An inflation catheter technique has been devised for providing general anaesthesia during laryngoscopy using the control module of the Nuffield Anaesthetic Ventilator driven by 50% nitrous oxide in oxygen. The system is simple and reliable in use and ensures adequate pulmonary ventilation.
The need for laryngoscopy aboard a spacecraft on a mission of one to three years is anticipated. Therefore, the inexperience of the potential endoscopists and the problems they face in the free-floating environment of space are considered. A simplified coaxial intubation technique employing a split-apart modification of a Jako-type laryngoscope is suggested.
The following report deals with the frequency of side-effects following suspension laryngoscopy and ways to avoid them, based on the evaluation of 800 cases. As there was only a low rate of complications established (4,5 per cent) which is mainly caused by tooth damages and intubation-lesions, the preoperative information may be limited to those two points.
The authors bring not only the results of their experience but a solution of the respiratory problems during anaesthesia in suspension laryngoscopy, thanks to the devising of a new intubation probe and a pre-set automatic insufflator with varying volum flow.
After reviewing the different methods of ventilation by pulsed oxygen used for anaesthesia during laryngoscopy in suspension in the child, the authors describe a technique of introduction of the endotracheal catheter which makes it possible to reduce the risk of alveolar hyperpressure at the time of installation of the laryngoscope and at the time of waking.
