Disappointing effect of endoscopic Teflon injection for female stress incontinence.

Twenty-two women were treated with endoscopic, transurethral, submucosal Teflon injection for moderate or severe urinary stress incontinence. The short-term effect was rather good, but he long-term results were disappointing. After 5 years, only 4 patients were completely or sufficiently continent and had not required any additional treatment for incontinence. Three major complications, paraurethral abscess, urethral diverticulum and Teflon granuloma with urethral wall prolapse occurred. Subsequent suprapubic urethropexy for the patients with inadequate response did not produce the usual good results.

[Osteosynthesis using resorbable plates in maxillofacial surgery: hopes and disappointments].

The use of absorbable osteosynthesis material has two advantages in maxillofacial surgery: suppressing the possible biological effects of implanted metallic foreign bodies, making second surgery for material removal useless. Although the biotolerance of currently used materials is well known, the authors intended to test their use in the specific conditions of maxillofacial surgery, in which simply extrapolating the data collected in general surgery leads to erroneous conclusion. The authors report the results of a preliminary clinical study on 21 cases of osteosynthesis of the upper third of the face using absorbable plates (18 fractures of the zygomatic bone, 3 maxillary osteotomies). The biotolerance of the material is very good, after more than 3 years for the oldest cases. The mechanical stability of the assembly is sufficient for the selected indications. As regards disappointments, these include the thickness of the plates, the difficulty to adapt them to bony surfaces, the fragility of the screws and the slow resorption of the material.

Biocompatible Materials

[My current options in cervicofacial lift. Results, disappointments and development from 1980 to 1990].

Based on a series of 100 cases of cervicofacial face lift selected at random over a period of 10 years, the author describes the development of her personal technique together with the results and disappointments responsible for this development. She distinguishes 3 surgical periods. The first period (1980-1984) was characterised by very extensive subcutaneous undermining associated with reduced detachment of the facial SMAS, systematic anterior and variable posterior platysmaplasty. The second period (1984-1986) was distinguished by abandonment of the systematic submental approach, a persistent extensive subcutaneous undermining especially in the neck, more pronounced dissection of the facial SMAS and a systematic posterior platysmaplasty. The third period (1986-1990) was marked by a trend toward more limited subcutaneous undermining and more dissection-section-resection-retention of the facial and cervical SMAS and the current tendency consists of total platysmaplasty.

Immunodiagnosis of neurocysticercosis. Disappointing performance of serology (enzyme-linked immunosorbent assay) in an unbiased sample of neurological patients.

To ascertain the reliability of serological diagnosis of neurocysticercosis in the everyday a priori situation of neurological consultation, the enzyme-linked immunosorbent assay test was used to predict the eventual diagnosis of neurocysticercosis in an unselected sample of 1064 consecutive neurological cases. Results showed 69% sensitivity and 71% specificity of the enzyme-linked immunosorbent assay test for the diagnosis of neurocysticercosis. In sharp contrast with publications that have proclaimed the excellent diagnostic performance of immunodiagnostic tests, our results suggest that identification of serum antibodies with standard enzyme-linked immunosorbent assay techniques is of little value when applied to a large and heterogeneous group of neurological patients in an endemic area of cysticercosis, and our results urge a reevaluation of currently used immunodiagnostic tests that are practiced in the serum of suspected cases.

Disappointing follow-up findings for indigent high-risk newborns.

Indigent populations have received little attention in neonatal follow-up studies. We conducted "blinded" evaluations one year past term for 204 indigent high-risk infants who were ventilator treated or had a very low birth weight (VLBW) (less than or equal to 1500 g) and 85 healthy term controls from families similar to those of the high-risk infants. Marked developmental delay (Bayley Mental Developmental Index, less than 70) or gross motor abnormality occurred in 2% of controls, 27% of VLBW infants, 33% of ventilator-treated infants, and 39% of ventilator-treated VLBW infants. Despite considerable effort to prevent attrition, 43% of high-risk survivors were unavailable for follow-up at the one-year visit. Even if all of these infants were assumed to be normal, the incidence of developmental delay exceeded that in 11 of 12 recent studies. Indigent high-risk infants deserve considerable follow-up attention because of their high rate of attrition and developmental delay.

Anthropometry

Advances and disappointments, indications and restrictions regarding HLA-matched corneal grafts in high risk cases.

54 corneas, 49 high risk cases and 5 low risk cases, were grafted with HLA-matched donor material. The importance of antigen compatibility especially in high risk cases is demonstrated. The percentage of irreversible rejections and reversible rejections in this group is markedly smaller as compared with our own "at random" material and the "at random" material of other authors. The follow-up period is 3--21 months.

The biochemical approach to mental handicap: 30 years of achievements and disappointments.

In the past half century, biochemistry has emerged as an independent discipline that provides essential and fundamental information for medical science. It has an increasingly important role in various branches of medicine and the biochemist is frequently called upon to apply his expertise to the solution of clinical problems, amongst them the role of biochemical disturbances in the aetiology of disorders associated with mental handicap, and the investigation of factors affecting their severity and clinical course. The expectation has been that the understanding of biochemical defects will lead promptly to the suggestion of remedies.
