[Cytokinetics of epidermal and dermal cells in allergic contact dermatitis and atopic dermatitis (author's transl)].

The cytokinetics of epidermal and dermal cells was examined in 16 patients with subacute or chronic allergic contact dermatitis and 11 with atopic dermatitis. In allergic contact dermatitis a H3-thymidine-labelling index (H3-1) of 4.55 +/- 2.4% was found in the epidermis and of 0.7 +/- 0.44% in the dermal infiltrate. In comparsion, in atopic dermatitis the values were 5.5 +/- 2.31% in the epidermis and 0.93 +/- 0.72% in the dermis. The DNA-synthesis-time (ts) was lengthened slightly with 11.4 +/- 2.4 h in allergic contact dermatitis and 11.75 +/- 3.68 h in atopic dermatitis. In comparsion with normal skin, the cell-cycle-time (tc) was slightly shortened in allergic contact dermatitis (244 +/- 110 h) and in atopic dermatitis (233 +/- 83 h). There was no significant difference between both types of dermatitis regarding cytokinetics.

Immunofluorescence of the skin in allergic diseases: an investigation of patients with contact dermatitis, allergic vasculitis and atopic dermatitis.

Skin biopsies for immunofluorescent studies were taken from patients with contact dermatitis (positive patch tests), atopic dermatitis and allergic vasculitis for comparison with normal-appearing skin from the same patients, and from healthy controls. A variety of deposits of immunoglobulins, complement components and fibrinogen were demonstrated in 6 out of 20 patients with contact dermatitis, 7 out of 10 with atopic dermatitis, 8 out of 10 with allergic vasculitis, and in 4 out of 20 control individuals. No diagnostic pattern of deposits was found. Elevated serum IgE and eosinophilic counts were found in patients with atopic dermatitis, and high serum IgA and fibrinogen levels were found in the allergic vasculitis group.

Antabuse treatment of nickel dermatitis. Chelation--a new principle in the treatment of nickel dermatitis.

Eleven nickel-hypersensitive patients with chronic, dyshidrotic hand eczema aggravated by oral challenge with 0.6-2.5 mg nickel were treated with 100 mg tetraethylthiuramdisulfide (Antabuse) two to four times daily for 4-10 weeks. Nine of the patients experienced a flare of the dermatitis shortly after initiation of the treatment. During the course of treatment the dermatitis of seven patients cleared, improvement was seen in two patients, and in two the dermatitis remained unchanged. Flare was seen in six patients when the treatment was discontinued. Seven patients experienced side effects such as fatigue, headache and dizziness. The treatment of four patients was discontinued due to side effects. During the treatment high levels of nickel were found in the serum and urine.

Allergens

Phototoxic textile dermatitis ("bikini dermatitis").

Phototoxic textile dermatitis with subsequent hyperpigmentation developed in two patients after they wore bikini bathing suits. After extraction of the dye from the bathing suits, 15 fractions could be visualized by two-dimensional thin-layer chromatography. Two of these fractions are found in Disperse Blue 35, an anthraquinone dye known to give rise to occupational phototoxic dermatitis, but which, to our knowledge, has never been reported to cause dermatitis in consumers. One of the two fractions was also found to cause phototoxic reactions in normal subjects.

Adult

Occupational dermatitis from Lactuca sativa (lettuce) and Cichorium (endive). Simultaneous occurrence of immediate and delayed allergy as a cause of contact dermatitis.

Four patients with occupational contact dermatitis to Lactuca sativa had cross-sensitivity to Cichorium endivia. One of the patients also had contact urticaria to Lactuca and Cichorium, and another reacted positively to scratch tests with these plants as a sign of immediate allergy. In two cases such immediate allergy was considered the cause of a vesicular, intense itching eruption within a few minutes of contact with fresh leaves of Lactuca on previously eczematous skin. The severe chronic dermatitis of the hands of these patients is ascribed to combined delayed and immediate allergy.

Adult

Perioral dermatitis and rosacea-like dermatitis: clinical features and treatment.

