Epidemiology and control of diphtheria in the Republic of Moldova, 1946-1996.

In 1994-1996, the Republic of Moldova was stricken with an epidemic of diphtheria after >30 years of routine diphtheria immunization and the near absence of the disease for nearly 20 years. The intensity of the epidemic gradually grew, reaching a peak in 1994-1995. The epidemiology of diphtheria in Moldova during this period is described along with laboratory findings and control measures. Pharyngeal diphtheria was the predominant clinical form of the disease (97% of cases), and it most often developed in a localized form (70%), with 20% in the toxic form. The clinical diagnosis of diphtheria was bacteriologically confirmed in 91% of cases: Of the cases tested for biotype, 91.3% were gravis, 8.5% were mitis, and 0. 2% were intermedius. Of 494 toxigenic isolates from cases and carriers at the beginning of the epidemic, 47% were nonphagotypeable strains, and 25.7% were phagotype VI strains. Aggressive population-based diphtheria control measures, a mass immunization campaign, rapid case identification, antibiotic prophylaxis and supplemental immunization of close contacts in clusters of infection, and high coverage with routine immunization rapidly controlled the epidemic within Moldova.

HIV type 1 genetic diversity and genotypic drug susceptibility in the Republic of Moldova.

HIV-1 genetic diversity and, for the first time, genotypic drug susceptibility was investigated for strains circulating in the Republic of Moldova (of the former Soviet Union). Eighty-three samples from adults recently infected by intravenous drug use (IDU) (n = 60), heterosexual contact (n = 8), and from blood donors (n = 15) that tested positive from 1997 to 1998, and originating from different regions of Moldova were serotyped. By group-specific and subtype-specific peptide ELISA, patients were infected by serotype A (n = 65), serotype B (n = 1), or were nontypable (n = 17). Heteroduplex mobility assay (HMA) confirmed 11 subtype A and the one subtype B infection. Analyses of pol and env sequences for six of the IDUs confirmed that they were infected with subtype A strain. These strains clustered tightly with subtype A strains isolated from the former Soviet Union in phylogenetic analysis. No mutations associated with drug resistance were detected. The Republic of Moldova is culturally more closely related to Romania (where subtype F dominates the epidemic), but depends economically on Russia (where subtype A is established among IDUs). Thus, our results suggest that the spread of HIV in this region is driven by drug networks rather than being due to cultural similarities.

Study of Corynebacterium diphtheriae strains isolated in Romania, northwestern Russia and the Republic of Moldova.

A selection of 167 Corynebacterium diphtheriae strains isolated in Romania, the Russian Federation and the Republic of Moldova were analysed by biotyping, phage typing, the toxin production test and by molecular techniques such as ribotyping, pulsed field gel electrophoresis and random amplified polymorphic DNA, in order to establish the epidemiological relatedness, genetic divergence and strain circulation within and between the bordering countries. Using a set of five digoxigenin-labeled oligonucleotides and BstEII digestion, 34 ribotypes were identified. The strains isolated in the epidemic areas (Russia and Moldova) were very closely related but different from those isolated in Romania. C1 and C5 were the main ribotypes identified in these areas. Neither ribotype was found in Romania, where the main circulating types were C3 and C7. Field inversion gel electrophoresis was more discriminative than ribotyping and revealed 54 macrorestriction profiles after SfiI restriction. Both methods showed a significant homogeneity of the strains from epidemic areas and a large diversity among the Romanian strains. Random amplification was useful as an identification method for the epidemic strains, but not for the Romanian ones which displayed a large number of amplification profiles. The phenotypic methods associated with molecular typing techniques enabled distinguishing between strains, detecting the epidemic clone, and sustaining the absence of transmission across borders.

Prevalence of hepatitis B, D and C virus infections among children and pregnant women in Moldova: additional evidence supporting the need for routine hepatitis B vaccination of infants.

