Multi-locus allelic architecture underlying natural variation in leaf rolling in japonica rice.

Leaf rolling is a key component of rice canopy architecture that affects light interception, microclimate formation, and planting density. The contribution of naturally occurring allelic variation to quantitative variation in leaf rolling within cultivated rice remains poorly understood, while extreme leaf rolling caused by loss-of-function mutations often results in detrimental pleiotropic effects. Herein, we examined how multi-locus allelic variation contributes to natural variation in leaf rolling within japonica rice. Leaf rolling was quantified based on the leaf rolling index (LRI) using a panel of 201 japonica accessions. The phenotype was transformed using the Yeo-Johnson method to reduce strong right skewness and improve the distributional properties of the data, thereby facilitating subsequent regression modeling. Haplotype analyses were performed for previously reported leaf rolling-associated genes and genome-wide association study (GWAS) lead loci, leading to the identification of five loci exhibiting substantial haplotype-dependent phenotypic variation. Phenotypically defined allelic groups represented these loci were subsequently evaluated using multiple linear regression (MLR), with the first two principal components derived from genome-wide SNP data included as covariates to account for population structure. The final MLR model identified four loci (qALR1, OsYABBY1, OsSLL2, and OsSRL10) as the independent contributors to leaf rolling variation, collectively explaining 21% of the variance in the transformed phenotype after accounting for population structure. Model diagnostics and ten-fold cross-validation supported the statistical validity of the framework and indicated stable model performance across validation folds. Analysis of multi-locus allelic combinations showed 13 distinct configurations that clustered into three phenotypically differentiated groups. This reflected the cumulative dosage of high-leaf rolling alleles. Thus, the natural variation in leaf rolling in japonica rice is governed by the additive effects of multiple moderate-impact loci. The multi-locus allelic framework established here provides a statistically sound and biologically interpretable basis for dissecting polygenic canopy traits and practical guidance for developing genetic materials aimed at optimizing rice plant architecture.

[Evaluation of effectiveness and tolerance of the long-term treatment with bromoperidol decanoate in psychotic disorders].

An open study in 20 patients (7 M, 13 F, mean age 40 years) has been performed in order to evaluate the effectiveness and the safety of bromperidol decanoate in the long-term treatment of psychotic disorders. Patients were selected according to DSM-III-R diagnostic criteria (schizophrenia, residual type: 6 patients; disorganized type: 4; paranoid type: 5; undifferentiated type: 2; atypical psychosis: 3) and treated with bromperidol decanoate 150 mg i.m. (single administration) every month for 6 months. The BPRS scores significantly decreased at the end of the therapy with respect to the beginning (-39.5%; p less than 0.01 Friedman analysis between times); this improvement was already significant at the first month control. The results obtained from a clinical global impression, evaluated from the Visual Analogical Scale (VAS), showed that all patients improved at the end of the therapy (p less than 0.01 ANOVA between times). The side effects, mainly extrapyramidal ones, were mild and did not interfere with the therapy; in fact none patient dropped-out from the treatment. Bromperidol decanoate showed to have a good efficacy and safety in the therapy of psychotic disorders, with a stabilizing effect on the depressive mood.

The problem of diagnostic variability in general practice.

