The role of community health workers in malaria control in the Philippines.

The barangay health workers (BHWs) or government-trained health volunteers have been operating in rural villages in the Philippines since 1981. Although malaria is a consistently recognized health problem, the BHWs do not contribute optimally to control activities even in endemic areas. This is so because of inadequate training, insufficient logistic support, poorly sustained motivational schemes and lack of community support. Prospects for involvement in malaria control are numerous, however there is need to enhance the existing BHW Program.

[Community health workers are capable of determining reliably the target population for health programs].

The target population is a key concept for the planning and evaluation of health services. However, in developing countries, health professionals in the field are often uneasy with the determination of the population denominator. In Zaire, where reliable demographic data are hardly available, we make use of two different methods aimed to the determination of the size of the target population: (1) an exhaustive population based survey performed by trained interviewers, and (2) a rapid census of the target population performed by community health workers. This paper presents both the results of a demographic survey organized in 1984, and two consecutive census performed by community health workers in 1986 an 1988, in the same area (Northern-Kivu, Zaire). Our results suggest that community health workers under close supervision are able to perform rapidly and at low cost a reliable collection of the demographic data needed for the implementation and monitoring of health programmes at the local level.

An introductory course in prescription drugs for community health workers developed by a systems approach.

A 16-hour introductory course in prescription drugs for community health workers was developed by a systems approach. It featured an emphasis on: (1) learning the broad indications, side effects, and adverse reactions of the 30 most commonly prescribed drugs in the outpatient clinic, and (2) aiding the workers in patient education. Teaching materials were simple and inexpensive. An evaluation based on a pretest of the course's objectives compared to a final test documented the success of the instruction.

Adopting the primary health care approach in malaria control in Malaysia: lessons in community participation.

The Primary Health Care approach in Malaysia was first tried out in Sarawak, East Malaysia in 1982. In 1984, the Vector Borne Disease Control Program, Kelantan decided to adopt the Primary Health Care approach as an additional strategy in its effort to control malaria in the state, which then experienced an increase in malaria cases. Much effort was directed at creating the awareness and stimulating the interest of health staff and communities to adopt the strategy. Kelantan was made the model state. The paper gives an outline of the process involved and some characteristics of PHC workers. A study was carried out among health workers, community members and health staff on their knowledge and involvement in PHC in three states, including Kelantan. In view of the overall success of this approach, and the weaknesses which have been identified possible solutions have been suggested and should be acted upon.

Women's liberation groups as a primary preventive mental health strategy.

As a preventive mental health worker in a community mental health center, I considered it part of my role to explore the possible therapeutic value of women's liberation groups in the community. I was particularly concerned with countering the sense of isolation and powerlessness prevalent among female adults in our suburban catchment area. Subsequent investigation and participant observation of women's groups bore out my expectation that many of the goals, premises, and methods of preventive mental health were compatible with those of women's liberation groups. These groups provided a very effective framework for personal as well as interpersonal and sociocultrual change in the direction of psychological growth.

Based on cultural strengths, a school primary prevention program for Asian-American youth.

As Asian-American mental health workers in a community mental health center where there is very little being done to provide appropriate services to its Asian-American population, we felt a need to find effective mental health models for the unique cultural and linguistic characteristics of our people. The high-school primary prevention program described in this paper provides Filipino youth with a positive view of their cultural identity and a supportive place to examine how their cultural values and behavior differ from those of mainstream Americans. Recognizing that ethnic minorities need to rely on their own subculture for the kind of psychological and social support required for a positive mental health, the program is aimed at mobilizing the strengths and resources of that particular subculture. In this way, its members can more effectively cope with the larger culture.

General principles in delegation of maternity functions to community-based health workers.

Following a review of the general principles in delegation of responsibility, the functions to be delegated are presented together with minimal criteria for training to acquire required skills. Problems arising from delegation are reviewed as well as possible solutions which include modification of some standard procedures. It is concluded, drawing from case summaries, that effective delegation needs strong leadership of maternal health teams buttressed with national political, professional, and community support.

The interfacing of constituencies.

Within a public human service organization interaction occurs among diverse constituencies such as clients, donors, and professionals. Several recent trends affecting relationships among constituencies are identified and problems emerging from these trends are considered. Since various changes within human service organizations have occurred during the 1960s and early to mid-1970s, one may speculate about future developments which may result from these trends. It is suggested that each constituency must adapt to changing circumstances in a spirit of cooperation if the organization is to be effective.
