[Observation on therapeutic effect of Jingjin therapy on migraine].

OBJECTIVE: To observe therapeutic effect of Jingjin therapy on migraine and search for an ideal way for non-medical treatment of migraine. METHODS: One hundred cases of migraine were randomly divided into a observation group and a control group, 50 cases in each group. The observation group were treated with Jingjin therapy, and the control group with oral administration of Nimodipine. RESULTS: The total effective rate was 100.0% in the observation group and 70.0% in the control group with a significant difference between the two groups (P < 0.01). The therapeutic effect for attacking times, attacking lasting time, headache index and accompanied symptoms in the observation group was better than that in the control group (P < 0.01). CONCLUSION Jingjin therapy has an obvious therapeutic effect on migraine.

[Re-cognition of the Jinglin theory in the Internal Classic].

OBJECTIVE: To summarize the Jijgiin theory and study the essence of Jingjin. METHODS: Analyze the meaning of the word, anatomical basis, symptoms and treatment of Jingjin. CONCLUSION: The Jingjin theory in the Internal Classic is formed on the basis of 12 division method of the surface, reflecting internal relations among the skeletal muscles and the crux of injury, and summarizing treatment methods of Jingjin diseases; and the meaning of essence of the Jingjin is nerve and muscles.

[Observation on therapeutic effect of aponeurotic system penetration needling on peripheral facial paralysis].

OBJECTIVE: To observe therapeutic effect of aponeurotic system penetration needling on peripheral facial paralysis. METHODS: One hundred and ten cases of peripheral facial paralysis were randomly divided into a Jingjin group (n=68) and a control group (n=42). The therapeutic effects of acute stage, resting stage and sequela stage, and the relation between the facial nerve lesion degree and the therapeutic effect were investigated. RESULTS: The effective rates of the two needling methods were respectively 98.5% and 90.5%, the Jing1in group being better than the control group (P < 0.05); at the acute stage, the therapeutic effect of acupuncture was obvious, and the therapeutic effect at the sequela stage and for the patient of nerve faulty type in the Jingjin group were better than that of the control group (P < 0.05). CONCLUSION: Acupuncture and moxibustion has definite therapeutic effect on facial paralysis at the acute stage and in the patient of nerve active type, and aponeurotic system penetration needling can be used for the patient of facial paralysis at the sequela stage or with nerve faulty type.

Safety and efficacy of Meridian sinew tuina (MST) for post-surgical upper limb lymphedema: a systematic review and meta-analysis.

BACKGROUND: Complex Decongestive Therapy (CDT) is the non-operative standard for breast cancer-related lymphedema (BCRL), but many patients experience persistent subcutaneous stiffness, pain, and restricted mobility. This study systematically reviews the safety and clinical efficacy of Meridian Sinew Tuina (MST) protocols for BCRL. METHODS: Global and regional databases (PubMed, Cochrane Library, Embase, Web of Science, CNKI, Wanfang, VIP) were searched from inception to January 15, 2026, with alerts monitored through April 30, 2026. Randomised controlled trials (RCTs) evaluating MST (deep tissue mobilisation along the six-hand meridian sinew [Jingjin] lines via plucking, kneading, and pressing) were included. Two reviewers independently extracted data, evaluated risk of bias using Cochrane RoB 2, and assessed evidence certainty via GRADE using a random-effects model. RESULTS: Fifteen RCTs were included. For the primary anthropometric outcome, MST significantly reduced upper limb circumference compared to controls (SMD = 1.59; 95% CI: 1.44 to 1.74; Z&#x2009;=&#x2009;20.81; p&#x2009;<&#x2009;0.0001; I2=0.0%; N&#x2009;=&#x2009;924; GRADE: Moderate certainty). The Clinical Response Efficacy Rate (&#x2265; 30% swelling reduction and symptom relief) favoured MST (RR = 1.69; 95% CI: 1.54 to 1.87; Z&#x2009;=&#x2009;10.62; p&#x2009;<&#x2009;0.0001; I2=0.0%; N&#x2009;=&#x2009;1,114; GRADE: Moderate certainty). Trial Sequential Analysis confirmed sample size sufficiency. For secondary outcomes (N&#x2009;=&#x2009;924; GRADE: Low to Very Low certainty due to performance bias and clinical heterogeneity), MST showed favourable 3-month improvements in DASH functional scores (SMD&#x2009;=&#x2009;-1.81; 95% CI: -2.11 to -1.51; I2=45.1%), pain intensity (SMD&#x2009;=&#x2009;-2.44; 95% CI: -2.93 to -1.95; I2=50.4%), and quality of life (SMD = 1.04; 95% CI: 0.79 to 1.29; I2=0.0%). No serious adverse events occurred. CONCLUSIONS: MST protocols are associated with favourable short- and mid-term reductions in upper limb swelling. However, confidence is tempered by unblinded performance bias and control group variations. MST cannot be unconditionally recommended for standalone implementation but represents a promising, optional supportive adjunctive intervention within oncological rehabilitation.