STUDY OBJECTIVE: The aim was to examine the scale, source, and relevance of variation between general practices in respect of the rates with which patients consulted with illnesses falling in each of several diagnostic groups. DESIGN: This study involved a general practice morbidity survey conducted over two years, 1970-72. All patients who consulted their general practitioners were identified and the number of these who consulted with diagnoses attributable to each of the 18 main chapters of the International classification of diseases were counted. Patients who consulted for more than one diagnosis within a chapter were counted once only; those who consulted for one or more diagnoses in each of several chapters were counted once for each chapter. SETTING: This was a national survey involving general practitioners in England and Wales. SUBJECTS: The study involved 214,524 patients from 53 selected general practices (115 doctors) who were registered with their general practitioners for the whole of the year 1970-71 and for whom their morbidity data had been linked with their social data from the 1971 census. MEASUREMENTS AND MAIN RESULTS: Using the numbers of patients on the practice lists as denominators, practice patient consulting rates (PPCR) were calculated for each practice and for each ICD chapter. Variability in chapter PPCR was examined by calculating coefficients of variation and, after allowance for random variation, coefficients of residual variation. There were large interpractice (doctor) variations in all chapter rates. These variations were only marginally attributable to: chance; different age, sex and social class mixes of practice populations; geographical locations; and practice organisation. The rates were, however, consistent from one year to the next for any one practice. Approximately half of the interpractice (doctor) diagnostic variability was associated with overall patient consulting behaviour. When the effects of this behaviour were discounted, any major residual diagnostic variability was confined largely to ICD chapters I-V, XVI, and XVII, ie, those chapters where aetiology forms the basis of classification. CONCLUSION: Variations in recorded diagnostic rates are mainly due to the consistent but idiosyncratic and selective exclusion by practitioners of some components from the total set which often coexist in a new diagnosis. Because of the scale of interpractice diagnostic variability, the use of algorithms and information technology is largely precluded from outcome studies, auditing procedures, and studies of practice work loads in general. However, (1) the consistency of any individual doctor's pattern of diagnostic recording from one year to another permits studies of trends; and (2) given a reasonable number of recording practices, the population mean practice consulting rates can be estimated with sufficient accuracy for many epidemiological research and administrative uses.

Use of tetramethylammonium hydroxide to determine the total fatty acid composition of microorganisms.

A destructive chromatographic method of identifying microorganisms is described. The method is based on hydrolysis of the biomass of the microorganisms in the presence of tetramethylammonium hydroxide, followed by introduction of the hydrolyzate into the heated injector of a gas chromatograph. The main products in this case are methyl esters of fatty acids, the composition of which is used as a diagnostic criterion. Chromatograms of the residues after chloroform-methanol extraction and the dynamics of the change in total fatty acids in the process of growth can also be used for identification. The dynamics of total fatty acids was studied in E. coli strain K-12, and destructive chromatograms for bacteria and yeasts are cited.

The relationship of vallecular residue to oral involvement, reduced hyoid elevation, and epiglottic function.

The purpose of this investigation was to identify certain variables that may result in vallecular residue after swallowing. The relationship between vallecular residue and oral-stage dysphagia, reduced hyoid elevation, and movement of the epiglottis was assessed in 330 patients referred to the speech pathology section for evaluation of oropharyngeal swallowing function. Patients with vallecular residue were more likely to have oral involvement or deviant epiglottic function, characterized by an absence of epiglottic inversion or incomplete inversion, than patients without vallecular residue. Although the primary focus of the study was between vallecular residue and the other select variables, the interrelationships among the three other variables were also studied. Patients with deviant epiglottic function were more likely to have oral involvement, reduced hyoid elevation, or vallecular residue than patients without deviant epiglottic function and patients with reduced hyoid elevation were more likely to have oral involvement than those with normal hyoid elevation. Multivariate analysis revealed that the relationship between oral involvement and deviant epiglottic function, present in the bivariate analysis, was not significant when controlling for other relationships in the model. The relationship between primary medical diagnostic category and the presence/absence of vallecular residue as well as the other dichotomous variables varied between and within diagnostic categories.

Quantification of the impact of known risk factors on time trends in non-Hodgkin's lymphoma incidence.

The incidence of non-Hodgkin's lymphoma among white men in the United States was measured as 6.9/100,000 person-years in 1947-1950 and as 17.4 in 1984-1988. We have estimated how much the known and suspected diagnostic and risk factors might have contributed to this apparent increase of 152%. Firm conclusions cannot be drawn without more data on risk and changes in prevalence, but a reasonable range of impacts can be constructed. After accounting for the likely effects of misdiagnosis of Hodgkin's disease as non-Hodgkin's lymphoma, of the acceptance of new entities of non-Hodgkin's lymphoma, of familial factors, of human immunodeficiency virus and other immunosuppressive conditions or drugs, and of occupation, we estimate that the percentage increase in incidence was still 80% among all males and 42% among those aged 0-64. An agent carrying a relative risk of 2.0 rising in prevalence from 0 to 42% would account for the latter rise. Diet, hair dyes, and general environmental exposures to pesticides may be contributing, but currently estimated risks and changes in exposure levels do not appear large enough to account for the residual rise. Among men aged 75-84, some of the residual rise of 109% probably is diagnostic, but only further research will clarify the issue.

