[Current viewpoints on osteoarticular tuberculosis].

Nowadays the osteoarticular tuberculosis is a disease of adult and senile age. Among my own patients were most frequently affected the spinal column (55.7%), the hip-joint (10.1%) and the knee-joint (9.4%). If other diagnostic methods fail, to the interest of an early ascertainment of the diagnosis the performance of a test excision should not be delayed. On modern conditions a conservative treatment is sufficient in most cases and achieves good results. Here, the chemotherapy follows the usual directives, in which cases on account of the lacking proof of causative organisms frequently must be issued from the probable position of resistance. Excluding early diagnosed, purely synovial tuberculoses of the joint in young patients, even nowadays every tuberculous skeletal focus needs immobilisation. In case that conservative therapy does not lead to an improvement of the findings, operative measures are indicated. Here, the removal of the focus is the first place. A final solution of the problem of the osteoarticular tuberculosis is to be expected above all within the general combat against tuberculosis.

[Character of changes in protease inhibitory capacity of blood in experimental osteoarticular tuberculosis].

All the steps of osteoarticular tuberculosis evolution were accompanied by an increase in proteolytic activity in rabbit blood serum, which was compensated by activation of alpha 1-inhibitor of proteinases at the initial steps of osteitis. The system "proteinases-inhibitors" was imbalanced during the severe phases of arthritis independently on the rate of its development as follows: predominance of proteolytic potential over inhibitory activity, dissimilar activity of alpha 1-inhibitor of proteinases simultaneously with equal rate of alpha 2-macroglobulin content decrease. Dissimilar alterations in the system studied enabled to suggest that differential application of proteinases is suitable for treatment of inflammation with respect to the step of osteoarticular tuberculosis development.

[Surgical correction of skeletal deformations in children with osteoarticular tuberculosis].

On the grounds of the experience with the surgical treatment of 268 children of different age with tuberculosis of the spine and big joints the types of surgical interventions, aimed at the correction, prevention or elimination of marked skeletal deformities concomitant with the main disease are defined. A high effectiveness of early necrotomies in the prophylaxis of such deformities is emphasized. A critical assessment of resection of the main joints in children of the first 7 to 9 years of age is given, and the indications for this operation are restricted.

[Osteoarticular tuberculosis in children (author's transl)].

A group of sixteen children suffering from osteoarticular tuberculosis were seen over the past five years in the Belgrade Paediatric Surgery Clinic. This disease is now extremely rare and effects chiefly children born outside maternity departments and not receiving BCG vaccination at birth for different reasons. The osteoarticular tuberculosis rate in the Socialist Republic of Serbia over this period was thus one in 500,000 children. The disease is three times as common in male children, most often affecting the hip and presenting above all at about the age of nine. At the time of admission, eight children had tuberculosis lesions affecting the lungs, visible and active or latent, partially or completely. In five children, one of the parents was receiving treatment for active pulmonary tuberculosis at the time of onset of the disease. Conservative treatment (immobilisation, tuberculous bacteriostatic therapy and general measures) was used in fifteen children and only one underwent surgery, in addition to the standard treatment described. In only one child, there was ankylosis of the hip and the end of treatment, the others having less sequelae of returning completely to normal. In one girl, osteoarticular tuberculosis was complicated by exsudative tuberculosis pericarditis.

Age Factors

Serum electrophoretic pattern in osteoarticular tuberculosis.

A serum electrophoretic study was carried out in 37 established osteoarticular tuberculosis cases. The result of this study has been analysed according to severity and chronicity and also after treatment, including certain streptomycin-resistant cases. It has been observed that there was a decrease in albumin and an increase in Alpha2 and gamma-globulin fractions of serum protein as the disease became more advanced and chronic in nature. The administration of known antitubercular drugs reverses this phenomenon. The resistant cases revealed an increase in gamma-globulin fraction at almost the same level as observed in the untreated cases.
