Application of stereologic methodology to inflammatory infiltrates residing in oral mucous membrane lesions. Structural and morphometric comparison of benign mucous membrane pemphigoid and reticular lichen planus.

In the present study, stereologic procedures are employed to estimate quantitatively the composition of chronic connective tissue infiltrates characterizing various oral mucous membrane diseases. One case each of benign mucous membrane pemphigoid (BMMP) and reticular lichen planus (RLP) served as examples to demonstrate the applicability of this methodology. In both lesions which are described clinically, histologically and on the ultrastructural level and which display a markedly different infiltrate composition, fibroblasts were affected and either proliferated (BMMP) or became cytopathologically altered (RLP). The quantitative data presented, besides providing new information, argue in favour of the stereologic approach.

Resistance of Bordetella pertussis phase I to mucociliary clearance by rabbit tracheal mucous membrane.

Cultivated mucous membrane of rabbit trachea was point inoculated with Bordetella pertussis phase I or III. Phase I (virulent) bacteria were found to be infective at the point-inoculated site, but phase III (avirulent) bacteria rarely showed such behavior. After inoculation, homogenized segments of mucous membrane were spread on plates. Large numbers of phase I bacteria were recovered from the inoculated segment; however, the laryngeal segment was the site of recovery of large numbers of phase III bacteria. The difference was not due to the ciliostatic bacterial toxin. After coinoculation with phase I and III bacteria, phase III bacteria were recognized at the laryngeal end, whereas phase I bacteria were recognized at the inoculated site. Therefore, the adherence of the bacteria to the ciliated epithelium is considered to be the most probable mechanism of the resistance to the mucociliary stream. Scanning electron microscopy facilitated visualization of phase I bacteria adhering to the inoculated site.

[The reactions of human middle ear mucous membrane (author's transl)].

Healthy middle ear mucous membrane of newborns differs from the respiratory mucous membrane of the respiratory tract, in that it lacks a mucociliary epithelial pattern and because it has an absence of cells responsible for immunological resistance. Irritations over a short period of time and intensity cause a mucous membrane edema, which leaves behind, when the irritation disappeares, no permanent morphological changes in the structure of the mucous membrane. Severe irritations over a long period of time cause a characteristic change in the epithelium and submucosa: the single layered flat epithelium is replaced by respiratory epithelium; in the submucosa a proliferation of the connective tissue occurs simultaneously with the development of a local immunologically potent, cellular defense system. The actively secretive epithelial cells serve as the means whereby the antibodies are transported to the surface of the mucous membrane. Whereas at the time of mucous membrane edema, serum components (transsudate) are the primary source of the resulting "serotympanum", an increase in viscosity of the mucous allows one to recognize the active secretive work of the metaplastic epithelium. The biochemical composition of the various effusions givens a direct a direct indication of their origin: it is, however, no key to the cause! Only in the case of purulent secretions is it possible to recognize the cause by cellular or bacteriological identification. The same is valid for the norphological changes of the middle ear mucous membrane since the membrane will generally react in a similar manner, even though the types of irritation differ. Basement membrane changes under neoplastic oral mucous membrane. Ultrastructural observations, review of the literature, and a unifying concept.

This study explores the morphologic nature of the basement membrane under neoplastic oral mucous membrane. Of particular interest is the progression of changes associated with dysplasia, carcinoma in situ, and invasive carcinoma. On the basis of a critical review of the literature and our own ultrastructural observations, we present a unifying concept for the development of these basement membrane changes. This concept proposes that neoplastic epithelial cells may produce a collagenolytic enzyme which is released into the epithelial intercellular spaces. This enzyme permeates to the basement membrane, causes breaks in the basement membrane, and focal loss of stromal area contiguous with intercellular spaces. The neoplastic basal cells develop pseudopodia that eventually extend through the breaks in the basement membrane. This concept suggests that the basement membrane changes herald the progression of carcinoma in situ to invasive carcinoma.

An unusual case of benign mucous membrane pemphigoid.

This case of benign mucous membrane pemphigoid (BMMP) is unusual in that blistering, scarring lesions were confined to the skin for 15 years before mucous membranes were involved. The onset of this disorder at the age of 38 is also unusual. Detailed immunological investigation was performed on this patient but the results in no way clarify the present confusion regarding the immunopathological processes in BMMP related to those operative in bullous pemphigoid.

