Lifestyle, health and disease: a comparison between Papua New Guinea and Australia.

The proposition that lifestyle is a major determinant of community health is explored by contrasting the features of a rural subsistence community in the highlands of Papua New Guinea and the features of the community in urbanized, industrialized Australia. Reference is made to differences in physical environment, housing, work, social situation, human relationships, patterns of disease, population statistics, diet, growth, obesity, physical fitness, blood lipid concentrations, blood pressure, salt intake and the occurrence of hypertension, diabetes, cardiovascular disease and signs of degenerative changes in various tissues. The Papua New Guinea community is seen as a self-reliant, self-contained, socially cohesive subsistence society whose members are well adapted to their physical and social environment, free from major degenerative cardiovascular diseases, with little overt psychiatric illness, but with a heavy burden of infectious disease, with marginal nutritional levels of degenerative disease and disease from psychological stress. It is clear that health, in its fullest sense, is not the prerogative of any one type of society.

[Gastric diseases in foreign workers in the German Federal Republic (author's transl)].

Gastric biopsies from foreign workers were compared with those from German patients of the same age group. With reference to population statistics investigations showed that a far higher percentage of foreign workers are gastric patients than are Germans of the same age. Chronic atrophic antral gastritis occurs in foreigners twice as often as in a comparative group of Germans. 77% of the duodenal ulcers and 88% of the gastric ulcers are found in the group of foreign workers examined aged from 21 to 40 years. The German patients, on the other hand, show normal gastric mucosa with more than chance frequency compared with foreign workers of corresponding age and sex.

[Immunogenetic characteristics of the population of the Transcarpathian region of the UkrSSR ABO system erythrocyte antigen findings].

The phenotypes with their respective alleles frequencies of the ABO system were studied in 33 230 individuals of 9 ethnic groups of the Transcarpathian Region population. Statistically significant differences in allele frequencies were found in Gypsies, Germans and Slovaks as compared to those in the main Ukrainian population. There are significant differences between Hungarians and Gypsies of the Transcarpathian Region and analogous populations beyond the region. Absence of a reliable difference between gene pools of the Slav groups of the population and of Hungarians may point to the local origin of the later.

Longevity syndromes: familial hypobeta and familial hyperalpha lipoproteinemia.

Longevity and morbidity and death from myocardial infarction were examined in eight kindreds with familial hypobeta lipoproteinemia and in 18 kindreds with familial hyperalpha lipoproteinemia. Expectation of life for males and females from kindreds with hypobeta lipoproteinemia was 9 and 12 years longer (p less than or equal to 0.002) than that indicated by population statistics for U.S. white populations, whereas expectation of life for males and females from kindreds with hyperalpha lipoproteinemia was 5 and 7 years longer (p less than 0.02). Morbidity from myocardial infarction in 115 living first-degree adult relatives of probands with hypobeta and hyperalpha lipoproteinemia and in 364 living first-degree adult relatives of normolipemic spouse controls were compared. Nonfatal myocardial infarction (MI) was reported for 18 of 364 (5 per cent) relatives of normal spouse controls and in 0 of 115 relatives of hypobeta and hyperalpha subjects (p less than 0.05). The ratios (mean+/-S.E.) of C-LDL to C-HDL in familial hypobeta and hyperalpha lipoproteinemia were 0.79+/-0.06 and 1.21+/-0.06, as compared to 2.41+/-0.12 in a control population (p less than 0.001). If high-density lipoproteins confer protection against development of atherosclerosis, whereas low-density lipoproteins have opposite effects, then we speculate that the low ratio of C-LDL:C-HDL may be related to prolonged longevity and decreased morbidity from myocardial infarction in familial hypobeta and hyperalpha lipoproteinemia.

The clinical spectrum of systemic lupus erythematosus in childhood.

The onset and course of 108 children with systemic lupus erythematosus have been analysed. There were more black patients than one would expect from hospital population statistics. There was a greater preponderance of boys with onset of the disease at less than 12 years of age and there is a large number of familial cases. Major signs and symptoms differed from those observed in adults only in the greater degree of reticuloendothelial involvement and in a possibly greater propensity for children to change renal biopsy category. Diffuse proliferative renal lesions remain a major contributor to death both in children and in adults, but the importance of the extrarenal mortality factors plus the greater proportion of male deaths is emphasized.

[Consideration of modern demographic questions (author's transl)].

It is assumed that the decrease in the birth rate can be checked or that an increase in births can be stimulated by models or value judgements. On the basis of articles reflecting the official demographic opinion published by the Federal Office for Statistics, an attempt was made to reflect the intellectual foundation for such views. With the help of population statistics and model computations as well as vaguely intimated ideologies, changes in procreative activity considered necessary could be induced. The investigation concluded with the question of whether the detachment of procreative activity in highly industrialized, capitalistic countries with their specialized instruments, technologies, and institutions can be attributed to economic factors.

[Fatal kidney diseases due to analogesic abuse in Switzerland].

As part of an investigation into death by poisoning in Switzerland we were able to examine the register of deaths at the Swiss Federal Bureau of Statistics. All the medical reports which named analgesic nephropathy as being the basic illness and the main cause of death have been taken into consideration in this work. 331 deaths occurred in the five year period 1967-1971, and were studied as closely as the documents allowed. The earliest cases of death arose in the thirties, the maximum number however occurring between the age of 60 and 70. Between the onset of kidney damage and the actual death elapses a period of about 10 years. The ratio from woman to man is almost 2,8:1. The occurrence is particularly frequent among childless housewives. The victims are people of widely different occupations and social classes. The analgesic nephropathy resulting from addiction and leading to death is most frequently encountered among the population of the cities, and the German Swiss Cantons, as well as among people of protestant faith. The shocking number of 331 deaths in five years (in 1973 there were a further 79 cases) bears witness to the need for adequate preventive measures.

Designing case-control studies.

Identification of confounding factors, evaluation of their influence on cause-effect associations, and the introduction of appropriate ways to account for these factors are important considerations in designing case-control studies. This paper presents designs useful for these purposes, after first providing a statistical definition of a confounding factor. Differences in the ability to identify and evaluate confounding factors and estimate disease risk between designs employing stratification (matching) and designs randomly sampling cases and controls are noted. Linear logistic models for the analysis of data from such designs are described and are shown to liberalize design requirements and to increase relative risk estimation efficiency. The methods are applied to data from a multiple factor investigation of lung cancer patients and controls.

Production and consumption of alcoholic beverages in the USSR. A statistical study.

The Soviet government has published few official statistics on alcoholic beverages. Using Soviet statistical, commerical and technical sources, however, estimates can be derived on the production and consumption of alcoholic beverages during the 1957-71 period, including urban and rural consumption, expenditures, tax revenues, imports and exports and use of agricultural products.

[Epidemiology of breast cancer in Austria (author's transl)].

In cooperation with the Austrian National Institute for Statistics the incidence-rates of breast cancer in Austria in the period of 1971-1975 were investigated. The incidence-rate of breast cancer (50 new cases/year/100,000 women) is higher than the incidence-rate of cervical or endometrium carcinoma. The increasing risk of the higher age-groups, which is especially expressed by the incidence-rate of 145 in the age-group of 70 or more years, is pointed out. This findings are of great consequence to the system of preventive medical care. In urban areas the incidence of breast cancer seems to be higher than in rural district.
