[Some parameters of blood-stasis, yin deficiency, and yang deficiency in patients with remote myocardial infarction].

This paper demonstrates some investigations on the differentiation of symptom-complex for old myocardial infarction patients (OMI). Among total 100 cases, 20 cases of blood-stasis type, 28 cases of Yin deficiency type and 52 cases of Yang deficiency type. Several laboratory investigations had been carried out for them. The results indicated the level of HDL-C was decreased, LDL-C was increased, ratio of HDL-C/TC was also decreased, platelet aggregation test (PAgT) was increased, factor VIII related antigen (VIII R: Ag) was elevated, among the above 3 types, especially in Yin deficiency group, showed statistically significant. In Yin deficiency group, the ACG tracing demonstrated late bulge type or in a plateau form, A/E-O greater than or equal to 15%. Yang deficiency group, SV, CO, CI were decreased, when compared with Yin deficiency all P value less than 0.01. Among 3 differentiation symptom-complex, microcirculation changes and degree of blood-stasis were in same appearance.

[Study on relationship between TCM Syndrome types (Xin-qi and Xin-yang deficiency) and contents of cytokines (tumor necrosis factor-alpha and interleukins) in patients with congestive heart failure].

OBJECTIVE: To explore the relationship between TCM Syndromes types, Xin-qi deficiency (XQD) and Xin-yang deficiency (XYD), and contents of cytokines, including tumor necrosis factor alpha (TNF-alpha), interleukin-6 (IL-6), IL-1beta and IL-10 in patients with congestive heart failure (CHF). METHODS: Sixty-seven CHF in-patients with Xin-qi deficiency or Xin-yang deficiency syndrome were enrolled, their NYHA (New York Heart Association) cardiac function was assessed, ejection fraction (EF) and E peak/A peak (E/A) ratio were determined by Doppler ultrasonic echocardiograph, and serum TNF-alpha, II-6, IL-1beta and IL-10 were measured by double antibody sandwich ELISA assay. RESULTS: The cardiac function grading in patients of XQD was mostly of I and II grade, while that in patients of XYD was mostly of III and IV grade (by Ridit analysis, P<0.01). The E/A and EF values were higher in patients of XQD than those in patients of XYD (P<0.01). Levels of TNF-alpha, IL-6 and IL-1beta were higher and IL-10 was lower in patients of XYD than those in patients of XQD (P<0.05 or P<0.01) respectively. CONCLUSION: TNF-alpha, IL-6, IL-1beta and IL-10 probably play an important role in the development process of XQD to XYD, and could be taken as the microcosmic indexes for differentiation of the two syndromes.

[Effect of monoamine neurotransmitters in the hypothalamus in a cortisol-induced rat model of yang-deficiency].

Eight monoamine neurotransmitters in hypothalamus of normal and cortisol-induced Yang-deficiency rats were measured with HPLC-ECD. These neurotransmitters included: (1) Noradrenaline (NA); (2) Adrenaline (A); (3) 3-methoxy-4-hydroxyphenyl glycol (MHPG); (4) 3,4-dihydroxyphenylacetic acid (DOPAC); (5) Dopamine (DA); (6) 5-hydroxyindole-3-acetic acid (5-HIAA); (7) Homovanillic acid (HVA); and (8) Serotonin (5-HT). With the mobile phase of 10% methanol-0.15 M chloroacetic acid-sodium hydroxide buffer, the neurotransmitters might be well separated, the sensitivity was similar to that of gas chromatography-mass spectrogram with the lower limit of 0.1 ng, this assay was simple and might be accomplished with half an hour. The Yang-strengthening Chinese herbs Radix Aconiti Praeparata, Cortex, Cinamomi, Epimedium brevicorum and Herba Cistanchis were used in the study and the following were founded: (1) In the hypothalamus of cortisol-induced Yang-deficiency rat NA decreased and a increased when compared with that of the normal controls (both P less than 0.05). (2) After 4 weeks of administration of Yang-strengthening herbs, the NA and A of the Yang-deficiency rats returned to nearly normal and it suggested these herbs have the effect of "supporting the right and recovering the source"; DA markedly increased both in control rats and Yang-deficiency rats (P less than 0.05, P less than 0.001 respectively); DOPAC obviously decreased both in control rats and Yang-deficiency rats (P less than 0.001); 5-HT and 5-HIAA increased in the control rats (both P less than 0.001) and HVA significantly decreased in Yang-deficiency rats. (3) The ratio of DA/DOPAC and 5-HT/5-HIAA markedly increased both in the control and Yang-deficiency rats after using Yang-strengthening herbs (P less than 0.01-0.001) and it suggested Yang-strengthening herbs could inhibit the activity of monoamine oxidase in hypothalamus.

