Artificial intelligence-supported double reading in European population breast cancer screening: A systematic review and meta-analysis of prospective programs.

BACKGROUND: Most European population mammography screening programs rely on double reading with arbitration, a model that delivers mortality benefit but is increasingly challenged by radiologist workload, variable specificity, and interval cancers. Artificial intelligence (AI) is being evaluated to support or optimize these established European screening pathways. PURPOSE: To synthesize prospective or program-embedded evaluations of AI conducted within European-style population screening programs and to estimate exploratory program-level absolute risk differences (RDs) per 1000 examinations for cancer detection rate (CDR) and recall. MATERIALS AND METHODS: We performed a prespecified, focused evidence synthesis of three large studies embedded within routine population screening programs operating under European-relevant workflows: MASAI (randomized AI-supported risk triage within a national program), ScreenTrustCAD (prospective paired-reader evaluation with AI as an independent reader in a double-reading framework), and PRAIM (nationwide decision-referral implementation). Outcomes were harmonized as AI-control RDs per 1000 examinations. Random-effects pooling used Hartung-Knapp-Sidik-Jonkman models. For the paired-reader design, sensitivity analyses applied a Kish effective sample-size approach across plausible within-examination correlations (ρ = 0.3-0.8). Positive predictive value (PPV) and workflow/time outcomes were summarized descriptively. RESULTS: Across 597,419 examinations, the pooled CDR RD was +0.9 per 1000 (95% CI -0.0 to +1.8; I2 ≈ 12%), consistent with a modest directional increase with borderline statistical uncertainty. The pooled recall RD was -0.6 per 1000 (95% CI -3.1 to +2.1; I2 ≈ 41-43%), indicating no consistent recall increase across screening programs. Where reported, PPV was higher with AI-supported screening. Efficiency signals included 44.3% fewer total readings in MASAI and shorter reading times for AI-normal examinations in PRAIM; in PRAIM, a program-level safety-net mechanism recovered 204 cancers that would otherwise have been missed. CONCLUSION: In European population screening programs characterized by double reading and arbitration, prospective program-embedded evidence suggests that AI integration may yield a small absolute increase in cancer detection (≈1/1000) without a consistent increase in recall, alongside improved PPV and efficiency signals. These findings suggestAI primarily as a complementary reader within European screening workflows, with implementation requiring explicit quality assurance and monitoring of interval cancers and stage distribution.

Radiographic screening in the early detection of lung cancer.

A prospective study designed to detect early lung cancer in a high risk outpatient volunteer population is in progress. At present all 10,362 volunteers have been recruited into the project. A review of the radiographs of patients with lung cancer identified on the initial screen, or in retrospect, has led to the following conclusions: (1) Independent double reading is important in a screening project. (2) There are no reliable radiographic criteria to distinguish early lung cancer from benign disease. (3) The lateral chest radiograph is useful in the high risk patient. From these conclusions the authors have three recommendations for the practicing radiologist. (1) Since double reading improves sensitivity, attempt to doubly read a chest radiograph by removing your eyes from the film and look at it a second time before finalizing your report. (2) Consider any newly appearing, noncalcified lesion in the chest radiograph of a high risk individual as primary lung cancer until proven otherwise. (3) Consider the "routine chest radiograph" in a high risk patient as a challenge to detect early lung cancer rather than the drudgery of day to day clinical practice.

Estimated and true hinge axis: a comparison of condylar displacements.

The purposes of the investigation were: i) to compare the condylar displacements from centric slides for 74 sets of casts mounted with an estimated versus a true hinge axis; ii) to determine the error in hinge axes location on the recording of condylar displacements from centric slides; and iii) to define the reproducibility of the Mandibular Position Indicator (MPI), a method of measuring condylar (in vitro) displacements from centric relation position. The findings revealed that routine use of the true hinge axis to obtain MPI readings for analysis of diagnostic casts is not warranted. The mean standard error of MPI double readings attributed to both hinge axes location was 0.4 mm for each component in the sagittal plane. Of the 0.4 mm of mean standard error, 0.2 mm was contributed by the lack of reproducibility of the MPI. The centric relation bite registration technique outlined in this study always captured a superior articulator position of the condylar mechanism. The condylar distractions and deflections from centric relation to maximum intercuspation were primarily in an inferior direction with a smaller posterior component.

The radiology of lung cancer.

Double reading of radiographs increases the likelihood of detecting pulmonary disease and, accordingly, physicians have an obligation to view the radiographic examinations performed on their patients, either with the radiologist or independently. It is also essential that the radiologist be proved with the clinical findings before he interprets the radiographs, a practice which will result in significant improvement in the accuracy of radiologic reports.

["Cirrhotic's lund". A new radiological entity? 182 CASES (AUTHOR'S TRANSL)].

The authors studied the chest X-rays of 182 patients with alcoholic cirrhosis. They eliminated the abnormal films of patients with severe clinical or biological changes. After double reading, they isolated ten cases in which chest X-ray revealed changes in the pulmonary interstitium. The question as to the nature of this interstitial pneumopathy is posed.

