Diagnostic and differential diagnostic aspects in histiocytosis X diseases.

Diagnostic and differential diagnostic aspects of the known manifestations of histiocytosis X are discussed with respect to our own findings. It is our considered opinion that the term "histiocytosis X" should not be employed in isolation for diagnostic purposes. Rather, one should make an effort to designate the disease exactly in stating the prognosis for a particular patient. Both clinical and morphologic parameters must be considered in making this exact diagnosis.

[Cerebral sclerosis. Diagnostic criteria and differential diagnostic consideration in practice].

In"cerebral arteriosclerosis" the diffuse sclerotic involvement of the cerebral vessels may produce acute softening of cerebral tissue. However this paper concentrates mainly on the clinical symptomatology which, in the absence of major vascular accidents, is characterized from the psychopathologic viewpoint by acute confusional states, aggressive behaviour, fluctuating loss of memory, disturbances of concentration and finally dementia. The chief neurologic symptoms are motor disturbance with short-stepping gait, stooped position of the body, pseudobulbar symptoms with dysarthric speech and disturbances of swallowing, and increased perioral reflexes. A complete case history and a thorough neurologic and psychopathologic examination are the most important factors in diagnosis, while ancillary methods are of value only for differential diagnosis. Prophylaxis and therapy (cardiotherapy, treatment of diabetes and hypertension, lowering of serum cholesterol and sedation) are discussed. In the differential diagnosis of dementia in the elderly patient consideration should be given to chronic vascular diseases, degenerative cerebral atrophies, brain tumors, low pressure hydrocephalus, progressive paralysis and some other rare brain conditions.

[The diagnostic significance of immunoglobulin determination in chronic liver diseases. I. Differential diagnostic accuracy of immunoglobulin changes and enzyme activities (author's transl)].

715 patients with hepatobiliary diseases came for examination, who because of the morphological findings are classified in diagnostic groups. The immunoglobulin measurements of IGG, IGA, IGM were determined and assessed in combination with enzymatic investigations of GOT, GPT, AP and GGTP. Typical group-specific changes were only found in primary biliary liver diseases and toxic cirrhoses of the liver, all acute or chronic inflammatory liver diseases could not be separated and by inference from the final diagnosis showed great errors in classification. Data on the raised mean levels of immunoglobulins in the individual diagnostic groups were demonstrable, but the range limits were so widely scattered that their differential diagnostic valence is of no consequence. Immunoglobulins may appear to be of interest for the observation of the course of liver diseases, but they are unsuitable for diagnostic purposes.

[Differential diagnostic studies using the dermatoscope in selected diseases].

The Zeis dermatoscope can be used beyond its known applications for precise and detailed skin surface examinations of different types of dermatosis. Differential diagnostic problems are solved by comparative observation of efflorescence and efflorescence fields of similar appearance. The examination series presented, the results of which are discussed in detail, is concerned mainly with surface examinations of parapsoriasis en plaques, seborrhea en plaques and the clinically similar early forms of mycosis fungoides. Also mentioned are examinations of lichen sclerosus et atrophicus and circumscribed scleroderma, lichen planus, Hutchinson-Dubreuilh freckle and similarly appearing facial verruca seborrhoica, as well as fresh dispersions of foreign bodies. The didactic value is underlined by color photographs taken of diseased skin and nails with the dermatoscope at various magnifications. Finally, a description is given of possible instrument extensions.

Periodic acid Schiff-positive non-glycogenic globules in hepatocytes. Differential diagnostic aspects in screening for alpha-1-antitrypsin globules in an autopsy material.

Thirty-eight subjects with diastase-resistant PAS-positive cytoplasmic globules in hepatocytes were found among 238 autopsies. In 15 of the 38 subjects the globules were antigenically alpha-1-antitrypsin, in 23 subjects they were not. The latter globules were found in centrilobular regions, the alpha-1-antitrypsin globules mainly in periportal regions. The non-alpha-1-antitrypsin globules showed less differences in size (6-10 mu) and a smaller number per hepatocyte (1-7) than the alpha-1-antitrypsin globules (1-40 mu and 1-30 per cell). The non-alpha-1-antitrypsin globules were only demonstrated in livers with centrilobular sinusoidal dilatation having, in all cases but one, also centrilobular, confluent necrosis. This type of globules can be assumed to be of differential diagnostic importance mainly in an autopsy material. The nature of these globules is discussed.

Differential diagnostic features of the radionuclide scrotal image.

Differential diagnosis of scrotal lesions is aided by correlating radionuclide images with clinical findings. Subacute torsion is associated with peripheral hyperemia and can be mistaken for an inflammatory process; however, in a review of 128 studies, torsion and orchiectomy were the only processes encountered which had a center truly devoid of activity on the tissue phase compared to the normal side. Other lesions such as acute inflammation, abscess, hematoma, and hemorrhagic tumor may superficially appear to lack central activity but invariably contain at least as much activity when compared to the normal side.
