[Problems of cystic uretero-pyelitis (author's transl)].

The authors report on a series of 7 new cases of cystic uretero-pyelitis (C.U.P.). An historical and pathogenic review of this anatamo-radiologic entity is presented. The conditions for discovery were: renal colic in 4 instances, discovery during surgery in association with a pyeo-ureteral junction syndrome in one instance, and a hematuria in three instances (1 being accompanied by renal colic). Age varied from 22 to 78 years, with an average age of 61 years. The distribution between the sexes was equal: 4 women and 3 men. Urinary infection was observed in cases (colibacilluria), whereas it was absent in 3 cases. If hematuria is present, one should, as a rule, look for a possible association with an ureteral or vesical tumor (1 case). The final diagnosis can be established only through histological examination (this was the case in only 3 of our patients), but when there is a suggestive radiologic image and a reassuring clinical picture, one can recommend a simple medical treatment (1 recovery) and propose a steady radiologic surveillance. In case of doubt or hematuria, it is preferable to advocate surgical investigation: 2 patients out of the 3 showing a hematuria, were operated on, the third having refused. In one case, hematuria was due only to cystic uretero-pyelitis, while the second was known to have a vesical tumor with C.U.P. The real danger, in presuming a cystic uretero-pyelitis, is the failure to recognize a ureteral papillomatosis, hence the necessity of radiologic surveillance. The authors did not note any serious bilateral form. The only bilateral involvement seen in the 7 cases was not accompanied by reanl deficiency.

Frequency of pyelitis, pyelonephritis, renal perivasculitis, and renal infarction in dogs.

The frequency of pyelitis, renal perivasculitis, and renal cortical inflammation was determined in 104 apparently normal dogs, 50 dogs with disease that were necropsied, and 20 dogs with experimentally induced cystitis. Abnormalities occurred least often (19%) in the apparently normal group and most often (55%) in the cystitis group. The frequency of specific lesions was generally higher in males than in females. Bacteria were isolated from kidney and urine of some of the apparently normal dogs, but their relationship to the lesions observed was not established.

Urothelial changes of the renal papillae in Sprague-Dawley rats induced by long term feeding of phenacetin.

Thirty female Sprague-Dawley rats were fed 0.535 per cent phenacetin in the diet for up to 110 weeks. Twenty-six of these rats developed urothelial hyperplasia, partly papillary, of the renal papillae. Twenty-eight rats showed dilatation of the vasa recta frequently associated with thrombus formation and calcification. One phenacetin fed rat had epithelial hyperplasia associated with chronic pyelitis. In 2 of the 30 control rats urothelial hyperplasia was found to be associated with chronic pyelitis. The hyperplastic urothelial changes and vascular changes were often, but not always, present simultaneously. One control rat developed a mammary carcinoma, as compared with 5 rats in the phenacetin group. Four phenacetin fed rats developed carcinoma of the ear duct. The results of the present investigation provide evidence that phenacetin can induce proliferative lesions of the urothelium of the rat renal pelvis with weak carcinogenic activity in the ear duct and mammary glands.

Experimental Escherichia coli O6 pyelonephritis in rabbits. Effect on O6 antibody quantity and avidity of prior immunization with E. coli O2 bacteria.

Haematogenous pyelonephritis was induced in rabbits using Escherichia coli 06:K13:H1 bacteria and the amounts and avidities of antibodies to the 06 antigen were analysed by the ammonium sulphate precipitation technique of Farr. In a group of six animals preimmunized with E. coli 02:K2ab:H1, five developed pyelonephritis and one pyelitis, as determined by histological examination. All aminals showed a considerable antibody response to E. coli 06 antigen during the infection. The animal with pyelitis gave a slightly smaller response than the others. The antibody avidity showed a pronounced variation. In a second group of six rabbits not preimmunized, five animals developed pyelonephritis. The titres of antibodies against E. coli 06 antigen increased during the infection inall of the six animals. However, the increase was significantly smaller than for the animals preimmunized with E. coli 02:K2ab:H1 (P smaller than 0.01). The pattern of the antibody avidities in this group was also heterogenous. The results are consistent with previous findings that exposure to serologically heterologous E. coli bacteria can enhance the development of the homologous antibody titres. This could be of relevance for serological diagnostic work as well as in the determination of the protective capacity of the antibody.

Pregnancy in the overweight Nigerian.

One hundred pregnant patients weighing 80 kg or over at the 14th week of pregnancy were studied during their antenatal period and delivery. The incidence of pre-eclampsia, hypertension, pyelitis of pregnancy and operative delivery was higher in overweight patients than in an equivalent number of patients of average weight also studied. The age-related difference in antenatal weight gain in obese patients was confirmed. Two maternal deaths occurred among the overweight patients.

[Laboratory and clinical studies of cefamandole in children (author's transl)].