[Effects of eight methods of intelligent turtle on free radical metabolism in the kidney-yang deficiency model guinea pigs].

OBJECTIVE: To observe effects of moxibustion according to opening points on time in eight methods of intelligent turtle on aging and the mechanism. METHODS: Healthy and adult guinea pigs were randomly divided into 5 groups, a prevention group of kidney-yang deficiency model with eight method of intelligent turtle (group I), a prevention group of kidney-yang deficiency model (group II) with moxibustion at Mingmen (GV 4), a group of treatment group of kidney-yang deficiency model with eight methods of intelligent turtle (group III), a treatment group of kidney-yang deficiency model (group IV) with moxibustion at Mingmen (GV 4) and a control group of kidney-yang deficiency. Effects of moxibustion according to opening on time in eight methods of intelligent turtle and moxibustion at Mingmen (GV 4) on superoxide dismulase (SOD) activity and malondialdehyde (MDA) level were compared. RESULTS: After modeling, no signs of kidney-yang deficiency and no significant changes of SOD and MDA were found in group I and II; but obvious symptoms of kidney-yang deficiency and decrease of SOD (P < 0.01) and increase of MDA (P < 0.01 or P < 0.05) were found in group III, IV and V; after treatment, in the group III); the symptoms of kidney-yang deficiency basically disappeared, SOD increased (P < 0.01) and MDA decreased (P < 0.01), with no significant differences as compared with those before modeling (P > 0.05); in the group N, the symptoms of kidney-yang deficiency significantly improved and SOD increased (P < 0.05) with difference as compared with those before modeling (P < 0.05), and MDA decreased (P < 0.05), but with no significant difference compared with that before modeling (P > 0.05); the symptoms of kidney-yang deficiency, SOD and MDA did not significantly change in group V (P > 0. 05). CONCLUSION: Both moxibustion according to opening points on time in eight methods of intelligent turtle and moxibustion at Mingmen (GV 4) can effectively regulate free radical metabolism and have obvious action of preventing and treating kidney-yang deficiency in the model guinea pigs, and the therapeutic action of moxibustion according to opening points on time in eight methods of intelligent turtle is better than that of moxibustion at Mingmen (GV 4).

[Experimental study on effect of the water extract of dogwood fruits on the liver and testis in rats model of kidney-yang deficiency].

OBJECTIVE: To explore the effect of Dogwood fruits on tonifying kidney-yang. METHOD: The effect of the water extract of Dogwood fruits on rats model of kidney-yang deficiency with the hydrocortisone was observed. RESULT: The water extract of Dogwood fruits could make normal the liver weight, and mitigate hepatocyte pathologic changes, increase the heptocellular levels of RNA and hepatin, and decrease the malondialdehyde (MDA) in rats model of kidney-yang deficiency. It could also make the viscera quotiety return to normal way and increase the levels of RNA in the interstitial cells of testicle in rats model of kidney-yang deficiency. CONCLUSION: Water extract of Dogwood fruits can protect and improve the functions of the liver and testicle in rats model of kidney-yang deficiency.

[Effect of zhuchun pill on immunity and endocrine function of elderly with kidney-yang deficiency].

After randomly dividing 45 elderly males with Kidney-Yang Deficiency into Zhuchun Pill (ZCP) group and placebo (starch capsule) group by single blind method, and treating them for 3 months, we found that ZCP could greatly improve the main symptoms of Kidney-Yang Deficiency, P < 0.01 by comparison with control group: the 3H-TdR lymphocyte transformation (3H-TdR LCT) rate, level of serum C3, IgA, IgD, IgG & IgM went up (P < 0.01); contents of plasma adrenocortical hormone (ACTH), testosterone (T) and cyclic adenosine monophosphate (cAMP) increased (P < 0.01-0.001): the level of superoxide dismutase (SOD) in serum also increased while lipid peroxide (LPO) in plasma decreased (P < 0.01). But for control group, there were no remarkable changes after treatment, so it revealed that the improvement of Kidney-Yang Deficiency by ZCP is realized through regulating the organism's immunity and endocrine function as well as through scavenging the free radical agents.

