[Antibiotic resistance of Pseudomonas aeruginosa strains isolated from patients with infected burns].

Sensitivity to 15 drugs of 248 P. aeruginosa strains isolated from patients with infected burns was studied by the method of agar dilution. All of the strains were resistant to polymyxin M, ceporin, erythromycin and oleandomycin. Most of the strains were resistant to streptomycin, monomycin, ampicillin and rifadin. Moderate resistance of the strains to carbenicillin, morphocycline, vibramycin, kanamycin, tetraolean and tetracycline was observed: the maximum concentrations of these antibiotics (128 microgram/ml) inhibited the growth of 85, 69, 63, 51.8, 43.6 and 41.2 per cent of the strains respectively. Gentamicin proved to be most active against the strains of P. aeruginosa and inhibited 87 per cent of the strains when used in the therapeutic doses. The study provided recomendation of the drugs for parenteral and local use in treatment of burns infected with P. aeruginosa.

[Variability of the microfloral makeup of the pathological focus in suppurative infection].

During the period of hospital treatment a change in the composition of would microflora in 145 patients was found to occur, which affected the characteristics of microflora in respect of species, types and strains. This change consisted in the appearance of new (secondary) species, variants and strains and in the disappearance of some original (primary) ones. The populations of staphylococci were found to vary in the level and spectrum of their resistance to antibiotics (resistovars) and in the degree and spectrum of their sensitivity to staphylococcal typing phages (phagovars). Changes in the populations of staphylococci were caused mainly by the cange of S. aureus or by the additional appearance of their new variants which belonged, as a rule, to phage group II, had multiple resistance to antibiotics and corresponded to the phagovars of the hospital flora. The development of hospital variants in open wounds led to an increased general level of resistance of staphylococcal populations to antibiotics.

[Heterogeneity in phage sensitivity in populations of S. aureus vegetating in patients and carriers].

It was found that the S. aureus populations vegetating in patients and carriers were heterogeneous by phagegroup reference of the strains isolated in 31.1 and 23.9% of cases, respectively, and in 35.9 and 26.9%--phagovars. The frequency and the extent of variability of the populations in the patients depended on the character of the disease, the site of infection and the association of the focus with the external environment, and in carriers--on the localization and the carrier category. S. aureus populations habituating in the patients with the primary purulent processes and mastitis, as in the closed processes and in cases of extrahospital infection consisted, with rare exception, of the strains of one phagovar. In a considerable number of patients with purulent processes after the incision (38.0%), with infected wounds (37.2%), and particularly with burns (78.6%) of the S. aureus population consisted of 2, 3 and even 4 phagovars. Variable were also populations vegetating in patients with open processes (47.1%) and in hospital (26.5%) and mixed infection (52.6%). The phage-type composition of the S. aureus populations in carriers was also characterized by heterogeneity, which was more pronounced in chronic carriers and in case of localization of the processes in the nose. On the basis of the data obtained the minimal number of cultures which should be studied from the microbial focus in order to obtain objective data on the composition of phagovars in the population of various categories of patients and carriers was calculated.

[Indices of microorganism sensitivity to antiseptics in apical periodontitis].

High incidence of detection of staphylococci aur. and epiderm.; streptococci hemolyt. and virid., and gram-negative microorganisms in dental root channels in periodontic patients was established. Among antiseptic drugs, most toward major periodontal++ initiators were pervomur, iodopyrone, chlorhexidine, ethonium.

[The epidemiological surveillance of hospital infections linked to methicillin-resistant Staphylococcus aureus].

As revealed in the realization of the epidemiological surveillance of hospital infections caused by methicillin-resistant S. aureus (MRSA) in different types of hospital, MRSA strains causing purulent inflammatory diseases belong to different clones. The complex marking of MRSA made it possible to determine the presence of the same clone in different hospitals and to detect the outbreaks of hospital infections caused by different clones of MRSA in one hospital. It was found necessary to supplement the commercial international phage-typing set with pages permitting the detection of the specific system of restriction-modification in MRSA.
