Lanthanum inhibition of Vibrio cholerae and Escherichia coli enterotoxin-induced enterosorption and its effects on intestinal mucosa cyclic adenosine 3',5'-monophosphate and cyclic guanosine 3',5'-monophosphate levels.

Several trivalent cations, including lanthanum (La3+), inhibited the secretion (enterosorption) induced by the enterotoxins of Vibrio cholerae and Escherichia coli in the rabbit ileum in vivo. High concentrations (greater than 10 mM) of La3+ were required to inhibit cholera enterotoxin (CE)-induced enterosorption, probably because of the adsorption of the La3+ often potentiated the CE-induced enterosorption. If luminal La3+ exposure followed CE exposure, some recovery of the enterosorptive response was observed. The longer the lag between the CE exposure and the La3+ exposure, the greater was the recovery of the enterosorptive response. Lanthanum inhibited HCO3- secretion more than Cl- secretion. By altering the luminal fluid pH at the time of La3+ exposure, it was found that La3+ was adsorbed to negatively charged luminal sites, having an apparent pK between 2.5 and 3.0. Although La3+ antagonized the enterosorptive response to CE, it mimicked rather than antagonized the cyclic adenosine 3',5'-monophosphate elevation and cyclic guanosine 3',5'-monophosphate depression induced by the toxin. It is therefore concluded that the La3+ inhibition of the CE-induced enterosorption must have occurred at a site following the generation of the cyclic nucleotides. Cholera enterotoxin caused complex time-dependent changes in the mucosal cyclic adenosine 3',5'-monophosphate and cyclic guanosine 3',5'-monophosphate levels, as revealed by studying tissue cyclic adenosine 3',5'-monophosphate/cyclic guanosine 3',5'-monophosphate ratios. The possible roles these two cyclic nucleotides may play in the pathogenesis of the cholera diarrhea are discussed.

[The use of enterosorption in the combined therapy of patients with urolithiasis complicated by chronic kidney failure].

The results of conservative treatment of 51 patients with urolithiasis complicated by chronic renal failure in the intermittent stage have been analyzed. There were two groups of patients. The first one included 36 patients receiving enterosorption in addition to routine multimodality treatment. The second group was made up of 15 patients who did not undergo enterosorption which consisted in oral application of SKH-1K coal in a dose of 10 to 15 g 3 times a day 1.5 h after meal for 10 days. The treatment efficacy was judged from the patients' general health status, central hemodynamics (cardiac index and peripheral vascular resistance), the level of azotemia, and the content of medium-sized molecules. As shown by the data obtained, the multimodality treatment coupled with enterosorption produced an appreciable improvement of the patients' health status, reduction of the signs of intoxication, while the level of medium-sized molecules and creatinine reached the control. The hypokinetic circulatory syndrome seen before the treatment disappeared. At the same time the second group patients retained the high level of azotemia, whereas the content of medium-sized molecules remained increased. Besides, they demonstrated the hypokinetic circulatory syndrome. It is thus recommended that enterosorption be applied to the treatment of patients with chronic renal failure.

[Experimental and clinical validation of using enterosorption in mechanical jaundice].

Experiments in 119 rats with the model of mechanical jaundice have shown the efficiency of enterosorption with polyphepan. The development of the pathological process was found to improve, lethality of the experimental animals to decrease. Favourable changes of the laboratory indices characteristic of endotoxemia were noted. The mechanism of medical action of enterosorption is realized by means of intensification of transfer of masses of metabolites and toxins through the intestine wall and their excretion through the gastrointestinal tract. One of important mechanisms of the medical action of enterosorption is the stimulation of intestinal peristalsis which is confirmed by data of the dynamics of bioelectrical activity. Enterosorption performed in 60 patients with mechanical jaundice was followed by a more rapid improvement of the general state, decrease of the indices characteristic of endotoxicosis as compared with 60 patients of the control group. The frequency of postoperative complications and lethality were reliably lower.

Enterosorption in prolonging old animal lifespan.

The effect of repeated courses of enterosorption upon the mean and maximal lifespan and some functional and metabolic indices was determined in 28-month-old Wistar rats. Significant increase of mean and maximal lifespan of old rats was noted at certain regimens of enterosorption. The experimental animals demonstrated less marked age-related structural and ultrastructural changes in the liver, kidneys, myocardium, intestines, pancreas, as compared with control animals. Enterosorption leads to a reduction of pentobarbital-induced sleep, decrease of content of cytochrome P-450, blood cholesterol and triglycerides, cardiac and cerebral tissue cholesterol, total lipids, liver cholesterol and triglycerides. Enterosorption was found to increase the RNA and protein biosynthesis in the liver, kidneys and adrenals of old animals.

