On the mechanism of action of the antianginal drug nonachlazine on ischemic myocardium.

The activity of a new antianginal drug, nonachlazine, synthetized in the Institute of Pharmacology, Academy of Medical Sciences of the USSR, has been demonstrated using model myocardial ischemias on anesthetized dogs and conscious cats. Antianginal activity was evaluated by ECG, epicardial electrogram, lactate level, and lactate/pyruvate ratio in the venous blood flowing from the ischemic myocardial area. The study of the cardiotropic effect of nonachlazine provided the following findings: (1) nonachlazine enhances ino- and chronotropic functions of the heart via stimulation of its beta-adrenergic receptors; (2) nonachlazine's positive chronotropic effect is substantially less marked than the inotropic one; (3) nonachlazine decreases the intensity of chronotropic reactions of the heart induced by isopreterenol. Biochemical analysis showed that in addition to its activation of oxidative phosphorylation, the ability of nonachlazine to stimulate glycogenolysis is also of importance in the development of its antianginal effect. This conclusion has been suggested by the following: (1) in acute myocardial ischemia, nonachlazine decreased lactate level and increased ATP level up to the norm; (2) at day 3 after ligation of the coronary artery, nonachlazine did not change lactate content, increased ATP and NAD, and decreased NADH2; (3) in experiments on rabbit myocardial mitochondria in vivo and in vitro nonachlazine was found to stimulate oxidative phosphorylation; (4) nonachlazine was found capable of increasing the norepinephrine level and of increasing phosphorylase a activity and the rate of glycogenolysis.

[Effect of nonachlazine on transmembrane ionic currents in the frog atrial trabeculae].

The effect of the antianginal drug nonachlazine displaying antiarrhythmic properties on transmembrane ionic currents in the frog atrial fibers was studied in experiments on isolated trabeculae of the frog atria. The transmembrane ionic currents were measured by a voltage clamp technique based on a double sucrose gap arrangement. Nonachlazine (1.03 X 10(-5) mol/l) decreased the amplitude of the fast inward current whatever the magnitude of membrane potential. The drug inhibited the slow inward current and prevented the adrenaline-increased permeability of the slow sodium-calcium channel if external sodium ions were replaced by choline chloride. Nonachlazine (1.03 X 10(-5) mol/l) diminished the amplitude of the inward ionic current in a calcium-free medium as well. The stimulatory effect of prostacycline (2 X 10(-7) mol/l) on the fast inward ionic current was inhibited by nonachlazine. The data obtained suggest that the antiarrhythmic effect of nonachlazine might be linked with the inhibition of the fast sodium inward current and the slow calcium inward current.

[Effect of indomethacin and nonachlazine on myocardial and cerebral tyrosine hydroxylase activity in rats].

Nonachlazin (6 mg/kg) and indomethacin (10 mg/kg) intraperitoneally have a unidirectional effect on the activity of tyrosin hydroxylase, reducing it in the hypothalamus and in the brain stem and increasing it in the heart septum. In the brain, the effects of nonachlazin and indomethacin are not summarized whereas in the heart their effects are summarized. Unlike indomethacin, nonachlazin exerts a direct effect on the rate of tyrosin hydroxylase reaction but has no effect on the activity of cyclooxygenase and on synthetase of prostaglandins. It is concluded that prostaglandins produce a modulating action on the activity of brain and heart tyrosine hydroxylase and participate in the mechanism of action of nonachlazin.

[Effect of nonachlazine on the processes of uptake and release of nordrenaline].

Nonachlazine (2 micrometer) blocked significantly the uptake 1 of endogenic noradrenaline in the myocardial tissues and in adrenergic neurons of vas deferens. The uptake II, as well as the intensity and dynamics of the noradrenaline release were not affected by nonachlazine. These processes resulted in an increase of free noradrenaline concentrations in the synaptic space. The latter seems to be a result of the mediator's passage into the nerve fibers (through axonal membranes) and into the granule, this being due to the nonachlazine-induced block. It is suggested that the metabolic and functional nonachlazine effects are related to the beta-adrenergic receptors stimulation, mediated through the agency of noradrenaline.

[Effect of nonachlazine on the frequency and amplitude of isolated atrium contraction].

