[Vocational definition in the medical students: The influence of personality].

UNLABELLED: The vocational indefinition has been related by different authors with higher levels of personal immaturity, dependency sentiments or anxiety. In this paper, we analyses the possible rapport between specific personality traits and the vocational definition in the Medical Students. MATERIAL AND METHOD: It has been analyses 1484 students of 2nd year of Medicine in the Valencia University. They have responded an anonymous interview about their characteristic sociodemographics and their vocational interests. We determinate their Personality Profile with the 16 P.F. of Cattell. The done got has been analysed with Techniques of Regression Logistic. RESULTS AND CONCLUSIONS: The majority of the analysed population were female (62,7%) and with a medium age of 20 years. The majority of the students have definite vocationally in the 2nd year of Medicine: the 81% against the 19%. The students who haven't defined vocationally trend to be more unstable (C-), expedient (G-), strong minded (I-) and shrewd (N+). CONCLUSIONS: The results of our study seems to confirm the association between the career indecision in the Medical Students and specifics traits of personality, particularly the traits associated with the immaturity personal.

Psychiatric aspects of traffic accidents.

The authors review the literature pertaining to the roles of psychopathology and personality variables in traffic accidents. They review studies of nonpsychiatric and psychiatric samples and examine the roles of suicide, life events, alcohol, and drugs. Certain personality characteristics and psychopathology--such as low tension tolerance, immaturity, personality disorder, and paranoid conditions--appear to be risk factors for traffic accidents. Although the role of alcohol is relatively well established, the roles of most other drugs are less clear. The results of further investigation of psychopathology's role in traffic accidents may have implications for prevention.

[New synthesis empathogenic agents].

The use of synthesis drugs is the object of numerous written articles and TV programs in the last, decade. These synthesis drugs or "designer drugs", are well known for their ability to enhance, reinforce or appease social difficulties and relationships. In the research for empathetic and entactogenic relations one discover an obvious lack of communication and "warmth" in personal or professional relationship. An image of chemical "well being" has become a frequent stereotype of a society with an atrophying of performance and values while supposedly dedicating itself to individual performance. The youths are the first victims of these new drugs, the economical and social environment are the main reinforcing factors of this behaviour. The main characteristic of these drugs, is the non-recognition of their danger, some users go so far as to describe this category of substances as "drugs which are not drugs". As a characteristic, the use of a these synthesis drugs is almost recreative, during the week-end and holiday. The drug addiction is different than that of opiates or cocaine. One can observe some cases of real dependence--corresponding to the DSW IV criterion--when the personality of the users is the main characteristic (narcissic failure, immature personality, family and school problems). Many adverse effects--hypertension, kidney failure, psychoses--were declared. The mass-media has presented many articles concerning Ecstasy (MDMA). This is the most used drug during the rave parties. Its adverse effects are well known and proven. The authors would like to present other more recent synthesis drugs, also known as "analogs". These drugs, a kind of mixture between amphetamine-like (MDMA, MBDB, MDA) and misused medicines (ketamine, gamma OH, atropine) represent a real danger. GHB, 2 CB, HMB, are some of these recent substances. The possibility to procure them on the Web, or to produce them by oneself, add to their danger because of the lack of controls on toxicity and quality. The original danger signs were revealed by the FDA and currently a major preoccupation within french specialised services. The major problem for the practitionner is to inform the users, in order to prevent addiction and analyse the solutions.

[The personality of soldiers with inappropriate behavior patterns at the end of military service].

The end of military service is the turning point in young man's life. After the organized way of living in military environment, he is expected to enter the regular life duties for which he is not prepared to. The aim of this investigation was to study the basic features of soldier's personality that contribute to the maladapted behavioral patterns in frustrating situations occurring at the end of military service, as well as the close inspection of the behavioural changes in such persons, which occurred in that period. Thirty-three soldiers who came to the neuropsychiatric examination between the 10th and 12th month of compulsory military service (72.72% for the first time since joining the Army) were examined. The soldier's personality in EPI (Emotion Profile Index) test was characterized by: lower incorporation and reproduction, but raised level of destructiveness and aggressiveness. The graphic representation of personality profile in the circle of segment values demonstrates the similarity of structurality level to the average profile of emotionally immature person and in certain segments to the persons who attempted suicide. Manifested behavioural patterns in soldiers had the features of Short timer's syndrome. The results of our investigation have shown that the problems of maladapted behaviour in soldiers at the end of military service are dominantly associated with the poor personality integration preexisting chronic problems of the immediate family and differing starting positions after the discharge as well as the poorer organization of living and work of soldiers in the unit due to the inadequate leadership.

Kidney transplantation and liaison psychiatry, part II: A case of dissociative identity disorder.

