Morbid changes of the mandible in familial polyposis of the colon.

The study was made to ascertain whether the radiographs of the mandible of patients with familial polyposis of the colon show osteomas. Orthopantomography of the mandible was taken in 10 cases of familial polyposis of the colon and 6 cases of the unaffected family members. And radiographic and histological studies were performed in another autopsied case. As a result, radiographic studies revealed sclerotic masses of the mandible in 10 of the 11 cases. In some cases, the changes were also found in the maxilla. The number of sclerotic masses ranged from 2 to 11, and they were associated with odontomas in 2 cases and impacted teeth in one case. Two of the 6 unaffected family members were found to have one or two sclerotic masses. At autopsy of a case died of cancer of the sigmoid colon, a radiographic study of the excised mandible revealed 11 sclerotic masses measuring 5 mm in diameter each, as well as one impacted tooth. A bucco-lingual cross action through the mandible in the canine area revealed central osteomas. Histological examination showed the osteomas were composed primarily of mature, lamellar bony tissue.

[The role of intravenous digital subtraction angiography in the detection of the cause of cerebral ischemia].

A comparative analysis of two angiography methods applied in two unrelated groups of patients with brain ischemia symptoms is described. The first group of 452 patients suffering from extracranial cerebral artery disease was analyzed by an intravenous digital subtraction angiography (DSA); the second group of 500 patients was subjected to a conventional aortic arch angiography. Research was based on comparative analysis of angiograms as per type and location of morbid changes. In the area of common carotid artery and internal carotid artery the stenosis was considerably more frequently detected by the conventional angiography than by the intravenous DSA, i.e. in 105 (20.0%) as compared to 40 (8.8%) patients, respectively. Similar difference is indicated in a total number of morbid changes in these arteries detected in 248 patients (49.6%) by aortic arch angiography and in 154 (34.1%) patients by intravenous DSA. The dissimilarity is noticed with minor atherosclerotic changes, while medium and major stenoses are detected with equal frequency by both methods. The intravenous DSA more often indicated flexion of carotid artery, and considerably more often the vertebral artery flexion as found in 53 (11.7%) patients analyzed by intravenous DSA compared to 33 (6.6%) patients analyzed by conventional angiography. Advantage of intravenous DSA is less complications and ambulatory performance. The conventional angiography better indicates morbid changes in common carotid artery bifurcation and vertebral artery origin.

[Acute poisonings with Gramoxone (paraquat)].

The first two cases of acute intoxications with gramoxone (Paraquat) in P.R. of Bulgaria are described--a female of 34, in Toxicated by inhalation and male of 71, swallowed the poison per os. The author finds certain peculiarities in the disease course, differing from the literature data. No morbid changes developed in the lungs of the two patients, which, according to the foreign authors, were characteristic for that kind of intoxications. On the other hand--serious lesions of the upper-respiratory ducts and disturvance in their possibility were observed in the patient with the per os intoxication. The late development (6ty day) of morbid changes in the liver of the same patient are stressed upon, regardless the energetic preventive treatment with hepatoprotective remedies. The necessity of continuous follow-up of liver state and the respective treatment are also stressed.

Adjuvant immunotherapy with bacillus Calmette-Guérin in prostatic cancer.

Ninety-two patients with histologically proved prostatic cancer were studied. Forty-six patients were treated with bacillus Calmette-Guérin (BCG) adjuvant immunotherapy, and 46 other patients, matched for stage and hormone therapy, served as controls. Survival from the time of histologic diagnosis was sixteen and one-half months longer (thirty-seven and one-half versus twenty-one months) in the BCG-treated patients. There was no mortality and only minimal morbidity. Changes in some immunologic parameters (white blood cell count, skin tests) suggest an immune response.

The influence of dietary mackerel oil on the condition of organs and on blood lipid composition in the young growing pig.

A feeding experiment was carried out in which piglets were fed to a diet enriched with either mackerel oil or olive oil. The oil consumption amounted to about 100 g per animal per day. The aim of this experiment was to study the effect of feeding high amounts of fish oil rich in polyunsaturated and long-chain monoenoic acids in order to determine if any morbid changes occurred in the animals as a result of this addition. The piglets fed olive oil served as controls. After 4 weeks, blood hemoglobin, plasma glucose, and serum triglycerides were significantly lower in the mackerel oil group as compared with the control group. There was no difference in serum cholesterol and serum lipid composition. Very low density lipoproteins were lower in the mackerel oil group. The fatty acid composition of blood serum, heart muscle, and liver showed considerable differences, omega3 acids being higher and both omega6 and omega9 acids being lower in the mackerel oil group than in the control group. Some increase in the amount of triglycerides in the heart muscle was observed in the mackerel oil group. Some characteristics of "yellow fat disease" developed in the mackerel oil group. This type of vitamin E deficiency seems to be the result of the considerable amount of omega3 fatty acids present in the mackerel oil. No clinical symptoms due to ingestion of long-chain monoenoic acids were observed.