Rates of acute hepatitis B are high in Moldova, but the prevalence of chronic infection is unknown. In 1994, we surveyed children and pregnant women, collected demographic information, and drew blood for laboratory testing. Among the 439 children (mean age, 5 years), the prevalence of antibody to hepatitis B core antigen (anti-HBc) and hepatitis B surface antigen (HBsAg) were 17.1 and 6.8%, respectively. Among the 1098 pregnant women (mean age, 26 years), 52.4% were anti-HBc-positive and 9.7% were HBsAg-positive. Of the HBsAg-positive pregnant women, 35.6% were hepatitis B e antigen (HBeAg) positive and 18.3% had antibodies to hepatitis D virus. The prevalence of antibody to hepatitis C virus was 1.4% in children and 2.3% in pregnant women. The high HBeAg prevalence among HBsAg-positive pregnant women and the high anti-HBc prevalence among children indicate that both perinatal and early childhood transmission contribute to the high hepatitis B virus endemicity in Moldova.

Injections given in healthcare settings as a major source of acute hepatitis B in Moldova.

BACKGROUND: Reported rates of acute hepatitis B are high in many former Soviet Union republics and modes of transmission are not well defined. METHODS: Two case-control studies were undertaken in Moldova to identify risk factors for acute hepatitis B in people aged 2-15 years (children) and > or =15 years (adults). Serologically confirmed acute hepatitis B cases occurring between 1 January 1994 and 30 August 30 1995, were matched on age, sex, and district of residence to three potential controls who were tested for hepatitis B markers to exclude the immune. Stratified odds ratios (SOR) were calculated using bivariate and multivariate methods. RESULTS: In multivariate analysis, compared with the 175 controls, the 70 adult cases (mean age 25 years, 66% male) were more likely to report receiving injections in the 6 months before illness during a dental visit (SOR = 21; 95% CI: 3.7-120), a hospital visit (SOR = 35; 95% CI: 7.2-170), or a visit to the polyclinic (SOR = 13; 95% CI: 2.4-74). Among children, receiving injections during a hospital visit (SOR = 5.2; 95% CI: 1.2-23) was the only exposure reported significantly more often by the 19 cases (mean age 8 years, 68% male) compared with the 81 controls. CONCLUSION: These results, along with reported unsafe injection practices in Moldova, suggest that injections are a major source of hepatitis B virus transmission and highlight the importance of proper infection-control procedures in preventing transmission of blood-borne infections.

[Cholera in Ukraine and Moldova during the 7th pandemic].

The epidemic manifestations of cholera in the Ukraine and Moldova in 1970-1991 are analyzed. In the Ukraine the peak of cholera morbidity in 1970 was caused by infection brought to the ports of Odessa and Kerch with its subsequent spread to other territories. This infection was characterized by outbreaks transmitted through water, mainly sea water, with the isolation of Vibrio eltor toxigenic virulent strains and outbreaks of vibrio carriership of alimentary (dairy) origin with the isolation of avirulent strains. In Moldova local outbreaks of water origin were registered in 1970-1972. In both territories prolonged (up to 16 years and even longer) circulation of cholera vibrios, mainly nontoxigenic and avirulent, were registered. The retrospective epidemiological analysis and the epidemiological situation in some regions of the Ukraine in 1991 completely confirmed the prognostic importance of the division of territories into regions according to the type of epidemiological manifestations and the necessity of differentiating the measures to be taken in the system of epidemiological surveillance.

[The clinico-epidemiological characteristics of HIV infection in 93 adults in the Republic of Moldova].

We have examined 93 adults HIV serum-positive (citizens of Moldova Republic), admitted in 1994-1998 (months I-VIII) to the Hospital of Infections Diseases 'T. Ciorba'--Kishinev. Since 1996 the number of HIV infected people began to grow up rapidly. Most affected group was those of 16-29 years old people--78.4%. The major way of the HIV transmission was the use of intravenous drugs (81.7%). Number of men i.v. drug users was two time greater then women's (88.9%/66.7%). At the time of HIV infection diagnosis 77.9% of patients were in the clinical category A, 16.9% in B and 5.2% in C (1993 classification system, CDC-Atlanta). During the control period seven persons were redistributed from the clinical category A to B and two from A to C. A high prevalence value for tuberculosis in AIDS (71.4%) has been observed, this infection being the first manifestation of stadium C in 57.1% of cases. The circulation of hepatitis viruses is high in the drug users community: the markers of VHC were detected in 84.2%, HBsAg in 17.1% and VHD antibodies in 9.2%.

