Iodine intake and the pattern of thyroid disorders: a comparative epidemiological study of thyroid abnormalities in the elderly in Iceland and in Jutland, Denmark.

Thyroid abnormalities are common in all populations, but it is difficult to compare results of epidemiological studies, because different methods have been used for evaluation. We studied the importance of the population iodine intake level for the prevalence rate of various thyroid abnormalities in elderly subjects. Random samples of elderly subjects (68 yr) were selected from the central person registers in Jutland, Denmark, with low (n = 423) and, in Iceland, with longstanding relatively high (n = 100) iodine intake. Females from Jutland had a high prevalence of goiter or previous goiter surgery (12.2%), compared with males from Jutland (3.2%) and females (1.9%) and males (2.2%) from Iceland. Abnormal thyroid function was very common in both areas, with serum TSH outside the reference range in 13.5% of subjects from Jutland and 19% of those from Iceland. In Jutland, it was mainly thyroid hyperfunction (9.7% had low, 3.8% had high serum TSH), whereas in Iceland, it was impaired thyroid function (1% had low, 18% had high serum TSH). All subjects with serum TSH more than 10 mU/L had autoantibodies in serum, but antibodies were, in general, more common in Jutland than in Iceland. Thus, thyroid abnormalities in populations with low iodine intake and those with high iodine intake develop in opposite directions: goiter and thyroid hyperfunction when iodine intake is relatively low, and impaired thyroid function when iodine intake is relatively high. Probably, mild iodine deficiency partly protects against autoimmune thyroid disease. Thyroid autoantibodies may be markers of an autoimmune process in the thyroid or secondary to the development of goiter.

High incidence of multinodular toxic goitre in the elderly population in a low iodine intake area vs. high incidence of Graves' disease in the young in a high iodine intake area: comparative surveys of thyrotoxicosis epidemiology in East-Jutland Denmark and Iceland.

Little is known about the optimum level of iodine intake for iodine supplementation programmes, or about the effects of the high levels of iodine intake that are found in some countries. We compared the incidence of different types of hyperthyroidism in East-Jutland Denmark with a low average iodine intake but no endemic goitre, and the incidence in Iceland with a relatively high iodine intake. Hyperthyroidism was more common in East-Jutland than in Iceland, due to a much higher incidence of multinodular toxic goitre and also of single toxic adenoma. Most of the patients with these diseases were over 50 years of age. By contrast, the incidence of Graves' disease was significantly higher in Iceland than in East-Jutland. This difference was most marked in the younger age groups, in which hyperthyroidism was more than twice as common in Iceland as in East-Jutland. These results demonstrate that even mild iodine deficiency has a significant effect on population health, since it leads to a high incidence of autonomous thyroid nodules with hyperthyroidism in the elderly population. However, population iodine intake probably should not exceed a level much higher than that necessary to avoid iodine deficiency, otherwise Graves' disease may be induced in the young population.

[Increased frequency of CRD in the broiler production in northern Jutland. II. Influence on production and economy (author's transl)].

The consequences for production and economy caused by an increase in CRD outbreaks in broilers in northern Jutland are estimated by comparing production results from book-keeping farms to the average results of the whole country, where an improvement of 19 gram was found, while a similar increase did not occur in northern Jutland (Table I). The condemnation rate has risen 0.4% in northern Jutland. For the whole country 0.1% rise has been registered. Production results from 6 book-keeping farms with CRD have been compared to the results from all book-keeping farms in the region. It appears from Table III that the 6 farms were lacking 60 gram weight increase and 0.03 in feed conversion. Gross margins for production without problems have been calculated to 0.70 kr. per chicken against 0.42 kr. per chicken in the 71 CRD attacked flocks (Tables IV and V). If in addition also slaughterhouse expenses are added the total loss due to CRD in the 71 flocks sum up to 510.144 kr.

The results of treatment of epithelial ovarian cancer after centralisation of primary surgery. Results from North Jutland, Denmark.