Studies of the association and conformational properties of metal-free insulin in alkaline sodium chloride solutions by one- and two-dimensional 1H NMR.

One- and two-dimensional 1H NMR spectroscopy have been employed to probe the association and subsequent conformational changes of metal-free insulin in sodium chloride solution at pH 9 and 9.4. These studies establish that the proton resonances of His(B5) and His(B10) are useful signatures of aggregation and conformation. Changes in chemical shifts and areas of resonances due to the C2 protons of His(B10) and His(B5) and transfer of magnetization experiments served to identify the association as the assembly of tetramer from dimers under our experimental conditions (pH 9.4, [insulin] greater than 1 mM, [NaCl] = 0.1 M). Sodium chloride also alters the equilibrium distribution of species in favor of a tetrameric species. The association equilibrium constant was estimated from area measurements to be approximately 5 x 10(3) M-1 at pH 9.4, 26 +/- 0.1 degrees C, and 0.1 M sodium chloride. Under conditions of 0.1 M sodium chloride concentration, nuclear Overhauser effect experiments in the one- and two-dimensional modes revealed an operative nuclear Overhauser effect between the His(B5) C2 protons and the 2,6 ring protons of a Tyr residue provisionally assigned as Tyr(B16). We conclude that this interaction is a diagnostic signature of a conformational transition whereupon an extended chain from residues B1 to B9 (T-state) is transformed into an alpha-helix (R-state) thus bringing the rings of His(B5) and Tyr(B16) from adjacent subunits across the monomer-monomer interface into van der Waals contact. This conformational flexibility is an added consideration to the discussion of the relevant structure of insulin for receptor binding.

HIV seroprevalence in hospital patients in rural Georgia.

To determine human immunodeficiency virus (HIV) seroprevalence among hospital patients in three rural community-based hospitals in southern Georgia, we anonymously tested patients 15 to 54 years old for antibodies to HIV-1 from residual blood specimens collected for routine diagnostic purposes. Data collected included age, sex, race, hospital service, presenting condition, physician's knowledge of HIV infection status, and discharge diagnosis. Of 1319 patients tested, seven (0.5%, 95% confidence interval = 0.2% to 1.1%) had antibodies to HIV-1. Of those seven, five had HIV infection unsuspected by their physicians, and four had an infectious disease. HIV seroprevalence was 0.5% for both men and women, 1.0% for blacks, and 0.3% for whites. HIV-positive patients were found on all hospital services. These results suggest that in rural southern Georgia hospitals, HIV should be routinely considered in the differential diagnosis of conditions that may be related to HIV. In addition, these data demonstrate a need for medical services or referral networks for HIV-related illnesses and a need for continuing HIV counseling and testing offered by local health departments in rural southern Georgia.

[Post-extrasystolic potentiation for the study of regional left ventricular asynergy (author's transl)].

In 120 P. with ischemic heart disease, left ventricolar wall motion was analyzed from the left ventriculogram in a regular sinus beat as compared to the first beat following one or more premature ventricular contractions (PExP). The response of asynergic segment to PExP was determined with a qualitative as wall as a quantitative technique. Of a total of 225 asynergic segments, 62% showed a positive response to PExP. In the absence of pathologic Q waves in the electrocardiogram, the response was positive in 77% and negative in 23% of the cases. In the presence of Q waves, PExP was present only in 33% of the cases (p less than 0.001). In respect to the severity of asynergy, 68% of 145 hypokinetic segments and 52% of 80 akinetic-dyskinetic segments responded to PExP; in the presence of pathologic Q waves, the number of positive responses decreased to 36% in the case of segmental hypokinesis, and to 31% in the case of more severe asynergy. The study of PExP has appeared as an useful diagnostic technique, capable of detecting a residual myocardial function in left ventricular asynergic segments, even in the presence of electrocardiographic signs of infarction.

Diagnostic Implications and Correlates of Plasma Adenosine Deaminase 2 Activity and ADA2 Variants.

