IL-12 exacerbates helminth-mediated corneal pathology by augmenting inflammatory cell recruitment and chemokine expression.

Corneal inflammation (keratitis) is a major cause of visual impairment in Onchocerca volvulus infection. Previous studies showed that onchocercal keratitis can be induced in mice following s.c. immunization and intracorneal injection with soluble O. volvulus Ags (OvAg), and that the inflammatory response is dependent on T cells and IL-4. Since recombinant IL-12 impairs IL-4-dependent, Th2-mediated responses in other parasitic infections and in models of allergic asthma, the present study was undertaken to determine the effect of IL-12 on onchocercal keratitis. Mice were injected i.p. with IL-12 or saline at the time of initial sensitization to OvAg. Surprisingly, IL-12 treatment caused significant exacerbation of corneal pathology, which was associated with increased eosinophil and mononuclear cell infiltration into the corneal stroma. Consistent with the well-documented effect of IL-12 on Th1 cell development, corneas of IL-12-treated animals had elevated expression of the Th1 cytokine IFN-gamma and diminished expression of the Th2 cytokines IL-4, IL-5, IL-10, and IL-13. However, corneas from these animals also had marked elevation of alpha- and beta-chemokines known to be active on eosinophils and mononuclear cells, including IFN-gamma-inducible protein (IP)-10, macrophage inflammatory protein-1alpha, macrophage inflammatory protein-1beta, JE/monocyte chemotactic protein-1, RANTES (regulated upon activation, normal T expressed and secreted), and eotaxin. Together, these data indicate that IL-12 exacerbates OvAg-mediated corneal pathology by enhancing chemokine expression and recruitment of inflammatory cells.

Efficacy of phototherapeutic keratectomy in various superficial corneal pathologies.

PURPOSE: To study the use of phototherapeutic keratectomy in the treatment of various corneal pathologies. METHODS: We performed a nonrandomized consecutive study of patients who presented with superficial corneal diseases and poor vision. After slit-lamp microscopy, 31 eyes were included: corneal dystrophies, 20 eyes; pterygium, 1 eye; recurrent erosions, 3 eyes; superficial corneal opacities, 6 eyes; band-shaped keratopathy, 1 eye. All patients were treated with the Nidek EC-5000 excimer laser following manual epithelial debridement. A central 6.0 to 7.5-mm zone was treated with a surrounding 1-mm blend zone. Ablation zone diameter was determined by surgeon preference, presence of pannus, and overall corneal diameter. Follow-up ranged from 6 to 24 months, with a mean of 1 year. RESULTS: Epithelial healing occurred between 3 and 8 days in most eyes. Patients were followed for a minimum of 1 year. More than 50% of patients gained more than 2 Snellen lines; 12% for whom PTK was done for recurrent corneal erosions were symptomatically better. No eye was retreated. One patient with band keratopathy did not improve. Delayed epithelial healing occurred in three eyes. A trace to mild reticular subepithelial stromal haze was present in most eyes. Most patients showed a tendency to shift toward hyperopia. CONCLUSION: Phototherapeutic keratectomy may facilitate better visual acuity and reduced photophobia in eyes with various corneal pathology. Proper case selection is crucial.

Identification of an epitope of a recombinant Onchocerca volvulus protein that induces corneal pathology.

Ocular onchocerciasis results from immune recognition of parasite proteins released into the eye by degenerating microfilariae. Previous studies have shown that pathology similar to human ocular onchocerciasis can be induced in sensitized mice by intracorneal injection with Onchocerca volvulus antigens. In the current study, we used this murine model to map the segments of O. volvulus protein disulfide isomerase (OvPDI) associated with the development of corneal pathology. Subclones of OvPDI were constructed encompassing one or more predicted T cell epitopes. Keratitis was induced in BALB/c mice after subcutaneous immunizations with OvPDI, followed by intracorneal challenge of OvPDI constructs. Truncated OvPDI proteins containing amino acids 450-481 of OvPDI were found to induce keratitis, whereas constructs that did not include this region did not induce corneal pathology. Consistent with this observation, two peptides derived from the 450-481 region stimulated T cell proliferation to a greater degree than control carrier protein. DNA sequence analysis of cDNAs encoding OvPDI from blinding and non-blinding strains of O. volvulus indicated no differences in the primary amino acid sequence of the 450-481 domain. Immunization of animals with OvPDI induced antibodies recognizing a 55 kDa host protein, identical to the predicted molecular weight of the mouse PDI homologue. Together, these data implicate specific antigenic epitopes of OvPDI in the development of O. volvulus mediated corneal pathology.

[Analysis of corneal pathologic changes and laboratory parameters in herpes simplex karatitis patients with ganhuo-shangyanzheng or ganshengyinxuzheng].