OBJECTIVE: The study was performed to evaluate the results of treatment of ovarian carcinoma after the introduction of centralised primary surgery in the County of North Jutland, Denmark. METHOD: Prospective study of consecutive cases of ovarian cancer undergoing primary surgical treatment at the Gynecologic Oncologic Center after the introduction of centralised primary surgery. Results of treatment recorded up to the date of last examination or death. RESULTS: From 1999 to 2002, 107 patients with primary epithelial ovarian cancer underwent primary surgery at the Gynecologic Oncologic Center, Aalborg. This corresponds to 95.5% of patients with invasive carcinoma in the County of North Jutland. All patients with Stage I to Stage IIIB disease had a complete, macroscopically radical cytoreduction performed. In patients with Stage III and IV invasive tumors, the optimal debulking rate was 79.5%, and, in Stage IIIC and IV, the optimal debulking rate was 78.2%. Intra-operative and post-operative complications were generally few. Post-operative death, defined as death within 30 days after surgery, was observed in 4 cases (3.7%). After primary surgery, platinum-based chemotherapy was given in most cases. For Stage I to IV invasive cancer, the median survival was 46 months. In patients with Stage IIIC and IV disease, the median survival was 32 months. In optimally debulked Stage IIIC and IV disease, the median survival was 41 months. CONCLUSIONS: The results indicate a survival benefit after introduction of centralised primary surgery. Compared to existing national and regional data on survival in ovarian cancer, the results indicate an increase in median survival for all stages of approximately 15 months. Centralisation of primary surgery to centres with the necessary expertise may be the most significant way to increase survival in ovarian cancer in Denmark.

Use of lipid-lowering drugs during 1991-98 in Northern Jutland, Denmark.

AIMS: To examine a) the use of lipid-lowering drugs in North Jutland County in Denmark from 1991 to 1998 and b) the pattern of usage according to sex and age. METHODS AND RESULTS: We used the Pharmaco-Epidemiological Prescription Database in the county to identify all reimbursed prescriptions for lipid-lowering therapy from 1991 to 1998. One-year incidence rates (IR) and prevalence (P) of the use of lipid-lowering drugs were calculated. Both IR and P of patients in lipid-lowering therapy were stable until 1994, with the IR below 100 per 100 000 for both sexes. The IR then increased from 59.9 to 236.5 per 100 000 person-years in 1998 for women, and from 88.6 to 322.8 per 100 000 person-years for men. The utilization patterns were identical between the sexes. Thus, in both women and men the highest prevalence and incidence rates of lipid-lowering drug therapy were seen in the 60-69-year-olds. Furthermore, the marked increase in both prevalence and incidence of persons on lipid-lowering drug therapy between 1994 and 1998 was the result of an increased number of prescriptions in the 50-59, 60-69 and 70 + years olds, in both women and men. There was a remarkable 4-5 fold increase in the numbers of new patients who received statins during the same period. CONCLUSIONS: The overall use of lipid-lowering drugs has increased markedly over the last few years in Northern Jutland, Denmark. The increase began following publication of the first major trial documenting the benefit of therapy with statins.

Secular trends in incidence and 30-day case fatality of acute pancreatitis in North Jutland County, Denmark: a register-based study from 1981-2000.

BACKGROUND: The incidence rate of acute pancreatitis has been reported as having increased during recent decades in Western countries. Reported mortality lies around 10% and has improved during the past 20 years. The incidence rate and 30-day case fatality rate of acute pancreatitis in North Jutland County, Denmark were examined for the period 1981 to 2000. METHODS: Data were collected from the Hospital Discharge Registry of North Jutland County for the period 1981-2000. Sex- and age-standardized incidence rates and 30-day case fatality rate of a first attack of acute pancreatitis were calculated. Data on endoscopic procedures were assessed for the period 1992 to 2000 and on certain drugs for 1991 to 1999. RESULTS: The incidence rate of acute pancreatitis in women increased from 17.1 per 100,000 person-years in 1981 (95% confidence interval (CI), 12.6-23.2) to 37.8 per 100,000 person-years in 2000 (95% CI, 31.0-46.1). The corresponding increase in men was from 18 per 100,000 person-years in 1981 (95% CI, 13.3-24.2) to 27.1 per 100,000 person-years in 2000 (95% CI, 21.5-34.3). The incidence rate of acute pancreatitis increased with age in both sexes. The overall 30-day case fatality rate was 7.5% (95% CI, 6.5-8.7) increasing with age, adjusted odds ratio (OR) = 6.4 (95% CI, 3.5-11.6) and decreased with time, adjusted OR = 0.7 (95% CI, 0.4-1.0). CONCLUSION: The incidence of acute pancreatitis has increased, and in women surpassed that in men in 1999 and 2000. Short-term prognosis has improved.