[Expression of Smad1 and Smad5 in the testis of infertile rats with kidney-yang deficiency].

OBJECTIVES: To study the expression of Smad1 and Smad5 in the testis of infertile rats with adenine-modeled kidney-yang deficiency and the pathological mechanism of infertility with kidney-yang deficiency, attempting to obtain experimental evidence for the prevention and treatment of male infertility. METHODS: Forty-eight 60 d male SD rats were divided randomly into 6 groups with 8 in each: 7 d, 14 d and 21 d kidney-yang deficiency groups, and 7 d, 14 d and 21 d control groups. The experimental rats had been fed with adenine (300 mg/kg) and the expression levels of Smad1 and Smad5 were measured with immunohistochemical SABC method at the 7th, 14th and 21st day. RESULTS: Smad1 immunoreactivity was mainly located in the spermatogonia, spermatocytes and spermatids, and the reactive substance distributed in cytoplasm with negative nuclei. Sertoli cells and Leydig cells were negative. Compared with the control, the expression level of Smad1 was decreased significantly at the 21st day (P < 0.05), but with no significant difference at the 7th and 14th day (P > 0.05). Smad5 immunoreactivity was mainly located in the spermatogonia and spermatocytes, and the reactive substance distributed in cytoplasm with negative nuclei. Compared with the control, the expression level of Smad5 was not significantly different at the 7th day (P > 0.05). The expression of Smad5 was negative at the 14th and the 21st day. CONCLUSION: The weaker expression of Smad1 and no expression of Smad5 may be one of the pathological mechanisms of infertility with adenine-modeled kidney-yang deficiency.

[Effect of jingui shenqi pill on pituitary adrenocorticotropic hormone gene expression in shen-yang deficiency rats].

OBJECTIVE: To explore the effect of Jingui Shenqi pill (JGSQP) with various concentrations at different time points on pituitary adrencorticotropic hormone (ACTH) gene expression level in Shen-Yang deficiency rats. METHODS: The Shen-Yang deficiency rats were randomly divided into the model control group and the high, medium and low dosage of JGSQP groups. Reverse transcriptase polymerase chain reaction was used to observe the effect of JGSQP on the ACTH mRNA of pituitary tissue in rats treated at different time points (10 d, 20 d, 30 d). RESULTS: As compared with that in the model group, the ACTH gene expression level was significantly higher in the high dose JGSQP group (P < 0.05), and the increment in the medium dosage group was significantly higher in comparing with that in the high and low dosage groups (P < 0.05 or P < 0.01). CONCLUSION: Through up-regulation on ACTH gene expression is possibly one of the mechanisms of JGSQP in treating Shen-Yang deficiency.

[Body temperature changes in cancer patients with yin or yang deficiency and blood stasis syndrome].

The body temperature change of cancer patients with Yin or Yang deficiency and blood stasis syndrome varied within normal limits in a day. The researchers divided 60 cases of cancer patients into Yin or Yang deficiency and blood stasis syndrome groups. Their body temperature was measured 6 times daily with radio thermometer. The results showed: at different time during one day, temperature change of patients in Yin deficiency group fluctuated obviously. The mean temperature of patients in blood stasis group was higher than other two groups. The daily highest temperature of patients in the above two groups appeared at 6:00 pm. Temperature change of patients in Yang deficiency group was stable relatively and the highest temperature appeared at 2:00 pm. The palm temperature of patients who feel dysphoria with feverish sensation in chest, palms and sole was not higher than armpit temperature. These observations should provide new objective bases for clinical measurement time of temperature and syndrome diagnosis.

[Effect of Yangxue bushen tablet on ovarian function in animal model of Yang deficiency].