In order to investigate the relationship between Ganhuo shangyanzheng and Can-shengyin xuzheng in herpes simplex karatitis patients, we observed corneal pathologic changes and examined blood levels of prostaglandin F2 (PGF2), prostaglandin E2 (PGF2), tumor necrosis factor (TNF), arginine vasopressin (AVP), norepinephrine (NR), epinephrine (E) in sixty herpes simplex karatitis patients with Ganhou shangyanzheng or Gan-shengyinxuzheng. The results showed that the corneal pathologic changes were corneal ulcer infiltrating to stroma of cornea in Ganhuo shangyanzheng patients, and refractary corneal ulcer with large amount of corneal neovascularizaton and infiltration of corneal stroma in Gan-shengyinxuzheng patients, the blood levels of PGF2, PGE2, TNF, AVP, NE, E in Ganhuo shangyanzheng patients were higher than those in Gan-shengyinxuzheng patients or healthy persons. The results suggest that these parameters may be objective parameters for differential diagnosis of traditional Chinese medicine Zheng types in patients with herpes simplex karatitis.

Vision through an abnormal cornea: a pilot study of the relationship between visual loss from corneal distortion, corneal edema, keratoconus, and some allied corneal pathology.

Visual function was assessed by classic acuity measures as well as contrast thresholds over a wide spatial frequency range for subjects with experimentally induced corneal distortion and induced corneal edema. These results were compared with similar results for patients with uniocular keratoconus, bilateral keratoconus, and allied corneal pathology. Distortion and edema were found to produce characteristically different types of contrast attenuation at threshold (amplitude) and have quite different suprathreshold abnormalities (phase) for objects within the resolution limit and therefore could form the basis of a useful functional classification of the visual degradation from corneal pathology.

A systematic method of drawing corneal pathologic conditions.

We describe a method of indicating corneal pathologic conditions in the patient record by color-coded frontal and slit sketches. The practice improves biomicroscopic skill, provides an easily reviewed record of the disease's course, helps monitor observations of ophthalmologists in training, and creates a standardized method of notation.

[Corneal pathology in Crohn's disease: electron microscopic study of a case].

Corneal pathology in Crohn's disease is relatively rare. We had the opportunity to take a biopsy of a bullous mass in the cornea of a man with Crohn's disease. Analysis of electronic pictures shows an aberrant structure of the nucleus, a very dense nuclear membrane and fragmented nucleoles. The cytoplasm contains many patches without membrane; the basal membrane is composed of filaments, and the stroma possesses figures of fingerprint-type. This unclassified description of the cornea might be typical for Crohn's disease.

[Corneal pathology in rheumatoid arthritis and Sjögren-syndrome with special reference to "rheumatoid keratomalacia" (author's transl)].

The corneal pathology in rheumatoid arthritis and Sjögren's syndrome is described with emphasis on rheumatic keratomalacia. This particular disorder whether affecting the central and paracentral or the peripheral parts of the cornea is characterized by thinning of the corneal stroma, loss of epithelium, ulceration and eventually by corneal perforation. 7 cases are reported. Reduced tear production and simultaneous corticosteroid treatment seem to be the essential pathogenetic factors of this corneal disease. Therapeutic consequences and differential diagnosis are discussed.

Contributions of electron microscopy to the study of corneal pathology.

Most of the electron microscopic studies of pathological corneas have been done only recently. Keratoplasty has been the most important source of specimens for ultramicroscopic investigation. With the introduction of electron microscopic techniques, we have been able to confirm many light microscopic studies in pathological corneas. This contribution has been most valuable in the identification of the sites and types of pathological changes in corneal dystrophies and degenerations. This review of electron microscopic studies describes the present concepts on the nature of the histological changes in dystrophies and degenerations of the anterior and posterior corneal layers and corneal stroma. It also includes a review of some corneal inflammatory conditions as well as metabolic disorders affecting its transparency.

[Current State of Corneal Pathology in Germany: Results of a Survey Conducted Among Members of the Cornea Section of the German Ophthalmological Society (DOG)].

BACKGROUND: The current situation of corneal pathology is widely unknown. Especially, there are no data available how corneal tissue is processed after penetrating keratoplasty. MATERIAL AND METHODS: In November 2002 a questionnaire with 15 questions was sent to all registered members of the cornea section of the German Ophthalmological Society (DOG) ( www.sektionkornea.org ) via email. Only one answer per eye clinic was allowed. RESULTS: Answers were obtained from 40 eye hospitals in which 3070 penetrating keratoplasties had been performed in 2001. 90 % of the surgeons believe that ophthalmopathological investigations of excised corneal specimens are always or at least in some cases important, and 80 % examine 75 to 100 % of all corneal buttons. However, the corneoscleral rim is investigated only exceptionally. 45 % of all corneal buttons are examined by a pathologist within the hospital while 33 % of the corneal specimens are investigated by an ophthalmopathologist in an ophthalmopathological laboratory. In most cases the hospital itself has to bear the cost. Clinics with a high number of corneal transplantations do not differ from the average regarding the fate of corneal buttons. Morphological investigations and publications concerning the cornea are more frequent when an ophthalmopathological laboratory is available in the hospital. CONCLUSIONS: For most eye hospitals performing keratoplasties, the histopathological examination of excised corneal buttons is very important. It is carried out to a large extent by pathologists, but specialised ophthalmopathological laboratories are consulted when a profound expertise is needed. Thus, this survey is an argument for the preservation of these laboratories too. Based on the obtained data and on several comments by the section members initiatives to intensify histopathological investigations of excised corneal specimens should be undertaken.