Deafness complicating mucous membrane pemphigoid: a case report.

A patient with moderately severe mucous membrane pemphigoid is described. The onset of deafness was associated with the appearance of postero-superior retraction prockets in both eardrums. It is postulated that this was due to mucous membrane pemphigoid involving the mucosa of the middle ear. We are not aware of this complication having been described previously.

Pili-mediated attachment of Corynebacterium renale to mucous membrane of urinary bladder of mice.

Corynebacterium renal strain 115 with numerous pili became attached in vivo to the mucous membrane of the urinary bladder of mice 10 to 30 times more frequently than did that of C renale American Type Culture Collection 19412, which showed few pili. Antipili serum-treated C renale strain 115 was not recovered from the membrane in as large amounts as was untreated bacteria. Antisomatic serum-treated strain 115, on the other hand, was recovered from the membrane in amounts similar to untreated bacteria. Untreated organisms became attached to the mucous membrane of the urinary bladder more effectively than did the antipili serum-treated bacteria, as seen on scanning electron micrographs. It may be concluded that C renale strain 115 attaches itself to the mucous membrane of the urinary bladder om mice by the pili.

Benign mucous membrane pemphigoid: immunopathological study of three cases.

Three cases of benign mucous membrane pemphigoid were investigated by direct and indirect immunofluorescence. The findings of the direct immunofluorescent examination of perilesional tissue are studied in detail. Finally the relationships between bullous pemphigoid and benign mucous membrane pemphigoid are discussed.

Realization of genetic information programming the synthesis of pepsinogen-pepsin in the mucous membrane and tumors of the stomach in man.

Up to 19--20 fractions of separate RNA species and groups have been revealed in malignant tumors of the stomach and mucous membrane of patients with stomach cancer, ulcer and polyposis of the stomach by means of the method of analytical and preparative electrophoresis in 2.5% polyacrylamide gel. A more intensive incorporation of 14C uridine both into the nuclear and separate fractions of cytoplasmic RNA was observed in stomach tumors in comparison with the stomach mucous membrane of man. Pepsinogen-pepsin was synthesized by bound polysoms of the stomach mucous membrane. In stomach malignant tumors of man the polysomes were not capable of synthesizing this enzyme. Fractions of messenger RNA with sedimentation constants 16S-17S, possessing the ability to stimulate pepsinogen-pepsin synthesis in vitro, have been isolated from cytoplasmic RNA of a pig stomach mucous membrane.

Effects of local anesthetics on tactile sensitivity thresholds for cutaneous and mucous membranes.

New, reliable, and precise methods for measuring absolute pressure sensitivity in mucous membranes and on intact skin are described. Studies were conducted to determine how local anesthetics (phenol and sodium phenolate and Benzocaine) affect tactile sensitivity thresholds in the oropharynx and on the intact skin of the volar surface of the forearms, ankles, knees, elbows, and dorsum of the hands. Gargling and expectorating a solution containing phenol had a significantly greater anesthetic effect on the mucous membranes of the oropharynx than spraying and swallowing, which, in turn, had a greater effect than drinking the solution. Compared with ethanol and petroleum jelly (Vaseline), the topical application of Benzocaine (0, .5, 1, 3, and 5% solutions in 100% ethanol) significantly increased the tactile sensory thresholds on the volar surface of the forearm. A highly significant loss of tactual sensitivity of the oropharynx was demonstrated in smokers.

[Mucous membrane of the mouth and prostheses in old people (author's transl)].

The majority of people of advanced age at present have either very many gaps in their dentures or they have no teeth at all. For this reason they can only wear broad-base plate-type bridged, which can lead to pathological changes of the mucous membrane. The author uses the complex etiology of prothetic stomatopathy, in which not only changes in the mucous membrane of the mouth associated with ageing but also hormonal changes (menopause), general diseases and oral candidiasis play a role, to illustrated the necessity of collaboration between stomatologists and geriatricians. Oral and prosthetic hygiene degenerates with increasing age. The average period of wearing prostheses also increases with the age and the danger of chronic mucous membrane damage also increases. But since old people seldom if ever consult a stomatologist, every geriatrician should also have a look at the mucous membrane of the mouth when examining old patients.

Epithelium--connective tissue interface of oral mucous membranes.

