Erythrokeratodermia variabilis. A family study.

Erythrokeratodermia variabilis is a rare genodermatosis; American authors have reported only four previous cases. It had been a problem to obtain a large pedigree for clinical investigation. We studied a family with 12 involved members in five generations. Symmetrically distributed migratory patches and scaling plaques are characteristic and were found to involute with a combination of keratolytic agents and topical steroids. Exacerbations of these patches and plaques were noted in our female patients during such high estrogen states as pregnancy or oral contraceptive usage.

Sodium pyruvate treatment for hyperkeratotic disorders.

We have successfully used 5% sodium pyruvate in Eucerin for the treatment of congenital disorders with hyperkeratosis. It was most effective in those disorders with the most hyperkeratosis, and it was nonirritating even after extensive use. The efficacy of pyruvate may be due to the relatively low pKa of pyruvic acid.

The effect on rhino mouse skin of agents which influence keratinization and exfoliation.

The skin of the rhino mouse, an allelic variant of the hariless mouse, contains deep dermal cysts and huge numbers of hornfilled utriculi which resemble comedones. Chemicals which influence either differentiation or desquamation of horny cells were applied topically twice daily for up to 6 weeks. Except for the dermal cysts, the gross epithelial abnormalities were almost completely corrected by retinoic acid in a dose-dependent fashion. Salicylic acid caused partial emptying of the horny masses, but the utriculi did not regress. Lactic acid, propylene glycol and benzoyl peroxide had minor effects on keratinization and exfoliation. The rhino mouse is a suitable model for assessing chemicals which affect epithelial differentiation (retinoids)or which promote loss of cohesion between horny cells (descaling agents).

Newer treatment in dermatology.

The future of a drug depends upon what it can do in the hands of the practitioner. Medicine is practiced on the basis of probabilities, and treatment must be selected which has the best chance of helping the patient, with the least amount of harm. There are many new drugs available for dermatologic therapy in other developed countries which are not available in this country, due to peculiarities of federal drug regulations.

Acne Vulgaris

Lichen spinulosus: response to therapy.

Two cases of lichen spinulosus and the response to therapy with Keralyt under occlusion are presented. It is evident that Keralyt under occlusion for two consecutive weeks will clear these lesions. However, twice weekly application of Keralyt must be used in order to keep these areas free of lesions. An undesirable side effect in the two patients presented was a burning, stinging sensation immediately following application of Keralyt.

Mohs' chemosurgery and skin cancer.

Experience in treating 1, 275 skin cancers with Mohs' chemosurgery is presented. The Mohs technic uses microscopic examination to avoid excision of normal tissue and assure complete removal of the tumor. Chemosurgery is indicated for tumors which are poorly demarcated or which arise in areas known to have a high recurrence rate. It is the procedure of choice for almost all recurrent lesions.

Treating acne vulgaris.

While there is no single treatment plan that will work for every patient, acne can be controlled and long-term scarring can be minimized. Treatment must be individualized. Remember, there may be a "flare" when intensive therapy begins. Useful treatment methods include ultraviolet light, cleansers and soaps, peeling and drying agents, benzoyl peroxide preparations, abrasive soaps and scrubs, retinoic acid and oral antibiotics. The therapeutic choice depends upon the extent and severity of the disease.

Regression of warts. An immunological study.

Altogether 173 patients with warts were under observation for at least 3 and in most cases 6 months. In 80% of the patients wart-virus antibodies were present and could be measured by immunodiffusion (I.D.) and in 20% also by complement-fixation (C.F.) techniques. The mean duration of the warts in patients with C.F. antibodies was 0-6 years and in the others 1-9 years. The occurence of C.F. antibodies (IgG) was associated with rapid healing; 75% of these patients were cured during the first 2 months of the observation period. In contrast, of the patients with antibodies measurable only by the I.D. technique (IgM and/or low titres of IgG), only 16% were cured during the first 2 months and they had a fairly constant cure-rate (approximately 9% per month) during the 6 months' observation period. The results indicate that the cure of warts is partly connected with immunological phenomena, especially with the presence of C.F. antibodies. In other cases wart regression may be mainly a non-immune process, perhaps due to a limited lifespan of wart cells.

A new formula for depigmenting human skin.

Complete depigmentation of the noraml skin of adult male blacks was procured by the daily application for five to seven weeks of a formula consisting of 0.1% tretinoin, 5.0% hydroquinone, 0.1% dexamethasone, and hydrophilic ointment. Depigmentation was not attainable when any one of the components was omitted. The formula was therapeutically effective in treatment of melasma, ephelides, and postinflammatory hyperpigmentation. Senile lentigines were resistant to this therapy.

[Clinical and experimental studies on the effect of benzoyl peroxide in the treatment of acne].

In recent years benzoylperoxide has become increasingly popular in the treament of acne. Beside of a dermal irritative and keratolytic effect this was attributed to the bacteriostatic quality of the substance. By in vitro autoradiography and determination of skin surface lipids in benzoylperoxide=treated skin also a sebostatic effect could be observed. Scanning electron microscopy showed that benzoylperoxide influences the horny layer in a similar way like salicylic acid.

Bowen's disease of the nail bed: a case presentation and review of the literature.

Since 1919, when Bowen first described intraepidermal squamous cell carcinoma of the nail bed, only 11 cases of this rare disease have been reported in the literature. The 12th case of Bowen's disease, presented here, offers an alternative method of treatment using 5-fluorouracil combined with a keratolytic ointment.

Treatment of actinically damaged skin with a keratolytic gel.

Rough-textured, actinically damaged skin of the extremities was treated with a keratolytic gel to restore the surface to a more normal appearance. The excellent improvement observed suggests that an abnormality of cell attachment may be the defect in some so-called dry skin states.

A contribution to a new test method for dandruff-inhibiting and "keratolytic" action of drugs.

Free cholesterol in lipids from the scalp and hair is predominantly a constituent of epidermal lipids. Therefore, a reduction in cholesterol content induced by a drug indicates a reduction in cell turnover in the epidermis. As, according to the literature, increased cell turnover in the epidermis results in formation of dandruff, a reduction in the proportion of cholesterol should indicate inhibition of the formation of dandruff. Conversely, an increase in free cholesterol should generally indicate a "keratolytic" effect. So unequivocal an interpretation has not so far been possible in persons with dandruff, as it was not known whether free cholesterol was increased or decreased. In addition, this interpretation was not possible after use of antimicrobial substances, as in vitro investigations had failed to exclude microbial esterification of cholesterol on the scalp. The present investigation has shown that correlation of free cholesterol level with cell turnover is permissible in patients with dandruff, even if antimicrobial drugs are being tested.
