1. Josamycin concentrations in human serum and dental granuloma after a single oral administration of josamycin (600 mg) were assayed by an agar diffusion (paper disc) method. 2. The mean peak josamycin concentrations in serum and dental granuloma occurred at an identical time, approximately 90 min, and were 0.88 micrograms/ml and 1.61 micrograms/g, respectively. 3. The mean concentration ratio of dental granuloma to serum at the peak time was 2.24. 4. Josamycin concentration in dental granuloma at the peak time exceeded MIC80 for clinically isolated strains of Streptococcus group A, Peptostreptococcus spp., and Bacteroides spp.
Ampicillin concentrations in human serum and dental granulomas of 31 patients were determined after a single oral dose of talampicillin (equivalent to 500 mg of ampicillin) was administered to each. The specimens were taken at 1.5, 2.0, 2.5, 3.0, and 3.5 h after the administration of talampicillin. The mean peak ampicillin concentrations in serum and dental granulomas occurred at identical times, 2.5 h, and were 8.29 micrograms/ml (range, 1.81 to 13.20 micrograms/ml) and 2.94 micrograms/g (range, 1.14 to 7.16 micrograms/g), respectively. The mean dental granuloma/serum ampicillin concentration ratio at the peak time (2.5 h) was 0.42 (range, 0.29 to 0.56). Ampicillin concentrations in dental granulomas exceeded most of the MICs for the bacteria commonly isolated from odontogenic infection.

OBJECTIVE: The purpose of this study was to determine the immunolocalization of the interleukin (IL) 4, IL-6, and lymphotoxin (LT) alpha in dental granulomas and correlate their expression with the intensity of the inflammatory infiltrate. Study design Fifteen paraffin specimens of dental granulomas were selected, and the streptavidin-biotin complex stain was used to detect IL-4, IL-6, and LT-alpha. RESULTS: Our results revealed a significant statistical correlation between the intensity of inflammatory infiltrate and the percentage of mononuclear cells positive for IL-4. Moreover, we observed a statistically significant correlation between the frequency of cells expressing IL-6 and LT-alpha. CONCLUSION: These data suggest that the predominance of a helper T cell subtype 2 response in dental granulomas is correlated with the exacerbation of the inflammatory reaction and its evolution. Moreover, a correlation between the frequency of IL-6-positive and LT-alpha-positive cells suggests that the synergistic activities of these 2 cytokines may be involved in the pathogenesis of this inflammatory condition.
Three Torulopsis have been isolated from two hundred or so dental granulomas. Torulopsis glabrata has been observed in cases of ophthalmopathy, infections of the oral cavity, lung infections, endocarditis and septicaemia. Apical lesions due to Torulopsis glabrata are not typical of mycosis but are generic of dental granulomas. The histological granulomatous character prevails. Experimental infections have been obtained from this fungus. Human infections by Torulopsis glabrata have been described. These infections are induced particularly in subjects with their immunity systems depressed by prolonged cortisone therapy. Their systematic position and morphology in optical microscopy have been examined in accordance with current views.
Thirteen Cladosporium have been isolated from 221 dental granulomas. Cases have been observed of cerebral lesions both in man and cats. Also various types of lesions, encapsulated abscesses, etc. to be classified under the group of pheoifomycoses. Cutaneous allergies and cases of keratitis and chromoblastomycosis have been observed, too Cladosporium cladosporioides is very resistant to mercuric and phenolic compounds and radiations. The systematic position and morphology of Cladosporium have been fully examined in accordance to the latest scientific views.
Three Aspergillaceae have been isolated from a hundred or so dental granulomas. Their systematic position and morphology in optical microscopy have been fully examined as for the latest scientific opinion and in relation to histopathological studies.
Exophiala jeanselmei (Lang) De Hoog was isolated from a granuloma of a dental decay. Its systematic position and morphology in optical microscopy are widely discussed in accordance with current views.