OBJECTIVE: To study the therapeutic mechanism of Yangxue Bushen tablet (tablet for nourishing blood and tonifying Kidney) on the Kidney deficiency type of functional amenorrhea and infrequent menstruation. METHODS: Applied with orimeten, models of Yang deficiency in female rabbits were made, ikaclomine was taken as the control drug. Group I was the group of animal model of Yang deficiency; Group II was the ikaclomine treatment group; Group III was the high dosage treatment group; Group IV was the low dosage group; Group V normal rabbit treated with distilled water. The functional effect of gonadal axis was evaluated by the changes of the signs of animal histomorphology of ovary and uterus, and the level of beta-endorphin. RESULTS: The animals of Yang deficiency in Group II, III and IV recovered as a whole, and the changes of different levels of folliculi, morphosis of endometria and deciduous vaginal epithelial cell in group II, III and IV were more obviously than those in group I and V (P < 0.05). The content of beta-endorphin in blood plasma in group I was 106.6 pg/ml, which was lower than those in the other groups (P < 0.05), and the contents of beta-endorphin in blood plasma, in group III, IV and II were similar to that in group V (P < 0.05). CONCLUSION: Yangxue Bushen tablet has the effect on regulating menstruation by regulating the function of gonadal axis, regulating and promoting the ovarian function.

[Moxibustion combined with Jianpi Wenshen formula for cancer-related anorexia of spleen-kidney yang deficiency: a randomized controlled trial].

OBJECTIVE: To observe the clinical efficacy of moxibustion combined with Jianpi Wenshen formula in treating cancer-related anorexia of spleen-kidney yang deficiency. METHODS: A total of 80 patients with cancer-related anorexia of spleen-kidney yang deficiency were randomized into an observation group and a control group, with 40 cases in each group.Jianpi Wenshen formula was given orally in the control group. On the basis of the treatment in the control group, moxibustion was applied at bilateral Zusanli (ST36), Neiguan (PC6) and Zhongwan (CV12), Guanyuan (CV4), Mingmen (GV4) in the observation group, once daily for the first 15 days, then once every other day. Both groups were treated for 4 weeks. The scores of appetite, Karnofsky performance status (KPS), nutritional risk screening 2002 (NRS-2002), and patient-generated subjective global assessment (PG-SGA), as well as the serum levels of interferon-&#x3b3; (IFN-&#x3b3;) and C-reactive protein (CRP)were observed before and after treatment, and the clinical efficacy was evaluated after treatment in the two groups. RESULTS: After treatment, the scores of appetite, NRS-2002, PG-SGA, and the serum levels of IFN-&#x3b3; and CRP were decreased compared with those before treatment in the two groups (P<0.001, P<0.05, P<0.01), and the above indexes in the observation group were lower than those in the control group (P<0.05, P<0.001); the KPS scores were increased in the two groups compared with those before treatment (P<0.001, P<0.05), and the KPS score in the observation group was higher than that in the control group (P<0.001). The marked response rate was 57.5% (23/40) in the observation group, which was higher than 30.0% (12/40) in the control group (P<0.05). CONCLUSION: Moxibustion combined with Jianpi Wenshen formula can effectively improve appetite and overall status, and reduce the levels of IFN-&#x3b3; and CRP in patients with cancer-related anorexia of spleen-kidney yang deficiency.

[Effects of coumarins from cnidium monnieri on the function of pituitary-adrenocortical axis in kidney yang deficiency rats].

OBJECTIVE: To observe the effect of Cnidium monnieri in Kidney Yang Deficiency rats. METHODS: Rats were injected intraperitoneally with hydrocortisone acetate to replicate the animal model of Kidney Yang Deficiency. Osthol, total-coumarins (TCR) from the fruit of Cnidium monnieri and positive control drug (PCD) were administered orally to model rats. The plasma levels of corticosterone (B), adrenocorticotropic hormone (ACTH), renin (PRA), angiotensin-II (AT-II) and aldosterone (ALD) were determined by radioimmunoassay. RESULTS: Compared with those in normal rats, the levels of B and ACTH in the plasma of model rats lowered significantly (P < 0.01), the levels of B and ACTH of treatment groups were higher than those of model group, while the levels of PRA, AT-II and ALD had no significant difference (P > 0.05). CONCLUSION: The osthol, TCR have the effects in protecting and strengthening the function of pituitary-adrenocortex axis.