Nine cases of perioral dermatitis and 16 cases of rosacea-like dermatitis have been treated for the last 3 years at our clinic. These cases were all females whose ages ranged from 18 to 61 years with a mean of 45 years. All had been under treatment with fluorinated steroids for a period of from 3 months to 10 years, about 3 years on the average. Tetracyclines were beneficial as remedies for these conditions, bringing about healing or remission in approximately 3 months. About 1 year was required for the recovery from atrophy of the skin and telangiectasia. Hydrocortisone 17-butyrate was more useful as a topical medication than hydrocortisone acetate but gave rise to withdrawal rebound eruptions in some cases.

First epidemiological study of contact dermatitis in Spain - 1977. Spanish Contact Dermatitis Research Group.

The present work is the first epidemiological study carried out by the Spanish Contact Dermatitis Research Group during 1977. During this year 2806 patients were studied with patch test among 30873 dermatological patients. The 60-62% of the totality had reactivity to one or more patches. Four major groups of allergens were able to consider, following the incidence in their power of sensitize. First group with strong incidence include: Nickel, Chromate, Cobalt, T.M.T.D.,P.P.D.A., Mercapto mix., and Wood tars. Second and third groups with medium incidence contain: Caines, Carbonates, Neomycin, Balsam of Peru, Mercury, Lanolin, Naphtyl mix., Formaldehyde, Benzalkonium chloride, P. P. D. A. mix, and Turpentine. Four group show very low incidence substances, as: Epoxi, Sulfonamides, Etilendiamine, Parabens, Chinoform, Colophony and Cinnamon oil. Few comments about age and occupations are included.

Adolescent

Immediate hypersensitivity in hand dermatitis. Role of food-contact dermatitis.

A patient had a chronic hand eczema, presumably as a manifestation of atopy. Treatment resistance appeared due to handling certain foods that produced burning and stinging in the chronically eczematous skin and not in otherwise normal skin. Selected tests for delayed hypersensitivity and for immediate hyper sensitivity on intact skin of the back produced negative results, but on chronically inflamed skin of the arm and back, application of the pertinent foods produced a wheal and flare response. On intact skin, scratch tests with the foods produced positive results. Intradermal tests with commercial antigens were negative. Avoidance of these foods, as contactants, led to resolution of the dermatitis. Reappraisal of the role of immediate-type hypersensitivity in chronic hand eczema is important.

Eczema

Sulfur amino acid requirement and foot pad dermatitis in turkey poults.

Five experiments were conducted with Large White poults to determine the effect of methionine and other sulfur-containing compounds on the incidence and severity of dermatitis and on growth and feed efficiency to three or four weeks of age. The poults were housed in battery brooders with wire screen floors in four experiments" and in floor pens with litter in one experiment. Male turkeys were used in four experiments and female turkeys in one experiment. Either a corn-soy diet or a cornstarch-soy diet was used as the basal diet in the various studies. A high incidence of foot-pad dermatitis was observed in poults fed the basal diets unsupplemented with methionine. A high incidence of a dermatitis on the upper part of the beak was also observed in poults maintained in battery brooders but not in floor pens. Adding methionine to the diets significantly lowered the incidence and severity of dermatitis, but cystine and potassium sulfate failed to modify dermatitis. Some foot-pad dermatitis was still observed in poults fed levels of methionine more than adequate to meet the requirements for optimum growth and feed efficiency. The incidence and severity of foot-pad dermatitis generally increased with age during the experiment among poults fed methionine-supplemented diets. Although methionine deficiency is a major cause of foot-pad dermatitis in poults, other environmental or dietary factors also appeared to be involved in the development of the condition. The methionine requirement for optimum growth, feed efficiency, and prevention of dermatitis was approximately 0.6% or 2.1 g. per mcal. of metabolizable energy (M.E.). This is higher than the present recommendation of the National Research Council. With corn-soybean meal diets the requirement for total sulfur amino acids is approximately 1.05% or 3.7 g. per mcal. of M.E.