The authors examined the case of an adolescent patient with dissociative identity disorder secondary to psychological shock of a transplant rejection response. Psychiatric symptoms consisted of three components: visual hallucinations and delusions as a psychological defense against the anxiety of a transplant rejection; appearance of three personalities including proper self, the dead child (donor), and a prophet with strong predicting power; and a twilight state. These psychiatric symptoms may have been related to two psychological factors: immature personality characteristics formed during hemodialysis, and post-traumatic stress caused by a chronic rejection reaction from the patient's first transplant.

[Accounting failures in public officials or notaries. Medico-legal approach].

Considering 11 cases of psychiatric expert reports in criminal cases, the author's aim is to describe the criminological aspects of offences by notaries, mainly swindle and breach of trust. It appears that can be found in this professional category, personality types already described for "closely acquainted", or business crooks, as well as four obviously pathological cases, coming within the provisions of article 64 of the criminal code. Beyond the fact that there is a specific pre-criminal situation: dealing with contracts, assets, funds etc... the authors notices in several cases a discrepancy between the weak and immature personality of some notaries and the importance of the office with which they are entrusted. Thus the requirement of ability and sufficient professional qualification was not always met. The authors would be interested in further similar observations, to confirm or infirm their conclusions.

Reduplicative paramnesia: a disconnection syndrome of memory.

A case of post-traumatic reduplicative paramnesia (Capgras syndrome) is presented in which the patient experiences duplications in all three spheres of orientation (time, place, person). We postulate that these duplications are the result of a disconnection of new memory registration from past memory stores. The patient is unable to integrate present cues with premorbid experience, and bases judgments of his present orientation upon recollections from the past. A deep lesion at the right posterior hippocampus and temporo-parieto-occipital junction is suggested as the site of this postulated primary memory disconnection, based upon neuropsychological testing and CAT scan results. This right temporal atrophy and memory disconnection appears to have induced immature personality changes.

Guidelines for the use of antiretroviral agents in pediatric HIV infection. Center for Disease Control and Prevention.

Although the pathogenesis of human immunodeficiency virus (HIV) infection and the general virologic and immunologic principles underlying the use of antiretroviral therapy are similar for all HIV-infected persons, there are unique considerations needed for HIV-infected infants, children, and adolescents, including a) acquisition of infection through perinatal exposure for many infected children; b) in utero exposure to zidovudine (ZDV) and other antiretroviral medications in many perinatally infected children; c) differences in diagnostic evaluation in perinatal infection; d) differences in immunologic markers (e.g., CD4+ T-lymphocyte count) in young children; e) changes in pharmacokinetic parameters with age caused by the continuing development and maturation of organ systems involved in drug metabolism and clearance; f) differences in the clinical and virologic manifestations of perinatal HIV infection secondary to the occurrence of primary infection in growing, immunologically immature persons; and g) special considerations associated with adherence to treatment for children and adolescents. This report addresses the pediatric-specific issues associated with antiretroviral treatment and provides guidelines to health-care providers caring for infected infants, children, and adolescents.

Juvenile delinquency and drug dependence in Hungary.

It is regretable that the delinquency has increased in Hungary over the past 15 years. This fact occurred because criminality increased to a greater extent than average, and this means a steady supply of adult criminals. According to the surveys of the forensic psychiatrical records carried out over the 4 years between 1987-1990 in archival material of our Department, the criminological factors which occurred most frequently in juvenile delinquency among the 14-18-year-old age group are immature personality, endangered family background and inadequate school attendance. Thirty percent of criminal acts could be connected with alcoholism, 65% with theft, and 22% with aggressive delinquency. An important role in the increasing juvenile delinquency must be attributed to the crisis of the previous regime, including the faulty endeavours in child and youth protection and the inefficient school system. The decrease in living standards and the employment of women in work contributed to the loosening of family relations and the dereliction of children's education. The proportion of institutional young people among the perpetrators is high. Another factor with regard to the disadvantageous situation is the migration of young people to the towns from villages. Without the necessary conditions and with the poor company of friends, this presents an intensified criminological danger. Prevention is obviously of outstanding importance. The improvement of the economic situation, stability of families, substantial changes in school reform, the fight against alcoholism and the modern judicial regulation can be the means to achieve this goal.

Behavioral aspects of pediatric epilepsy syndromes.

