Spontaneous fracture of hypertrophied genial tubercles.

A case of spontaneous fracture of hypertrophied genial tubercles is reported. This is an uncommon event with only six previously reported cases. This entity is usually associated with severe edentulous bone resorption, a condition related to the pathogenesis of this fracture. Clinical complaints include painful swelling of the floor of the mouth and impaired tongue function. If left untreated, mucosal inflammatory changes may appear and a biopsy should be performed. Total recovery is obtained after removal of the fractured fragments.

Spontaneous fractures of the olecranon process in rheumatoid arthritis.

Spontaneous fractures through the olecranon process occurred in 3 patients with long-standing rheumatoid arthritis. In each case the elbow was severely damaged, particularly in the midportion of the trochlear notch, which was thinned either by erosion or cyst formation. The thin trichlear notch is the weak pivot point of a lever (the olecranon process) which pivots on the distal humerus. Excess stress on the lever, by the action of the triceps on the olecranon process, will produce a fracture at the weakest point: the midtrochlear notch.

Spontaneous fracture of the odontoid process in rheumatoid arthritis.

Six cases of spontaneous fracture of the odontoid process in rheumatoid arthritis are presented. Fifty-one patients with atlantoaxial subluxation in rheumatoid arthritis underwent surgery between 1981 and 1990. This included six patients (in 10%) who had subluxation accompanied by fracture of the odontoid without apparent trauma. The mean patient age was 58 years and all had a long history of rheumatoid arthritis. No trauma was considered to be the cause of the fracture. This is a fracture caused by erosion and osteoporosis of the odontoid process due to rheumatoid synovitis, aging and steroid therapy. In addition, another cause is a dynamic load produced from the instability accompanying atlantoaxial subluxation working on the odontoid in cervical extension. It is important remember that the odontoid process is susceptible to spontaneous fracture.

Spontaneous fractures in the brain-crippled, bedridden patient.

This report deals with the treatment of 31 spontaneous fractures which occurred in 50 institutionalized patients who were bedridden primarily because of severe cerebral palsy associated with brain injury. More than one-half of the group sustained a spontaneous fracture. Satisfactory healing of all fractures with a minimum of complications occurred without any immobilization or realignment. In 4 patients with delayed compound wounds, the treatment consisted of resection of the protruding portion of the bone after allowing the bony protrusion to wall itself off.

[Spontaneous fractures and tumors (author's transl)].

During childhood, pathologic fractures are found in systemic diseases of the skeleton (e.g. osteogenesis imperfecta or rachitis), but especially when bone tumors or tumorlike changes are present. Conservative measures such as continuous traction and application of a plaster cast lead to healing of the fracture only in a limited number of cases. Treatment of benign and potentially malignant and tumors and of tumorlike growths by excochleation and by resection and subsequent filling of the defect with autologous spongiosa and in some cases metallic fixation respectively has proved useful. The surgical treatment of malignant tumors depends on the king of tumor and its localization.

Cushing's disease with pituitary apoplexy leading to hypopituitarism, empty sella, and spontaneous fracture of the dorsum sellae. Case report.

A patient with a pituitary adenoma secreting adrenocorticotropin hormone manifested panhypopituitarism after an episode of pituitary apoplexy. The previously elevated urinary levels of 17-ketogenic steroids dropped sharply, and plasma cortisol became undetectable. The apoplexy also resulted in a partially empty sella on which the dorsum sellae collapsed. Recurrent Cushing's disease developed and was cured by transsphenoidal resection of a microadenoma.

["Spontaneous" fracture of femur following damage to the iliotibial tract].

Gravity apart, muscles act on the skeleton in balancing the usually excentric weight. This raises the load of the skeletal element but reduces the flexion strain. Loss of the traction-effect of a muscle or its aponeurosis produces overloading of the bone involved and may lead to fracture. This theortetical concept is supported by a clinical case of fracture of the femur which occurred a long time after the iliotibial tract had been eliminated.

Biomechanical Phenomena

Spontaneous fracture of the navicular bone in the horse.

After a short review of previous literature about fractures of the navicular bone in horses, the symptoms and the pathological-anatomical changes of the condition are described. The most important clinical symptom is acute severe lameness without significant swellings, but with pronounced pain reaction to rotation of the coffin joint. After rest the lameness is reduced considerably, but without treatment it can persist for several years. Adherences between the deep flexor tendon and the site of the fracture and eventual damage to the coffin joint are considered to be the cause of the persisting lameness. Three case reports are given where the horses were shoed with full bar shoes with clips and high calks, and were given two months rest in a box. After that the rehabilitation was started with increasing load on the leg in question in spite of some initial lameness. The three horses all regained their full working capacity after ca. 6 months' treatment. There is no formation of callus at the site of the fracture, but only a firm formation of fibrous tissue which does not bother the horse unless the fragments are too much dislocated giving rise to a greater destruction of the coffin joint. Radiographically the fracture line persists for the rest of the horse's life.

Spontaneous fractures of the humerus during pitching. A series of 12 cases.

We report the recent occurrence of spontaneous humeral shaft fractures in 12 pitchers. The subjects were interviewed over the telephone using a standard questionnaire. Radiographs and medical records were solicited from their physicians. Their average age was 36 years and they had pitched an average of 11.4 years with an average layoff of 14 years (range, 7 to 24). Mean time between games pitched was 21 days (range, 3 to 56). The average number of pitches before the fracture occurred was 38 (range, 10 to 100). Pain was experienced at some point before the fracture in 75% of the pitchers and 75% of the fractures were spiral. These fractures were most probably spontaneous fractures brought on by accumulated fatigue damage. The period of buildup after a prolonged period of layoff was probably insufficient time for proper bone remodeling to occur in these men. While a large percentage of these men had pain before their injury, suggesting a predisposing stress fracture, there is no doubt that their fractures could still be caused by a sudden torsional load without the presence of a stress fracture.

[Spontaneous fracture of an implanted anterior chamber lens].

This report deals with an 83-year-old patient who presented with visual deterioration. The patient had had an intracapsulary extraction with anterior chamber lens implantation. The break in the anterior chamber lens happened without any history of traumatic accident and led to dislocation of the lens and corneal decompensation. Scanning electron microscopy of the break area after explantation of the fragments revealed changes in the break zone that were characteristic of alternating tensile and compressive stress. Localization of the break at the transition of the optical part and the haptic where there is the greatest stress in the material points to insufficient strengthening in this critical part of the intraocular lens.