Laboratory and clinical studies of cefamandole (CMD), a new semisynthetic cephalosporin, were investigated and following results were obtained. 1) Absorption and excretion study following 25 mg/kg intravenous administration was carried out in pediatric patients. In 6 cases, mean serum levels of 116.7 +/- 24.0 micrograms/ml, 62.1 +/- micrograms/ml, 12.2 +/- 2.7 micrograms/ml, 2.9 +/- 1.1 micrograms/ml, 0.6 +/- 0.6 micrograms/ml and 0.1 +/- 0.2 micrograms/ml obtained after 15, 30 minutes, 1, 2, 4 and 6 hours administration. In 4 cases, mean urinary recovery of 68.2 +/- 17.2% (0 approximately 8 hours) was obtained. The mean half life of serum level was 0.36 +/- 0.08 hours. 2) The transfer of cefamandole was poor in infants with meningitis. 3) Cefamandole was given to 22 children with acute pyelitis (1 case), acute pneumonia (19 cases), and meningitis (2 cases). The dosage was 80.0 approximately 284.2 mg/kg/day, and it was divided into 4 approximately 6 times and given intravenous or intravenous drip. The duration of administration was from 3 to 17 days. The overall efficacy rate in 22 cases was 95.2%, i.e., excellent in 5, good in 15, poor in 1, and unknown in 1. In bacteriological examination, there were eradication of the organisms in 9 (52.9%), decrease in 4, unchange in 4 out of 17 strains. 4) Any noticeable adverse reaction was not observed.

[Localizationstudies in female patients with recurrent urinary tract infection. II. Patients with ureterovesical reflux (author's transl)].

In 17 girls and 5 women with urinary tract infection and low-pressure reflux we performed 23 bladder-washout-tests. 12 were interpreted as supravesical, 11 as vesical bacteriurias. All patients with vesical bacteriurias had a normal IVP. The majority of patients with supravesical bacteriurias had clubbing and scarring. The possibility of interpreting the supravesical bacteriurias as a sign for pyelitis rather than pyelonephritis is discussed.

[Clinical trial of the antibacterial combination sulfamoxole/trimethoprim (CN 3123). 2. Results of a multicenter clinical trial of CN 3123 in infections of the kidneys and urinary tract].

During the clinical trial of N1-(4,5-dimethyl-2-oxazolyl)-sulfanilamide (sulfamoxole) and 2,4-diamino-5-(3,4,5-trimethoxy-benzyl)-pyrimidine (trimethoprim) (CN 3123, Nevin, Supristol) results from 925 treated cases of bacterial infections of the urinary organs were documented. The analysis revealed the following conclusions: 1. On the basis of all the assessable cases, there was no clinical effect in less than 10% of patients and no bacteriological effect in only 13% of patients. The therapeutic response was clinically good in 76% and bacteriologically good in 68% of the patients. The rest of the patients showed a fair clinical response, that is to say they showed a definite improvement in the clinical picture, or some bacteriological response, i.e. a definite reduction in the organism counts or, in mixed infections, not all the strains of pathogen were eradicated. 2. There was a higher success rate in acute urinary tract infections which had not previously been treated than in chronic or previously treated cases. 3. The therapeutic results in the principal indications were as follows: Pyelonephritis: 73.9% good and 16.5% fair effect clinically; 63.6% good and 21.6% some effect bacteriologically. Pyelitis: 81.1% good and 18.9% fair effect clinically; 70.4% good and 25.9% some effect bacteriologically. Cystitis: 81.3% good and 8.6% fair effect clinically; 68.9% good and 17.6% some effect bacteriologically. Postoperative urinary tract infections: 98% good effect clinically and bacteriologically. Infections of the urinary organs (not specified in more detail): 71.8% good and 16.1% fair effect clinically; 65.0% good and 18.8% some effect bacteriologically.

Acute Disease

Hydronephrosis of pregnancy.

This review was done because most texts and some recent reviews do not recognize the obstructive cause. It is important only because of the high incidence of acute pyelonephritis during pregnancy. This is so common that it has been given a special name, "pyelitis of pregnancy". We have shown in the monkey that bacteria when introduced into the ureter under low pressure does not often cause pyelonephritis unless associated with ureteral obstruction. The presence of bacteria even with partial ureteral obstruction increases the chances of acute pyelonephritis. Care must thus be taken to eradicate bacteriuria during pregnancy to prevent acute pyelonephritis as has been shown by Kass, especially since vesicoureteral reflux occurs more frequently in the pregnant than nonpregnant primate. When we understand the obstructive cause of hydronephrosis of pregnancy and the effect of positional change, therapy will be more effective. The left lateral position then will not only relieve the pain of acute hydronephrosis of pregnancy but will also greatly assist in the therapy of acute pyelonephritis of pregnancy since it will relieve the associated ureteral obstruction.

[The value of renal phlebography in the differential diagnosis of arteriographically hypo- or avascular lesions in the kidney (author's transl)].

The present paper is an investigation into the diagnostic value of renal phlebography for differentiating between arteriographically hypo- and avascular malignant lesions in the kidney and benign renal masses. In three examinations carried out on eight patients with renal masses, phlebography was of crucial diagnostic importance. In ten renal cysts a presumptive diagnosis made by arteriography was confirmed by phlebography. In another eight renal cysts, a case of haemorrhagic pyelitis and in two cases of unilateral pseudotumerous pyelonephritis, phlebography was of no value.

