Comparisons of tactile thresholds between implant-supported fixed partial dentures and removable partial dentures.

This study compared the tactile sensitivity of splinted abutment and denture teeth of 16 fixed partial dentures (FPD) supported by blade implants and 16 removable partial dentures (RPD) in patients with Kennedy Class I and Class II edentulous conditions. No significant differences were noted between the tactile thresholds of the natural abutment teeth and artificial teeth in the FPD and RPD groups. The splinted abutment teeth required 45.4 g, or 5.4 to 5.8 times higher occlusal loads than did those needed for the comparable nonsplinted teeth, to detect the stimulus. A further increase of 54% in thresholds with the FPD and over 100% with the placement of the RPD indicated the superiority of the RPD in terms of load distribution as a result of the cross-arch splinting and mucosal support. Moderate positive correlations (r = 0.37 to 0.46; P < .05) between tactile thresholds and masticatory performance were found, signifying that reduced tactile perception was not responsible for the incomplete restoration of the masticatory function with RPDs or FPDs but might be contributing to increased masticatory performance within both treatment groups.

Analysis of Variance

A 5-year longitudinal study of cantilevered fixed partial dentures compared with removable partial dentures in a geriatric population.

Prosthesis function and dental conditions were observed for 5 years in 27 elderly patients treated with mandibular cantilevered fixed partial dentures (FPDs) and in 26 elderly patients treated with distal-extension removal partial dentures (RPDs). All patients were treated with a complete upper denture. The patients were assigned randomly into two treatment groups that had the same composition with regard to sex, age, and distribution of teeth. The patients were under supervised oral hygiene and prosthodontic care. Clinical examination of prostheses, masticatory system, periodontal status, and caries was carried out yearly. Oral hygiene was good, and the periodontal status was maintained in both groups. Caries was observed six times more frequently in the RPD group than in the group with fixed restorations, however. Occlusal and functional conditions deteriorated in the RPD group only. Eight of 42 fixed partial dentures (19%) failed; of these, six were recemented with composite resin. Generally the need for dental and prosthetic follow-up treatment was more pronounced in the RPD group than in the FPD group.

The prevalence of Candida albicans in complete denture and removable partial denture wearers: a comparative study.

In this study, 60 complete denture wearers, 53 removable partial denture wearers and 50 dentate subjects were examined to determine the Candidal carrier state. The influence of local factors such as denture type, smoking habits and sex on Candidal carrier rate were investigated. Wearing complete or removable partial denture was determined as an important factor increasing Candidal carrier rate. Cigarette smoking is another factor which increases Candidal growth in complete denture and removable partial denture wearers. In contrast to denture wearers Candidal carrier rate was considerably less on dentate smokers. Candidal colonization rate was found to be higher on the dorsum of the tongue. This suggests that the tongue is the primary oral reservoir of Candida albicans in the mouth.

Duplicate casts in fabrication of temporary removable partial dentures.

The use of blocked-out duplicate casts in the fabrication of temporary removable partial dentures has been described. Temporary removable partial dentures produced in this controlled manner fit properly and aid in minimizing intraoral damage. Accuracy of fit is assured because all relief is accomplished by block out on the surveyor during the laboratory phase rather than by random grinding of the prosthesis at insertion. In addition, the original cast is salvaged and can be used as a meaningful reference during the finishing process in the laboratory (Fig. 5).

The utilization of intracoronal or extracoronal removable partial denture attachments in conjunction with a fixed retainer applies additional stresses to the structural durability of the fixed retainer and its integrity with the underlying abutment. Therefore, the utilization of this attachment with a fixed retainer requires understanding and diligent application of the basic principles of fixed prosthodontics.The primary objective of prosthodontics must be the preservation and health of the remaining existing tissues and structures: teeth, periodontal tissues, temporomandibular joint, and oral facial muscles. This must be of primary importance as we achieve our goals of (1) eliminating existing pathology, (2) restoring morphology and function, and (3) improving esthetics and thereby heightening self-esteem and the psychological well-being of the individual. These objectives make us sensitive to the considerations we have discussed in preparation design and the tissue management required in the fabrication of fixed retainers.

The fabrication of an aluminum-based temporary removable partial denture has been presented. The advantages of this type of prosthesis include compatibility with oral tissues, accuracy of fit, decreased bulk, and improved thermal conductivity.

A technique for constructing a provisional removable partial denture before and during construction of an anterior splint bar prosthesis is described. The provisional prosthesis is easily adapted to fit the splint bar prosthesis after it is cemented in place. The technique assures that the patient will enjoy the benefits of a well-fitting and esthetic provisional restoration through all phases of construction of the splint bar and the final removable partial denture.

Occlusal plane modification of an existing maxillary complete denture prior to removable partial denture construction: a case report.

Success in fabricating removable partial dentures, Kennedy Class I and Class II in particular, is highly dependent on proper orientation of the occlusal plane. The closer to the ideal, the better the prosthesis will function and provide proper esthetics and phonetics. This case report presents the diagnosis and occlusal modification of an existing maxillary complete denture to improve the patient's masticatory function with the fabrication of a removable partial denture.

Correction of prognathism with fixed and removable partial dentures.

Prognathism in those patients for whom surgical orthodontic procedures are contraindicated can be successfully treated by the use of fixed and removable partial denture prosthodontics. Careful neuromuscular evaluation of the patient is necessary for the success of the treatment rendered. The nonsurgical treatment of prognathism by this method was described.

[Structural designs in the removable partial dentures].

Today the removable partial dentures are used as permanent prosthetic devices. Thus, their designs must be considered the prevention of breakage or deformation. A metal plate denture consisting of double structure has been designed with the intention of improved mechanical strength and durability. Principal investigations of this study were to introduce structural design techniques and apply them to denture design, to analyze the double structure using the finite element method, to judge reliability using a statical bending test, to compare the breaking strength of each structure, and to measure movement during function of mandibular extension base removable partial dentures on the simulation model. The summary of results is shown below: 1. Calculated values and experimental values were a fairly good approximation. 2. The reinforcement and Lightweight method of double structure was made evident. 3. Regarding the packing resins, METADENT was slightly superior in mechanical strength to ACRON. 4. Maximum stiffness of the double structure was approximately 7-10 times that of skeleton types, and breaking strength was about 9 times. 5. Tensile strain generated at the double structure was statistical less than for skeleton types. 6. When loads of 10 kg applied, the difference of distal displacement between double structure and skeleton types was between 70-80 microns. 7. The stiffness of the skeleton types decreased remarkably by shortening the denture flanges, but the double structure did not show significant change.

A procedure has been described for using porcelain-fused-to-metal crowns in place of denture teeth on a removable partial denture. Improved esthetic results are achieved when replacement denture teeth are made simultaneously with, and visually match, adjacent tooth crowns. This procedure provides a superior treatment result, particularly where multiple unilateral anterior teeth are lost and a removable partial denture is indicated.

An orderly sequence of steps for construction of an interim acrylic resin partial denture has been presented. The technique allows the dentist to fabricate an effective restoration that has a definite path of insertion and removal that can be placed in the patient's mouth with little time spent on adjustment and correction. This technique may be used with heat- or cold-curing acrylic resin.
