Prophylaxis of endophthalmitis with topical povidone-iodine.

The authors conducted an open-label nonrandomized parallel trial to examine whether the preoperative application of povidone-iodine to the ocular surface reduces the incidence of endophthalmitis after intraocular surgery. During an 11-month period, topical 5% povidone-iodine was used to prepare the conjunctiva in 1 set of 5 operating rooms, while silver protein solution was used in another set of 5 rooms. In all cases, surgeons continued to use their customary prophylactic antibiotics. A significantly lower incidence of culture-positive endophthalmitis (P less than 0.03) was observed in the operating rooms using povidone-iodine (2 of 3489 or 0.06%) compared with those using silver protein solution (11 of 4594 or 0.24%). Use of topical povidone-iodine in over 3000 cases was not associated with any adverse reactions. In a majority of the observed cases of endophthalmitis, some form of intraoperative communication with the vitreous cavity existed.

The antimicrobial activity of embalming chemicals an topical disinfectants was evaluated to determine the degree of disinfection achieved during the embalming of human remains. The administration of arterial and cavity embalming chemicals resulted in a 99% reduction of the postmortem microbial population after 2 hours of contact. This level of disinfection was maintained for the 24 hours test period. Topical disinfection of the body orifices was also observed. Therefore, it is probable that present embalming practices reduce the hazard from transmission of potentially infectious microbial agents within the immediate environment of embalmed human remains.

Inflammation patterns after laser trabeculoplasty measured with the laser flare meter.

Anterior chamber inflammation was measured in 71 eyes after argon laser trabeculoplasty with the laser flare-cell meter (Kowa FC-1000). Visual acuity, tonometry, and laser flare measurement were performed before and 3, 6, and 18 hours and 2, 4, 7, 14, 30, and 90 days after argon laser trabeculoplasty. Anterior chamber inflammation peaked 2 days after argon laser trabeculoplasty. Forty-nine percent of the eyes showed significant inflammation, with a flare increase of more than 10 photon counts per millisecond over prelaser values. Inflammation was significantly more frequent in pseudoexfoliative glaucoma (69%) and pigmentary glaucoma (100%) than in primary open angle glaucoma (23%). Post-argon laser trabeculoplasty relative pressure decrease was significantly more important in the group of patients with inflammation than in the group without inflammation in the early and late follow-up periods. Topical nonsteroidal anti-inflammatory treatment (diclofenac sodium) was needed in 17 patients, who readily responded to treatment within 72 hours.

Interaction of benzo [A] pyrene and a hyperplastic agent in epidermal nuclear enlargement in the mouse. A dose response study.

Experiments were conducted on the effects of various dose levels of benzo [a]pyrene (BP) on nuclear size in mouse interfollicular epidermis over a 3-day period. Topical application of BP was made with or without croton oil (CO) (0.1 or 0.5%) in the vehicles acetone, toluene and methyl ethyl ketone (MEK). Nuclear size was measured on histological sections either manually or by Quantimet Image Analyser. Vehicle controls treated with 0.1 or 0.5% CO in acetone or MEK gave rise to epidermal hyperplasia with some nuclear enlargement and toluene without CO produced a similar response. It was found that when BP was applied in a vehicle capable of inducing hyperplasia, the nuclear enlargement produced was greater than that produced by either the vehicle control or BP in a non-irritant vehicle. The enhancement of response to BP when tested in the presence of a hyperplastic agent resulted in lower concentrations of BP being detectable. As the levels of BP detectable by nuclear enlargement under these conditions compared reasonably well with those detectable in long-term tests, this system might be usable as a basis for a short-term test for carcinogens.

[Use in anesthesiology of a new beta-2-stimulator, Salbutamol. Original technic and considerations].

Notes on drugs relating to the adrenergic system are followed by an account of the chemical, pharmacological and clinical features of a new beta2-stimulant, Salbutamol. Employment of this drug to prevent bronchospastic crises in 10 asthmatic patients subjected to narcosis is reported. An original technique was used: oral administration preoperatively, inhalation in the immediate preoperative period, topically on the trachea, and inhalation and oral adminstration post-operatively. Satisfactory results were observed in all 10 cases and support for the use of Salbutamol in anaesthesiology is given.

Mental illness topics in popular periodicals.

Recent emphases on educating the general public about mental illness have included the encouragement of the publication of plainly written, lay-oriented information about mental disorders and their treatment in popular periodicals. To examine the extent to which coverage of mental health topics in popular magazines is being accomplished, the present study looked at the numbers and types of articles related to mental health indexed in the Readers' Guide to Periodical Literature and changes in indexed articles between 1965 and 1988. The number of articles and number of headings related to mental illness and treatment have increased significantly since 1965, particularly those describing specific disorders. In addition, many of the changes in headings used by the Readers' Guide are consistent with changes in the field (e.g., the introduction of new diagnostic categories) and with desired changes in terminology (e.g., using less stigmatizing terms). These general findings are encouraging for mental health education advocates, although more detailed analysis of the content of articles is needed to be sure that the increased periodical information available to the public is accurate and up-to-date.

Developing drug literatures. 1. Bibliometrics of baclofen and dantrolene sodium.

The literatures of two antispastic drugs, baclofen and dantrolene sodium, were studied bibliometrically for their first decade and were found to be generally similar. Baclofen had 93 papers and dantrolene sodium had 70. About a quarter of the baclofen papers were in a foreign language, whereas almost all dantrolene sodium papers were in English. Baclofen literature had a lower nonscholarly content, but the scholarly increase, 5%, was the same for both. Both drugs had an average of 12 references per paper. The literatures of both drugs had a similar degree of internal cohesiveness; almost half of their papers referred to other papers in the same drug collection. The first human paper for baclofen was the second published; for dantrolene sodium, it was the eleventh. The distribution of journals carrying papers on each of these drugs followed Bradford's law. Two authors per paper was the average for each drug. The productivity of authors approximated Lotka's law for both drugs. About two-thirds of the papers of both drugs had a drug-word in their titles. The literature of both drugs contained about 15% legendary papers, typical of clinical pharmacology. The most intense papers, 15 for baclofen and 11 for dantrolene sodium, were identified, using citation, bibliographic coupling, and co-citation frequencies. A generalization predicts what might be expected from the literature of future antispastic drugs.

Using bibliometric analyses of patent literature for predicting the clinical fates of developing drugs.

Certain bibliometric features of the early literatures of developing drugs can be used to predict their ultimate clinical fates. The chronological sequence of publications is expressed as a binary vector with 1 for a patent and 0 for a nonpatent. The decimal equivalents for standardized vector lengths provide scalar values for comparing one drug with another. In order to incorporate concordant patents, fuzzy subsets are employed, with the number of attempts required to achieve transitive closure being the values for comparison. The methods involved are described using minoxidil as an example.