A prospective study of liver enzyme and other changes following repeat administration of halothane and enflurane.

A prospective study of liver enzymes and other measurements following repeat administrations of halothane or enflurane was carried out in patients undergoing minor urological operations. The patient populations were similar with respect to frequency of factors which might influence liver function, social habits, drug therapy and time intervals between administrations. Sixty-three received two or more administrations of halothane and 66 received two or more administrations of enflurane, both drugs given with nitrous oxide in oxygen. There was a greater frequency of increased enzymatic activity following repeat administrations of halothane than following enflurane and the average alanine aminotransferase and gamma glutamyl transpeptidase concentrations were increased to a greater degree following halothane than enflurane. There was no change in the eosinophil count and no significant postoperative morbidity. Change in alanine aminotransferase, lactate dehydrogenase and gamma glutamyl transpeptidase occured more frequently in obese patients receiving halothane.

A review of the evidence concerning the impact of medical measures on recent mortality and morbidity in the United States.

Because it still is widely believed that one deadly disease after another is being eliminated, or diminished, largely because of medical interventions, there is little commitment to social change and even resistance to a reordering of national priorities. In this article we examine the contribution of medical measures to recent mortality changes in coronary heart disease, cancer, and stroke, which together account for two-thirds of total U.S. mortality and consume the vast majority of available resources. Morbidity changes are also examined and found to be not declining in a manner congruent with mortality and, in fact, increasing for some subgroups. Using a combined measure of mortality and morbidity (the probability of a life free of disability), it is demonstrated that although overall life expectancy has increased over several decades, most of this increase is in years of disability. Our late 20th century approach to the emerging AIDS pandemic (the frantic search for a "magic bullet"--either a treatment or a vaccine) belies any suggestion that the arguments and data presented concerning the modest contribution of medical measures are now passé.

Disablement in society: towards an integrated perspective.

Demographic and morbidity changes have undermined traditional responses to disease and disability. An expanded model of disablement is required, integrating consequences as well as causes. Three main concepts involved are impairment, disability, and handicap. These constitute a continuum ranging from bio-medical phenomena (impairment) through functional limitation and activity restriction (disability) to social disadvantage (handicap). Though complete sequences can occur, analytical and practical problems arise from the fact that their interrelationship is not always straightforward or predictable. From an analysis of available data it is calculated that 34% of the adult population in Great Britain are impaired. A tenth of these are severely disabled. The age structure and underlying causes of disablement challenge prevailing stereotypes: disablement becomes more frequent with age and it is caused in large part by chronic diseases. An attempt to assess handicap direct, rather than as a derivative of disability, displays severe disadvantages among the disabled. The model allows the possibility of gathering information relevant to different aspects of disablement and warns against reductionist or naive responses in health care and social policy.

Dermatological aspects of so called vinyl chloride monomer disease.

The pathological findings revealed in 30 subjects with so called vinyl chloride disease has been presented. The most often observed symptoms were: changes in histopathological picture of the skin, history of the symptoms of Raynaud's phenomenon, positive results of functional proofs (pletysmography, vibration perception test, cooling test), and scleroderma like skin lesions. Acroosteolysis was found only in 3 cases, thrombocytopenia only in one. The constellation of singns may be related to the high level of exposure of examined persons. The authors point to the significance of dermatological methods of investigation for detection of morbid changes induced by VCM action.

Idiopathic pulmonary fibrosis. A rare cause of scintigraphic ventilation-perfusion mismatch.

A case of idiopathic pulmonary fibrosis with multiple areas of mismatch on ventilation-perfusion lung imaging in the absence of pulmonary embolism is presented. Idiopathic pulmonary fibrosis is one of the few nonembolic diseases producing a pulmonary ventilation-perfusion mismatch. In this condition, chest radiographs may not detect the full extent of disease, and xenon-133 ventilation imaging may be relatively insensitive to morbid changes in small airways. Thus, when examining patients with idiopathic pulmonary fibrosis, one should be aware that abnormal perfusion imaging patterns without matching ventilation abnormalities are not always due to embolism. In this setting, contrast pulmonary angiography is often needed for accurate differential diagnosis.

[Epidemiologic characteristics of viral hepatitis. I. Analysis of registered cases of viral hepatitis in Vojvodina].

According to reported morbidity rate, viral hepatitis still represents a major epidemiological problem in Voivodina, as well as in the entire country, although some of the epidemiological features of these diseases are changing in the course of time. The present paper contains an analysis of morbidity, changes in the age distribution, the degree of differentiation of the viral hepatitis, participation of hepatitis A and B in the total number of the differentiated cases, as well as their seasonal occurrence.

