We used the standard latex fixation test to screen 71 patients with bladder tumors and appropriate controls for the rheumatoid factor. The rheumatoid factor was present in transitional and squamous cell carcinoma of the bladder. Positivity was correlated with staging and histological grading of the tumor. In control subjects it was present in squamous cell carcinoma of the penis and anal canal. The possibility of the circulating immune complex with tumor antigen and its antibody has been discussed.

Fifty-five patients with extramammary Paget's disease were the source of material for this study. Step-sections were done through most of the specimens. Clinical information, including follow-up, was obtained on 45 of the 55 patients. Extramammary Paget's disease could be divided histologically according to where Paget cells were found, namely: 1) wholly within the epidermis and the epithelial structures of adnexa, and the dermis; 3) within the epidermis, the epithelial structures of adnexa, and contiguous epithelia of other organs such as the genitourinary and gastrointestinal tracts. Our conclusions are that extramammary Paget's disease is more than one disease and in most instances begins in the epidermis as an adenocardinoma and extends from there into contiguous epithelium of hair follicles and eccrine sweat ducts. Uncommonly, Paget cells extend from the epidermis into the dermis and from there may metastasize. Rarely, extramammary Paget's disease results from direct extension into the skin of an adenocarcinoma in a contiguous organ such as the genitourinary or gastrointestinal tract.

The pharmaceutical industry is not supplying the penicillin preparations that are required for the treatment of syphilis. For those in whom penicillin hypersensitivity is suspected there is a need for a safe injectable alternative that is effective if given once daily or, preferably, at two- or three-day intervals. Existing treatments for chancroid, lymphogranuloma venereum, and granuloma inguinale are described, but even collectively there are few cases and treatments for other sexually transmitted diseases merit priority. Treatments for scabies and pediculosis pubis, although not perfect, are reasonable. There is a need for better local treatment for condylomata acuminata and systemic immunological methods, including those that increase cell-mediated immunity, deserve attention. The same is true for molluscum contagiosum. There is an urgent need for an effective, safe treatment of herpes genitalis that is able to eradicate the virus from the host. If it is proved that the herpes virus is responsible for carcinoma of the cervix this could then be the most serious sexually transmitted disease as in many countries such carcinomas are responsible for approximate seven times more deaths in women than is syphilis in men and women together. The limitations of prophylactic methods in preventing all possibility of infection with one or more of the sexually transmitted diseases are discussed.

Between 1960 and 1969, 7150 major operations have been performed in the gynecological departements of West-Berlin on female patients of 60 years of age or more. Of 6658 cases the case-history was at our disposal. In contrast to reports in literature, the largest group was that of 3111 malignant tumors (46.7%). The postoperative mortality of the entire group was 7.7%, in cases of malignant tumors 12.1%. Discussion on surgical methods for the most frequent gynecological malignomas, and on possibilities to reduce the postoperative mortality, as well as on some aspects pertaining to the gynecological geroprophylaxis.
Lymphocyte transformation under phytohaemegglutinin (PHA), spontaneous rosette formation and immunoglobulin concentration were measured in women with genital or breast carcinoma. It was found that (1) DNA synthesis of lymphocytes under PHA was normal in women with stage I and II cervical carcinoma, slightly decreased in stage III; (2) DNA synthesis of lymphocytes under PHA was slightly but significantly decreased in untreated patients with breast carcinoma; (3) after radiotherapy, both for cervical and body of the uterus carcinoma, there was a significant decrease of PHA-stimulated DNA synthesis of lymphocytes compared with healthy females and with patients at the same stage of tumour spread not treated by radiotherapy; (4) there was no significant difference in spontaneous rosette formation between radiated patients with cervical or breast carcinoma, on the one hand, and healthy controls and non-radiated patients, on the other; (5) IgG levels were slightly but significantly decreased in patients with ovarian carcinoma while in cervical, breast and body of the uterus carcinoma they were normal, IgA and IgM levels were significantly raised.
The determination of carcinoembryonic antigen (CEA) in plasma has been of much interest currently concomitant with the search for an immunologic diagnosis test. Recent reports have shed some doubt on the specificity of carcinoembryonic antigens for gastrointestinal tract malignancies. This report details the plasma CEA values in 341 patients with varying gynecologic malignancies. These studies have demonstrated that plasma CEA is elevated in close to 50 per cent of patients with invasive gynecologic cancer. The incidence of positive values is appreciably higher in the advanced stages of disease. Of particular interest was that 84 per cent (21 of 25) of the patients with recurrent squamous-cell carcinoma of the cervix had a positive CEA value. Similar results were found in patients with cancer of the vulva, ovary, and endometrium.

Levels of misonidazole in human tumours, normal tissues and blood have been measured in patients given a 1g oral dose of drug before surgery or biopsy. The results show that 50--70% of the blood level was found in a wide range of tumours and that similar levels were found in adjacent normal tissues. Good penetration of drug was achieved within tumours, and up to 90--100% of the blood level was found in the necrotic cyst fluid at the centre of some tumours. CSF studies showed free diffusion into the CNS, which was confirmed by finding 50--70% of the blood level within brain tumours. A delay of passage of drugs into the CSF was noted, which was not found for drug diffusion into bile and saliva.