Clinical and molecular evaluation of warming and tonic herb treatment for sibling patients of a typical kidney-yang deficiency family.

It is essential to explore the molecular therapeutic effect of warm and tonic herb treatment for individuals with typical kidney-yang deficiency. In this report, we have identified members of a family with a history of suffering from cold and kidney-yang deficiency syndromes. First, we have employed the accumulated scores of the 40-items clinical scoring indicators for kidney-yang deficiency and cold syndromes to clinically assess the presence or absence of the deficiency for 15 family members. We then proceeded to compare the gene expression profiles of RNA isolated from blood samples, prior to and post-herbal treatment, of a sibling (brother and younger sister) that are suffering from the deficiency using cDNA microarrays with 18816 genes. Following treatments with the warming and tonic herb, the accumulated clinical scores obtained from the 40-items clinical scoring indicators were compared to those obtained pre-treatment. It was observed that the accumulated clinical scores were reduced by 1/3 for the brother and 2/3 for his younger sister following the treatments. Furthermore, we have demonstrated that the level of gene expression for a total of 33 genes at pre-treatment was modulated after treatments with the warming and tonic herb and correlated well with the clinical improvements of their syndromes. These results suggest that the combination of gene profiling and the accumulated clinical scores obtained from the 40-items clinical scoring indicators may provide an accurate clinical assessment and a way to monitor the therapeutic efficacy of the warming and tonic herb treatment.

[Ia antigen expression of peritoneal macrophage in mice model of yang deficiency and the effect of aconitine].

The immune associated (Ia) antigen expression (IAAE) of macrophage is one of the important indicators of the specific immunity in the body. In this paper, mice of Yang Deficiency model was made by injecting corticosterone in the hind legs of mice to inhibit the function of hypothalamus -pituitary-adrenal axis. Authors observed the changes of IAAE in model mice and the effect of Aconitine on it. The results indicated that: (1) In model mice, the IFN-gamma induced IAAE of peritoneal macrophages was inhibited. The difference of IAAE between the model and the normal mice was significant (P < 0.01). (2) Aconitine could significantly increase the IAAE of model mice (P < 0.01). These data implied that the immuno-suppression in the model mice of Yang Deficiency might be due to the lowered IAAE of macrophages under this condition. The promoting effects of Aconitine on IAAE enhanced the antigen presenting ability of macrophages and the immune response of the model mice. It might be related to the therapeutic mechanism of Aconitine on Yang Deficiency Syndrome.

[Effect of osthol and total coumarins of fructus cnidii on thyroid hormone and thyrotropic hormone in kidney-yang deficiency rats].

Osthol and total coumarins of Fructus Cnidii were given to the model of Kidney-Yang Deficiency rats induced by hydrocortison acetate injection, the changes of serum concentration of thyroxine T3, reverse T3 (rT3), T4 and thyroid stimulating hormone (TSH) were observed. Results showed that in comparison with normal rats, all of the criteria mentioned above were lowered significantly (P < 0.01) in model rats. After treatment, the criteria observed were all increased significantly (P < 0.01 or P < 0.05). It was suggested that the osthol and total coumarins of Fructus Cnidii could elevate the pituitary-thyroid axis function of Kidney-Yang Deficiency rats.

Familial characteristics of kidney-yang deficiency and cold syndrome.

Family investigation is a reliable model to study the effects of both genetic and environmental factors on human health. This article studies kidney-yang deficiency syndrome and cold syndrome through family investigation and cDNA microarray technology, exploring the effects of both genetic and environmental factors on the health of family members. Particularly, these two syndromes were first assessed by the accumulated clinical scores measured by 40-item scoring tables among 15 family members. The family patterns were obtained and the correlation of these two syndromes was determined. Then the gene differential expression profiles among 12 family members were obtained using an 18,816 clones cDNA microarray. The profiles of the patients with typical kidney-yang deficiency syndrome and cold syndrome were compared to those of normal members and 89 differential expression genes were found. Further, only 22 genes were identified as known functions, and most (16 genes) were associated with the regulation of metabolism, temperature feeling, and growth. Therefore, the formation and development of these two syndromes have not only genetic but also environmental factors, including living conditions and lifestyle.
