The usefulness of the serum transferrin receptor in detecting iron deficiency in the anemia of chronic disorders.

BACKGROUND AND OBJECTIVE: Recent studies have shown that the serum transferrin receptor (sTfR) is a sensitive, quantitative measurement of tissue iron deficiency. The objective of the study was to evaluate the diagnostic efficiency of some laboratory tests, including sTfR measurements, in the diagnosis of iron depletion in patients with anemia of chronic disorders. DESIGN AND METHODS: The patient population consisted of 37 anemic patients: 10 hypoferritinemic patients (serum ferritin < 25 micrograms/L), and 27 anemic in-patients with hyperferritinemia (serum ferritin > 200 micrograms/L) and clinical/analytical criteria of anemia of chronic disorders, who were submitted to a bone marrow aspirate with iron stain. The sensitivity and specificity of serum TfR was evaluated according to the results of bone marrow iron status. Statistical analysis employed Student's t-test, one way analysis of variance and a logistic regression model using the Wald test. RESULTS: Serum TfR was high in all the patients with hypoferritinemic anemia. In 12 patients with low bone marrow iron, the mean sTfR was 5.63 mg/L. In 6 of these 12 patients the sTfR was normal. On the other hand, sTfR was high in 4/15 patients with normal or increased iron stores. On multivariate analysis the most sensitive predictor of true iron deficiency was MCH (mean corpuscular hemoglobin). No other variables remained independently significant, including sTfR, after the inclusion of MCH in this model. INTERPRETATION AND CONCLUSIONS: In our opinion, the iron status of patients with anemia of chronic diseases can not be accurately assessed by sTfR, as its sensitivity and specificity are low. In these patients, the gold standard for iron stores evaluation continues to be bone marrow aspirate and Perls stain.

Teenage childbearing and welfare: preventive and ameliorative strategies.

The results of seven computer simulations suggest that strategies to prevent teenage childbearing may be more effective in reducing the number of young women who require welfare assistance than are strategies to improve the circumstances of teenagers who have already given birth. The first simulation constitutes a baseline projection, in which current levels and patterns of adolescent childbearing are assumed to continue to 1990. Three "preventive" simulations assume that no births or fewer births occur among teenagers during the projection period; and three "ameliorative" simulations assume that changes occur in the completed family size, marriage rate and educational attainment of teenage childbearers. Compared with the baseline projection, the three preventive strategies are estimated to reduce by 22-48 percent the number of adolescent childbearers who, as 20-24-year-olds in 1990, will be receiving welfare payments; the three ameliorative strategies cause only a 6-12 percent drop. The strategy with the least impact is the education scenario, in which adolescent mothers are assumed to be no more likely to drop out of school than are other comparable teenagers. The primary reason for the surprisingly small effect appears to be the relatively low earnings of women--even when they are high school graduates. All of the experimental scenarios tested, however, bring about at least some reduction in projected government spending for the three major public assistance programs considered (Aid to Families with Dependent Children, Medicaid and Food Stamps).

Women's childhood experience of parental separation and their subsequent health and socioeconomic status in adulthood.

The long term consequences for women of parental divorce and separation in childhood are explored using data from a national, prospective, longitudinal survey. In comparison with women who suffered no parental loss, parental divorce is associated with lower educational attainment and occupational status, poorer mental health, higher alcohol consumption and higher rates of remarriage. Links with earlier signs of emotional disturbance and current levels of stress are explored.

A path analysis of some determinants of infant growth in Khartoum.

The interrelationships between some socioeconomic and behavioural characteristics of mothers and the growth and illness experience of their infants in poor Khartoum townships have been examined by path analysis. For both infant body weight and supine length it is shown that weaning age and illness experience are important determinants of growth. It is also shown that greater maternal income has disadvantageous effects through encouraging early weaning. Most importantly the infants of housewives have later weaning, less illness and greater weight and length at 1 year of age than the infants of mothers with jobs, showing the value of time for caring in the prevailing environmental circumstances.

Use of contraceptives by women of upper socioeconomic status.

This was a descriptive correlational study of the contraceptive knowledge, methods, and consistency of contraceptive use by women of upper socioeconomic class. Socioeconomic class was determined using Hollingshead's Four-Factor Index of Social Status (1975) and was based on the woman's own educational and occupational status. The mean socioeconomic score for participants in this study (N = 83) was high in the second of five socioeconomic classes. Study participants were found to be consistent users of their contraceptive method and had fewer unplanned pregnancies in their history than the general population. They were more knowledgeable about contraceptives and reproduction than others tested with the same contraceptive knowledge instrument. They used barrier methods predominantly, with the use of less theoretically effective methods correlating significantly with a greater number of contraceptive methods used in the past.