The influence of the Battle of Jutland on plastic surgery.

The Battle of Jutland was fought between the British and German fleets in the gray wastes of the North Sea in May 1916. Because the battle was fought close to English shores, burn victims fared better than previously had been possible. From the Battle of Jutland came major advances in the clinical care of the acutely injured, as well as new concepts of reconstruction after healing had taken place.

Results of 312 pregnancies among white class B-F mothers in northern Jutland from 1976 to 1992.

STUDY OBJECTIVE: To determine the outcome of all pregnancies among White class B-F mothers in the county of Northern Jutland from 1976 to 1992. DESIGN: Retrospective, hospital record based study. SETTING: Eleven hospitals, of which the Department of Obstetrics in Aalborg has been appointed regional centre for care of all diabetic pregnancies in the county, covering approximately 550,000 inhabitants. PATIENTS: 312 consecutive, unselected planned pregnancies among 223 women with White class B-F diabetes mellitus delivering in the county of Northern Jutland from 1976 to 1992. MAIN RESULTS: Thirty-two (10.2%) pregnancies terminated with spontaneous abortions and ten (3.2%) abortions were induced for medical reasons. There were eleven (4.0%) perinatal deaths of which one had lethal malformations. Ten (3.6%) babies had major malformations. HbA1c values varied from 7.4% (n = 171) in the first to 6.6% (n = 216) in the third trimester. CONCLUSION: The perinatal mortality is in the upper range of contemporary published series with late intrauterine stillbirths as the main problem. To improve detection of fetal distress in late pregnancy, our observational program has been intensified.

Farmers' involvement in landscape activities: an analysis of the relationship between farm location, farm characteristics and landscape changes in two study areas in Jutland, Denmark.

Farmers are important agents in rural landscape management as they modify landscape elements to suit their needs. The purpose of this study is to investigate if patterns of landscape activities undertaken by farmers are related to certain farm characteristics and if landscape activities vary from one location to another. With this purpose, we investigated the differences between the level of farmers' involvement in landscape activities in two study areas in central Jutland, Denmark. Particular attention was given to the analysis of the type and extent of landscape activities and their relationship with farm characteristics. In both study areas, landscape activities leading to a more extensive type of land use predominate. However, a multivariate analysis implemented on the set of landscape activity data reveals significant differences between the two study areas. Hedgerow planting and removal is more common in Sønder Omme, while pond digging and cultivation of permanent grass lands are characteristic activities in Gadbjerg-Givskud. Moreover, the proportion of farmers involved and the area affected by different landscape activities is larger in Sønder Omme than in Gadbjerg-Givskud. The farms of the two areas are different in terms of production type, size and socio-economic characteristics. These characteristics represent significant relationships with landscape activities. However, they are not sufficient to explain the differences between landscape activities undertaken in the two areas. The differences are caused by unique cultural and biophysical environment, which influence and modify the relationship between farm characteristics and landscape activities in a distinct manner for each study area.

Analysis of changes in a shelterbelt network landscape in central Jutland, Denmark.

This paper investigates the changes in a shelterbelt network between 1950 and 1997 in a study area in central Jutland, Denmark. The analysis consists of two parts. Firstly, the changes in key parameters which are of landscape ecological importance (shelterbelt density, connectivity, node type and landscape grain size) are examined. The results of this analysis show that shelterbelt density varied considerably during the period due to changes in biophysical and socioeconomic factors. A GIS analysis shows that landscape grain size decreased in 45 percent of the area. A major change of shelterbelt type occurred in the area with landscape ecological and visual scenery implications. It appears that public-subsidised shelterbelt planting programmes were the most important factors influencing changes in the shelterbelt network in the study area while differences in soil type were less important. Secondly, data from a questionnaire survey were used to examine the driving forces behind shelterbelt activities over a 10 year period. Hobby farmers were the most active farmer group in shelterbelt planting and agronomic reasons were the most important motives for shelterbelt planting in the study area during this period. Furthermore, it appeared that microclimate effects and aesthetical considerations have become important reasons for shelterbelt planting.

Cannabis plants illicitly grown in Jutland (Denmark).

