[Hypertrophy of the inferior labial frenum].

Hypertrophy of the inferior labial frenum, which is present in approximately 8% of the population, can give rise to prosthodontic or periodontal problems. In contrast to hypertrophy of the superior labial frenum, there is no relation to orthodontic problems. The indications for extirpation of a hypertrophic inferior labial frenum are discussed.

[Diastema verum and persistent labial frenum].

In the management of diastema verum associated with persistent labial frenum it is important to distinguish, on the basis of clinical testing, between a normal and abnormal labial frenum. The authors believe that from an orthodontic standpoint, frenotomy ad frenectomy are contraindicated in the deciduous dentition. The latter procedure is indicated in a mixed dentition and is best carried out before the eruption of lateral incisors.

Histologic features of the superior labial frenum.

Histologic examination was performed on the superior labial frenum obtained from 11 fresh biopsy and three autopsy specimens. The frena contained considerable dense collagen, loose connective tissue and elastic fibers. There were no muscle fibers in any of the frenum sections except the autopsy specimens, where it was seen in the vestibular tissue but not in the frenum proper.

[Indications for therapy of labial frenum].

Reporting some cases and literature researches, the Authors point out the indications of labial frenum therapy. The results show no correlations between teeth diastema and frenum: surgical therapy of frenum is recommended after complete eruption of permanent teeth. The correct surgical procedure must be radical and conducted in a short time: frenectomy prolonged to the incisive papilla, is recommended.

The superior labial frenum: a histologic observation.

Superior labial frena obtained from cadavers or frenectomies were examined histologically. The frena were covered by stratified squamous nonkeratotic, orthokeratotic, or parakeratotic epithelia, depending upon the gingival extent of the frenum. In addition to the normal components of dense, irregular, collagenous connective tissue, almost all frena contained myelinated nerve fiber bundles and small vascular channels. Additionally, 35% of the frena examined contained a few small skeletal muscle fibers.
