Against the background of the planned statutory Pflegeversicherung (insurance for nursing care) in Germany, a report is presented on experience gained to date with the new Leistungsbestand "Schwerpflegebedürftigkeit" (situation requiring social benefit "urgent need of nursing care") (Para. 53 ff. SGB V). The experience gained so far is of particular interest since, for the first time on a large scale, monetäre Erstattungsleistung (the provision of a monetary entitlement) replaces the Sachleistungsprinzip (benefit in the form of services) that is otherwise applied in the area of health insurance. It is intended that this form of entitlement will form the core of the planned nursing care insurance scheme.
The use of nurse practitioners and other non-physician health care professionals is a cost-effective means of providing health care services. Since NPs, PAs, CNMs and others provide services comparable to physicians, their use in ambulatory care settings can also significantly increase the availability and accessibility to health care services, especially to low-income individuals and those requiring services in areas of physician shortage. While current obstacles such as reimbursement policies, legislative restrictions and prescriptive authority limit the use and effectiveness of non-physician health care providers, more physicians, hospitals, clinics, HMOs and public health care systems will view their use as a means of containing escalating health care costs. To the patient this will mean an increase in the availability of comprehensive health care services and the prevention and early detection of medical problems through wellness care. A discussion is presented regarding the ways in which use of NPs and other non-physician professionals can reduce costs and increase the quality and availability of health care services.
The purpose of this study was to explore the nature of health insurance coverage research universities offer their employees and the extent to which these employers offer options providing for reimbursement of services of independent nurse practitioners. A request for health insurance documents mailed to 77 public research universities resulted in a response rate of 83 per cent. A total of 75 per cent sent usable descriptions of insurance coverage for analysis. Among the respondents, 69 per cent (n = 40) reported offering insurance through Blue Cross/Blue Shield companies. Nursing services reimbursed generally included traditional nursing care such as private duty nursing and home health care. Coverage of nursing services that overlap with traditional medical practice, such as prenatal care and labor and delivery services of nurse midwives, anesthesia services of nurse anesthetists, psychiatric services of nurse specialists, and primary care nursing services of nurse practitioners, was less evident. Findings generally indicated that in spite of the presence of enabling or mandatory state legislation for third-party reimbursement of nursing services, legislation is not being implemented in many states by employers in their purchase of group policies.
Today's rapidly changing health care environment directly affects nurse practitioners. NPs will be practicing in increasingly competitive situations and with mounting pressures for cost-effective services. Ignoring the issues in the health care marketplace can threaten NP survival. This article will analyze the changes in the health care system, relate the economic effects of these changes to nursing practice, and apply a homeostatic model of interprofessional competitive behavior. As NPs gain perspective on the dynamics of competition and the influence of economics, they will be better able to successfully position themselves in the health care marketplace.