[The joint use of plasmosorption and enterosorption in the combined treatment of patients in the acute period of a myocardial infarct].

Plasmosorption and enterosorption were assigned to 35 acute myocardial infarction (AMI) patients. Plasmosorption was performed within an hour since AMI onset followed by a 5-day course of enterosorption (enterosorbent YO-A in a dose 30 g/day). The above combination produced a positive effect on hemodynamics and blood rheology resultant from a reduction in hematocrit value, plasma levels of fibrinogen and its derivatives, plasma viscosity and platelet aggregation. Ibustrin (200 mg) combined with heparin (100000 U) introduced into extracorporeal circulation effectively prevents thrombogenesis. AMI patients treatment with enterosorption plus plasmosorption enhances the response to nitroglycerin, reduces the rate of complications and lethal outcomes.

[Use of enterosorption in biliary tract cancers complicated with mechanical jaundice].

Enterosorption using polyphepan in experimental mechanical jaundice was followed by improvement of the course of the disease, lower mortality and improvement in endotoxemia indices. The therapeutic effect of enterosorption is due to enhanced transfer of metabolites and toxins through the intestinal wall and subsequent excretion via the gastrointestinal tract. Enterosorption performed in 60 patients with malignancies of the bile tracts and mechanical jaundice was followed by a more rapid improvement in general condition and reduction in the indices of endotoxemia, as compared with 60 controls. Postoperative complication frequency and mortality showed a significant decrease.

Association of Escherichia coli with the small intestinal epithelium. II. Variations in association index and the relationship between association index and enterosorption in pigs.

The association between small intestinal epithelium and enteropathogenic Escherichia coli (EEC) was studied in ligated intestinal loops of pigs and rabbits. The association indexes (degree of association) for each of two porcine EEC strains varied widely among pigs and independently of each other. Significant litter-to-litter variations in association indexes among colostrum-deprived newborn pigs were interpreted to be the result of congenital resistance to association with specific EEC in some pigs. Since enterosorption occurred in loops with low association indexes, it was not necessary for EEC to establish a high association index for them to cause enterosorption in ligated intestinal loops. Two strains of EEC which are enteropathogenic for humans caused enterosorption in ligated loops in pigs 3 weeks old or less but not in 6-week-old pigs.

Effect of enterosorption on animal lifespan.

Experiments were performed on Wistar male rats, starting from the 28th month of age. The effect of dietary sorbent (non coated nitrogen-containing carbon administered as 10 day courses at 1 month intervals in dosage of 10 ml/kg) on lifespan and a number of biological indices were studied. Enterosorption resulted in the increase of mean and maximal lifespan by 43 and 34% respectively. Analysis of the effect of enterosorption on activity of microsomal enzymes, intensity of total RNA and protein biosynthesis, lipid metabolism, formation of free radicals etc. showed that it produced a positive influence on the functional state of the studied systems and increased the organism's adaptive capacities. Enterosorption was found to delay the rate of onset of age-related structural changes in the organs and tissues.

[Effect of enterosorption on EPR signals in the liver of rats of different ages].

Liver EPR signals were studied in adult (8 months) and old (24 months) Wistar male rats after the 30 days course of enterosorption carried out daily. The signals were analyzed at the values of g = 1.94, 2.00, 2.25, 2.035, 2.16, 2.10 and 1.97. Age-dependent differences were found in liver EPR signals of intact animals, i. e. the intensity of signals of g = 1.97, 2.00, 1.97 and 2.25 was decreased in old animals. In adult animals enterosorption caused a decrease of signals of g = 2.25, 1.94, 2.00, 2.16 and 2.10. In old animals the treatment led to a dissimilar effect: the intensity of signals was decreased at the ranges of g = 2.25, distinctly increased at g = 1.97 and unaltered at the other EPR signal values studied. Geroprotective action of enterosorption is discussed considering the decrease in enzymatic activity of electron transport systems due to the treatment studied.

[The efficacy of enterosorption in bacterial intestinal infections].

The article studies clinic manifestations after the prescription of polyphepanum, SKN-1P (based on recovered carbon) and enterodesum in 35 patients with acute dysentery, 104 patients with acute gastroenterocolitis, and 123 patients with typhoid. The first two drugs turned out to be highly effective for treatment of these infections. Enterosorption produced an insignificant positive effect in cases of typhoid. An early prescription of enterosorbents in enhanced doses could rapidly stop the basic proliferation of acute intestinal diseases. It is reasonable to start enterosorption even at the pre-hospital stage.