Apart from control of the seizures, two of the most important factors in determining how well a child with epilepsy progresses toward independence are cognition and behavior. The diagnosis of the correct epilepsy syndrome often provides information with regard to probability of good seizure control and intellectual outcome. However, relatively little has been published on the behavioral aspects of the various epilepsy syndromes. In West syndrome there is emerging evidence that early effective treatment might improve outcome in terms of both cognition and behavior. The work on this syndrome in children with tuberous sclerosis has demonstrated an association between temporal lobe tubers and autism. In Dravet syndrome, a variety of psychiatric disorders have been reported, including hyperactivity and autistic features. This is another epilepsy syndrome that tends to be resistant to treatment, implying that the prognosis has to be guarded. The behavioral problems reported with Lennox-Gastaut syndrome also include autistic features, as well as generally sluggish behavior. It is very likely that these characteristics largely reflect the effect of ongoing seizure activity. Autistic features, aggression, and hyperkinesis have been described with Landau-Kleffner syndrome. The behavior may improve dramatically with appropriate medical treatment or after multiple subpial transection. Although the syndrome of benign partial seizures with centrotemporal or rolandic spikes is said to have a very good prognosis, it is becoming increasingly evident that behavioral problems such as concentration difficulties, tempers, hyperactivity, and impulsivity might occur. Juvenile myoclonic epilepsy has been associated with very variable behavioral traits, sometimes with immature personality features and poor social adjustment suggesting frontal lobe dysfunction. Because many of the reports of behavioral disturbance associated with epilepsy syndromes are anecdotal and do not include validated measures of behavior it would be unwise to draw firm conclusions from them at this stage. Carefully conducted prospective studies, paying particular attention to any behavioral improvements that occur with successful treatment of the epilepsy, are required.

Psychosomatic study of self-excoriative behavior among male acne patients: preliminary observations.

BACKGROUND: Studies of the psychosomatic correlates of self-excoriative behavior in acne have involved mainly women with acne excorieé. Little is written about the psychosomatic factors that affect men with acne. Excessive self-excoriation of acne lesions is an important clinical factor because it can prolong the course of the disease and exacerbate the deeper inflammatory process with an increase in the severity of scarring. METHODS: Thirteen men (mean +/- SE: age: 22.2 +/- 1.4 years) with mild to moderate facial acne, whose self-excoriative behavior was not severe enough to result in acne excorieé, completed a battery of self-rated questions assessing their self-excoriative behaviors, the severity of their acne, and various psychologic factors. RESULTS: Certain aspects of self-excoriative behavior (e.g., a tendency to pick or squeeze the acne lesions when stressed) correlated directly with depression (brief symptom inventory (BSI)) (Pearson r = 0.64, P = 0.02) and anxiety (BSI) (Pearson r = 0.61, P = 0.03) scores. The dermatologic indices of acne severity such as inflammation (Pearson r = 0.82, P = 0.0004) and pustules (Pearson r = 0.62, P = 0.03) were the strongest correlates of self-excoriative behavior. CONCLUSION: Self-excoriative behavior in men with acne may be exacerbated by a coexisting depressive or anxiety disorder. In contrast, women with acne excorieé have been reported to suffer from an immature personality where the cutaneous condition may serve as "an appeal for help." Men who excessively pick their acne will benefit from aggressive dermatologic therapies and should be assessed for underlying depressive and anxiety disorders.

A rare case of Capgras syndrome observed in wake-amine induced psychosis.

An adolescent girl showed the apparent Capgras syndrome in the course of wake-amine induced psychosis. The patient, then 18, committed homicide in close connection with the syndrome and also with the distorted judgment due to personality immaturity. As far as the syndrome was concerned, the extension was so extreme that it involved her neighbors and even her dwelling city. The detailed case history was reported along with a discussion about the psychodynamism.

Induction of anti-human immunodeficiency virus type 1 (HIV-1) CD8(+) and CD4(+) T-cell reactivity by dendritic cells loaded with HIV-1 X4-infected apoptotic cells.

T-cell responses to X4 strains of human immunodeficiency virus type 1 (HIV-1) are considered important in controlling progression of HIV-1 infection. We investigated the ability of dendritic cells (DC) and various forms of HIV-1 X4 antigen to induce anti-HIV-1 T-cell responses in autologous peripheral blood mononuclear cells from HIV-1-infected persons. Immature DC loaded with HIV-1 IIIB-infected, autologous, apoptotic CD8(-) cells and matured with CD40 ligand induced gamma interferon production in autologous CD8(+) and CD4(+) T cells. In contrast, mature DC loaded with HIV-1 IIIB-infected, necrotic cells or directly infected with cell-free HIV-1 IIIB were poorly immunogenic. Thus, HIV-1-infected cells undergoing apoptosis serve as a rich source of X4 antigen for CD8(+) and CD4(+) T cells by DC. This may be an important mechanism of HIV-1 immunogenicity and provides a strategy for immunotherapy of HIV-1-infected patients on combination antiretroviral therapy.