The effect of nonachlazine, a new antianginal drug, on the rate and the amplitude of contractions of the isolated auricle was studied on guinea pigs and albino rats. Unlike the experiments conducted on intact animals, in the experiments on the isolated auricle nonachlazine proved to decrease the above-mentioned indices. This effect of the drug in the case of the isolated auricle is likely to be due to the ability of nonachlazine to cause a direct effect on the myocardium and the conductive heart system. No specific distinctions in the character of nonachlazine action were revealed.

[Mechanism of the antianginal action of nonachlazine].

Experimental data on the mechanism accounting for the action of a new antianginal drug nonachlazine are presented. Nonachlazine was found to intensively (120+/-9.5 per cent) and protractedly (30--40 min) augment the blood flow in the coronary vessels, increasing the oxygen reserve of the myocardium. The contractile function of the heart and beat ejection considerably gain in their intensity, after an initial short-lived fall (by an average of 26.5+/-3.4 per cent), with the resistance of the coronary vessels declining (21+/-1.0 per cent). Administration of nonachlazine made against the background of a propranol (0.5 mg/kg) block of the beta-adrenergic structures of that by practolol (5 mg/kg) is not accompanied by the above effects. The prevalent influence of nonachlazine on the blood circulation and the function of the heart muscle may be explained by its action on the extravascular factors of the venous blood flow regulation (changes in the metabolism activity of the heart, caused by the stimulation of the beta-adrenergic structures of the myocardium).

[Mechanism of action of nonachlazin on nervous regulation of coronary circulation].

In experiments on cats and dogs under general anesthesia nonachlazine, a new antianginal agent, depressed the reflex decrease of the blood flow to the coronary arteries (electromagnetic recording). Nonachlazine also depressed the reflex decrease of the blood flow in the system of common carotic artery, and the pressure vasomotor reflexes. During recording the tonic and reflex activity in the heart sympathetic nerves nonachlazine proved to selectively inhibit the vasoconstrictor impulses from Adelta-afferent fibers of the spinal nerves, that is it influenced the vasomotor component of the "primary" pain reaction. This mechanism could underlie the block by nonachlazine of the pain syndrome during the heart ischemia disease.

[Action of nonachlazine on the creatine phosphokinase and acid phosphatase activity and on the lysosomal membrane stability of the ischemic myocardium in rats after preliminary reserpinization].

Experiments on the ischemic myocardial tissue of the rat in vitro showed that despite the fact that reserpine-induced depletion of the tissue catecholamine storage failed to affect the stability of lysosomal membranes, a combination of reserpine pretreatment of the animals with subsequent administration of nonachlazine at the low dose yielded stabilization of lysosomal membranes to a greater extent than administration of nonachlazine alone. The effects of nonachlazine (0.25 mg/kg) on the activity of creatine phosphokinase and total activity of acid phosphatase with and without reserpine pretreatment were similar.

[Mechanisms of the hemodynamic effects of nonachlazine in ischemic heart disease patients].

During course therapy with nonachlazine of patients with ischemic heart disease, stable angina pectoris a negative inotropic effect of the drug in persons with eukinetic and in particular hyperkinetic type of hemodynamics was revealed. Enhancement of beta-adrenoblocking effect of nonachlazine with an increase of its daily dose was found. Nonachlazine effect on hemodynamic parameters was shown to be mediated through adrenoreactive structures.

[Effect of nonachlazine on the size of an experimental myocardial infarct].

The effect of intravenous injection of nonachlazine on the infarction size in rabbits was studied planimetrically 7 days after coronary artery occlusion. The first group rabbits received the drug 5 times every 30 minutes, beginning from the 30th minute after occlusion (the total dose 15 mg/kg). The second group rabbits were given nonachlazine 3 times a day (the total dose 12 mg/kg) during 6 days, beginning from the 2nd day after coronary artery occlusion. In both the animal groups, the drug induced respectively a 22.8 and 39.4% decrease in the necrosis size as compared to control. The effect of nonachlazine is presumably linked with stimulation of the collateral coronary circulation in the occluded zone and with the drug direct action on myocardial metabolism.

[Anti-inflammatory, analgesic and antipyretic properties of nonachlazine].

Nonachlazin diminishes aseptic inflammation provoked by subplantar administration of carrageenin to rats. Nonachlazin increases the sensitivity pain threshold of the rat paw affected by carrageenin-induced inflammation, decreases the reaction of mice to intraperitoneal injection of phenylchinoin, and the ability of the rabbit central nervous system for impulse summation. Nonachlazin prevents carrageenin-induced temperature rise in rats.

[Effect of nonachlazine on brain and myocardial noradrenaline content in normal rats and rats subjected to stress].

