[Nd-YAG laser photocoagulation of scrotal sebaceous cysts].

Scrotum's sebaceous cyst is an usual pathology, not life threatening, but susceptible of infection, producing abscess with their associated cosmetic and psychological disturbance. The classical management is the complete excision, under local anaesthesia, to prevent its recidive. Until today, the use of de Nd-YAG laser for the management of the scrotum's sebaceous cysts as not been reported. We report a case of a young man with several scrotum's sebaceous cysts, treated successfully with the use of a Nd-YAG laser.

Sebaceous cyst excision with minimal surgery.

Sebaceous cysts are common benign tumors found on the scalp, ears, retroauricular areas, scrotum, face and thorax. A technique involving minimal surgical trauma and good cosmetic results is described. In a series of 302 patients, the recurrence rate was extremely low (0.66 percent) during 18 months of follow-up. Experience with a minimal-incision technique suggests that this method is a satisfactory alternative to conventional, more invasive surgery.

Familial adenomatous polyposis (FAP): genotype correlation to FAP phenotype with osteomas and sebaceous cysts.

Gardner syndrome is characterized by the triad of colorectal adenomas, soft and hard tissue tumors. This disorder was regarded as a separate disease until the identification of the APC gene when it was recognized that mutations in the APC gene were the underlying cause of both Gardner syndrome and familial adenomatous polyposis (FAP). The present study aimed at examining whether a particular APC genotype could be delineated in FAP patients with benign extracolonic manifestations: sebaceous cysts and/or osteomas. A questionnaire was sent to all Danish FAP patients (N = 234) asking for occurrence of sebaceous cysts and palpable osteomas. Medical records later verified positive findings, when possible. The results for each patient were correlated to the position of his or her mutation in the APC gene. Positive participation compliance was 77% (N = 180), and in 105 of these patients the pathogenic APC mutation was known. Palpable osteomas were reported in 17 of the patients in whom a pathogenic mutation had been identified. Osteomas were only identified in patients with mutations between codon 767 and 1513, a gene area also associated with congenital hypertrophy of the retinal-pigmented epithelium (CHRPE) and hepatoblastoma. Sebaceous cysts were reported in 51% of the patients, and their APC mutations were evenly distributed in the gene with no particular hotspot. Osteomas appeared most frequently in patients with sebaceous cysts, odds ratio 6.6, P < 0.001. The study provides molecular evidence that Gardner syndrome is a variant of FAP and essentially obsolete in clinical practice.

Porcine sebaceous cyst model: an inexpensive, reproducible skin surgery simulator.

BACKGROUND: Surgical simulators are an established part of surgical training and are regularly used as part of the objective structured assessment of technical skills. Specific artificial skin models representing cutaneous pathology are available, although they are expensive when compared with pigskin. The limitations of artificial skin models include their difficulty in representing lifelike cutaneous pathology. OBJECTIVE: Our aim was to devise an inexpensive, reproducible surgical simulator that provides the most lifelike representation of the sebaceous cyst. materials and methods: Pigskin, either pig's feet/trotters or pork belly, was incised, and a paintball was inserted subcutaneously and fixed with cyanoacrylic glue. RESULTS: This model has regularly been used in cutaneous surgical courses that we have organized. Either adding more cyanoacrylic glue or allowing more time for the paint ball to absorb fluid from surrounding tissue can also adjust the degree of difficulty. CONCLUSIONS: The degree of correlation with lifelike cutaneous pathology is such that we recommend that all courses involved in basic skin surgery should consider using the porcine sebaceous cyst model when teaching excision of sebaceous cysts.

Keratin 17 mutation in pachyonychia congenita type 2 with early onset sebaceous cysts.

BACKGROUND: Pachyonychia congenita (PC) is a group of autosomal dominant ectodermal dysplasias caused by mutations in four differentiation-specific keratin genes. Two major clinical subtypes of PC have been generally recognized. Symmetrically thickened fingernails and toenails are the defining characteristic of PC type 2 (PC-2) with onset at infancy. Pilosebaceous cysts are the best hallmark of PC-2, but they usually occur at puberty. OBJECTIVES: To report a Chinese pedigree of PC-2 with unusually early onset sebaceous cysts and to explore the genetic mutation and its phenotype. METHODS: Exon 1 of keratin 17 was amplified by polymerase chain reaction (PCR) from genomic DNA from the three patients in the pedigree, the proband, his half-sister and his younger son, two unaffected members in the pedigree and 50 unrelated and unaffected people. PCR products were directly sequenced to detect the mutation. RESULTS: Direct sequencing of the PCR products revealed a heterozygous 275A-->G mutation in all three affected members. This mutation predicts the substitution of asparagine by serine in codon 92 (N92S) located in the 1A domain of keratin 17. CONCLUSIONS: Mutation in the 1A domain of keratin 17 underlies the affected members' phenotype, PC-2 with early onset sebaceous cysts and late-onset thickened fingernails and toenails. The onset of the cysts is very early in some people within this family and the age at onset of thickened fingernails and toenails is variable within the family, implying the existence of modifying factors.

False-positive iodine-131 whole-body scans due to cholecystitis and sebaceous cyst.

False-positive whole-body 131I scans are not frequent but have serious consequences in the management of patients with thyroid cancer. They can be classified in four main groups: elimination of iodine in body fluids, infection or inflammation, cysts or transudates and nonthyroid tumors. We report on two patients with false-positive post-therapy 131I scans. The first patient had uptake projected in the right pelvic area which was later proven to be a large gluteal sebaceous cyst. The second patient had uptake in the gallbladder area that did not disappear after 131I treatment; she underwent exploratory laparotomy which revealed extensive chronic cholecystitis. These cases illustrate two new causes of false-positive 131I whole-body scans (sebaceous cyst and cholecystitis), which highlights two mechanisms (elimination in body fluid and inflammation).

Dermatobia hominis myiasis masquerading as an infected sebaceous cyst.

In an apparently routine case, what appeared to be infected sebaceous cysts of the scalp turned out to harbour botfly larvae imported from a tropical country. Although the parasite Dermatobia hominis is not indigenous to the United States, cutaneous myiasis caused by the fly is fairly common in Central and South America, and several cases have been reported in people returning or emigrating from these regions. The authors therefore suggest that infection resulting from D. hominis invasion be considered in persons who present with furuncular lesions and give a history of travel to endemic areas.

A syndrome of leukonychia totalis and multiple sebaceous cysts.

We report on a family with leukonychia totalis, koilonychia and multiple sebaceous cysts segregating as an autosomal dominant disorder. This condition has only previously been described in two kindreds, and this report documents the natural course of the disorder and provides further evidence for pancreatitis being an associated syndromic feature.

Treating a sebaceous cyst: an incisional technique.

The author describes an incisional technique for removing a sebaceous cyst. This technique considerably reduces the size of the cyst, and if excision of the balance of the cyst is later pursued, the resulting scar is not as conspicuous as the scarring that occurs from conventional excisional surgery.

Helcococcus kunzii as sole isolate from an infected sebaceous cyst.

Helcococcus kunzii was isolated in pure culture from pus drained from an infected sebaceous cyst associated with marked cellulitis. The cyst was excised one month later after the inflammation had subsided with flucloxacillin treatment. This is the first report of the isolation of H. kunzii as the sole pathogen from an infected site.
