Epidermal nicotinamide adenine dinucleotides in psoriasis and neurodermatitis (lichen simplex hypertrophicus).

Epidermal nicotinamide adenine dinucleotides were measured in subcorneal and basal epidermal layers in patients with psoriasis and neurodermatitis and in healthy controls. Lowry's microtechniques utilizing enzymatic cycling were used. The total NAD content in these groups was except for the involved psoriatic skin about 1.1 mmoles/kg dry weight. Between 40 and 50% of this content consisted of the reduced form. In the involved psoriatic skin the total NAD content was increased to 1.5 mmoles/kg, this increase being mainly due to a rise in NAD+. There was no difference in NAD content between epidermal layers in the various groups studied. The total NADP content was near 0.15 mmoles/kg in healthy controls and in patients with neurodermatitis. The subcorneal layers contained 10% more of the dinucleotide than the basal layers, but in the two layers the reduced form amounted to about 80% of the total. In both non-involved and involved skin of the psoriatic patients the total NADP content was significantly increased above the control level, by up to 20% in the former and up to 65% in the latter. In the two layers studied the NADP+ content was increased by about 75% in both non-involved and involved areas. In contrast, the NADPH content rose only in the basal layers of the lesion, by 55%. The increased levels of NADP+ and NADPH found in psoriasis might suggest an accelerated or differently conducted NADPH dependent biosynthesis in this disease.

Neurodermatitis and intralesional steroids.

The present work describes the results of treatment of 30 cases of neurodermatitis circumscripta by intralesional injections which were given by Dermo-Jet. 15 cases were treated with triamcinolone acetonide (0.25 mg/cm2 of the skin) and the rest with aqua distillata every week for 8 weeks and were followed for another 4 weeks. The results of treatment were excellent in 66% of triamcinolone treated cases and 46% of aqua distillata treated cases. The difference in the response was statistically insignificant. So we conclude that aqua distillata might have worked as suggestion therapy in emotionally labile individuals.

[Atopic neurodermatitis, basic concepts in pediatrics].

Atopic neurodermatitis (AND) is the first or second most frequent skin diseases in childhood. It represented 15% of all skin diseases seen at the Hospital del Nino, IMAN. The clinical and epidemiologic aspects of AND, the different hypothesis of its pathogenesis, management and treatment are discussed. The importance of emotional factors in AND have been over stressed, resulting in therapeutic abuses of psychyatric drugs and psychotherapy.

Neurodermatitis: a skin condition that affects surgical wound healing.

Neurodermatitis, or lichen simplex chronicus, generally found in females between the ages of 25 and 55 years, is highly inflammatory and may become generalized. The patients are hypersensitive and are not good candidates for surgery as a Köbner-like phenomenon may occur at the site of the incision with consequent breakdown of the incision, wound dehiscence, and infection. If these patients are to respond normally to treatment, it is sometimes necessary for the surgeon to treat their tensions.