A new simple technique for the identification of antibodies against double stranded DNA: preliminary communication.

A new, simplified assay for detecting double stranded DNA antibodies by indirect immunofluorescence using fixed human metaphase chromosomes as substrate is described. The technique has the advantage of ease of preparation, clarity of reading, specificity for double stranded DNA and apparently greater sensitivity than immunofluorescent assays currently available. Thirty of 92 systemic lupus erythematosus sera were found to have double stranded DNA antibodies using the metaphase preparation, compared with 23 positive sera detected with the Crithidia luciliae technique. No double stranded DNA antibodies were detected in 90 sera from patients with rheumatoid arthritis. The human metaphase chromosome assay could replace existing techniques for identification of double stranded DNA antibodies.

Diurnal rhythms in airway obstruction.

Diurnal patterns of variation in peak expiratory flow rate were studied in patients with all types of airway obstruction. Most regular patterns fall into one of two types: morning dip (M) with the lowest reading in the morning and (P) or double dip with the lowest readings morning and evening. Regular patterns were most frequently seen in asthmatics, but occurred also in bronchitis, particularly those with a history of wheeze. Treatment with bronchodilators reduces the prevalence of all regular patterns but does not abolish them. M pattern is characteristic of asthma but P pattern is the most frequent regular pattern seen in bronchitis. Amplitude of variation is greater in asthmatics than in simple bronchitis. It is reduced by about 20% with bronchodilators. Random observation of magnitude of response of peak flow rate to a bronchodilators is not a good indicator of amplitude of variation over the day. Because of the occurrence of a peak pattern in many patients the magnitude of diurnal variation may be underestimated by twice-daily peak flow rate readings. Treatment with corticosteroids does not appear strongly to influence either characteristic patterns or amplitude of variation.

Single bolus versus drip infusion for intravenous cholangiography.

Biliary tract visualisation in 50 patients examined by infusion of methylglucamine iodipamide was compared, by double-blind technique, to cholangiograms obtained in 50 other patients by a single bolus injection. The infusion technique did not give better results than the single bolus method. In another group of 20 patients, each individual was examined by both methods. The radiographs, read in a double-blind manner, showed both methods to be comparable in demonstrating the biliary system. It seems that other means have to be designed and employed in order to improve the radiographic delineation of the biliary tract.

A quantitative examination of the use of fluorescein in predicting viability of skin flaps.

The use of fluorescein as a predictor of skin viability in the rat flap model of avascular skin necrosis is well known in the field of plastic surgery. Inherent in this model are several faults that must be corrected if it is to become a valid standard. Fluorescence readings are one of the main variables in flap models. By carefully shaving rat backs with a double-edged razor, taking fluorescence readings in a uniformly darkened space, carefully tattooing the exact extent of both the bright and the mottled areas of fluorescence on the flap, and placing a plastic barrier between the flap and the body to prevent any part of the flap from acting as a graft, we were able to quantitatively predict which portions of a flap would survive and which would die after fourteen days. The bright fluorescent area (mean = 11.99 cm2, SEM +/- 0.53 cm2) is equal to the amount of tissue that will survive (mean = 11.91 cm2, SEM +/- 0.50 cm2; NS), while the sum of the nonfluorescent area and the mottled fluorescent area (mean = 27.69(2), SEM +/- 0.52 cm2) is equal to the amount of tissue that will die (mean = 27.99 cm2, SEM +/- 0.52 cm2; NS).

[Behavior of the "double erythrocyte sedimentation rate" in neoplastic pathology].

Special aspects of a particular ESR reading have been evaluated in a series of 273 patients admitted ver a number of years to the Departments of Surgical Anatomy and Operations Course and, thereafter, Surgical Symptomatology of the University of Milan (Head: Prof. Ugo Ruberti). The inhibitory phase of sedimentation observable in vitro is particularly and typically scanty in patients with malignant tumours. The negative results are also analysed critically, with mention of particular factors (metastasis, arteriopathies, etc.) of use in diagnosis and prognosis. In the light of present-day knowledge it is therefore considered that this "double" ESR reading points, with sufficient specificity, to an ongoing tumoral process.

Social class and preschool language skill: IV. semantic mastery of shapes.

Sociolinguists have contended that children, and especially black children, from families of poverty commonly fail in school not because they have either a cognitive or language deficit but because they have a dialect problem. Being unfamiliar with standard English, they are forced to undertake a double task while learning to read. In order to test the validity of this contention, 54 Head Start and 33 nursery-school children of four years were examined with a test of shape identification. The children of Head Start identified six shapes perceptually by matching them about as well as did the nursery-school children. On the other hand, a much smaller percentage of Head Start than of nursery school named correctly all the shapes as they were indicated by the examiners or indicated correctly all the shapes when they were named by the examiners. Sex differences in semantic mastery of shapes are absent; and among the children of Head Start, race differences in semantic mastery of shapes are also absent. The deficit in semantic mastery for shapes is clearly evident, and demonstrating the existence of such deficits can be useful in guiding compensatory education.