Female education, age, parity, and reproduction cessation in Ghana.

Data from the 1988 Ghana Demographic and Health Survey is employed to determine the existence of a relationship between the amount of education a woman has and her intent to cease childbearing. The findings are that with increasing levels of education, a female's desire to cease reproduction also increases. However, this relationship is obscured if parity is not controlled.

Trends in attitudes toward abortion: 1972-1976.

Trends in attitudes toward abortion are examined over the 1972-1976 period. While an overall tendency of more liberal attitudes is noted, important differences over time are apparent by gender, education, occupational prestige, and religiosity.

Factors associated with maternal mortality in Addis Ababa, Ethiopia.

A housing probability survey in which 9315 women were interviewed was conducted in 1983 to detect the incidence and aetiology of maternal mortality in Addis Ababa, Ethiopia. Maternal mortality for the two-year period from 11 September 1981 was 350/100,000 livebirths (excluding abortions). A logistic regression analysis selected antenatal care, occupation and income as risk factors for maternal mortality, after adjusting for age, parity, education and marital status. Odds ratios were 2.5 for unbooked women compared to those receiving antenatal care, about 3 for students, and maids/janitresses compared to housewives, and between 3 and 5 for those earning less than US$25 monthly, compared to those earning US$150 or more.

Heterosexual transmission of HIV-1 among employees and their spouses at two large businesses in Zaire.

To better understand the reasons why up to 80% of all HIV-1 infections in Zaire, but less than 5% in North America and Europe, are acquired through heterosexual transmission, and to assess the impact of HIV-1 infection on a large urban African workforce, we enrolled 7068 male employees, 416 female employees and 4548 female spouses of employees at two large Kinshasa businesses (a textile factory and a commercial bank) in a prospective study of HIV-1 infection. The HIV-1 seroprevalence rate was higher in male employees (5.8%) and their spouses (5.7%) at the bank than among male employees (2.8%) and their spouses (3.3%) at the textile factory. At both businesses HIV-1 seroprevalence was higher among employees in managerial positions (5.0%) than among workers in lower-level positions (3.0%; P less than 0.0001). In a multivariate analysis of male employees, receipt of a transfusion, a history of genital ulcer disease, working at the bank, urethritis, or being divorced or separated were independently associated with HIV-1 infection. During 1987 and 1988, AIDS was the most common cause of death among recently employed workers, accounting for 20 and 24% of all deaths at the textile factory and the commercial bank, respectively. The HIV-1 seroprevalence rate was higher among female workers (7.7%) than among the spouses of male workers (3.9%; P = 0.001). In multivariate analysis of the wives of workers, having an HIV-1-seropositive spouse, receipt of a blood transfusion, or a history of genital ulcer disease were independently associated with HIV-1 infection.(ABSTRACT TRUNCATED AT 250 WORDS)

Birth spacing patterns and correlates in Shaanxi, China.

This study shows that most women in Shaanxi Province, China try to have their first birth as soon as possible after their first marriage, and that the length of the interval between marriage and first birth is strongly correlated with the woman's age at first marriage. The length of the second and third birth intervals and the likelihood of going on to have a second or third birth are strongly influenced by the sex composition of children already born, the survival time of the child initiating the interval, the duration of breastfeeding, and the woman's occupation. There is significant regional variation in the length of birth intervals and in the prevalence of second and third births in Shaanxi. The findings indicate that China's one-child policy is far from being universally accepted in Shaanxi, including its urban areas. The persistence of many social, economic, institutional, and cultural factors promoting high fertility poses many obstacles to further fertility decline.

Document delivery capabilities of major biomedical libraries in 1968: results of a national survey employing standardized tests.

The standardized Document Delivery Tests (DDT's) developed earlier (Bulletin 56: 241-267, July 1968) were employed to assess the capability of ninety-two medical school libraries for meeting the document needs of biomedical researchers, and the capability of fifteen major resource libraries for filling I-L requests from biomedical libraries. The primary test data are summarized as statistics on the observed availability status of the 300 plus documents in the test samples, and as measures expressing capability as a function of the mean time that would be required for users to obtain test sample documents. A mathematical model is developed in which the virtual capability of a library, as seen by its users, equals the algebraic sum of the basic capability afforded by its holdings; the combined losses attributable to use of its collection, processing, relative inacessibility, and housekeeping problems; and the gain realized by coupling with other resources (I-L borrowing). For a particular library, or group of libraries, empirical values for each of these variables can be calculated easily from the capability measures and the status statistics. Regression equations are derived that provide useful predictions of basic capability from collection size. The most important result of this work is that cost-effectiveness analyses can now be used as practical decision aids in managing a basic library service. A program of periodic surveys and further development of DDT's is recommended as appropriate for the Medical Library Association.
