Tumorous syphilid.

An unusual form of secondary syphilis, characterised by oozing tumours on the forehead and scalp concomitantly with plaques in the genital area, is presented. The patient was diagnosed as having secondary syphilis by dark-field examination, serologic tests for syphilis, and demonstration of spirochetes in the tissues by the immunoperoxidase technique with avidin-biotin peroxidase complex. Treatment with benzathine penicillin led to rapid resolution of the skin lesions one month later. This case of tumorous syphilid, a variant of papular syphilid, which appears not to have been described in the literature previously.

Ringworm-like late syphilides.

A peculiar form of late, superficial, serpiginous, non-ulcerative syphilides, leaving no visible scars, resembling clinically the ringworm of the glabrous skin, showing the granulomatous histopathological structure and relatively abundant T. pallidum, predominantly spread around the blood vessels is described.

[Ulcerovegetating syphilid of the buccal mucosa (author's transl)].

The authors report a case of ulcerovegetating lesions of the buccal mucosa in a patient with confirmed cutaneous and biological secondary syphilis, and discuss the diagnostic features of ulcerovegetating mucous syphilids which correspond to the lesion originally described by Milian, and to the diagram illustrating the work by Grinspan quoted in the references.

Miliary pustular syphilid.

A case of secondary syphilis is presented in which a dramatic generalized eruption of pustules and an elevated temperature were prominent features. Biopsy of the skin lesions showed dermal granulomata, which are occasionally seen in the papulopustular lesions of secondary syphilis. The classification of pustular syphilids is reviewed.

Horny syphilid.

A thirty-seven year old man presented with volar lesions resembling widespread clavi. Careful physical examination and serologic tests established a diagnosis of secondary syphilis. The unusual lesions best fit the rare clinical entity of horny syphilid. The case emphasizes the importance of maintaining a high index of suspicion of syphilis and the value of the serologic tests in evaluating cutaneous eruptions.

[Skin syphilids, only incidental findings].

Six cases of syphilides- rarely seen cutaneous manifestations today--are reported and the difficulties in finding the correct diagnosis are stressed. Considerations with particular reference to the differential diagnosis of clinically and histomorphologically similar diseases are discussed. Beside case history and serological tests it is of overall importance to bear in mind the possibility of late syphilis.