OBJECTIVE: Deficiency of adenosine deaminase 2 (DADA2) is a monogenic autoinflammatory disease manifested as polyarteritis nodosa, stroke, and bone marrow failure. Leveraging an international cohort of 200 DADA2 cases, we aimed to characterize the diagnostic utility of a plasma ADA2 enzyme activity assay and understand the implications of residual ADA2 activity. METHODS: Data were collected from individuals who underwent ADA2 testing from 2018 to 2025. Plasma ADA2 activity was determined using an established spectrophotometric assay. ADA2 variants were analyzed in transfected cells by enzyme assay and western blotting. RESULTS: We determined that plasma ADA2 activity is 99.0% and 96.0% sensitive and 99.7% and 98.8% specific in distinguishing genetically confirmed DADA2 cases from controls and carriers, respectively. Eighteen individuals with DADA2 (9%) possessed detectable ADA2 activity, including several cases with levels seen in carriers. Residual ADA2 activity was associated with the vasculitis/inflammatory phenotype but not with disease severity. Genotype analysis revealed that 14 of 18 cases with residual plasma activity possessed at least one hypomorphic missense variant with greater than 20% residual ADA2 function when overexpressed in 293T cells, often occurring in trans with a more deleterious variant. In vitro analysis revealed that missense ADA2 variants exert variable dominant-negative effects by forming large intracellular protein aggregates via disulfide bond formation at a cysteine residue (Cys408). CONCLUSION: We confirmed the utility of plasma ADA2 activity as a diagnostic assay and showed that the inflammatory phenotype of DADA2 occurred in cases with residual activity. In vitro findings illustrate potential interactions of ADA2 variants to synergistically disrupt protein function.

The residual meniscus as a cause of persistent ache of the knee is a rare condition. Permanent trouble demands an active instrumental diagnostic approach. A reliable diagnostic measure is repeated arthrography. Operative removal of the residual menicsus is the treatment of choice.

Capillary permeability of 99mTc-DTPA and blood flow rate in the human myocardium determined by intracoronary bolus injection and residue detection.

The aims of the present study were to quantitate blood flow rate and capillary permeability of 99mTc-DTPA in the human myocardium and to assess whether capillary permeability is influenced by the presence of small degree atherosclerotic lesions. Myocardial blood flow rate and capillary permeability of 99mTc-DTPA were quantitated during coronary angiography by the single injection, residue detection method. Eighteen patients undergoing a diagnostic coronary angiography because of uncharacteristic chest pain were studied. In 12 patients with angiographically normal epicardial coronary arteries, a mean value of the capillary permeability-surface area (PS) product of 54.0 ml.(100 g.min)-1 (SD 13.0) was determined from a capillary extraction of 55.0% (SD 9.4%) and a regional myocardial plasma flow rate of 74.6 ml.(100 g.min)-1 (SD 6.3) equivalent with a regional myocardial blood flow rate of 121.7 ml.(100 g.min)-1 (SD 13.0). Similar values of blood flow rate, capillary extraction fraction and the PS product were determined in 6 patients with localized coronary atheroma without hemodynamically significant coronary artery stenosis (25-50% luminal narrowing). The values for the regional myocardial blood flow rate in the human heart are in accordance with values obtained by other methods, and the PS product of 99mTc-DTPA is similar to results obtained in canine hearts. Localized coronary atheroma leading to insignificant coronary artery stenosis does not produce any measurable dysfunction of the coronary microcirculation, in terms of capillary permeability and blood flow rate, during resting conditions.

Massive infestation by Ascaris lumbricoides of the biliary tract: report of a successfully treated case.

This is a report of a 25 years old black woman from the city of São Paulo, Brazil, who developed acute obstructive cholangitis of Ascaris lumbricoides with septicemia and multiple hepatic abscesses. The patient had sickle cell trait and normal delivery 3 months ago. Massive infestation of the biliary tract by Ascaris lumbricoides was diagnosed by abdominal ultrasonography and endoscopic retrograde cholangiography. Sixty worms were removed from the common bile duct and hepatic abscesses were drained by surgery. The infectious process was polymicrobial. The patient's recovery was complete after a long evolution with a wide spectrum antibiotic therapy. New surgeries were needed to remove residual worms in the biliary tract. The diagnostic methods, clinical-biochemical features and also the clinical and surgical management are presented. The biliary ascariasis pathophysiology is commented.

[Results of computerized tomography after Whipple operation of chronic pancreatitis].