[Effect of bonding materials and methods on microleakage around the edge of stainless-steel brackets: an in vitro study].

PURPOSE: To observe and compare the effect of different types of adhesive materials and different types of etching methods on microleakage around bonded metal brackets. METHODS: 36 healthy premolars extracted for orthodontic reason were selected in the study. The samples were divided into three groups, 12 in each. Group A was etched with 37% phosphate and bonded with Jingjin adhesive; Group B was with 37% phosphate+3M Transbond; Group C was self-etching primer+3M Transbond. All samples were thermalcycled for 500 times, then put into 1% methylenum solution for 24 hours. Microleakage of samples was observed under stereomicroscope. Multiple comparison and Student t test were used for the comparison of data samples(alpha=0.05). RESULTS: (1) Significant difference of microleakage was found among three groups (F=22.462, P<0.01); Multiple comparison showed no significant difference between Group A and Group B (P>0.05), significant differences of microleakage were found between group A and C (P<0.05), group B and C (P<0.05), respectively. (2) Significant differences between occlusal microleakage and gingival microleakage were found in group A (P<0.05), group B (P<0.01) and group C (P<0.01), respectively. CONCLUSIONS: (1) The type of adhesive had no effect on microleakage. (2)Different etching methods are relevant to microleakage, the microleakage of self-etching groups is much more than that of phosphate-etching groups. (3) Gingival microleakage is significantly larger than occlusal microleakage.

Conditional mixed models adjusting for non-ignorable drop-out with administrative censoring in longitudinal studies.

In this paper, a class of conditional mixed models is proposed to adjust for non-ignorable drop-out, while also accommodating unequal follow-up due to staggered entry and administrative censoring in longitudinal studies. Conditional linear and quadratic models which model subject-specific slopes as linear or quadratic functions of the time-to-drop-out, as well as pattern mixture models are both special cases of this approach. We illustrate these models and compare them with the usual maximum likelihood approach assuming ignorable drop-out using data from a multi-centre randomized clinical trial of renal disease. Simulations under various scenarios where the drop-out mechanism is ignorable and non-ignorable are employed to evaluate the performance of these models.

R2R3-MYB transcription factor MYB113 specifically regulates anthocyanin accumulation in Lycium ruthenicum.

LrMYB113 drives anthocyanin biosynthesis in Lycium ruthenicum by forming an MBW complex and directly activating LrDFR and LrANS promoters, providing a genetic target for enhancing flavonoid production. Lycium ruthenicum Murray (black goji berry), a Solanaceae medicinal plant, is valued for its high flavonoid content. However, the transcriptional regulation of flavonoid biosynthesis in L. ruthenicum remains unclear, hindering its pharmaceutical development. Here, we identified and characterized LrMYB113, an R2R3-MYB transcription factor, as a key regulator of anthocyanin biosynthesis in L. ruthenicum. Phylogenetic analysis grouped LrMYB113 into the anthocyanin-associated S6 subgroup of MYBs. Heterologous expression of LrMYB113 in tobacco induced pigment accumulation and upregulated anthocyanin pathway genes. LrMYB113 overexpression in L. ruthenicum hairy roots enhanced accumulation of four acylated anthocyanins and activated anthocyanin pathway genes. Yeast two-hybrid and bimolecular fluorescence complementation assays showed LrMYB113 interacts with bHLHs (LrJAF13/LrAN1b) and WD40 (LrAN11) to form an MBW complex. Promoter binding and transactivation assays demonstrated LrMYB113 directly binds to and activates LrDFR and LrANS promoters. Dual-luciferase assays showed LrMYB113 alone strongly activates LrDFR and LrANS promoters; MBW complexes enhanced activity compared to individual bHLH/WD40 but not to LrMYB113 alone. Our findings identify&#xa0;LrMYB113 as a critical regulator of anthocyanin biosynthesis in L. ruthenicum, shedding light on flavonoid molecular mechanisms and supporting genetic improvement for pharmaceutical use.

Association between adherence measurements of metoprolol and health care utilization in older patients with heart failure.

