[Gastric diseases in foreign workers in the German Federal Republic (author's transl)].

Gastric biopsies from foreign workers were compared with those from German patients of the same age group. With reference to population statistics investigations showed that a far higher percentage of foreign workers are gastric patients than are Germans of the same age. Chronic atrophic antral gastritis occurs in foreigners twice as often as in a comparative group of Germans. 77% of the duodenal ulcers and 88% of the gastric ulcers are found in the group of foreign workers examined aged from 21 to 40 years. The German patients, on the other hand, show normal gastric mucosa with more than chance frequency compared with foreign workers of corresponding age and sex.

[Polychlorinated biphenyls (PCB) in food. The situation in the Federal Republic of Germany (author's transl)].

In a coordinated research program of the Federal Ministry of Research and Technology (BMFT), a screening study was carried out with representative samples. In spite of rather large ranges of PCB concentrations, the most important food-stuffs show clearly defined clusters with mean values of approx. 0.005 microgram/g in low fat food components of plant origin such as cereals or potatoes, approx. 0.05 microgram/g in vegetable fats, approx. 0.3 microgram/g in fat from milk, butter and cheese, approx. 0.15 microgram/g in animal fat, approx. 0.03 in chicken eggs and approx. 10 microgram/g in fish fat. Considering the mean diet in the FRG, a daily PCB intake of about 29 microgram from animal fat and of 6 microgram from the other food-stuffs results. The total intake of about 35 microgram per day and capita is almost the same as the figure conditionally suggested by the WHO as the acceptable daily HCB (hexachlorobenzene) intake.

This survey on the gerontological research shows the topical main accents of work in this field in the Federal Republic of Germany: Social services for aged, old-age-security, social and health situation, and older workers' problems are the most important items of the essay. Moreover, there is a prospect on the different types of projects, research institutions and where they are distributed in the different areas and the different ways of financing this research.

[Rehabilitation in France: a comparison with the Federal Republic of Germany (author's transl)].

The purpose of the following article is to present the rehabilitation efforts in France. It describes legal provisions, especially the French Rehabilitation Act of 1975, the "Loi d'orientation", with some of its most important rules of application, as well as some concrete individual measures. It describes approaches which differ from those in the Federal Republic of Germany. The article focuses particularly on the activities of the voluntary organisations and the efforts towards a barrier-free architecture.

[Poliomyelitis in the German Federal Republic 1974-1975 (author's transl)].

The virological surveillance of poliomyelitis in 1974-1975 led to the detection of specific characteristics of wild viruses in 57 out of 221 cases of poliovirus isolation. The disease symptoms were typical for poliomyelitis in 36 of these cases, less characteristic in 13. Polio wildvirus was isolated 8 times from the surroundings of the patients. In 36 foreign patients and one German the causative agent was imported from an endemic area. In these cases and in a further 7 German patients the disease was sporadic. In contrast 5 cases in German children in late autumn 1975 constitute a local epidemic of poliomyelitis.

Adolescent

[Statistical survey on laparoscopy in gynaecology up to 1977 in the Federal Republic of Germany (author's transl)].

1385 questionaires were sent to gynaecologists in West Germany. 682 (49.2%) were answered anonymously. 302 answers (44.2%) revealed that the gynaecologists did not do endoscopy. 265.900 pelviscopies or laparoscopies were included in the statistics. The laparoscopies or pelviscopies were done from 1949--1977 by 380 gynaecologic surgeons or gynaecological departments. In Germany to the end of 1977 unipolar high frequency current was used for hemostasis in 52%, bipolar high frequency current in 42%, and modern endocoaglation in 25% of the departments. In 240 departments (63%) the five to 8 mm. pelviscope was used. In 124 department (33%) the 11 mm. laparoscope was used. In the total of the cases 949 (3.56/1000) complications occurred. These required 690 laparotomies (2.59/1000). The cause of the complications were injuries to vessels or high frequency current burns. A total of 24 (0.09/1000) deaths or 1 death per 11,000 pelviscopies was reported. The operative mortality was 13 deaths (0.09/1000) in 140,977 (53%) purely diagnostic pelviscopies or laparoscopies, 3 deaths (0.08/1000) in 37,639 (14.2%) combined diagnostic and operative pelviscopies and 8 deaths (0.09/1000) in 87,284 (32.8%) laparoscopic tubal ligations. Since 1974 the operative mortality is less than 1:100,000. Since 1973 approximately 30--35,000 pelviscopies are done yearly in West Germany.

[Sugar-free, tooth-protecting chewing gum and candy. Results of a 7-year study].

Experiences of 7 years with sugarless chewing gums and lozenges (tab. I and II) regarding their tooth protective properties are reported. Telemetry of interproximal plaque pH allows to assess acid formation from carbohydrates by plaque bacteria under almost natural conditions. Altogether, 5 chewing gums and 8 lozenges containing sorbitol or mixtures of sorbitol and hydrogenated oligosaccharides were investigated. Lowest pH values during and after chewing sugarless gums varied between pH 6.0 and 7.3. When sucking sugarless lozenges the recorded pH values were between 5.8 and 7.0. In contrast to lozenges, the consumption of sugarless chewing gums becomes particularly important due to their greater stimulation of saliva and buffering capacity of oral fluid. All products tested did not acidify interproximal plaque below the critical pH and therefore comply with the regulations of the Swiss Federal Health Authorities with respect to the labeling or marketed sweets with "safe for teeth". New non-fermentable sugar-replacing substrates are being developed. Their utilization in foodstuffs and sweets is being discussed.