Four hundred forty-nine fresh cannabis plants and 26 fruiting tops harvested in Jutland (Denmark) from July to September 1988 were characterized according to weight, height, marihuana yield, and cannabinoid content. The median weights were 308 g and 584 g for plants grown outdoors (n = 418) and in greenhouses (n = 31), respectively. The average marihuana yield was 8.7% for the plants grown outdoors and slightly lower for the greenhouse plants. Great variations, however, were seen both between and within the individual harvests. The mean concentration of total THC (tetrahydrocannabinol) was 0.87% for the plants grown outdoors. An increase according to the month of harvest was observed. For plants grown in greenhouses the mean value of total THC was 1.35%, while the mean concentration of fruiting tops was 2.13%. All plants contained cannabidiol (CBD), but only negligible concentrations of other cannabinoids. In approximately 80% of the plants the THC content was higher than the CBD content (drug type), while the rest either contained equal concentrations (intermediate type) or most CBD (fiber type).

Outcome of 328 pregnancies in 205 women with insulin-dependent diabetes mellitus in the county of Northern Jutland from 1976 to 1990.

STUDY OBJECTIVE: To determine the degree of centralization and to describe the outcome of all pregnancies among Type 1 (insulin-dependent) mothers in the county of Northern Jutland from 1976-90. DESIGN: Retrospective, hospital record based study. SETTING: Data were collected from all departments of surgery (n = 8) and obstetrics (n = 3) in the county covering 550,000 inhabitants (approx. 10% of the total Danish population). PATIENTS: Three hundred and twenty-eight consecutive, unselected pregnancies among 205 women with Type 1 diabetes mellitus admitted to the hospitals in the county from 1976-90. MAIN RESULTS: Thirty-two (11%) pregnancies were terminated with spontaneous abortions and ten (3.3%) abortions were induced for medical reasons. There were eleven (4.3%) neonatal deaths. There was no additional fetal loss within the first year of life. Eight (3.1%) babies had major and additionally six (2.4%) minor malformations. Total fetal loss was 53/297 = 18%. Twenty-eight percent of the babies were large-for-gestational age. No pregnancies terminated at primary level hospitals and only 6% at the non-sub specialized obstetrical Departments. CONCLUSION: We have achieved an almost complete centralization of the management of pregnant women with diabetes mellitus and our outcome is comparable to the results published from other regional surveys taking care of these patients.

Landscape changes in agrarian landscapes in the 1990s: the interaction between farmers and the farmed landscape. A case study from Jutland, Denmark.

Recent landscape changes in a farmed landscape are analysed and related to farm and farmer characteristics. It is assumed that farm and farmer characteristics serve as mediators of large scale or macro driving forces of change-in the present case, a changing farming context including demands for a more environmentally friendly farming practise and a reduced output. The results are based on multivariate analyses of data collected from structured interviews of 160 farmers in a case study area, in central Jutland, measuring 5000 ha. The analysis shows that farmers are highly involved in landscape changes. The investigated landscape changes include creation and removal of landscape elements as well as certain management changes. The most common activity was creation of elements: hedgerows, small woodlands and conversion of rotational arable land to permanent grassland, whereas removal of elements, mainly hedgerows and semi-natural grasslands, were seen less frequently. Management changes like abandonment of permanent grassland were widespread. The results indicate a general extensification of the land use and the authors interpret the results partly as an indication of a change from productivism to a more multifunctional agricultural regime. The observed landscape changes at the farm level show a low, but structured relationship with the current farm and farmer characteristics, meaning that landscape changes were undertaken by various farmers and on various farms. On a general level, however, the age of the farmer and the duration of farm ownership seem to have a major influence on the landscape changes.

Distribution of Puumala hantavirus in Denmark: analysis of bank voles (Clethrionomys glareolus) from Fyn and Jutland.