Double-staining procedure for the fluorescent treponemal antibody absorption (FTA-ABS) test.

The fluorescent treponemal antibody absorption (FTA-ABS) double-staining procedure was reproducible, comparable to the conventional test, and easy to read. We recommend the use of the FTA-ABS double-staining procedure for microscopes with incident illumination, the 100 x/1.30 oil achromatic objective and the 6.3 x ocular to obtain optimal fluorescence, and the KP560 as a barrier filter to exclude rhodamine emission when fluorescein fluorescence is read. With this system, errors related to poor focusing or failure to visualise treponemes on all smears should be eliminated.

Effect of sumatriptan, a new selective 5HT1-like agonist, on liquid gastric emptying in man.

Paired studies were carried out on 12 healthy male subjects to compare the effect of intravenous doses of the 5-HT1-like agonist sumatriptan (GR43175; 3 mg), 10 mg metoclopramide and saline control on the rate of gastric emptying of a radiolabelled liquid test-meal. Intravenous administration of metoclopramide accelerated gastric emptying by decreasing the lag period, while intravenous administration of sumatriptan delayed gastric emptying by increasing the lag period. The observation that sumatriptan causes a delay in gastric emptying in normal healthy volunteers, but relieves nausea and vomiting during migraine attacks, suggests that sumatriptan may be acting via a central mechanism to relieve symptoms of nausea and vomiting associated with migraine.

Engineering and comparison of cas12a-based genome editing systems in plants.

While Cas9 and Cas12a are both RNA-guided endonucleases used for genome editing, only Cas12a is able to process pre-crRNA via its additional ribonuclease activity. This feature reduces the complexity of Cas12a versus Cas9-based genome editing systems thus providing an attractive alternative for generating site-specific mutations in plants. Here we aimed to improve the efficiency of the cas12a-based generation of two double-strand breaks flanking the open reading frame of a target gene, leading to its full deletion. To this end, we compared the relative impact of different components on cas12a-based gene deletion efficiency in three different eudicotyledons, Arabidopsis thaliana, Lotus japonicus, and Nicotiana benthamiana. We detected the highest cas12a-based editing efficiency with a combination of suitable promoters for crRNA and cas12a expression, a tandem terminator to control cas12a expression, a re-coded cas12a, adapted to the codon usage of Arabidopsis and engineered to carry introns, and encoding a Cas12a flanked by a nuclear localization signal at both ends. Our work revealed the high potential for improving cas12a-based genome editing systems for plant genetic research.

Diabetic retinopathy study. Assessment and comparison of retinal lesions for computer analysis.

A method of documentation of a diabetic retinopathy (dR) is described. Panorama fundus photographs (PPhs) of each eye are taken every 3-6 months. The retinopathy is classified according to qualitative, quantitative, and topographic criteria. The retinal lesions are assessed according to a subjective grading scale. Data of direct readings and of comparisons between two PPhs of one eye are transferred to punch cards for storage and computer processing. In a double evaluation of 200 PPhs, the overall reliability of readings was found to be approximately 95%.

Diabetic Retinopathy

Re-examination of the effects of periodic and aperiodic stimulation on the auditory-evoked vertex response.

It has been hypothesized that aperiodic stimulation will produce larger auditory-evoked vertex responses than periodic stimulation. Attentiveness toward an acoustic stimulus by human subjects has been shown in the past to enhance the auditory-evoked response. The unpredictability of aperiodic intersignal intervals has been assumed by previous investigators to produce more attentive listening behaviour and has therefore been used clinically to obtain 'enhanced' evoked responses. In order to examine this question further, auditory-evoked vertex responses were obtained with variable-interval (aperiodic) and with fixed-interval (periodic) schedules of stimulus presentation that had geometric mean intersignal intervals of 5 or 10 s. The evoked responses were obtained from normally-hearing adults while they were engaged in a counting task and while they were reading. Tone bursts were presented at 30 and 60 dB sensation level. No significant differences in peak-to-peak amplitude measures of components with latencies shorter than 300 ms could be attributed to aperiodic stimulation. When the time-base on the signal averager was extended to include components with latencies longer than 300 ms, no significant changes in component amplitudes could be attibuted to aperiodic stimulation, even though peak-to peak amplitudes during an attentive state (counting) were nearly double those obtained during an inattentive state (reading). It was concluded that aperiodicity in intersignal intervals is not a useful technique to employ in order to produce larger and more identifiable evoked responses for clinical diagnosis.

Phonological processing in reading: data from alexia.

The reading of four subjects suffering from a phonological reading impairment as a result of a cerebral lesion was tested. A double dissociation observed in their results is strong evidence for the existence of two functionally independent kinds of phonological processing in reading--a graphemic and a phonetic one.
