[Stereotaxic brain operations in erethitic imbecility (author's transl)].

Long-term results of stereotaxic operations in 23 patients with severe erethitic (aggressive-destructive) imbecility were evaluated. Target areas in the thalamus, electrocoagulated in separate bilateral operations, were the dorsomedial nucleus and the internal medullary lamina. Definite tranquilization was achieved in 50% and acquiescence in 27% of patients, resulting in a positive effect on more than 75% of patients. The postoperative course showed a constant level of quiet behavior in only 25% of patients ("static course"), whereas 50% of patients showed an oscillating course leading to subsequent improvement and later stabilization ("undulant course"). The final outcome was improvement and later stabilization ("undulant course"). The final outcome was not improved in 25% of patients. Additional therapeutic measures involving adaptation of psychopharmacotherapy improvements in surroundings, and play and working therapy were occasionally effective (Fig 3). In the pneumoencephalograms of these patients a new distinctive variant was encountered that awaits confirmation: typically, a definite increase in thalamic height, a decrease in ventricular width, and a relative diminution of base line length was found. Stereotaxic thalamotomy in erethitic imbeciles is a therapeutic method with a 75% long-term effectiveness and should be considered a proven therapeutic alternative.

'It begins with the goose and ends with the goose': medical, legal, and lay understandings of imbecility in Ingram v Wyatt, 1824-1832.

A number of historians have recently suggested that we need to get out of the asylum if we are to fully understand attitudes to insanity in the nineteenth century. Arguing that accounts of the medicalization of madness have ignored the importance of non-medical attitudes to lunacy and idiocy, recent studies have stressed the need to explore family attitudes and responses in more detail. Unfortunately, efforts to escape the asylum have to some extent been hampered by a persistent reliance on institutional records. This institutional dependence is understandable: certificates of insanity, reception orders, case-books, and asylum registers, together with published documents, constitute the major record of historical constructions of the lunatic and idiotic mind. However, there may be more resourceful ways of exploring both medical and non-medical attitudes to idiocy. In this paper, I want to use the records from a contested will case, Ingram v Wyatt, to demonstrate that records from the ecclesiastical courts can provide access to a domain where the definitions and meanings of idiocy and imbecility were routinely considered by lawyers, lay witnesses, judges, and doctors. I shall argue that such cases constitute a fruitful site for excavating lay, professional legal, and medical attitudes to imbecility, for exploring the complex relationship between medical and non-medical understandings of capacity, and for situating those understandings within the context of professional developments in law and medicine and contemporary concerns about inheritance.

[Comparative-age aspect of severe imbecility].

The study covered 3 age groups of patients with marked imbecility: 9-12, 13-15, and 16-18 years of age. The study was conducted with the help of psychological experiments as well as clinical and pedagogical observation. The patients' household skills and abilities to provide self-care were also analyzed. The results obtained are indicative of the positive development of individual functions which, however, is possible only under conditions of constant stimulation and corresponding work. Nevertheless, potential abilities of the patients are poor and they should be brought together in groups in boarding houses to allow them to learn at a level compatible with their intellectual abilities.

[Rehabilitative pedagogic aid for imbecile children with an auditory defect (author's transl)].

Basing on the results of diagnostic activities during eight years in a neuropsychiatric institution the image of the multiple injury imbecility plus auditory defect is introduced. Problems of rehabilitation of children injured in this way are discussed. A pretest-posttest control group procedure is followed by the discussion of the model of a stationary advancement by the help of rehabilitative pedagogy. In the front we consider an atmosphere with gestures and simultaneous speech, which is able to give a chance of communication to the multiple injured child.

[Imbecile children visit geriatric patients. A possibility for clinical sociotherapy].

130 observation minutes of organized visits of imbecile girls in school age to geriatric patients accommodated for a long time (17 chronic schizophreniacs and 8 cerebro-organic patients) were analysed. Singing, ball and party games for children and dancing two by two to recorded music proved as suitable means of communication. This kind of contacts led to a social activation especially by schizophreniacs who had a lack of drive and seemed to be regressive, also caused an increase of drive and self-reliance by formerly timid, reserved girls. The staff was even relieved by the presence of the children, because these activated the geriatric patients. The arrangement can be realized without great effort in big psychiatric hospitals.