Automated lamellar therapeutic keratoplasty (ALTK) in the treatment of anterior to mid-stromal corneal pathologies.

PURPOSE: To evaluate the efficacy of automated lamellar therapeutic keratoplasty (ALTK) in the treatment of anterior to mid-stromal corneal pathologies. METHODS: A prospective clinical study was undertaken in 15 eyes of 15 patients who underwent ALTK using the ALTK System (Moria/Microtek Inc., Doylestown, Pennsylvania, USA). The parameters evaluated were uncorrected and best corrected visual acuity (BCVA), keratometry, pachymetry, contrast sensitivity, glare acuity, intraoperative surgical time and complications, postoperative time to epithelialization, graft clarity and postoperative complications. RESULTS: The mean surgical time was 46 +/- 9 mins. The BCVA, contrast sensitivity and glare acuity significantly improved at 6 months postoperatively. The mean decimal BCVA was 0.08 +/- 0.03 preoperatively, which improved to 0.33 +/- 0.30 at 6 months, when 11 eyes had BCVA > or = 6/18. The median epithelialization time was 3 days (range 1-10 days). CONCLUSIONS: Automated lamellar therapeutic keratoplasty appears to be safe and effective for diseases affecting the anterior to mid-stroma of the cornea.

Excimer laser phototherapeutic keratectomy for anterior corneal pathology.

The argon-fluoride 193 nm excimer laser is currently under investigation in the United States for both refractive and therapeutic indications. We report the results of phototherapeutic keratectomy (PTK) on 20 eyes of 19 patients. All patients had corneal pathology in the anterior one-third of the cornea. Mean follow-up was 7.8 months. While results are difficult to quantify because of varied indications, visual acuity improved in nine of 20 eyes (45%), remained unchanged in 10 eyes (50%), and worsened in one eye (5%). We judged 15 eyes (75%) to be functionally improved and one eye (5%) to be functionally worse. Our results demonstrate the effectiveness of excimer laser PTK for a wide variety of anterior corneal conditions. With continued improvement in patient selection and refinements in hardware, software, and surgical technique, PTK holds promise as an attractive alternative to more invasive and aggressive surgery.

Corneal pathology and aniridia associated with partial trisomy 2q, due to a maternal (2;6) translocation.

A newborn with partial trisomy 2q, due to a maternal (2;6) translocation is presented. The baby had microcephaly, a prominent forehead, hypertelorism, a broad nose with a flat nasal bridge, a long philtrum, micrognathia, and low set ears which are common to cases with partial 2q trisomy. Additional findings of aniridia and marked corneal pathology are reported.

Different strains of Pseudomonas aeruginosa isolated from ocular infections or inflammation display distinct corneal pathologies in an animal model.

PURPOSE: The present investigation sought to define the responses of mouse eyes to challenge with three different strains of P. aeruginosa isolated from human corneas or contact lenses: two different strains produced an ulcerative keratitis, and one strain produced contact lens-induced acute red eye (CLARE). METHODS: The corneas of BALB/c mice were inoculated with three different strains of P aeruginosa. The strains were allowed to interact with the corneas for up to 24 h. In addition, strain Paerl, isolated from CLARE, was subjected to in vitro assays to measure its ability to invade corneal epithelial cells, or to produce cytotoxicity in these cells. Both these assays used cultured rabbit corneal epithelial cells. RESULTS: Both MK isolates were able to infect the corneas of mice, but the CLARE isolate was non-infective. The predominant response to infection with the cytotoxic strain was severe corneal edema and infiltration of the corneal stroma with polymorphonuclear leukocytes (PMNs). The predominant response with the invasive MK isolate was corneal ulceration and infiltration with PMNs. The CLARE strain produced only low levels of PMN infiltration. In in vitro assays the CLARE strain was non-invasive and non-cytotoxic. CONCLUSIONS: This study has identified that P. aeruginosa produces at least three different types of corneal pathology and that not all strains are able to infect mouse corneas.

[Corneal pathology in the cat and dog].

Corneal diseases in the Cat and the Dog are of great variety and present similitudes with human corneal disorders, as demonstrated by the present report which discusses dystrophies, degenerations and inflammations of the cornea successively. Deep endothelial dystrophies are rare, poorly understood, and related to certain breeds; lipid dystrophies are frequent and either primary, occurring in some breeds only, or secondary to hyperlipemia; calcareous dystrophies are breed-related affections. Primary corneal degeneration has been described in the cat only and is a very rare affection. Keratitis is by far the commonest corneal lesion reported, and may be of herpes virus origin in the Cat and due to adenovirus in the Dog. Dry keratitis as in humans has also been observed. Two types of keratitis of allergic origin are described which are closely related to two breeds of dog: Alsatians and the long-haired dachshund. Recurrent corneal erosions, poorly recognized in veterinary medicine, are now described; their etiology and histological appearances are comparable with those reported in humans. These findings emphasize the interest of using animal models for human pathology investigations.