53 patients were controlled with CT after Whipple resection. The median age of the patients was 49 years, the median time after surgery nine years. Diagnostic criteria were the identification of the residual pancreas, signs of pancreatic atrophy and radiological signs of chronic pancreatitis. CT was performed with slice thickness of 4 mm and steps of 4 mm. The residual pancreas was visible in all 53 cases. Pancreatic atrophy was found in 26 cases, in four of these the residual pancreas was only 2 mm in size. Signs of chronic pancreatitis were found in 16 cases. By comparing preoperative and postoperative CT-scans, seven progresses of pancreatitis were detected. Signs of acute pancreatitis were not found in the 53 patients. Fatty liver degeneration was found in 7.5%. We found that CT is the most reliable technique to visualize the pancreas after Whipple resection. CT should be performed in all patients with clinical symptoms of an acute pancreatitis. CT should also be performed in patients with residual pancreas pseudocysts. There is no indication for CT in the routine examination of asymptomatic patients after Whipple resection.

The enigma of post-radiation edema and recurrent or residual carcinoma of the larynx.

Persistence of significant edema of the larynx beyond six months after completion of radiotherapy presents the laryngologist with a diagnostic dilemma. Review of the results of 43 cases demonstrated a high incidence of residual or recurrent carcinoma. Based upon this experience earlier and frequent laryngeal biopsies are recommended for these cases.

The use of laparoscopy to determine the results of chemotherapy for ovarian cancer.

The chemotherapeutic management of patients with epithelial tumors of the ovary is hampered by a lack of diagnostic tests that are sufficiently sensitive for detecting residual or recurrent tumor. At the completion of a chemotherapy program, direct visualization of the peritoneal cavity is necessary to accurately assess the amount and location of residual tumor. Laparoscopy, with visually directed biopsies of residual tumor masses, can spare some patients a laparotomy and yet provide a safe and easy route for assessment of intraabdominal disease. If residual tumor is not seen at laparoscopy, than laparotomy with multiple peritoneal biopsies must be done so that residual tumor will not be missed and chemotherapy prematurely stopped. Laparoscopy is not an alternative to "second look" laparotomy but is a useful adjunct for determining the presence of resectable, unresectable or diffuse disease after chemotherapy.

Sex differences and distinct subgroups in schizophrenia. A study of 54 chronic hospitalized schizophrenics.

Sex differences with regard to age at first hospitalization and residual symptomatology were investigated in 54 long-term hospitalized chronic schizophrenics. Patients fulfilled diagnostic criteria of DSM-III, Kraepelin's dementia praecox and Leonhard's group of schizophrenias as well. The severe residual psychopathology necessitated continuous hospitalization in all the patients. The 27 women and 27 men neither differed in duration of illness or period of hospitalization nor in their social environment. We found that women were older than men at their first hospitalization and exhibited more marked positive symptomatology during the course and in the residual state than men. However, regardless of the residual symptomatology, men generally received more neuroleptics. A classification of the schizophrenic patients by means of the Leonhard criteria revealed that unsystematic schizophrenics (affect-laden paraphrenia, periodic catatonia, cataphasia) of both sexes were significantly (p less than 0.001) more often married at the time of first hospitalization than were systematic schizophrenics. Further, there was an overwhelming preponderance of women among the group of affect-laden paraphrenia and, conversely, of men in the group of periodic catatonia. Presuming different etiology in affect-laden paraphrenia and periodic catatonia, our findings suggest a significantly varied frequency in the rate in which women and men are afflicted by heterogeneous subgroups of schizophrenia.

[Research in schizophrenia: necessity to include patients of multiple diagnostic systems].

The discrepancies of studies on symptomatology and treatment of schizophrenia could be related to the selection of different patients diagnosed by one diagnostic system, different from a study to another. Therefore, we tested whether 14 diagnostic systems could include 51 patients differently as regard to the intensity of positive, negative or depressive symptomatology and to the phase of illness. The distribution of the patients in different sets of diagnosis has been carried out by a computer program and the symptomatology has been evaluated with PANSS and MADRS. Some diagnostic criteria like DSMIII-R, Langfeldt, Taylor, ICD 9 include negative and depressive patients preferentially. Others systems like Berner, Catego, ICD 9, New-Haven, Schneider, include more patients with acute than residual symptoms. These results show the importance of the choice of one or more diagnostic criteria depending on the aim of the study.
