Vein of Galen malformation. Endovascular management of 43 cases.

Since 1984, 43 patients with true vein of Galen aneurysmal malformations have been referred to us and managed according to our patient selection, technique, and follow-up guidelines. Thirty-four were embolized transarterially with bucrilate (isobutyl cyanoacrylate) or enbucrilate (N-butyl cyanoacrylate) embolization. No cutdown or hypotension during or after the embolization was used and no balloon catheter was employed. Forty-seven percent of the children had a completely occluded lesion which was confirmed when the child was at least 6 months of age at the follow-up angiographic examination; 52.9% were found to be completely normal or only to have mild cardiac failure that could be treated medically or moderate macrocephaly without neurological symptoms or mental retardation. In the embolized group 5.8% died as a result of the wrong treatment (1 case) or poor timing of embolization 3 days after ventricular shunting (1 case). The overall mortality (embolized and non-embolized groups) in the neonatal children was 27.7% with a total of 18.6% for all ages. Complete morphological exclusion of the arteriovenous malformation was accomplished in 41.9%; 74.4% of all children referred are now clinically normal or present moderate mental retardation which is diminishing. There was 3% neurological morbidity in the embolized group (only following the venous approach) in 78 sessions and more than 100 arteries embolized. These results compare favorably with surgical or other techniques of arterial embolization (balloon or particles), as well as transvenous (transtorcular or transfemoral) embolization, where the morbidity and mortality are significantly higher and the late clinical evaluation is seldom satisfactory.(ABSTRACT TRUNCATED AT 250 WORDS)

Variable histotoxicity of histoacryl when used in a subcutaneous site: an experimental study.

Histoacryl (butyl-2-cyanoacrylate) is one of the least histotoxic cyanoacrylate derivatives and is used as a tissue adhesive. Clinical applications primarily include skin closure (blepharoplasty incisions, etc.). In a recent study, we demonstrated that Histoacryl elicits minimal histotoxicity when used to glue bone grafts to rabbit-ear cartilage. Acute inflammation was limited to areas where Histoacryl escaped from between the bone graft and ear cartilage to contact well-vascularized soft tissue. In this study, Histoacryl was applied between bone graft and cartilage in one rabbit ear and adjacent to well-vascularized soft tissue with no graft in the opposite ear. Histologic analysis revealed minimal if any inflammation when small amounts of glue was used in the nonvascular region between bone graft and cartilage. However, subcutaneous implantation contacting well-vascularized soft tissue resulted in increased acute inflammation and prolonged foreign-body giant-cell response. Further studies are required to rule out any long-term problems associated with subcutaneous implantation of Histoacryl.

Tissue adhesives inhibit experimental new bone formation.

The effect of the cyanoacrylate tissue adhesive Histoacryl on new bone formation was studied in rats. Experimental heterotopic new bone formation was induced by implanting pieces of demineralized allogeneic bone matrix (DABM) in the abdominal wall of 10 growing Sprague-Dawley rats. This produces cartilage formation within and around the implants after 10 days, followed by enchondral ossification and the formation of an ossicle with remodelling bone and bone marrow after three weeks. Prior to implantation, the DABM-implants were treated with increasing amounts of the tissue adhesive n-Butyl-2-cyanoacrylate-monomer (Histoacryl). New bone formation was quantified at three weeks by assay of the ash content of the implants as a measure of net bone formation, by 45calcium uptake prior to sacrifice, and by histology. Treatment of DABM with the cyanoacrylate caused an intense inflammatory process with a foreign body reaction, and abolished bone induction and new bone formation. Tissue adhesives should be used with caution in fracture surgery since they inhibit new bone formation, cause a foreign body reaction, and may impede fracture healing.

Aneurysmal bone cysts treated by superselective embolization.

A series of 7 cases of aneurysmal bone cysts (ABC), all but 1 located in the pelvic bones, which were treated by transcatheter embolization is presented. Five embolizations were performed for primary treatment, while in 2 patients they were performed preoperatively. In all patients a definite histological diagnosis was established by open biopsy, and plain radiographs as well as computerized tomography (CT) were applied for evaluation of the results. The embolizing materials were tissue adhesive for permanent, and Gelfoam for preoperative vessel occlusions. Of 5 lesions treated for the purpose of definitive embolization, 3 (all located in the superior pubic ramus) showed complete ossification; in one, partial remineralization and progression occurred. In a further patient with a secondary ABC, partial reconstitution was recorded. Both preoperative embolizations resulted in successful en bloc resection with minimal blood loss. Transcatheter embolization is the treatment of choice for the highly vascular ABC in unfavorable anatomical locations.