WHO MONICA Project: assessing CHD mortality and morbidity.

In the WHO MONICA Project mortality and morbidity from acute myocardial infarction is being monitored in 39 Collaborating Centres in 26 countries for a period of 10 years. The myocardial infarction registration procedures have been standardized and suspect coronary events are classified into diagnostic categories according to common criteria. This paper presents ischaemic heart disease mortality figures, based on routine statistics for the study populations and discusses cross-sectional comparability of morbidity data from event registration. Because of differences in availability of data used for diagnostic classification of events (such as autopsy data) the proportions of different diagnostic categories vary from centre to centre. There are, therefore, problems in cross-sectional comparison of morbidity data between the centres. The primary target of event registration in the project is to monitor morbidity changes within centres during the ten-year period. This goal should not be affected by the problems described in this paper.

[Hepatorenal lesions in acute acetone poisoning].

Lesions of the parenchymal organs, not described so far, were found in two males and two females with acute acetone intoxication, besides the well known morbid changes in the gastrointestinal tract and ZNS, regardless the energetic hepato--and nephroprotective treatment. In all patients (one of them inhaled acetone vapours and the rest swallowed the poison per os) liver lesion is present with the corresponding clinical manifestations and changes in the laboratory liver indices. In two of the per orally intoxicated, renal lesion was also found, manifested in a milder degree. The state of the parenchymal organs is necessary to be followed up carefully in case of acute acetone intoxications and a timely treatment with hepato- and nephroprotective drugs in large doses to be applied: glucocorticoids, glucose and levulose solutions, cocarboxylase, lipovitan, vitamin "C", "B1", "B2", "B12", calcium gluconate.

[Critical assessment of angiographic findings in malignant tumors of the extremities (author's transl)].

Five selected cases out of a total of 36 patients who had been investigated by angiography for tumors of the extremities are demonstrated in order to show the difficulties which can arise in the interpretation of their angiograms. In the subsequent discussion, the classical criteria of Dos Santos for malignant tumors are analyzed; in agreement with other authors, we state, that only unequivocal tumor vessels are characteristic of malignant processes. With certain reservations - especially in the differentiation of malignant and inflammatory processes - the parenchymal phase and the functional behaviour of the circulation permits conclusions to be drawn on the nature of the morbid changes.

Enhancement of survival in acute experimental fowl typhoid in chicks by the administration of iron dextran.

Chicks aged 15 days were infected orally with 10(6-4) living Salmonella gallinarum. Iron from iron dextran or ferric ammonium citrate (DFe and CFe respectively), in doses of 2-0 or 0-4 mg/kg given intramuscularly at the time of infection, had no effect on subsequent mortality compared with infected controls. Increasing the amount of CFe to the maximum (20 mg/kg) that was non-toxic intramuscularly was also ineffective but when the dose of DFe was increased to 20 or 50 mg/kg the survival rate rose sharply. If given at other times in relation to the time of infection, or more frequently, 50 mg/kg or more of DFe was less active. The increased survival rate among infected chicks given 50 mg/kg of DFe at the time of infection was accompanied by decreases in the severity of the morbid changes in the disease, and by reductions of between c 10- to 100-fold in the numbers of viable S gallinarum in the liver, spleen or blood at the height of the acute infection. DFe (50 mg/kg) given at the time of infection also eliminated the difference in the survival rate noted previously between infected chicks fed respectively with diets containing fish or meat meal as the sole source of supplementary protein.

Patterns of demographic change in the Americas.

Considerable scholarly debate has focused on the nature of demographic change in the Americas before and after 1492. Recent research on human skeletal samples and related archeological materials suggests that morbidity and mortality were increasing throughout much of the Western Hemisphere before 1492 in response to increased population density, increased sedentism, and changing subsistence. The evidence suggests that after 1492 population reduction was caused not by continental pandemics but by localized or regional epidemics augmented by social and economic disruption. The twentieth century has witnessed remarkable Native American population recovery, fueled both by improvements in health care and changing definitions of "being Indian."

Trends in perinatal and neonatal mortality and morbidity in India.

Although the infant mortality rate (IMR) has reduced by 50% during the past century, it compares poorly with the advanced countries and some developing countries. The observed fall in IMR has been mostly in post-neonatal mortality, with the result that neonatal deaths now account for over 60% of all infant deaths. The overall perinatal mortality rate (PMR) in India is still over 50 per 1000 and has shown virtually no decline during the past decade, However, PMR differs widely in different states, urban/rural areas, different hospitals and so on. PMR is seen to correlate better with social development than economic development of the representative community. The causes of perinatal deaths suggest poor health of mother and poor health facilities and are hence potentially preventable. Various studies have shown that PMR can be significantly reduced within a short span of time. The registration of vital statistics continue to be highly unsatisfactory especially in rural areas.