The interface between the epithelium and connective tissue of mucous membranes and skin represents a tissue site that is affected by a host of diseases which alter its normal morphologic features. Many differences exist in the normal morphologic pattern of this interface between various anatomic sites. It is essential, therefore, to have a thorough understanding of these differences before a meaningful study and assessment of pathologic changes affecting the interface can be made. This study was undertaken to establish a set of morphologic standards which can be used as a basis for interpreting pathologic changes which occur at the epithelium--connective tissue interface of oral mucous membranes.

[Influence of reserpine on the nasal mucous membrane (author's transl)].

The influence of reserpine on the nasal mucous membrane in the various conditions of its autonomic denervation was investigated in the rabbits. Histochemical analysis of a acetylcholine esterase and quantitative measurements of acetylcholine were performed. The results point out that reserpine influence is found only on the vascular elements in the mucous membrane and not on the subepithelial glands. Analogically it was concluded that in some patients with arterial hypertension who were treated with reserpine, its side effect, nasal obstruction, was caused only by vasodilatation. In rhinitis vasomotorica, in the addition to the vasodilatation the abounding secretion of subepithelial mucous glands is present. The performed experiments also point out that resection of the vidian nerve in resistant cases of vasomotor rhinitis with aboundant secretion is reasonable.

A study of mitotic activity and the diurnal variation of the epithelial cells in wounded rectal mucous membrane.

Excision ulcers of the rectal mucous membrane were made in two groups of rats. One group was wounded at 09.00 hours and the second group at 21.00 hours. Mitotic counts were carried out in the glandular epithelium at the ulcer edges at 2 hour intervals over a period of 24 hours. Mitotic activity increased in 2-4 hours and thereafter showed a peak-and-trough pattern. The wounded rectal epithelial cells exhibited a diurnal variation with a peak of activity during the day and a low period of activity at night. It would seem unlikely that the adrenaline-chalone complex acts on the rectal epithelium, as this would entail maximal mitotic activity during periods of rest, when the circulating levels of adrenaline in the rat are at their lowest. The experiments clearly showed that the diurnal variation was not abolished by wounding. The increased mitotic activity occurred in the epithelial cells in the lower and mid thirds of the colonic glands; dividing cells were rarely seen in the top twenty cells or so of the glands, or in the surface epithelium. Mitotic activity was often lower in the first one or two glands at the immediate wound edge, which is difficult to explain by present theories of mitotic control.

Microscopic and ultramicroscopic alterations of the respiratory mucous membrane in experimentally provoked bronchitis.

The noses of four experimental dogs were surgically obstructed, and thus their entire respiration was disturbed, and this affected the mucous membranes of the lower respiratory passages. Normal histological examinations and electronic microscopy found chronic irritation of the bronchial mucous membranes. It was possible histologically to follow the most subtile alterations on the membranes from the very beginning of the chronic bronchitis. The importance of nasal obstruction as a contributing factor of chronic bronchitis is stressed.

[Peptic ulcers evoked by retention of the antral portion of mucous membrane at the duodenum after a Bilroth II stomach resection].

From the authors' data the retained mucous membrane of the antral region at the duodenum following gastric resection for peptic ulcer results in the development of gastric ulcers. It has been found that only the removal of the retained antral mucous membrane stops the production of HCl and promotes the healing of ulcer. The reflux of the duodenal contents into the isolated pylorus does not constitute a cause of the gastrin production, the continuation of which is mainly due to the vagal denervation of the glands producing the antral hormone.

Tobacco smoking habits and attitudes of Australians with oral mucous membrane keratoses.

The purpose of this study was to ascertain whether or not a relationship existed between the presence of oral mucous membrane keratoses and tobacco smoking habits. Two groups of tobacco smokers, one group exhibiting oral mucous membrane keratoses, and the other no such changes, were chosen for their similar smoking habits. Detailed tobacco smoking histories of each group were ascertained. Similar results for both groups, in terms of the number of years of smoking, quantity of tobacco consumed dialy, type of tobacco smoked and the method of inhalation and exhalation were found. Eighty per cent of patients wanted to stop smoking with the fear of cancer being the major reason in the subjects with oral keratoses and the continued cost of smoking the major reason in the control subjects. In the affected patients the prevalence of white patches was not related to the quantity of tobacco consumed but rather to the number of years the subject had smoked.