[Butylcyanoacrylate--an adhesive for bonding strain gages to non-fixed biological materials (author's transl)].

Creep, hysteresis, stability, elongation capability and repeatbility of butylcyanoacrylate as a proper adhesive for bonding strain gages to non-fixed tissues have been tested with good results. Due to its excellent bonding performance butylcyanoacrylate provides the only means for bonding electrical resistance foil strain gages to biological materials for the time being. This opens new aspects for strain measurements with strain gages in the biomechanical field.

[Experimental studies in animals concerning adhesive orchiopexy (author's transl)].

In an experimental study in rabbits, histoxicity of butyl-2-cyano-acrylic-tissue adhesive for orchiopexy was comapred with the usual suture type fixation of the testis. There was no difference in tissue damage between the two methods in respect to local inflammation, diameters of the tubules and the number of spermatogonic cells. Using several established cell lines, a significant cytotoxicity was seen when dropping butyl-2-cyanoacrylic-adhesive into the tissue cultures. Mutagenic influence of butyl-2-cyanoacrylate should be ruled out in children prior to clinical application. In addition, the high cost of the adhesive should be considered.

Transcatheter embolization of the kidney with butyl-2-cyanoacrylate: experimental and clinical results.

The technique and efficacy of therapeutic catheter embolization of the kidney with butyl-2-cyanoacrylate (Histoacryl) were studied in 80 rabbits (including control groups) and in 10 dogs. A mixture of butyl-2-cyanoacrylate, 50% glucose, and tantalum powder was used for the embolization. Complete and permanent vascular occlusion was found in nearly all cases. The main complication observed was a reflux of embolizing material into the lumbar arteries, which occurred in seven rabbits. Clinically therapeutic embolization was performed in six patients with hypernephroma. The indication for embolization in these patients, as well as in two others with iatrogenic lesions, was pronounced hematuria. Cessation of bleeding was achieved in all cases. For embolization the coaxial catheter technique is recommended; in special cases with extensive arteriovenous shunts, adjunctive balloon occlusion would be advisable.

Diminished genotoxicity of mitomycin C and farmorubicin included in polybutylcyanoacrylate nanoparticles.

The genotoxic effects of mitomycin C (MMC) and farmorubicin (FR) in a free form and included in polybutylcyanoacrylate nanoparticles (PBCN) were studied employing the Salmonella/microsome mutagenicity assay and the micronucleus test in mouse bone marrow as well as in mouse fetal liver. The data obtained clearly indicated that MMC (0.25-2.00 micrograms/plate) was a strong mutagen in S. typhimurium TA102, while the same concentrations of this compound in PBCN were ineffective in inducing his+ revertant mutations in bacterial cells. A similar total suppression of mutagenic activity of FR (1.0-20.0 micrograms/plate) was registered in S. typhimurium TA98 when the drug was included in PBCN. Furthermore, the incorporation of MMC (2.0 or 4.0 mg/kg, i.p.) into PBCN strongly diminished or even abolished its clastogenic activity in the bone marrow of virgin and pregnant mice as well as in mouse fetal liver, respectively. In addition, a lack of genotoxic effect of PBCN only was also established. The toxic activity of MMC in mouse bone marrow was significantly reduced or completely abolished after its inclusion in PBCN. A conclusion might be drawn that the genotoxic activity of some antitumor drugs might be markedly diminished or even abolished after their incorporation in PBCN.

3-Methoxybutylcyanoacrylate: evaluation of biocompatibility and bioresorption.

The biocompatibility and bioresorption of 3-methoxybutylcyanoacrylate (MBCA) was evaluated in vivo using female Wistar albino rats. MBCA was found to elicit slight to moderate tissue reaction similar to isobutylcyanoacrylate (iBCA) which has been sold commercially as a surgical adhesive (Bucrylate, Ethicon). MBCA was judged less reactive to tissue than ethylcyanoacrylate (ECA). The MBCA implants in rat gluteal muscles also resorbed within approx. 16 wk while iBCA implants remained essentially unchanged at 36 wk in vivo. In vitro resorption in phosphate buffer (pH 7.2) at 37 degrees C showed the same trend. The MBCA performed similarly to iBCA as a haemostat on excised rat livers and as an adhesive on rat skin incisions and had comparable adhesive bond strength.

The role of an adhesive (Histoacryl) in tendon repair.