OBJECTIVE: Data from electronic dosing monitors and published pharmacokinetic parameters were used to derive medication adherence measures for immediate-release metoprolol and examine their association with health care utilization of outpatients aged 50 years or older with heart failure. METHODS: We used a 1-compartment model and published population pharmacokinetic parameters to estimate mean plasma metoprolol concentrations for patients treated for 6 to 12 months. In the absence of directly measured plasma concentrations, we calculated the intended mean plasma concentration (Cp'(ave)) under the assumption of perfect adherence to the prescribed dose and frequency of administration. Projected mean plasma concentrations (Cp(ave)) were estimated by use of data from recorded dosing times. In addition to taking adherence (percentage of dose taken) and scheduling adherence (percentage of doses taken on schedule), we calculated the deviation from the intended exposure (DeltaCp(ave) = Cp'(ave) - Cp(ave)) and the proportion of intended exposure achieved by the patient (Cp(ave) /Cp'(ave)). We assessed the association between the adherence measures and the numbers of emergency department visits and hospital admissions experienced by the patients. RESULTS: Patients (N = 80) were aged 62 +/- 8 years. Mean DeltaCp(ave) and Cp(ave)/Cp'(ave) were 7.9 ng/mL (SD, 10.7) and 0.6 (SD, 0.3), respectively. Log-linear models adjusted for patient functional status indicated that greater deviation from the intended metoprolol exposure (DeltaCp(ave)) was associated with increased numbers of emergency department visits ( P < .0001) and hospital admissions (P < .0001). A higher proportion of intended exposure (Cp(ave) /Cp'(ave)) corresponded to a reduced number of emergency department visits (P = .0204) and hospital admissions (P = .0093). Taking adherence was univariately associated with both emergency department visits and hospital visits (P < .0001 and P = .0010, respectively). Scheduling adherence was associated with the number of emergency department visits (P = .0181) but not with the number of hospital admissions (P = .1602). Model selection procedures consistently chose the proposed measures over taking adherence and scheduling adherence. CONCLUSION: Deviation from the intended exposure and proportion of intended exposure achieved by the patient are valid adherence measures for immediate-release metoprolol and are associated with health care utilization. The potential utility of these measures for other beta-adrenergic antagonists and perhaps other cardiovascular drugs should be investigated.

Telephonic case-finding of major depression in a Medicaid chronic disease management program for diabetes and heart failure.

OBJECTIVE: Major depression is common in low-income and chronically ill persons and is a barrier for effective chronic disease care. We evaluated a Medicaid-sponsored strategy for detecting depressive symptoms in adults with diabetes or congestive heart failure. METHODS: Using a two-item screening tool, 890 adults enrolled in the Indiana Chronic Disease Management Program were assessed by telephone for depressive symptoms between December 2003 and March 2004. A subset of 386 participants also completed the eight-item Patient Health Questionnaire (PHQ-8) depression measure. Antidepressant use was examined using pharmacy claims. RESULTS: Depressed mood or anhedonia was reported by 51% of participants. About one in four participants had a PHQ-8 score indicating a high risk for major depression (score >or=10). The two-item screen was 96% sensitive [95% confidence interval (CI), 89-99%] and 60% specific (95% CI, 54-65%) for identifying members at high risk for depression by the full PHQ-8 instrument. Only half of participants with high-risk PHQ-8 scores had a pharmacy claim indicating that an antidepressant medication was filled within 120 days of the depression screening. CONCLUSIONS: A two-stage, telephonic approach involving the PHQ-8 instrument for Medicaid members with either depressed mood or anhedonia could identify two clinically depressed persons for every nine members screened.

Statistical methods for chip calibration and saturation effects in antibody-spiked gene expression data.

Oligonucleotide microarrays are amongst, a set of technologies that allow for high throughput assessment of vast numbers of gene expressions. In order to evaluate gene expressions given detection limits, antibody spiking is often used providing one with an expression curve relating antibody treated expression and non-antibody treated expression. These curves can exhibit different functional shapes across chips and hence need to be standardized. In addition, each curve is subject to saturation effects, which are typically dealt with by extrapolating a linear fit to the subset of the data not visually subject to saturation. In this paper we introduce methods for the non-parametric standardization of expression curves using univariate smoothers. We also explore parametric methods for more efficient analysis of the standardized curves. We demonstrate an alternate method of parametric analysis using a weighted linear mixed effects model that does not arbitrarily delete data beyond an observed saturation point; allows for natural grouping of genes and provides significantly more accurate predictions than naive linear extrapolation. Both methodologies are studied through sets of simulations.

Structural mechanism governing the quaternary organization of monocot mannose-binding lectin revealed by the novel monomeric structure of an orchid lectin.