[Enterosorption in complex therapy of bronchial asthma].

Generation of active oxygen forms in whole blood was studied in 84 patients with bronchial asthma (BA) by luminol-enhanced chemiluminescence (Chl) registration. Chl intensity depended on inflammatory process expression; groups of patients with high and low blood Chl (the 1st and the 2nd group, respectively) were distinguished. Enterosorption was used in complex therapy of 8 patients of the 1st group and 15 patients of the 2nd group. Indices of free radical oxidation and content of average molecular oligopeptides in blood serum normalized in the patients who took enterosorbent; the remission in these cases was more complete. The results of the study suggest that enterosorption increases effectiveness of treatment in patients with BA.

[Enterosorption and nutritious support with pectin-containing preparation in the treatment of stress immunodeficiency in peritonitis].

Fifty-eight patients aged from 18 to 65 years with peritonitis of different etiology in toxic and terminal phases were treated. Nasoenteral intubation, enterosorption and early nutritious support with pectin-containing preparation (PCP) were carried out in all the patients just after surgery with standard course of 5 days. Baseline data was compared with that after PCP-supported treatment. Powder products of red beet were used as PCP. Immunodeficiency in peritonitis is characterized by imbalance of stress-realizing and stress-limiting mechanisms of immunocompetent cells. Enterosorption and early nutritious support with PCP in peritonitis decrease the level of plasmic cortisol and enhance stress-limiting reception that reduces a harmful effect of cortisol and reestablishes immunity.

[The tolerance of diabetic patients for physical loading before and after enterosorption treatment].

Use of enterosorption in 58 patients with diabetes mellitus of different severity grades allowed to establish that a course of treatment with SKN sorbents resulted in an increase of the tolerance to physical load, improvement of myocardial hemodynamics after veloergometric loads. This was evidenced by an increased volume of work performed on the veloergometer, reduction of the index of energy expenditures of the heart coefficient of expenditures of the myocardial reserves. The used enterosorbents produced a hypotensive effect. Enterosorption furthers increase of tolerance to physical load in a relatively short time period--10 days. During this time traditional treatment including sugar-reducing agents, angio- and hematoprotectors did not result in essential changes of hemodynamic indices.

[Enterosorption in the combined treatment of patients with multiple sclerosis].

Fifty-nine patients exhibiting an exacerbation of multiple sclerosis were examined for efficacy of enterosorption with the fibrous coal sorbent vaulen. The solvent powder was administered 3 times a day in a dose of 50-60 mg/kg for 20 days between meals. The relief of neurological symptoms started on treatment days 3-4 with subsequent attenuation within the course of treatment. This agreed with discontinuation of liquor hypertension and recovery of normal immunological spectra of serum parameters. The method of enterosorption is simple, safe and readily arrests the disease exacerbation.

[The use of enterosorption in pulmonary tuberculosis patients with side effects from antibacterial preparations].

Data are reported on 66 tubercular patients. In 59 of them enterosorption along with traditional treatment were used to control side-effects of antibacterial treatment, correction of liver function, reduction of tubercular intoxication. The obtained results indicate efficiency of enterosorption that removes during a short time period the side-effect allowing to treat efficiently the main disease.

[Enterosorption and therapeutic plasmapheresis in the intensive therapy of patients with acute leukemia and of persons who were victims of the accident at the Chernobyl Atomic Electric Power Station].

The paper reports on the preliminary experience of treatment of those patients having been the victims to the reactor accident at the Chernobyl NPP as well as of those with acute and chronic leukoses, having received courses of modern cytostatic therapy. The patients were exposed to therapeutic plasmapheresis and enterosorption as part of a complex conventional treatment programme. The conclusion the authors have come to is as follows: enterosorption strikingly reduces manifestations of "cytostatic disease"; the use of plasmapheresis in those persons having been previously exposed to small dose chronic radiation, makes for improvement of clinical and laboratory values but further studies are warranted to determine indications for therapeutic plasmapheresis.

[Enterosorption as a method for correction of biogenic amine levels in experimental bronchospasm].

The mechanism of enterosorption positive influence on the course of bronchospastic condition was studied on the model of allergic bronchospasm. The using of the sorbent (polyphepan) during the period of sensitization of rats prevented the beginning of bronchospasm or reduced its extent. The results of investigation proved that a decrease the of biogenic amines concentration in biological liquids and tissues depending on their transport and sorption in intestine is one of the main mechanisms of enterosorption.