[Characteristics of the gene pool of the Gagauz population of Moldova].

Population genetic data on Gagauzes from Moldova are reported for the first time. Blood groups AB0 and Rh and biochemical markers of genes HP, TF, GC, and PGM1 were determined in 190 Gagauzes. The following allelic frequencies were determined: AB0*0, 0.5241; AB0*A, 0.3279; RH*d, 0.4571; HP*1, 0.3544; TF*C1, 0.7472; TF*C2, 0.1770; TFC3, 0.0730; TF*B, 0.0028; GC*1F, 0.1025; GC*1S, 0.5932; GC*2, 0.3043; PGM1*1+, 0.5286; PGM*1-, 0.1000; PGM1*2+, 0.2607; and PGM1*2-, 0.1107. The data obtained indicate that the gene pool of Gagauzes is similar to those of neighboring southeastern European populations.

[Results of serologic screening for hemorrhagic fever with kidney syndrome in Moldova].

Results of serologic screening of small mammals for antigen of the virus of hemorrhagic fever with the renal syndrome (HFRS) and of testing human sera for antibodies to this virus in Moldova are reviewed. A total of 9946 animals belonging to 22 species and blood sera of 792 healthy donors were examined in 1983-1990. HFRS virus antigen was detected in 30 animals of 9 species, with the highest incidence of the antigen in gray voles. Two serotypes of Hantavirus were found to circulate in small mammals: Puumala and Hantaan. Screening of healthy humans revealed antibodies to HFRS virus in 7.8%. Natural foci of HFRS were detected in 23 of the 44 administrative territories of the Republic situated in 5 landscape geographical provinces.

[Genetic structure of the residents of Central Moldova].

Distributions of the marker genes for (1) blood groups ABO, Rhesus, MN, P, and Lewis; (2) some allotypes of G1m immunoglobulin; and (3) serum proteins Hp, Tf, and Gc; and (4) erythrocytic isoenzymes Acp and EsD were studied in 10 rural populations of central Moldova. For each local population, an empirical distribution of genotypes was obtained; and on this basis, allele frequencies, expected distributions of genotypes, and observed and expected heterozygosity were estimated. Significant differences between the studied populations with respect to variations of allele frequencies were found for the following loci: Hp, AcP, Gc, P, G1m, and Lewis. The results were compared with data on some populations of Western Europe and the CIS. Close relationships between indigenous Moldovans and other Romanic peoples, as well as the Russians and the Ukrainians, were demonstrated.

[The detection of antibodies to the hepatitis C virus and to HBsAg in donors with viral hepatitis and cancerous diseases in the Republic of Moldova].

Large-scale and uneven distribution of hepatitis C and B (HC and HB) among free blood donors of different regions of the Moldova Republic was established. The rate of detection of anti-HCV (South 6.2%, North 3.6%) correlated with that of HBsAg detection (south 12.5%, north 4.5%). A great role of HCV in the etiology of acute (among them posttransfusion) hepatitis and chronic hepatitis was demonstrated. A significant frequency of detection of anti-HCV (15%) in children treated in oncohematology wards and receiving multiple hemotransfusions as well as the detected cases of acute posttransfusion HC attest to active realization of this virus transmission in the transfusions of blood and/or its components. The necessity of urgent introduction into the Blood Transfusion Service of the republic of the examination of blood donors for HBsAg, alongside with their examination for the presence of anti-HCV and vaccination prophylaxis of HB in high risk groups is substantiated.

[The characteristics of salmonellosis in the Republic of Moldova at different periods of adverse epidemic situations].

The epidemiological analysis of salmonellosis morbidity in the Republic of Moldova for 20 years (1970-1989) was made. The analysis revealed that the time course of morbidity was characterized by several sharply defined periods of seasonal rises. The main epidemic features of Salmonella infections at the periods of unfavorable epidemic situation were established and their essential differences shown.

