[Traumatic diaphragmatic hernia in children].

The article presents results of treatment of traumatic diaphragm hernias in 6 boys. The clinical picture, diagnosis and treatment of children with such a disease have been studied. The main cause of the appearance of the traumatic diaphragm hernia is shown to be late diagnosis of ruptured diaphragm associated with fractures of pelvic bones.

Adolescent↗

[Incarcerated diaphragmatic hernias in children].

Results of the treatment of 19 children aged from 1,5 months to 8 years with different forms of hiatal hernias are analyzed. Specific features of the clinico-roentgenological picture of the disease is described with special reference to the child's age and type of hernia. Methods of the surgical treatment are proposed.

Child↗

[Esophagocele with elevated esophagus in children].

The clinics of the Department of Surgery in Children of the Leningrad Pediatric Medical Institute during the period of 1951-1983 dealt with 29 children aged from some days to 9 years with hiatal hernias and the elevated esophagus. Results of the observation of patients with hiatal hernia and elevated esophagus have shown the clinical manifestations of the disease to be various. They can be divided into 4 clinical groups: 1) acute course, 2) subacute course, 3) incarcerated hernias, 4) asymptomatic course.

Acute Disease↗

[Clinico-x-ray characteristics of hernias of the esophageal hiatus of the diaphragm in children].

A classification of hiatal hernias has been developed on the basis of an analysis of the clinical picture and rentgenological examination of 63 children. For the diagnosis of hiatal hernias a method of rentgenological and endoscopic examination is proposed. Remote results of the operative treatment within the period of from 1 to 23 years of follow-up observations are favourable.

[Remote results of surgical treatment of intestinal trauma in children].

The authors studied long-term results of the surgical treatment of 67 children followed-up for 1 to 22 years. Good results were noted in 32 operated patients operated upon for isolated intestinal injuries mostly within the first hours after trauma and subjected to a complex postoperative management, devised in the clinic. 17 children operated 12 and more hours after trauma who developed various complications showed fair results. 50% of them underwent antiadhesive therapy; full volume infusional therapy was not carried out. Poor results were observed in 18 patients operated mainly for a combined intestinal trauma. These patients were not subjected to a complete complex of therapeutic measures and developed various complications.

Abdominal Injuries↗

[Relaparotomy in abdominal injuries in children].

An analysis of 18 relaparotomies in 12 children with injuries of the abdominal organs is presented. Inadequate measures against paresis immediately after laparotomy are felt to be the main cause of reoperations for an adhesive-dynamic form of the obstruction. While accomplishing relaparotomy, it is recommended not to resort to reconstructive operative procedures in cases where a palliative operation promptly liquidating complications would suffice.

Abdominal Injuries↗

[Subcutaneous injuries of the duodenum in children].

In Child Surgery clinics of the Leningrad City since 1955 to 1974 eight children, aged from 3 to 15 years, were operated upon for this kind of trauma. The diagnosis of duodenal injuries intraoperatively is based on the presence of retroperitoneal hematoma, emphysema in the retroperitoneal space, yellow-green staining of the posterior peritoneal leaf.

Abdominal Injuries↗

[Characteristics of general anesthesia in video thoracoscopic surgery in children].

Videothoracoscopic operations were carried out in 54 patients aged 6 months to 14 years. Two variants of general anesthesia were used: bolus injection of fentanyl in combination with calypsol and infusion of fentanyl in combination with subnarcotic doses of fluothane. One-lung ventilation was carried out in all children. Hemodynamic status, gaseous composition of the blood, and cerebral bloodflow were monitored during general anesthesia.

[Choice of the optimal scale for evaluation of the severity of sepsis in children].

Quantitative assessment of the severity of clinical status was carried out and prognostic values of PRISM III, PRISM, SOFA, APACHE II scores and scores proposed by A. Castellanos et al. and K. L. Goitein was evaluated in 105 children (2 months-14 years) with sepsis. Clinical status evaluated in score during the first day of intensive care was correlated to the disease outcome. Sensitivity, specificity, expected values of positive and negative results were evaluated for each score and their discrimination capacity was assessed by ROC analysis. Use of quantitative scores (PRISM, PRISM III, SOFA, APACHE II, and A. Castellanos') is permissible for prospective evaluation of the efficiency of intensive care in children with sepsis, PRISM being the most informative.

APACHE↗

[Hypoxic changes in metabolism and effectiveness of therapy of surgical sepsis in children].

Examinations of 64 children with surgical sepsis, 12 of whom developed multiorgan insufficiency, showed that reduced oxygen transport and hypoxic changes of metabolism aggravate the endogenous intoxication. Hence, normalization of gas exchange and liquidation of oxygen debt by means of artificial ventilation of the lungs performed according to modern schemes becomes an important and obligatory component of intensive care for grave surgical sepsis.