Two isoforms of an antifungal protein, gastrodianin, were isolated from two subspecies of the orchid Gastrodia elata, belonging to the protein superfamily of monocot mannose-specific lectins. In the context that all available structures in this superfamily are oligomers so far, the crystal structures of the orchid lectins, both at 2.0 A, revealed a novel monomeric structure. It resulted from the rearrangement of the C-terminal peptide inclusive of the 12th beta-strand, which changes from the "C-terminal exchange" into a "C-terminal self-assembly" mode. Thus, the overall tertiary scaffold is stabilized with an intramolecular beta-sheet instead of the hybrid observed on subunit/subunit interface in all known homologous dimeric or tetrameric lectins. In contrast to the constrained extended conformation with a cis peptide bond between residues 98 and 99 commonly occurring in oligomers, a beta-hairpin forms from position 97 to 101 with a normal trans peptide bond at the corresponding site in gastrodianin, which determines the topology of the C-terminal peptide and thereby its unique fold pattern. Sequence and structure comparison shows that residue replacement and insertion at the position where the beta-hairpin occurs in association with cis-trans inter-conversion of the specific peptide bond (97-98) are possibly responsible for such a radical structure switch between monomers and oligomers. Moreover, this seems to be a common melody controlling the quaternary states among bulb lectins through studies on sequence alignment. The observations revealed a structural mechanism by which the quaternary organization of monocot mannose binding lectins could be governed. The mutation experiment performed on maltose-binding protein-gastrodianin fusion protein followed by a few biochemical detections provides direct evidence to support this conclusion. Potential carbohydrate recognition sites and biological implications of the orchid lectin based on its monomeric state are also discussed in this paper.

Regulation of the interaction of inosine monophosphate dehydrogenase with mycophenolic Acid by GTP.

Inosine monophosphate dehydrogenase (IMPDH), a rate-limiting enzyme in the de novo synthesis of guanine nucleotides, is a major therapeutic target. A prototypic uncompetitive inhibitor of IMPDH, mycophenolic acid (MPA), is the active form of mycophenolate mofeteil (CellCept), a widely used immunosuppressive drug. We have found that MPA interacts with intracellular IMPDH in vivo to alter its mobility on SDS-polyacrylamide gels. MPA also induces a striking conformational change in IMPDH protein in intact cells, resulting in the formation of annular aggregates of protein with concomitant inhibition of IMPDH activity. These aggregates are not associated with any known intracellular organelles and are reversible by incubating cells with guanosine, which repletes intracellular GTP, or with GTPgammaS. GTP also restores IMPDH activity. Treatment of highly purified IMPDH with MPA also results in the formation of large aggregates of protein, a process that is both prevented and reversed by the addition of GTP. Finally, GTP binds to IMPDH at physiologic concentrations, induces the formation of linear arrays of tetrameric protein, and prevents the aggregation of protein induced by MPA. We conclude that intracellular GTP acts as an antagonist to MPA by directly binding to IMPDH and reversing the conformational changes in the protein.

Progress in accelerated measles control in the People's Republic of China, 1991-2000.

Measles incidence decreased dramatically following widespread use of measles vaccine in China in 1965. To evaluate continued progress in accelerated measles control, data on measles cases reported to the National Notifiable Disease Reporting System during 1991 to 2000 were analyzed. From 1991-1995 to 1996-2000, average annual measles incidence decreased from 9.0 to 5.7 cases per 100,000 population, mortality rates fell from <0.3 to 0.1 deaths per million population, and the percentage of China's total population residing in provinces with a measles incidence of <2 cases per 100,000 population and having a measles elimination goal increased from 21% to 29%. Incidence rates were highest in western provinces and in infants and young children. Additional attention must be focused on western provinces and toward ensuring that all infants are immunized. Achieving high routine two-dose coverage with measles vaccine and enforcing school entry requirements may be highly effective strategies to support further gains in measles control.

An outbreak of poliomyelitis caused by type 1 vaccine-derived poliovirus in China.

