Trimethobenzamide HCl in the treatment of nausea and vomiting associated with antineoplastic chemotherapy.

Fifty-five patients undergoing chemotherapy for various neoplastic processes were treated with trimethobenzamide or placebo to control nausea and vomiting. Trimethobenzamide was shown to be significantly better than placebo in relieving periodic and total nausea over the 48-hour study. Vomiting incidence was also reduced in the group receiving trimethobenzamide. Differences were noted in the degree of emetic stimulus associated with different chemotherapeutic regimens. No side effects attributable to trimethobenzamide were recorded.

The relationship between serum and saliva trimethobenzamide concentrations in man.

Six healthy volunteers provided simultaneous serum and mixed saliva specimens at various time intervals after receiving three different concentrations of trimethobenzamide HCI (total dose 800 mg) via rectal syringe on three separate occasions. Specimens were analyzed for trimethobenzamide content and the results subjected to pharmacokinetic analysis and other statistical procedures to identify characteristics of the relationship between distribution processes in these specimen types. On the basis of linear regression analysis there was a significant correlation (P less than 0.01) between saliva and serum drug content for both mean values (r = 0.94) at each sampling time and all individuals' paired data points (r = 0.72). Because of the large degree of scatter in individual pairs, the data were subjected to analysis of variance in an attempt to determine a potential source of this divergence. The relationship between serum and saliva drug concentrations was found to depend upon the time after drug administration. We conclude that a nonuniform process determines the relative degree of partitioning trimethobenzamide between serum and saliva.

Quantitative analysis of trimethobenzamide hydrochloride by ion-pair column chromatography and semiquantitative analysis of 3,4,5-trimethoxybenzoic acid by thin-layer chromatography.

An ion-pair column chromatographic/UV spectrophotometric method for assaying trimethobenzamide hydrochloride in capsules and injections is presented, as well as a method for the detection of 3,4,5- trimethoxybenzoic acid in trimethobenzamide hydrochloride bulk drug and dosage forms. Results obtained by the USP XX, Pharmacopeial Forum, and ion-pair column assay procedures are compared, and results of a collaborative study of the proposed assay and impurity detection methods are presented.

Effect of theophylline and trimethobenzamide when given during kainate-induced status epilepticus: an improved histopathologic rat model of human hippocampal sclerosis.

PURPOSE: The most common pathology in temporal lobe epilepsy (TLE) is hippocampal sclerosis. It is controversial whether status epilepticus (SE) or prolonged seizures plus secondary cerebral injuries are pathogenic mechanisms of hippocampal sclerosis. This study addressed this question in rat models of TLE. METHODS: Hippocampal neuron densities and supragranular mossy fiber sprouting were determined in adult rats subjected to systemic kainate-induced SE (KA-only) and KA-induced SE followed 75 minutes later by theophylline (KA/Theo) or trimethobenzamide (KA/Tri). These drugs probably decrease seizure-induced cerebral hyperemia or hypertension. RESULTS: Compared with controls and KA-only rats, KA/Tri and KA/Theo rats showed decreased CA3b and CA1 neuron densities (i.e., greater Sommer's sector injury). In addition, KA/Tri rats showed that increased trimethobenzamide dosages were associated with decreased hilar, CA3c, CA3b, CA1, and subiculum neuron densities. There were no significant differences in supragranular mossy fiber sprouting between KA-only, KA/Tri, and KA/Theo rats. CONCLUSIONS: Pharmacologic manipulations during KA-induced SE are associated with differences in hippocampal pathology, especially in Sommer's sector, and the final pattern of damage and axon sprouting shows histopathologic similarities to that in patients with hippocampal sclerosis. Our findings support the hypothesis that secondary physiologic insults during SE that are likely to decrease seizure-induced cerebral hyperemia and hypertension may generate greater hippocampal neuronal injury compared with SE alone, and this may be a pathogenic mechanism of human hippocampal sclerosis in patients with TLE.

The antiemetic drug trimethobenzamide prevents hypophagia due to acetyl salicylate, but not to 5-HT1B or 5-HT1C agonists.

Pretreatment with the antiemetic agent trimethobenzamide (TMB) prevented the hypophagic response of rats to acetyl salicylate (a known emetic in man and dogs). However, it did not affect the hypophagic responses to the 5-HT1B agonist RU24969, or to the 5-HT1C/5-HT1B agonists mCPP and TFMPP. The results therefore suggest that the hypophagic effects of the 5-HT agonists do not involve a malaise-dependent mechanism similar to that mediating the effect of acetyl salicylate.

Ion-pair column chromatographic determination of trimethobenzamide hydrochloride in capsule and injection dosage forms: collaborative study.

An ion-pair column chromatographic method was developed for the determination of trimethobenzamide hydrochloride in capsules and injection dosage forms. Detection is by UV spectrophotometry at 261 nm. Recoveries by the Associate Referee ranged from 98.3 to 101.0% for the drug substance. Results by 5 collaborators for capsules averaged 99.1% of labeled or theoretical with coefficients of variation (CVs) of 1.81% (reproducibility) and 1.17% (repeatability); results for injections averaged 100.4% of labeled or theoretical with CVs of 1.91% (reproducibility) and 0.69% (repeatability). The method has been adopted official first action.
