Long-term ECG in ambulatory clinical practice. Analysis and 2-year follow-up of 100 patients studied with a portable ECG tape recorder.

A portable tape recorder for long-term ECG monitoring is described. Its light weight (500 g) and small size (138 X 115 X 39 mm) make its usage in routine clinical practice a practical proposition. The most important application has been in the differential diagnosis of Adams--Strokes syndrome. The results from the first 100 patients with a 2-yr follow-up are presented. The importance of GCG recording during the patients' relevant subjective symptoms is stressed. The mean duration of recording was 2.8 days. In the 28 patients with histories which fitted the symptoms of Adams--Stokes syndrome this diagnosis was confirmed by an arrhythmia recorded simultaneously with the symptoms. In 36 other patients with a similar history the diagnosis was excluded becase of a normal ECG during subjective symptoms. Of the 28 patients with Adams--Stokes sydrome, bradyarrhthmia was the causal factor in 20 patients and these had a pacemaker implanted, whereas the remaining 8 patients had a tachyarrhythmia, which wa treated with antiarrhythmic drugs. The 2-yrs follow-up revealed an improvement and a disappearance of the Adams--Stokes attack in all the patients with an implanted pacemaker. In several of the 36 patients in whom Adams--Stokes syndrome could not be confirmed the syncopal attacks disappeared spontaneously. A large number of arrhythmias, including ventricular and supraventricular tachycardia, 2nd degree AV block and sinus bradycardia were observed during symptom-free intervals in these 36 patients. The introduction of long-term ECG recording routinely in patients with dizziness and syncope of unknown reason has resulted in an increase of the number of patients with a confirmed diagnosis of the Adams--Stokes syndrome, and it has contributed to an increase in the incidence of pacemaker implantation in Malmö from 130 per million inhabitants in 1971 to 220 in 1973, and 1974, respectively.

Adams-Stokes Syndrome

A 16-channel cassette tape recorder system for clinical EEGs.

A 16-channel EEG tape recorder system having a frequency response of DC-100 Hz for each channel is described. The system utilized standard commercially available highfidelity audio tape decks in conjunction with specially designed circuits for time-division multiplexing a balanced amplitude modulation

Electroencephalography

[Comparative evaluation of the response of rat-mothers to the natural pain squeaks of a ratling and to a tape recording of it].

Two kinds of stimuli were presented to the mother rat at random: the natural pain acoustic signal of its infant in response to electrical pain stimulation and the tape recording of the signal performed within the sound frequency region (up to 20 kHz). The mother rat was placed in a special long cage with two parts of different size. Her reaction was ethologically estimated. The natural pain signal proved to be more effective than its magnetic tape recording. The role of the spectrum ultrasound part of the pain acoustic signal is discussed.

ECG-tape recording for analysis of syncopal attacks before and during cardiac pacing.

ECG-tape recording has been utilized for diagnosis of transient attacks of syncope and dizziness both before and during cardiac pacing. The display unit can be utilized for automatic starting of the ECG writer according to markings made by the patient or bradycardia alarm. The method has proved extremely useful and convenient, which is illustrated by cases with different types of arrhythmia and pacemaker failure.

Ambulatory Care

Dynamic mini-computer graphics analysis of long-term electrocardiographic tape recordings.

The full graphic capabilities of a minicomputer have been applied to rapid and versatile analysis of long-term electrocardiographic tape recordings. Interfaced to a standard Avionics tape analyzer, the system's multiple display options provide a powerful tool for arrhythmia detection in a reasonable amount of time. Detection accuracy of the system compares very favorably with previously published figures for computer EKG monitoring. Quantitation of arrhythmic beats is an integral feature of the tape scanning. A method for analog display of randomly selectable EKG complexes is also described.

Arrhythmias, Cardiac

Simple device for the tape recording of complex behavioral situation on one track.

A low cost electronic device is described to record on one a.m. tape track the occurrence of any combination of four signals. The fast read out of recorded signals makes the apparatus reliable and helpful for coding behavioral situation and automatic averaging of event related phenomena. A detailed circuitry is included and discussed.

Electrophysiology

[The use of tape recorders in classes with motor handicapped pupils (author's transl)].

It is demonstrated that physical disabilities involving motor deficits automatically result in communicative limitations which become particularly evident in calsses. As a rule, the consequences of a slow writing performance due to physical disability are a decrease in performance. The supply of intellectually normal but severely physically handicapped pupils with adequate technical aids designed to compensate for their disabilities is, therefore, a necessity which improves their educational opportunities. With the help of an adjusted and technically supplemented tape recorder it is demonstrated in how far a technical apparatus can become the "tool" of a special education teacher.
