ejection fraction is frequently quantitated by M-mode echocardiography despite limitations, including 1) the ability to measure an accurate left ventricular short axis, 2) the mathematic and geometric assumptions used to derive the volumes from which the ejection fraction is calculated, and 3) the lack of applicability to asynergic ventricles. Nevertheless, using a cubed function or correction formula, ejection fraction can be calculated for symmetrically contracting, normally sized, and enlarged left ventricles. A table and accompanying comments are presented.

[Cryptococcosis in Venezuela. Comments on a clinical case with unusual location].

A tumor like case of Cryptococcosis with a rare location in the abdomen of a young healthy woman is commented. It gave the opportunity to refer to the pathogeny of the disease, making in that way a brief review of the cases known in Venezuela and some not still published from which we got information. That review showed that generally doctors found more frequently organic lesions within the fields they are specially working for. It is obvious that mycosis must be considered in the diagnosis of nearly every patient in many countries and that laboratories must get appropriate equipment and personnel to help in that diagnosis.

[Comments on the paper by D. Tausch et al. Experiments on the penetration power of various bullets into skin and muscle tissue (author's transl)].

The objections in the following comments on a recent paper by Tausch et al. (1978) are raised principally to the points that the mass of the projectile is given an importance for the penetration which is not justified, and that the inherent uncertainty of the measurement data and the scope of validity of the empirical formulas are not sufficiently taken into account. The discussion on the process of penetration and a discontinuity of the depth of penetration as a function of the velocity of the bullet is of fundamental significance, with consequences for the definition of the critical velocity.

The applied communication game: a comment on Muma's "communication game: dump and play".

We now have technology available to teach speech and language skills to children. We cannot always expect children to carry over or generalize this training and spontaneously use this "trained language" in communication situations, so we now must develop technology for teaching interpersonal communication skills. This report commented on a model presented by Muma (1975). Illustrations were presented that should help clinicians train more effective interpersonal communication. The applied communication game provides an objective procedure that utilizes the natural consequences in communication situations to improve communication skill in handicapped children.

Algorithm-derived, computer-generated interpretive comments in the reporting of laboratory tests.

The volume of laboratory and other data that physicians encounter daily is more than most can mentally process without oversight or error. Routine use of computer-generated reminders is one solution to this problem that is now widely suggested. We report our experience with the daily use of a user-written program of algorithm-derived, computer-generated comments in the reporting of chemical profiles and routine thyroid function tests in a busy medium-sized privately owned independent clinical laboratory.

Overview: the right to treatment--comments on the law and its impact.

The constitutional right to treatment has now become an accepted premise of litigation in the United States. It lacks only the imprimatur of the Supreme Court. The author presents a brief description of the history of right to treatment cases and comments on the problems and possibilities the law holds for psychiatry. He warns that psychiatric institutions must be aware of the costs and benefits of such litigation and must obtain skilled legal counsel to advise them of their rights and obligations. He calls on psychiatry as a profession to participate in right to treatment litigation and to attempt to shape legal standards so that future generations of psychiatrists can provide appropriate care to patients.