Two in vitro experiments have been done comparing various techniques of tendon repair. These include repairs using an adhesive, 2-butyl cyanoacrylate (Histoacryl), and repairs by the modified Kessler technique, using 4/0 braided polyester and 4/0 stainless steel. Mechanical analysis has shown that by replacing the circumferential suture of a modified Kessler tendon repair with Histoacryl, the tensile strength of the repair and the force needed to produce a gap can be increased by 30% to 40%. This improvement was statistically significant (P < 0.05).

Enbucrilate

[Catheter embolisation of metastasising renal carcinomas using butyle-2-cyano-acrylate (author's trabsl)].

Embolisation with butyle-2-cyano-acrylate was carried out in four inoperable renal carcinomas. By mixing this tissue adhesive with 50% glucose it remains suitable for embolisation for eight to ten minutes. Contact with blood produces the desired effect of immediate and permanent vascular occlusion. Since the material adheres to the vessel wall, it is not carried to any other vascular areas. No complications due to this material have been observed.

Adenocarcinoma

[Kidney damage during translumbar aortography treated by selective catheter embolisation (author's transl)].

During a low translumbar aortogram, a kidney was damaged leading to uncontrollable haematuria. The damaged segmental artery was selectively occluded by the injection of butyl-2-cyanoacrylate through a catheter. Bleeding stopped immediately, thereby avoiding a nephrectomy which had been planned. The complications and potential hazards of therapeutic embolisation are discussed.

Aortography

Histoacryl closure of eyelid skin grafts.

Histoacryl is virtually an ideal tissue glue. It has found application in corneal surgery, oculoplastic surgery, as well as many other surgical fields. A clinical series of 18 patients requiring eyelid skin grafts is presented. Each of the patients had skin grafts placed with a combination of sutures and Histoacryl. No complications were encountered. The postoperative course and results were identical to the authors' experience with skin grafts closed with suture alone. The advantages of Histoacryl skin closure are discussed. Other uses of Histoacryl in oculoplastic surgery are also reviewed.

Interfacial properties of polymethyl alpha-cyanoacrylate and polybutyl alpha-cyanoacrylate.

Several physical properties of two polyalkyl alpha-cyanoacrylates relevant to their use in pharmaceutical dosage forms have been investigated. Formation of polymer films at several oil-water interfaces reveals films of diverse morphology. The retardation of solute transfer across an oil-water interface caused by the presence of polymer films formed in situ from the monomers has revealed that the methyl derivative forms the more effective barrier to the test solute, gentian violet. The emulsion-stabilizing properties of the methyl polymer have been studied, and the film-forming properties of the monomers spread from benzene onto water surfaces have been examined using a surface balance.

Cyanoacrylates

Improvement of ocular penetration of amikacin sulphate by association to poly(butylcyanoacrylate) nanoparticles.

The main objective of this paper was to investigate the ability of polycyanoacrylate nanoparticles to improve the corneal penetration of hydrophilic drugs. Three different nanoparticle formulations were prepared by changing the nature of the stabilizer agent (Dextran 70000, Synperonic F 68 and sodium lauryl sulphate). The significant influence of the stabilizer type on the particle size, electrophoretic mobility and on the drug loading efficiency was proved. Moreover, the ocular disposition of amikacin was affected by its association to nanoparticles, displaying the most interesting results when Dextran 70000 was employed for preparation of nanoparticles. The increase of the amikacin concentration in cornea and aqueous humour was statistically significant for this nanoparticle formulation with respect to the other formulations and the control solution. The in-vitro release profiles obtained using a dialysis system were similar for all the nanoparticle formulations and for the control solution, indicating that drug molecules are desorbed from the nanoparticles quickly enough to maintain the equilibrium concentration in the dialysis system.

Histoacryl tissue adhesive in some types of retinal detachment surgery.

Preserved human scleral graft and histoacryl-blue tissue adhesive were used in four cases of retinal detachment surgery to obtain scleral buckling effect and to protect staphylomatous or necrotic scleral areas. The use of histoacryl produced a strong and resistant adhesion between the host and the preserved scleral patch. The postoperative inflammatory reaction was mild and disappeared within one week.

Enbucrilate

Transrenal ureteral embolization.

Transrenal closure of the ureter with butyl-2-cyanoacrylate using adjuvant balloon catheter occlusion was successfully performed for the palliative management of inoperable vesicovaginal and vesicosacral fistulas in 3 patients. Unilateral percutaneous nephrostomy provided supravesical urinary diversion as well as access for permanent embolization of the ureter. The contralateral kidney was nonfunctional in 2 patients. In the third, in addition to unilateral ureteral occlusion, the contralateral kidney was defunctionalized by embolization of the renal artery ("radiological nephrectomy") to avoid the need for bilateral nephrostomy drainage.