BACKGROUND: In May-July 2004, type 1 vaccine-derived poliovirus (VDPV) was isolated from 2 case patients with polio and a contact of a third case patient in Guizhou Province. METHODS: We conducted a field investigation of the outbreak, characterized outbreak isolates, and retrospectively reviewed national polio surveillance data for other VDPVs. RESULTS: Case patients were unimmunized children, 0.9-3.2 years old, living in 2 villages 40 km apart. Immunization coverage in the affected villages was very low. Isolates differed from the Sabin 1 type by 9-11 VP1 nucleotides (1.0%-1.2%); which indicated, on the basis of known rates of mutation of Sabin strains, that they had been circulating for <1 year. A province-wide immunization response targeting all children <5 years old was initiated in August, and the strain has not been isolated since. During 1997-2004, 10 VDPV strains (5 of type 2, 3 of type 1, and 2 of type 3) were isolated from >50,000 children with acute flaccid paralysis and their contacts; 8 (80%) were found in southern provinces, and 9 (90%) spontaneously disappeared. CONCLUSION: This is the first polio outbreak in China in over a decade and the first due to VDPV. The short duration of circulation demonstrates the rapidity with which attenuated Sabin strains can revert to a wild phenotype. One to two VDPVs have been identified each year, primarily in densely populated subtropical regions of southern China. This outbreak highlights the need to consider risks of paralysis from vaccine-derived strains in development of national poliomyelitis immunization policy.

A chronic inflammatory response dominates the skeletal muscle molecular signature in dystrophin-deficient mdx mice.

Mutations in dystrophin cause Duchenne muscular dystrophy (DMD), but absent dystrophin does not invariably cause necrosis in all muscles, life stages and species. Using DNA microarray, we established a molecular signature of dystrophinopathy in the mdx mouse, with evidence that secondary mechanisms are key contributors to pathogenesis. We used variability controls, adequate replicates and stringent analytic tools, including significance analysis of microarrays to estimate and manage false positive rates. In leg muscle, we identified 242 differentially expressed genes, >75% of which have not been previously reported as altered in human or animal dystrophies. Data provide evidence for coordinated activity of numerous components of a chronic inflammatory response, including cytokine and chemokine signaling, leukocyte adhesion and diapedesis, invasive cell type-specific markers, and complement system activation. Selective chemokine upregulation was confirmed by RT-PCR and immunoblot, and may be a key determinant of the nature of the inflammatory response in dystrophic muscle. Up-regulation of secreted phosphoprotein 1 (minopontin, osteopontin) mRNA and protein in dystrophic muscle identified a novel linkage between inflammatory cells and repair processes. Extracellular matrix genes were up-regulated in mdx to levels similar to those in DMD. Since, unlike DMD, mdx exhibits little fibrosis, data suggest that collagen regulation at post-transcriptional stages mediates extensive fibrosis in DMD. Taken together, these data identify a relatively neglected aspect of DMD, suggest new treatment avenues, and highlight the value of genome-wide profiling in study of complex disease processes.

Indiana chronic disease management program risk stratification analysis.

OBJECTIVE: The objective of this study was to compare the ability of risk stratification models derived from administrative data to classify groups of patients for enrollment in a tailored chronic disease management program. SUBJECTS: This study included 19,548 Medicaid patients with chronic heart failure or diabetes in the Indiana Medicaid data warehouse during 2001 and 2002. MEASURES: To predict costs (total claims paid) in FY 2002, we considered candidate predictor variables available in FY 2001, including patient characteristics, the number and type of prescription medications, laboratory tests, pharmacy charges, and utilization of primary, specialty, inpatient, emergency department, nursing home, and home health care. METHODS: We built prospective models to identify patients with different levels of expenditure. Model fit was assessed using R statistics, whereas discrimination was assessed using the weighted kappa statistic, predictive ratios, and the area under the receiver operating characteristic curve. RESULTS: We found a simple least-squares regression model in which logged total charges in FY 2002 were regressed on the log of total charges in FY 2001, the number of prescriptions filled in FY 2001, and the FY 2001 eligibility category, performed as well as more complex models. This simple 3-parameter model had an R of 0.30 and, in terms in classification efficiency, had a sensitivity of 0.57, a specificity of 0.90, an area under the receiver operator curve of 0.80, and a weighted kappa statistic of 0.51. CONCLUSION: This simple model based on readily available administrative data stratified Medicaid members according to predicted future utilization as well as more complicated models.

The Indiana Chronic Disease Management Program.

The Indiana Chronic Disease Management Program (ICDMP) is intended to improve the quality and cost-effectiveness of care for Medicaid members with congestive heart failure (chronic heart failure), diabetes, asthma, and other conditions. The ICDMP is being assembled by Indiana Medicaid primarily from state and local resources and has seven components: (1) identification of eligible participants to create regional registries, (2) risk stratification of eligible participants, (3) nurse care management for high-risk participants, (4) telephonic intervention for all participants, (5) an Internet-based information system, (6) quality improvement collaboratives for primary care practices, and (7) program evaluation. The evaluation involves a randomized controlled trial in two inner-city group practices, as well as a statewide observational design. This article describes the ICDMP, highlights challenges, and discusses approaches to its evaluation.