The majority of hantaviral infections in Denmark since 1957 have occurred on the island of Fyn. We have recently shown the presence of Puumala hantavirus (PUUV) in bank voles (Clethrionomys glareolus) on Fyn. To learn more about the distribution and prevalence of PUUV in Denmark, 310 small mammals, belonging to six rodent and two insectivore species, were trapped in four localities on Fyn and three localities on the Jutland Peninsula (mainland Denmark), where only a few cases of nephropathia epidemica (NE) have been confirmed so far. Serum samples (heart extracts) from all animals were initially analyzed for the presence of hantavirus-specific antibodies (Ab) by enzyme immunoassay, and lung tissue samples of bank voles for the presence of the viral nucleocapsid protein antigen by immunoblotting. Altogether 11 Ab-positive rodents were found, all of them bank voles from Fyn. PUUV genome sequences (nucleotides 2,168-2,569 from the medium genomic segment) were recovered by reverse transcription-polymerase chain reaction from seven bank voles. They showed an overall diversity up to 5% and formed a well-supported genetic lineage on the phylogenetic tree with a clustering of two strains from southeastern Fyn together and a separation from the one strain from western Fyn. Our data confirmed the circulation of PUUV in both the southeastern and western parts of the island, known to have the highest numbers of human NE cases.

Cauda equina syndrome in lumbar spinal stenosis: case report and incidence in Jutland, Denmark.

BACKGROUND: Cauda equina syndrome in spinal stenosis is a rare complication. During a 5-year period (1996-2000), 340 cases of spinal stenosis were diagnosed in the County of South Jutland, Denmark. The annual incidence of spinal stenosis was 272 per million inhabitants. During that period, only one patient with acute cauda equina syndrome in spinal stenosis was diagnosed: that is, the patient described herein. METHODS AND RESULTS: A 74-year-old woman appeared with urinary retention and fecal incontinence for the previous 24 hours. Computed tomography scan showed spinal stenosis from L2 to L4. She underwent an urgent operation and recovered within 5 days from her anal sphincter paresis and within 5 weeks from her bladder paresis.

Risk factors associated with retinopathy of prematurity (ROP) in northern Jutland, Denmark 1990-1993.

This population-based, historical, follow-up study analysed possible risk factors for retinopathy of prematurity (ROP) and resulting visual impairment in newborns over a period of 3 1/2 years in the County of Northern Jutland, Denmark. The study subjects were 141 infants with birth weight < or = 1500 grams and gestational age < 35 completed weeks who survived to a first eye examination at 5 weeks. The incidence of ROP (all stages) was 18%, and the frequency of severe visual impairment from ROP was 14/100,000 live births. A logistic regression analysis identified low gestational age, multiple births, continuous oxygen treatment, and male sex as risk factors for ROP. The main factor in the prevention of ROP is to prevent preterm births.

The North Jutland County Diabetic Retinopathy Study (NCDRS) 2. Non-ophthalmic parameters and clinically significant macular oedema.

BACKGROUND: The influence of non-ophthalmic parameters on the prevalence of clinically significant macular oedema has not been unambiguously established. The present study was initiated with the aim of clarification. METHODS: This cross-sectional study comprised 656 type 1 and 328 type 2 diabetic subjects undergoing retinopathy screening in the county of North Jutland. The association between the presence of clinically significant macular oedema and blood pressure, HbA1c, BMI, age, onset of diabetes, duration of diabetes, blood-pressure-reducing medication, lipid-lowering medication, neuropathy and urinary albumin excretion was explored using multiple logistic regression analysis. RESULTS: We found no significant association between the presence of clinically significant macular oedema and any of the examined parameters in type 1 diabetic subjects. In type 2 diabetic subjects, the duration of diabetes, HbA1c, neuropathy and increased urinary albumin excretion was significantly associated with the presence of clinically significant macular oedema. CONCLUSIONS: The risk factors for clinically significant macular oedema differ in type 1 and type 2 diabetic subjects and can account only in part for this manifestation.

Congenital heart malformations in Jutland, Denmark: a three year necropsy study in children aged 0-14 years. Epidemiology and classification according to sequential segmental analysis.

The use of sequential segmental analysis for describing congenital heart malformations was the method of assessment used in a prospective necropsy study covering Jutland, a well defined geographical and demographic area of Denmark. The study group was 1,154 children of whom 261 (22.6%) had a congenital heart malformation. The most common malformations were ductus arteriosus and ventricular septal defect and there were 77 cases in which connections between chambers or between chambers and great arteries were anomalous (68 liveborn; 37 male and 31 female: nine stillborn; two male and seven female). No difference in sex distribution or seasonal variation was found between those with congenital heart disease and those without. Extracardiac malformations and chromosomal abnormalities were more often seen in children with congenital heart malformation than those without (30.3% vs 16.6%). The sequential segmental analysis is a logical and precise way of describing congenital heart malformations and it should be routinely used to classify cases of congenital heart malformation.