[An analysis of group salmonellosis morbidity in the Republic of Moldova].

The epidemiological analysis of data on the outbreaks of Salmonella infections registered in the Republic of Moldova in 1983-1989 were analyzed. Significant differences in the epidemic parameters of the outbreaks occurring in 1983-1985 and in 1986-1989 were revealed. The deterioration of the epidemiological situation in recent years was shown to be linked with the alimentary route of the transmission of the infective agent with the increased circulation of S. enteritidis among agricultural animals and poultry.

Hepatitis C virus genotypes in different regions of the former Soviet Union (Russia, Belarus, Moldova, and Uzbekistan).

The prevalence of HCV genotypes in four republics of the former Soviet Union (Russia, Belarus, Moldova, and Uzbekistan) was investigated. Overall, 197 HCV isolates from 66 blood donors and 131 patients with chronic hepatitis were typed. Viral sequences from sera of infected subjects were amplified by nested RT-PCR using primers from the core region and typed by one or two techniques: (1) DNA enzyme immunoassay (DEIA) and (2) PCR with a set of type-specific primers. Only three major HCV genotypes were identified in this study population. HCV 1b was found to be the predominant virus type both among blood donors and chronic hepatitis patients, followed by 3a, 2a, and 1a (chronic hepatitis patients: 1b-82%; 3a-10%; 2a-4%, 1a-5% and 2c-1%; blood donors: 1b-77%; 3a-17%; and 2a-6%). No significant difference in genotype distribution was observed between different countries or between blood donors and chronic hepatitis patients within the same country. Results of the genotyping procedures were confirmed by direct sequencing of 216 nt PCR fragments corresponding to part of HCV core gene. Phylogenetic analysis of HCV 1b sequences from this study and from the Genbank demonstrated that the sequences from the former Soviet Union do not form evolutionary lineage(s) different from those of strains of the same subtype circulating in other geographical regions.

[S allele of the L-myc oncogene is associated with lung cancer metastases in patients from Moldova].

Race is widely believed to be a factor in the relationship between S allele of L-MYC oncogene and disseminated lung cancer. In particular, the clinical significance of L-MYC genotype was demonstrated in the Japanese while the results for the white US, Australian and Norwegian cohorts were negative. The present study was concerned with distribution of L-MYC oncogene alleles in 43 patients with lung cancer and 77 healthy subjects in Moldova. L and S allele frequency in both groups were nearly identical. However, the SS genotype was registered much more frequently in patients with metastasis (10/28; 36%)(p < 0.05) than in those with localized tumor (0/12). Moreover, overall frequency of S allele was significantly higher in lung cancer patients with node involvement (35/56; 63%)(p < 0.02) than in those with localized tumors (8/24; 33%)(p < 0.02). Finally, a significant correlation was found between S allele occurrence and distant metastases (M1: 19/28; 68%; M0:26/58; 45%)(p < 0.05). Similar data were reported in Russia. (ABSTRACT TRUNCATED)

Comparative studies of P- and hobo-element activity, fitness components and recombination parameters in two natural populations of Drosophila melanogaster in Moldova.

The genomic frequency of crossovers, fitness components, and activity of P- and hobo-transposable elements were studied in two natural populations of Drosophila melanogaster from regions with different pesticide pressure (forest and orchard populations). A significant increase in rf (recombination frequency) in the orchard population (as compared with the forest population) has been found within segments al-b, b-cn, cn-vg (chromosome 2) and for some chromosome 3 segments, th-st, st-cu, cu-sr, sr-e. None of the populations exhibited P-properties. However, the forest population showed weak hobo activity. Both populations contained probably defective hobo copies. With respect to average fitness (as estimated using a compound-autosome strain) and fecundity, the forest population was superior to the orchard one, whereas the latter had a higher locomotor activity and resistance to etherization. The difference in rf between the populations may be due to differences in their environmental conditions. Selection for pesticide resistance may favor the propagation of genotypes with higher rf, thus, possibly, ensuring release of adaptively significant genetic variation.