Within the scope of an additional micro-census inquiry--carried out in 1974--the Federal Statistical Office in Wiesbaden assessed the number of physically, mentally and psychologically impaired children. The results are differentiated according to the types of disability and therapy, age, in-patient treatment and special school attendance. Furthermore, the estimated results are compared with earlier surveys of the Federal Statistical Office and other estimates.

[Surgery of pancreatic endocrine tumours in the German Federal Republic: results of a survey (author's transl)].

Within a ten-year scan (1967-1976) 207 insulinomas, 50 gastrinomas, 8 Verner-Morrison tumors, 5 glucagonomas and 12 endocrine pancreatic tumours with associated MEA syndrome (multiple endocrine adenomatosis) were treated surgically at various university hospitals (information obtained by questionnaire). Half of the insulinomas were treated by enucleation, one third by resection of the tail of the pancreas. Total gastrectomy was the procedure of choice in 80% of patients with gastrinoma, but sometimes pancreatic resection to remove the tumour was added. An new therapeutic concept of using histamine-H2 receptor antagonists for treating patients with Zollinger-Ellison syndrome is discussed. In the eight patients with a Verner-Morrison syndrome removal of the tumour or distal pancreatic resection was the procedure of choice.

Acute Kidney Injury

[The conservative treatment of acute pancreatitis in the Federal Republic of Germany in 1977 (author's transl)].

A questionnaire on the conservative treatment of acute pancreatitis was answered by 312 surgical [n = 139] and medical [n = 173] departments from all over Western Germany and from West-Berlin. Nearly total positive agreement was found about routine administration of parenteral fluids and on the prohibition of oral food and fluids. The application of glucocorticoids, glucagon, heparin in small doses and of a carboanhydrase inhibitor [Diamox] is rejected by most of the departments. Renal failure is treated in 63% by hemodialysis and in 37% by peritoneal dialysis. There is great disagreement between the answers about the administration of atropine, antacids and aprotinin [Trasylol]. Antibiotics are applied routinely by 63% of the surgical and 70% of the medical departments.

Institutional data commons: a federated Data Use Certification-aware architecture for secure and scalable data use in biomedical data ecosystems.

BACKGROUND: Modern biomedical data ecosystems increasingly rely on global cloud platforms to coordinate access to large-scale genomic and clinical datasets. However, operational governance remains largely investigator-centric, shifting the responsibility for complex security, compliance, and infrastructure management to individual laboratories. As data volumes and regulatory requirements expand, this approach fails to scale across the research enterprise. This disjointed approach creates a substantial governance burden and can slow down scientific progress. In centralized cloud environments, investigators face siloed identity management and high costs, leading to inefficient data use and increased risk when integrating local and global datasets. MATERIALS AND METHODS: We examine limitations in the current infrastructure and propose reframing institutional data commons as governance-aware intermediaries to ensure secure, efficient and sustainable use of controlled-access biomedical data. RESULTS: This federated architecture decouples storage from authorization, enabling dynamic access linked to active certifications, whether data are analyzed in situ on global platforms or in local governance-aware institutional access environments. DISCUSSION: Shifting governance from investigators to institutional infrastructure ensures that biomedical research remains both secure and economically sustainable.

[Causes and outward forms of poverty in old age (author's transl)].

In the Federal Rupublic nearly 20% of the elderly people live on the edge of below the property-line, which is well-marked by public assistance. In spite of legal claims 75% of the needy elderly do not explore all possible avenues. The ascertainment of this high percentage which had been never estimated exactly-about which, hitherto, impressionistic or opportunistic declarations have been made rather than reliable assertions-is a result of a representative empirical research. The authors have conducted structured interviews, analysis of official files and explorative interviews with old people and with those who are, in a broad sense, competent regarding their need. The analysis of the old peoples' social biographies significantly showed the invariance regarding the class the necessitous belong to; generally workers or people of the farming section either become or remain poor. Deficient vocational education is the beginning of a lifelong outstanding external determination. Their non-problematical replaceability at work surrenders them to the enterpreneur's calculation. When growing older they often endure professional depreciation and financial reduction. After they have retired, need encreases. Individual failure is not the cause for poverty, however the belonging to the working-classes; all those belonging to these classes are potentially poor, due to their replaceability and exchangeability at work. At the lower end of the class-hierarchy potential poverty changes into actual poverty. Causative regarding formation of these classes is the fact, that the material possibilities of the social production cannot be used by the actual producers. The causes for poverty which reproduces itself always within the same classes can't be removed by legal means which can only relief poverty.

Age Factors

[Possibilities and problems in the rehabilitation of aged patients with mental illness (author's transl)].

In the Federal Republic of Germany as in other Western Countries institutions suitable for the rehabilitation of aged patients with mental illness are lacking. As long as "psychiatry of the higher age" is synonymous to "psychiatry of demented patients" the term "rehabilitation" means only a slogan, not a maxime. Jurisdiction in the Supreme Court though, has shown the way of going on. It will depend also on the attitude of the doctors in charge of the patients to fulfill the expectations posed in that jurisdiction and to reach so the far set marks in the interest of the patients. Big institutional deficits have to be ameliorated as far as possible; of prime importance here is the necessity to place the opportunities for the employment of qualified employees. Rehabilitation instead of custodial care here is synonymous for efficiency and humanity. The change from an obsolete and nonorganized "system" of care to a modern network of rehabilitation should be obtained by voluntary cooperation and by voluntary coordination of medical and social institutions. So an integration of all possibilities of help to a network could be reached.