Nonachlazin causes a rapid and significant increase in the noradrenaline content of the rat myocardium and brain stem. The noradrenaline level falls in these tissues under stress reaction induced by immobilization and electric pain stimulation. Nonachlazin pretreatment before stress reaction prevents the decrease of the myocardial noradrenaline level and the development of cardiac ischemic disturbances and arrhythmias (according to the ECG data). However, nonachlazin fails to diminish the noradrenaline level in the brain stem under these conditions.

[Effect of nonachlazine and oxyfedrine on central processes regulating vasomotor tone].

Effect of the antianginal drugs nonachlazin and oxyfedrin on tonic and evoked activity in sympathetic renal nerves and on vasomotor reflexes induced by stimulation of the tibial nerve was studied and compared in anesthetized cats with intact brain. Low doses of nonachlizin facilitated somatosympathetic reflexes. High doses of the drug induced a selective inhibition of somatosympathetic reflexes evoked by stimulation of high-threshold afferent A-fibers. The inhibition of somatosympathetic reflexes induced by oxyfedrin had no such a selectivity and was less prolonged. The effects of nonachlazin and oxyfedrin attained after the administration of reserpine and propranolol allow a conclusion that the central effects of nonachlazin and oxyfedrin are related to the fact that they modulate the adrenergic brain structures.

[Effect of nonachlazine on ATP, ADP, and lactic acid concentration in intact and ischemic myocardium].

It has been shown that nonachlazin in a dose of 6 mg/kg injected intravenously to cats 15 minutes after occlusion of the left descending coronary artery produced a 42% decrease in the content of lactic acid in the left heart ventricle as compared to its content in ischemic myocardium. Nonachlazin brought to normal the content of ATP that had been decreased under acute myocardial ischemia. Concurrent recording of ECG readings showed them to be improved. The parameters studied did not change in the animals with intact myocardium after the drug administration. It is suggested that the favourable clinical effect seen is accounted for by nonachlazin-activated glycolysis and oxidative mitochondrial phosphorylation in the myocardium.

[Effect of nonachlazine on ino- and chronotropic function].

As a result of the stimulation of beta-adrenostructures of the myocardium nonachlazin intensifies ino- and chronotropic functions of the heart. This effect does not manifest after administration of practolol and is less pronounced in animals anesthesized with urethane and alpha-chloralose since the sensitivity of the beta-adrenostructures of the myocardium decreases under the influence of the anesthetics. Apart from the stimulation of ino- and chronotropic function of the heart nonachlazin reduces concurrently the intensity of positive chronotropic effect of isoproterenol. Such a combination of the efects called a "partial" beta-adrenoblockade seems likely to play an important role in the manifestation of the antianginal action of nonachlazin.

[Experimental data on the toxicity of nonachlazine, a new antianginal agent].

The results obtained by a study of acute and chronic toxicity of nonachlazine--a new original coronary-active agent endowed with antianginal properties are reported. Nonachlazine has been found to have a sufficiently wide range of therapeutic action. With its repeated (for 36 days) oral introduction to rats in an effective dose of 10 mg/kg the drug does not produce any adverse effect on the general condition of the animals, reduces but insignificantly their weight gain, fails to exert any damaging effect on the blood system, does not change biochemical characteristics featuring the function of the liver, nor evokes any pathohistological changes of the internal organs. Nonachlazine has no locally irritating effect on the gastro-intestinal tract and is a compound producing but a slight stress action.

[Effect of nonachlazine on the adenylate cyclase system of the rabbit heart].

The influence of nonachlazine on the adenylate cyclase system of the rabbit heart was investigated. The faint beta-blocking activity (displacement of up to 20% of [3H]-dihydroalprenolol and inhibition of isoproterenol-stimulated activity) as well as the ability to stimulate basal activity in micromolar concentrations has been observed. Such combination of properties may be important in the realization of some positive effects of nonachlazine pharmacological activity.

[Effect of nonachlazine on the temperature sensitivity of the lysosomal membranes of the heart muscle in rats in vitro].

It has been shown in vitro that nonachlazine stabilizes rat myocardial lysosomal membranes and activates total acid phosphatase in homogenates of rat myocardium. Preliminary addition of nonachlazine to homogenates in two different final concentrations produces unequal changes in total and free activity of acid phosphatase and cathepsin D during 2.5-hour incubation of homogenates at 37 degrees C.
