Histological features of the palatal mucosa in denture sore mouth. Densities of mast cells in the lamina propria and histological features in the epithelium of the denture bearing and non-denture bearing human palatal mucosa in comparison with patients suffering from denture sore mouth.

A histological investigation was carried out on biopsy specimens taken from patients suffering from denture sore mouth. The results were compared with those obtained in previous investigations on denture and non-denture-wearers. The sections were studied by standardized quantitative morphometric methods. After 4 years the denture bearing palatal epithelium from both groups, the denture wearers and the patients suffering from a denture sore mouth did not show changes in mean thickness of the epithelium from both groups, the denture wearers and the patients suffering from a denture sore mouth did not show changes in mean thickness of the epithelium compared with the controls. The mitotic index in denture bearing epithelium from patients suffering from denture sore mouth was three times lower than in the epithelium of the normal denture wearers. The three groups of patients investigated did not show differences in density of mast cells in the lamina propria of their palatal epithelium.

[Method of correction of microstomia and deviation of the mouth angles by rotation of trapeziform and triangular flaps of the mouth mucosa].

In the paper, the author suggests an essentially new method of dilation of the oral orifice and elimination of mouth angles deviations basing on the principle of new redistribution of local tissues by means of rotation of a trapeziform graft of the mucosa with its submucous layer from the buccal and adoral region. In this method one uses more adequate tissues, which reserves in case of microstomia are located in the buccal and adoral region. The successful utilization in 60 patients of the suggested method of elimination of microstomia arising due to different causes enabled the author to recommend it widely for reparative surgery.

Custom finger guard-mouth prop as an alternative to conventional mouth props.

Conventional mouth props are an essential part of a dentist's armamentarium and are irreplaceable in certain dental situations. These props, however, may provide too harsh an approach or be injurious in other situations. The custom-made finger prop is a valuable alternative to the other mouth props when treating the young or handicapped.

[Results of primary and combined radiotherapy of mouth carcinomas (tongue, tonsil, maxilla, mandible, palate, floor of the mouth cheek mucosa)].

Responses to highvoltage radiotherapy of 320 patients with carcinomas of the oral cavity are discussed. Five year survival rate according to the stage of the, disease were the following ones: in carcinoma of the anterior 2/3 of the tongue and floor of the mouth 20-60%, buccal mucosa, upper and lower jaw 30-50%, base of the tongue 26% and tonsil 30-100%. After simultaneous irradiation of the efferent lymphatics in parimary N0 patients, whose primary tumor subsequently was under control, a significantly lower frequency of metastases to the lymph nodes was found than without coirradiation of the lymph flow. Clinically verifiable metastases of the lymph nodes are also controllable to a high percentage by radiotherapy alone: Only in a little more than five per cent of the primary N1-3 patients with the parimary tumor being under control, additional metastases of the lymph nodes developed later on. The complication rate was 6.8 per cent. Most frequently, osteonecrosis was involved, indeed occurring chiefly in cases, where a primary skeletal manifestation of the tumor already was existing.

[Impression taking in the edentulous mouth with special regard to closed mouth impression with simultaneous determination of the jaw relation].

In view of the increase in mean life expectancy and in the need for care in the gerostomatological sphere, more attention must be paid to such methods which optimize the construction of full dentures from the anatomico-pathological viewpoint, exert a rationalizing effect, and are likely to improve the success rate in the treatment of edentulous patients. The method of closed mouth impression and simultaneous determination of the jaw relation refers to well-proved principles of impression making, considers new findings concerning the determination of the jaw relation, incorporates modern materials and instruments, and forms the system character of clinico-technological measures.