Adenocarcinoma

Experimental model of corynebacterium renale pyelonephritis produced in mice.

Corynebacterium renale type I (strain 115), 1.7 X 10(7) to 4.5 X 10(7) organisms, introduced intravenously into mice disappeared from the blood less than 24 h after inoculation and did not produce pyelonephritis. The same strain, 1 X 10(7) to 5 X 10(7) organisms, inoculated into the urinary bladder of mice was not recovered from the blood in any of the mice, but caused pyelonephritis accompanied by ureteritis and cystitis in 16 of 21 (76%) mice. Pyelonephritis and cystitis in mice were histopathologically similar to those found in cows. The antibody response was observed only in the mice with pyelonephritis or pyelitis, but not in those with only cystitis or in those without lesions, as found in cows. Similar diseases were produced in mice by C. renale types II and III but less frequently than by type I. It is suggested, therefore, that mice may be useful in the study of bovine C. renale infection.

Trigonal-colonic anastomosis: a urinary diversion procedure in dogs.

Trigonal-colonic anastomosis for diversion of urine into the colon was performed in 12 clinically normal dogs and in 10 incontinent dogs with diseases of the urinary bladder or urethra. Dogs were studied from 1 to 30 months after surgery. The surgical procedure was technically satisfactory. Fifteen of 22 dogs were studied with intravenous urography, and only 1 case of hydronephrosis was found. Pyelitis was a common histopathologic finding in both groups of dogs. Pyelonephritis developed in 30% of dogs, regardless of duration of anastomosis. Glomerular filtration rate was reduced in all dogs studied, but renal failure was infrequent. Values for blood urea nitrogen and serum inorganic phosphorus were elevated due to intestinal recycling of nitrogenous products and phosphate. Electrolyte imbalances were not a problem, but gastrointestinal disturbances developed in 3 of the 10 diseased dogs. Six of 10 diseased dogs survived from 9 months to more than 3 years. Trigonal-colonic anastomosis appears to be a satisfactory salvage procedure for incontinent dogs with diseases of the urinary bladder or urethra that do not respond to other forms of therapy.

[Clinical and laboratory studies on S-6437 (a new longacting granule of cephalexin) in pediatric field (author's transl)].

Clinical and laboratory studies on S-6437 were made, and the following results were obtained. 1. Thirty pediatric patients with various types of infections such as tonsillitis, bronchitis, pneumonia, cystitis, pyelitis, lymphadenitis colli and pyodermia were treated with S-6437 at the daily dosage of 50 mg/kg orally, the clinical effectiveness was 89.3%. 2. The peak blood level of cephalexin after a single oral administration (25 mg/kg) was observed after 4 hours and the average peak blood level of 10 cases was 7.4 microgram/ml. 3. The average urinary excretion rate of 5 cases was 71.2%. 4. Mild side effects were noticed in a few cases.

[Pathology of renal tuberculosis (author's transl)].

Introduction of specific chemotherapy and vaccination leads to a remarkable recession of renal tuberculosis in the younger age group. Renal tuberculosis is the result of a haematogenous spread of tubercle bacillus. Haematogenous spreading occurs immediately after primary infection, or, in elderly patients, in combination with recurrency of tuberculous foci in lungs and hilar-lymphnodes. Simultaneous metastasis in the skeleton, especially in the vertebrae, are observed in 30%. The incubation period between tuberculous spread and clinical manifestation of renal tuberculosis lasts several years, in the average 5-8 years, for calcareous kidneys it may last as long as 20 years and more, for tuberculous pyelitis only a few months. Today it is possible to treat renal tuberculosis with drugs (Streptomycin, PAS and INH). In 13% the cicatrisation is combined with obstruction of calices and partial hydronephrosis, in 7% with obstruction of the ureter and total hydronephrosis. Early chemotherapy may prevent the development of tuberculous hydronephrosis.

Aged

Leukocyte response to somatostatin in normals and patients with bacterial leukocytosis.

In normals and patients with acute bakterial leukocytosis synthetic somatostatin decreases cell count of stab neutrophils, neutrophils and eosinophils. After termination of somatostatin infusion a rebound phenomenon occurred. In healthy normals lymphocyte count increased during somatostatin infusion. This effect outlasts the end of infusion for more than 24 h. No significant effect could be demonstrated in patients with a bacterial leukocytosis.

Radiolucent defects within renal pelvis.

Radiolucent filling defects within the renal pelvis are common findings in diagnositc urography, and because of their myriad causes the diagnostician is often faced with a challenging problem. Several of the more unusual causes of renal pelvis filling defects are described and their diagnosis, pathophysiology, and therapy discussed.

Adenocarcinoma

Uncommon radiologic observations in renal milk-of-calcium stone.

Milk-of-calcium stone is a rare finding in the kidney. It is diagnosed radiologically based on the fact that the opacity takes a semilunar shape and layers according to the position of the patient. An unusual observations is that layering may require some time to take place and that the semilunar configuration may be found incidentally in the supine position as well.
