In 13 patients with hydrarthrosis of the knee, samples of synovial fluid and blood were drawn at regular intervals, following a single oral dose of 600 mg proquazone. The concentrations of the unchanged drug were measured fluorimetrically and those of its three principal, active metabolites by high pressure liquid chromatography. The absorption and distribution of proquazone and its metabolites were rapid. Measurable--in some cases considerable--concentrations were to be found in both the synovial fluid and serum as early as 30 minutes after intake. High concentrations were still present up to 7 hours after intake. Considerable variations were observed, both between patients and over time for each patient, so that only qualitative rather than quantitative kinetic conclusions could be drawn from the results. The concentrations were generally lower in the synovial fluid than in the serum, which may be ascribed to the drug's high protein binding.

Gray-scale ultrasound images of the shoulders of 6 rhesus monkeys were obtained before and after fluid instillation to assess the ability of ultrasound to diagnose joint disorders. The normal bony landmarks and muscular structures were shown. After the fluid was introduced, fluid collections were identified in the axillary pouch and subscapular bursa. Ultrasound is a promising new method of detecting intra-articular effusions of the shoulder.
The authors present a dynamic observation of a patient with advanced rheumatic polyarthritis that developed considerable hydrathrosis of the left knee and popliteal deep cyst of the Backer type. The general treatment was completed with radioisotopic synoviorthesis with Au198 that administered directly in the articulation and in the cavity of the cyst, resulting in the rupture of its wall. The symptomatology of the dissecting popliteal cyst was similar to that of thrombophlebitis. Posterior incision and drainage of the cyst collection have resulted in the disappearance of the local phenomena.

The authors report the pathological and clinical findings in a case of histiocytosis X in a child. A rapidly progressive neurological syndrome of brain stem damage, occurred 5 years after the onset. Blockage of the 3rd ventricle led to surgical exploration of the posterior fossa. Lesions of the brain stem and cerebellum, in addition to the granuloma of the hypothalamus, are described. The neurological symptoms in histiocytosis X often suggest a space-occupying lesion but usually occur in the later prolonged forms in the adult. Association of thesaurismosis due to P.V.P., of therapeutic origin, produced numerous clinical skin and joint signs.
